<strong>Hip</strong> <strong>and</strong> <strong>knee</strong><br />

<strong>implant</strong> <strong>prices</strong> <strong>rise</strong> <strong>7.0%</strong><br />

Manufacturer list <strong>prices</strong> for hip <strong>and</strong> <strong>knee</strong> <strong>implant</strong>s will increase<br />

7% between 2000 <strong>and</strong> 2001 according to a survey performed by<br />

<strong>Orthopedic</strong> <strong>Network</strong> <strong>News</strong>. Porous coated hip systems (socalled<br />

“high dem<strong>and</strong>”) rose 5.7% to an average of $6,550;<br />

medium dem<strong>and</strong> cemented hips rose 6.4% to $4,899, <strong>and</strong> low<br />

dem<strong>and</strong> hips with polyethylene cups rose 6.7% to $2,263 in a<br />

survey of nine manufacturers. Porous coated <strong>knee</strong> <strong>implant</strong>s rose<br />

8.0% to an average of $6,191, cemented <strong>knee</strong> <strong>implant</strong>s, which<br />

are more widely used than porous coated devices, rose 7.9% to<br />

$5,037, <strong>and</strong> low dem<strong>and</strong> <strong>knee</strong> <strong>implant</strong>s, with a polyethylene<br />

tibial component rose 8.3% to $3,623. Partial hip <strong>implant</strong>s,<br />

generally used in elderly fracture cases, rose 7.4% for a bipolar<br />

hip <strong>implant</strong> to $2,577, while low dem<strong>and</strong> endoprostheses rose<br />

5.2% to an average of $1,093 in the survey.<br />

This year’s survey includes nine manufacturers, compared to<br />

last year’s twelve, because of the consolidation of Depuy <strong>and</strong><br />

Johnson & Johnson <strong>and</strong> Howmedica with Osteonics, as well as<br />

the exit of Implex from the direct sales market. Implex’ <strong>implant</strong>s<br />

have been absorbed by Zimmer which will sell their Hedrocel<br />

cups to their hospital clients, which increased the price about<br />

20-25% from the price charged by Implex in February 2000.<br />

It should be stated that the increases reported are the increases<br />

in manufacturer list <strong>prices</strong>, <strong>and</strong> not necessarily the <strong>prices</strong> paid<br />

by hospitals. Many manufacturers argue that increasing the<br />

<strong>prices</strong> allows them more flexibility in discounting. “Mark them<br />

up, mark them down,” as one puts it, in describing the cycle of<br />

manufacturer price increases followed by subsequent hospital<br />

discounts. The <strong>7.0%</strong> price increase follows an 8.7% manufacturer<br />

price increase between 1999 <strong>and</strong> 2000. Surveys of hospital<br />

subscribers also report significant price increases in their<br />

orthopedic <strong>implant</strong>s (see page 11) in the last several years.<br />

The manufacturers recording the smallest price increases are<br />

those that had the largest ones last year: last year Biomet<br />

reported a 16.3% price increase, this year’s was 3.4%, the<br />

lowest of the surveyed manufacturers. Smith & Nephew<br />

reported a 15.2% increase last year, compared to a 6.2%<br />

increase this year. Encore <strong>Orthopedic</strong>s, meanwhile had a 10.9%<br />

increase last year which was followed by a 10.6% increase this<br />

year, the largest reported by any manufacturer. Sulzer’s 7.9%<br />

increase last year was followed by a 9.1% increase between<br />

2000 <strong>and</strong> 2001.<br />

The introduction of new technology plays a partial role in these<br />

price increases. The “high dem<strong>and</strong>” hip category includes a<br />

cross-linked polyethylene liner, designed to withst<strong>and</strong> wear<br />

etter than normal polyethylene, is included in the <strong>prices</strong> of<br />

Depuy, Osteonics, Zimmer, <strong>and</strong> Smith & Nephew. These<br />

polyethylenes typically cost $300-$500 more than “normal”<br />

polyethylenes. These additions have pushed the cost of a porous<br />

coated hip system from DePuy to $7,170, the highest of those<br />

surveyed.<br />

Other hip <strong>and</strong> <strong>knee</strong> technologies have not been factored into the<br />

calculated <strong>prices</strong>, including the metal-on-metal <strong>implant</strong>s sold by<br />

Biomet (M2a), <strong>and</strong> Sulzer Orthopaedics (Metasul). The Biomet<br />

version has a list price of $7,412, <strong>and</strong> the Sulzer version is<br />

$6,865. Soon to be released are more metal-on-metal hips,<br />

ceramic on ceramic hips (Biomet), ceramic on ceramic <strong>knee</strong>s<br />

(Smith & Nephew), mobile bearing <strong>knee</strong>s (DePuy <strong>and</strong> Smith &<br />

Nephew, among others).<br />

A high dem<strong>and</strong>, porous coated hip system ranged in price from<br />

DePuy’s $7,170 for a Prodigy stem with a Marathon crosslinked<br />

polyethylene liner to $5,720 for Exactech’s AcuMatch<br />

porous coated system. Cemented hips ranged in price from<br />

Zimmer’s Versys/Trilogy at $5,254 to Exactech’s Acumatch at<br />

$4,420. Infrequently used low dem<strong>and</strong> hip systems ranged in<br />

price from Smith & Nephew’s Matrix at $2,874 to Exactech’s<br />

AcuMatch with an all poly cup at $1,910.<br />

High dem<strong>and</strong> <strong>knee</strong> <strong>implant</strong>s ranged in price from Biomet’s<br />

Maxim <strong>and</strong> DePuy’s PFC porous at $6,650 to Exactech’s<br />

Optetrak at $5,500. Cemented <strong>knee</strong> systems ranged in price<br />

from Wright’s Advantim at $5,515 to Exactech’s $4,385 for an<br />

Optetrak, <strong>and</strong> low dem<strong>and</strong> <strong>knee</strong>s with an all poly tibia ranged in<br />

price from $4,000 for a Depuy PFC to $2,993 for a Smith &<br />

Nephew Genesis II.<br />

Manufacturer price lists were obtained, <strong>and</strong> <strong>prices</strong><br />

effective on January 1, 2000, were compared to those<br />

effective on January 1, 2001. This is the tenth annual<br />

survey of hip <strong>and</strong> <strong>knee</strong> <strong>implant</strong>s which has been<br />

conducted by <strong>Orthopedic</strong> <strong>Network</strong> <strong>News</strong>. For each<br />

category of <strong>implant</strong> system <strong>and</strong> dem<strong>and</strong> level,<br />

representative components were priced <strong>and</strong> compared<br />

for the time periods. There were approximately 260<br />

components surveyed by <strong>Orthopedic</strong> <strong>Network</strong> <strong>News</strong><br />

which were believed to be representative of the most<br />

commonly used systems <strong>and</strong> configuration of systems.<br />

Where possible, <strong>prices</strong> <strong>and</strong> usage were reviewed <strong>and</strong><br />

verified with the manufacturers of the systems.<br />
