What is a<br />
<strong>Home</strong> <strong>Care</strong><br />
Package?<br />
A <strong>Home</strong> <strong>Care</strong> Package funds are supplied by<br />
the Government to enable you to continue to<br />
live at home.<br />
<strong>Home</strong> care packages are delivered on a<br />
Consumer Directed <strong>Care</strong> (CDC) basis. This<br />
means you (and your carer) have more choice<br />
and control over what services are delivered<br />
and where and when they are delivered.
Who are we?<br />
Atlas <strong>Home</strong> <strong>Care</strong> is a private company<br />
specialising in in-home care for the elderly<br />
and those with chronic diseases. In-home care<br />
allows people to receive treatment in the<br />
comfort of their home, as opposed to<br />
residential care and prolonged hospital stays.<br />
Atlas <strong>Home</strong> <strong>Care</strong> strives to make this process<br />
uncomplicated and stress free, allowing the<br />
main focus of a client to be their health and<br />
wellbeing.
How do I<br />
Start?
Step 1<br />
Application<br />
To Start the application for a <strong>Home</strong> <strong>Care</strong><br />
Package call Atlas <strong>Home</strong> <strong>Care</strong> on 1300 85 44<br />

We will guide you and your family through<br />
each step of applying for the home <strong>Care</strong><br />
Package Subsidy.
Step 2<br />
Assessment<br />
Your <strong>Home</strong> <strong>Care</strong> Consultant will provide you and<br />
your family with education and assistance during<br />
each phase of the assessment process.<br />
As the application process can be intimidating as<br />
you have your comprehensive assessment we are<br />
here to support you and make the process stressfree.
Step 3<br />
Funded <strong>Home</strong> <strong>Care</strong><br />
Package<br />
After your assessment you will be notified as to<br />
which <strong>Home</strong> <strong>Care</strong> Package level you are eligible to<br />
receive.<br />
Your <strong>Home</strong> <strong>Care</strong> Consultant, in conjunction with<br />
you and your family, will assist you create a care<br />
plan which is tailored to your needs and wishes.<br />
