If you change your f<strong>am</strong>ily n<strong>am</strong>e, please forward your request to CARNA<br />
by mail along with the supporting documentation.<br />
According to the Health Pr<strong>of</strong>essions Act, members have a responsibility to<br />
notify CARNA as soon as possible <strong>of</strong> any changes related to their personal<br />
information such as address, telephone number and employer information.<br />
