aricocele, one <strong>of</strong> <strong>the</strong> <strong>lead<strong>in</strong>g</strong> <strong>causes</strong> <strong>of</strong> <strong><strong>in</strong>fertility</strong> <strong>in</strong> <strong>men</strong>, is known to<br />

be a life style disease which develops over <strong>the</strong> course <strong>of</strong> time.<br />

There are many surgical and non-surgical treat<strong>men</strong>ts available for<br />

<strong>the</strong> issue.<br />

<strong>Varicocele</strong> – <strong>One</strong> <strong>of</strong> <strong>the</strong> Causes <strong>of</strong> Infertility <strong>in</strong><br />

Men<br />

Male's testicles are covered <strong>in</strong>side a scrotum, which is a sac like<br />

structure.<br />

This also conta<strong>in</strong>s arteries and ve<strong>in</strong>s that supply blood for <strong>the</strong> sex<br />

organs.<br />

Sometimes abnormal enlarge<strong>men</strong>t occurs <strong>in</strong> <strong>the</strong> ve<strong>in</strong>s; this condition<br />

is known as <strong>Varicocele</strong>.<br />

It is a widespread disease that can be found <strong>in</strong> 15% <strong>of</strong> <strong>the</strong> total male<br />

population. This usually starts develop<strong>in</strong>g <strong>in</strong> puberty and most <strong>of</strong> <strong>the</strong> time<br />

<strong>the</strong> targets left side <strong>of</strong> <strong>the</strong> scrotum. These can also occur on both sides, but<br />

it is rare.<br />

Not all types <strong>of</strong> <strong>the</strong> <strong>Varicocele</strong>s affect <strong>the</strong> production <strong>of</strong> sperms.


How does <strong>Varicocele</strong> develop?<br />

Ve<strong>in</strong>s are ma<strong>in</strong>ly responsible for tak<strong>in</strong>g <strong>the</strong> blood towards <strong>the</strong> heart for<br />

purification. In ve<strong>in</strong>s, <strong>the</strong>re are some valves which prevent <strong>the</strong> blood to<br />

come back. Because <strong>of</strong> our lifestyle and habits, <strong>the</strong>re is some imbalance <strong>in</strong><br />

<strong>the</strong> Ph <strong>of</strong> <strong>the</strong> cell, and <strong>the</strong> valves start develop<strong>in</strong>g some problems.<br />

This leads to <strong>the</strong> accumulation <strong>of</strong> <strong>the</strong> blood <strong>in</strong> ve<strong>in</strong>s which give rise to<br />

<strong>Varicocele</strong>s.<br />

Symptoms <strong>of</strong> <strong>Varicocele</strong><br />

In many cases, we do not see any symptoms <strong>of</strong> <strong>the</strong> disease. But some<br />

symptoms may be <strong>the</strong> sign <strong>of</strong> <strong>Varicocele</strong>s. These symptoms are:<br />

Lump on one side <strong>of</strong> testicles<br />

Swell<strong>in</strong>g <strong>in</strong> <strong>the</strong> scrotum<br />

Visible enlarged and twisted ve<strong>in</strong><br />

Recurr<strong>in</strong>g pa<strong>in</strong> <strong>in</strong> testicles<br />

<strong>Varicocele</strong>s are present <strong>in</strong> <strong>the</strong> 35 % <strong>men</strong> with primary <strong><strong>in</strong>fertility</strong> and 80%<br />

<strong>in</strong> <strong>the</strong> <strong>men</strong> with secondary <strong><strong>in</strong>fertility</strong>.<br />

Detection <strong>of</strong> <strong>Varicocele</strong> and Classification<br />

It can be detected by:<br />

Physical exam<strong>in</strong>ation <strong>of</strong> <strong>the</strong> testicles while <strong>the</strong> man is stand<strong>in</strong>g up<br />

and ly<strong>in</strong>g down.<br />

Scrotal ultrasound<br />

Angiography<br />

CT scan / MRI scan<br />

This cause <strong>of</strong> <strong><strong>in</strong>fertility</strong> is classified <strong>in</strong>to three grades:<br />

Grade I - In this, <strong>the</strong> dysfunctional ve<strong>in</strong>s are not visible. It is only<br />

evident dur<strong>in</strong>g or after physical exam while <strong>the</strong> male performs<br />

Valsalva maneuver (a breath<strong>in</strong>g exercise used as a diagnostic tool.)<br />

Grade II - It is not visible but can be felt on <strong>the</strong> physical exam<strong>in</strong>ation<br />

even without Valsalva maneuver.<br />

Grade III - It can be identified through scrotum without any physical<br />

exam<strong>in</strong>ation.


When do you need to treat it?<br />

<strong>One</strong> would require seek<strong>in</strong>g treat<strong>men</strong>t for one <strong>of</strong> <strong>the</strong> <strong>lead<strong>in</strong>g</strong> <strong>causes</strong> <strong>of</strong><br />

<strong><strong>in</strong>fertility</strong> <strong>men</strong>:<br />

• when:<br />

it is pa<strong>in</strong>ful<br />

it is caus<strong>in</strong>g testicular atrophy<br />

It is caus<strong>in</strong>g issues with testicular function<strong>in</strong>g<br />

it is <strong>the</strong> cause <strong>of</strong> <strong><strong>in</strong>fertility</strong><br />

• you want to<br />

go for ART (Assisted Reproductive Techniques)<br />

<strong>in</strong>crease low testosterone levels<br />

Surgical Treat<strong>men</strong>ts available for <strong>the</strong> treat<strong>men</strong>t<br />

<strong>Varicocele</strong>ctomy:<br />

It is a procedure <strong>in</strong> which <strong>the</strong> specialist go through <strong>the</strong> abdo<strong>men</strong> to<br />

clamp <strong>the</strong> abnormal ve<strong>in</strong>s so that blood can flow around <strong>the</strong><br />

abnormal ve<strong>in</strong> to <strong>the</strong> normal ones.<br />

<strong>Varicocele</strong> Embolization:<br />

It is a less <strong>in</strong>vasive method as opposed to <strong>Varicocele</strong>ctomy.<br />

In this procedure, a ca<strong>the</strong>ter is put <strong>in</strong>to <strong>the</strong> neck ve<strong>in</strong> or gro<strong>in</strong>, and<br />

<strong>the</strong>n a coil is placed through <strong>the</strong> ca<strong>the</strong>ter <strong>in</strong>to <strong>the</strong> varicocele to block<br />

<strong>the</strong> blood from enter<strong>in</strong>g <strong>the</strong> abnormal ve<strong>in</strong>s.<br />

<strong>Varicocele</strong>ctomy has been found to be <strong>the</strong> cost-efficient, and <strong>the</strong> rate <strong>of</strong><br />

success <strong>in</strong> pregnancy after <strong>the</strong> <strong>Varicocele</strong>ctomy are found to be 36% (this<br />

value varied with <strong>the</strong> quality <strong>of</strong> se<strong>men</strong>.)


Selection <strong>of</strong> <strong>the</strong> Cl<strong>in</strong>ic for <strong>the</strong> Treat<strong>men</strong>t<br />

Selection <strong>of</strong> a good cl<strong>in</strong>ic can impact <strong>the</strong> success <strong>of</strong> <strong>the</strong> treat<strong>men</strong>t <strong>of</strong> one <strong>of</strong><br />

<strong>the</strong> <strong>causes</strong> <strong>of</strong> <strong><strong>in</strong>fertility</strong> i.e. <strong>the</strong> <strong>Varicocele</strong>s. Here are some suggestions that<br />

you should consider if you are go<strong>in</strong>g to treat <strong>the</strong> cause <strong>of</strong> <strong>the</strong> fertility issue.<br />

The cl<strong>in</strong>ic should:<br />

have reliable doctor which means m<strong>in</strong>imal risk <strong>of</strong> failure<br />

have an operative versatility<br />

be consistent with cost-conta<strong>in</strong><strong>men</strong>t (cost-effective)<br />

can perform with m<strong>in</strong>imal morbidity and complications<br />

have a uniform success rate
