What the laparoscopist next door is reading. Or should be.
Are you getting the macro view on minimally invasive? The publications of the Society of Laparo-
endoscopic Surgeons provide essential multidisciplinary perspective and a far richer cross-reference
of experiential knowledge than a single-specialty organization can provide.
The peer-reviewed JSLS, Journal of the Society of Laparoendoscopic Surgeons brings important new
developments in laparoscopic surgery to SLS members in print and online. The journal’s rapid
review process ensures that significant scientific findings are presented with minimal delay.
From SLS, MIRA & SRS
Dear Colleagues,
This first annual Minimally Invasive Surgery Week is shaping up to be an amazing, exhilarating,
information-packed meeting. Join SLS, MIRA, and SRS for professional development,
interaction, debate, and the sharing of experiences in a format not found in other conferences.
We look forward to seeing you all in Boston!
Sincerely,
Paul Alan wetter, MD
Professor Emeritus, University of Miami School of Medicine
Chairman, The Society of Laparoendoscopic Surgeons
Dear Colleagues,
As the 21st President of the Society of Laparoendoscopic Surgeons (SLS), I welcome you to
participate in the SLS • MIRA • SRS Joint Annual Meeting and Endo Expo 2012 in Boston.
This year, the Society of Robotic Surgeons and the Minimally Invasive Robotic Association
will join us and provide a new dimension of scientific interactions for the meetings. The
Society of Laparoendoscopic Surgeons traditionally has offered the most up to date information
in minimally invasive surgery to our participants. With the added insight from our
robotic colleagues, this meeting will promise to elevate discussion of the pros and cons of the
future technologies and laparoscopic surgery options. Your participation, interaction, and
expertise will be valued and appreciated in this very exciting joint venture. I look forward
to seeing all of you in SEPTEMBER for our own BOSTON ENDO PARTY.
Maurice K. Chung, RPh, MD, ACGE
President, Society of Laparoendoscopic Surgeons (SLS)
Dear Colleagues,
It gives me great pleasure to welcome you at the first joint SLS, MIRA, SRS Meeting here
in Boston. We are all united by the same vision: to perform patient-friendly surgery through
minimal access with the aid of scopes, video cameras, and specific endoscopic instrumentation.
Surgical robots are such specific tools, highly developed already, but still in their infancy.
The potential of robotic technology in endoscopic surgery, however, is huge and we are
here to share experience and thoughts on how we can use it best for making surgical procedures
even more precise and even less invasive. Robotic surgery only works in a functioning
team. For the success of all endoscopic surgery scientific societies in the field should
cooperate as well. This joint meeting is a step in this direction and is a great opportunity for
exchange of knowledge and development of common treatment strategies. I wish all participants
an exciting and interesting conference on laparoendoscopic and robotic surgery.
Johannes Bonatti, MD
President, Minimally Invasive Robotic Association (MIRA)
Dear Colleagues,
The first joint SLS, MIRA, SRS meeting in Boston represents a coming together of three
societies who have been integral in the global promotion of minimally invasive surgical techniques.
Over the last decade there has been an ever-growing interest in the use of cuttingedge
robotics in surgery. This meeting will provide a forum to discuss the rapid evolution in
techniques and outcomes as well as areas of controversy from some of the most experienced
robotic surgeons in the world. This year’s program has been designed to address the needs of
beginners all the way to the mature and seasoned robotics user. Join us in Boston for what
will be an exciting and informative event for everyone.
Smart Surgeons Learn From Their
Mistakes, Brilliant Surgeons Learn From
Other Surgeons’ Mistakes: Prevention
and Management of Laparoscopic and
Endoscopic Surgical Complications
8:00-11:30am; 3.5 AMA PRA Category 1 Credits TM
All abdominal and pelvic MIS procedures
carry an inherent risk of complications. This
interactive course will present a philosophy
for the prevention and management of
complications during minimally invasive
surgery of the abdomen and pelvis. Video
segments and case presentations will be
used to demonstrate principles and stimulate
discussion based on participant needs
identified by responses to the pre-course
questionnaire. Source material will be
obtained from students’ “real world”
experiences and supplemented with materials
selected by the faculty panel and customized
based on pre-course questionnaire
results. An interactive student and faculty
discussion format will be utilized. This
course will discuss management paradigms
to prevent, recognize, and treat complications
appropriately. Careful, methodical
assessment and strategies for appropriate
action will be stressed. Topical presentations
based on student input will highlight
detailed surgical anatomy, sound surgical
principles and careful technique.
NOTE: Syllabus materials are provided as a
reference resource and are not intended as
a verbatim summary of course content,
which is largely video-based. Participants
are strongly encouraged to anonymously
submit video or other case material prior to
the course to maximize topical relevance for
their individual practice and needs. Please
Advanced Laparoscopic Suturing for the
Gynecologist, Urologist, General and
Robotic Surgeon “THE Vertical Zone – The
Next Level beyond Triangulation”(Including
Primer for Effective Robotic Suturing)
8:00am-4:00pm; 7 AMA PRA Category 1 Credits TM
Be one of the 90% of participants who can
throw an intracorporeal knot in < 3 mins.
After 90 minutes instruction in the ‘vertical
zone’!
Participants will learn the uses and limitations
of laparoscopic suturing with the
‘Triangulation’ algorithm. For gynecologists
closing the uterus and vagina transversely,
urologists anastomosing urethra to bladder,
performing uretero-ureterostomy, surgeons
closing colon and rectum transversely – the
needle needs to move in the sagittal plane,
not the side to side or coronal plane of the
triangulation style. The ‘Vertical Zone’
describes our technique of suturing with
two hands ipsilaterally that allows the needle
to operate in the sagittal plane, while
permitting a restful and relaxed attitude of
the elbows, forearms and hands. Primer on
robotic suturing and knot tying will be
shown and discussed, so that the participants
truly understand suturing applications
across all platforms.
In numerous courses nationally and internationally
this algorithm has allowed over 80%
of participants to succeed in tying an intracorporeal
knot within 3 minutes after 2
hours of training.
With excellent fidelity, the relative hand
positions and movements are immediately
transferable from the trainer to the O.R. This
course equips all attendees with improved
suturing skills and insight into applications
during surgery.
ORReady is a worldwide,
multispecialty
initiative meant to encourage
steps that are
known to improve surgical
outcomes and
save lives. If the current
guidelines such
as Check Lists, Time
Outs and Warm Ups
are followed routinely,
it is estimated that six
million patients around the world could have
better outcomes through error reduction.
That's 2-3% of the approximately 250 million
surgeries that are performed worldwide each
year. This course is meant to improve awareness
of safety and outcome issues and focus
on how to implement systems that promote
the penetration of these measures into the
culture of the operating room environment
and beyond. Validated warm-up procedures,
introduction of empowering patients and
referring physicians in the peri-operative
process; and suggestions for better communication
amongst the peri-operative team
will be presented. Nurses, surgical technicians,
Robotics represents yet another revolution in the application of minimally invasive techniques to
surgery. While conventional video endoscopic techniques were revolutionary in their own right,
they were hampered by limited instrument maneuverability and two-dimensional visualization.
These technological shortcomings took away the wrist-like motion of the human hand and the
depth perception of human eyes and necessitated the design of “new procedures” which were
adapted to the technology. Robotics by virtue of wrist-like instrument maneuverability and threedimensional
visualization has returned the advantages of the human wrist and eyes to the field of
minimally invasive surgery. For the first time in the history of minimally invasive surgery, operations
which were designed to be performed by open incisions can be replicated using minimal
access techniques.
A series of brief presentations by experts in the field will outline the State of the Art in the application
of Robotics to the specific field in surgery. These lectures will help the attendee to develop a
critical eye and assist them in the adoption of robotics into their practice.
KEYNOTE LECTURE
WHEN WILL MINIMALLY INVASIVE
AND ROBOTIC SURGERY REPLACE
ALMOST ALL OPEN SURGERIES?
Wednesday, September 5, 2012
11:30am–12:30pm
Minimally invasive surgery is developing
fast, covering an ever growing number of
operations and disciplines. Dr. Camran
Nezhat’s keynote address, entitled, “When
Will Minimally Invasive and Robotic Surgery
Replace Almost All Open Surgeries?” is a
review of laparoscopic surgery from its
inception to the present day. Professor
Nezhat, often referred to as a “Wizard” in
Laparoscopy, will present his personal view,
in an odyssey of science and technology,
and explain where minimally invasive and
robotic surgery is heading. Laparotomy was
first developed in the eighteenth century with
a vertical incision. In the twentieth century, it
was proven that a Pfannensteil incision has
superior wound healing to a vertical incision.
In the twentieth century, curious minds
started to explore laparoscopy, but it was not
until Dr. Nezhat introduced video-assisted
endoscopy in the 1970’s and 1980’s and performed
the most extensive pathology by this
technique, that the true potential of the
method was realized. The talk ends with a
look to the future and the technological
advances in minimally invasive and robotic
surgery in recent years. Taking an interactive
approach, Dr. Nezhat hopes that the audience
will help him to answer the question
posed in the title. In his words, “The objectives
of the lecture are: to motivate the
younger physicians, bring hope to their work
and education and encouragement in their
careers.”
SPECIAL WORKSHOP
DO YOU HELP STUDENTS/
RESIDENTS BECOME BETTER
LEARNERS? There may be
nothing you do as a teacher
that is more important!
Wednesday, September 5, 2012
4:00pm–5:00pm
The worlds of medicine and surgery are
changing very rapidly. None of us would be
safe if we depended exclusively on what we
learned in medical school and residency.
More than ever, physicians need to be effective
learners, and able to make needed
changes throughout their careers. In this
workshop we’ll ask and explore these questions:
1. Do you help your students/residents
become effective learners? (Do you
know how to do it?)
2. How effective a learner are you? (What
makes you effective?)
3. What are some key characteristics of
effective learners?
4. What can get in the way of being
& becoming effective learners? (Do
any of these conditions exist in your
current program?)
5. What are some strategies for helping
learners develop and refine their learning
skills? (In individual encounters; in
the overall teaching program?)
6. Are there some high-priority changes
you would like to make in your program?
(In your own teaching; in the
overall program?)
WORKSHOP GOALS
In this workshop we’ll encourage you to
reflect on, and discuss, some basic questions
toward achieving the goals of:
1. understanding the importance of being
an accomplished learner;
2. clarifying your thinking about your own
learning strategies;
3. formulating some ideas for improving
the ways in which your students/residents/fellows
are helped to become
effective learners;
4. identifying any changes you feel are
indicated in your own and your colleagues’
day-to-day teaching and
learning activities.
SURGERY IS A TEAM SPORT
Thursday, September 6, 2012
8:30am – 9:45am
All championship teams in every sport
become victorious because EVERYONE
plays together; so should be the same when
taking care of people in the Operating Room
(OR). There can be several superstars in
your OR; however, you are only as good as
your weakest link. The surgeon, anesthesia
person, circulator, scrub person, central
supply, maintenance, administration AND
the patient are all part of the team. If a
patient does not follow their peri-operative
instruction, the surgeon is poorly prepared,
or the equipment is not well maintained are
all examples of the TEAM not working properly.
These examples pertain to EVERYONE
that works in an OR; regardless whether
these procedures occur in an office, surgery
center or tertiary care OR. Additionally, ALL
surgical specialties are involved and ALL
societies are involved in this common goal
of being OR READY for the patient.
This session will explain how important the
Surgical Team is for excellent patient care
from the perspective of an Anesthesiologist,
Hospital Administrator and Surgeon. Additionally,
the understanding of how your outcomes
will be measured from the insurers
view point will be explained. Finally, we will
provide insight into how to construct a
check list for safe surgical care and its
implementation
Thursday, September 6, 2012
10:30am – 11:30am
Leading authorities in the field of MIS will
describe, discuss, and defend the techniques
of minimally invasive and more minimally
invasive surgery. The four major
modalities will be presented in chronological
order of appearance in the surgical
arena. Comparisons and contrasts will be
made to enlighten participants relative to the
superiority of each technique vs. all of the
others. Traditional laparoscopic surgery will
be defended by Dr. James (Butch) Rosser, Jr.
In another corner, Dr. Gustavo Carvalho will
extol the benefits of minilaparoscopy. From
yet another corner, Dr. David Rattner will
champion NOTES. Finally, Dr. Kevin Stepp
will take the mat to promote the virtues of
single incision laparoscopy. Lively, hopefully
civil debate will ensue. Questions, comments,
I COULD USE A ROBOT – BUT
SHOULD I? WHO, WHY, WHEN
AND WHERE?
JOINTLY WITH MIRA & SRS
Thursday, September 6, 2012
11:30am – 12:30pm
Robotic surgery has gained huge inroads
over the past decade in many surgical disciplines.
Its attraction to the patient as well as
surgeon is its minimally invasive approach,
apparent improved perioperative course,
and recovery. It is fun and cutting edge.
However, for certain procedures robotic
surgery has been utilized extensively prior
to clear evidence-based studies supporting
its comparative or superior effectiveness to
more traditional (gold standard) surgical
options. Furthermore, comparative studies
evaluating overall costs of robotic surgery
have been sparse. This debate will evaluate
the application of robotic surgery in the
areas of Urology, Gynecology, and General
Surgery. More specifically, common procedures
from each surgical discipline will be
discussed with respect to comparison of the
robotic approach to the traditional open or
laparoscopic options
….WORK TIME DIRECTIVES
Friday, September 7, 2012
7:40-8:40am
Is our profession being reduced to a mere job
– punching the clock? Work hours are being
defined by politicians and lawyers, not within
our profession. Will the 80 hour work week be
extended to every surgeon, not just residents?
Will the United States adopt the
European Work Time Directive (WTD) level of
work hours not to exceed 59 hours, or even
the current limit in Netherlands of 39 hours?
What is the real evidence on “fatigue”, and will
the surgeon be required to get consent from
patients that includes that they did not rest
enough before performing their surgery, as
proposed in the NEJM article? These and
other questions regarding the future of surgeons’
independent role in healthcare will be
discussed by a panel of experts – from here
and abroad – who have been involved with
these issues on behalf of our leading medical
Marc Raibert: presents Dynamic Legged Robots
Two areas that have held the imagination of everyone is that of robots
and the virtual world (as brought through the world of entertainment).
To bring the latest in advanced robotics, Marc Raibert of Boston
Dynamics will present some of the most revolutionary advances in
robotics to date As you watch these remarkable machines in his lecture
“Dynamic Legged Robots”, ask yourself how these could be
applied to advancing our care of patients.
We all enjoy being mesmerized by the special effects in the movies,
and marvel at the illusion of reality that is created. Mark Covey is from
Digital Domain, the company that brought such extraordinary movies
such as, I, Robot, Transformers, Tron, Titanic, Pirates of the
Caribbean, Lord of the Rings and many others. These technologies
could provide enormous benefit to not only our patients but to the
education of our surgeons. Tickets are required for accompanying
guests. See Registration Form.
CONTINUING MEDICAL
EDUCATION PROCESS FOR THE
SOCIETY OF LAPAROENDOSCOPIC
SURGEONS
In order to provide a high level of continuing
medical education for its members, The
Society of Laparoendoscopic Surgeons
(SLS) follows the Essential Areas and
Criteria of the Accreditation Council for
Continuing Medical Education in planning
and developing CME activities.
To help as a planning guide SLS has a CME
mission statement, which includes the CME
purpose, content areas, target audience, and
type of activities provided. SLS also identifies
the expected results of its CME activity.
SLS identifies the educational needs and
desired results for all CME activities.
SLS has an organizational structure that
supports our CME activities.
In order to achieve a level of CME excellence
for our members, SLS has developed a
“Template of Excellence.” This document
lists various examples of innovative ideas
that SLS utilizes to exceed the basic level of
compliance required by the ACCME. SLS
looks forward to receiving input from you in
order to continue to improve these processes
and provide you with a meaningful continuing
medical education in minimally invasive
surgery.
The complete SLS CME Process for
Continuing Medical Education is available
online at www.SLS.org.
