Georgia; 6-hour drive from Atlanta; and 20 miles from
the Jacksonville International Airport.
BY AUTO
From Jacksonville International Airport
Take I-95 North to Exit 373 (Route A1A East). Go East
11.2 miles to Amelia Island Parkway, (turn right at 1st
traffic light, just over Intracoastal Waterway Bridge).
Go 3.4 miles to traffic light and turn right. Go .7 miles
to the round-about and follow Route A1A South. Go
2.7 miles to resort entrance on the left, (third roundabout
on left). Follow signs to “Check-In”
Scenic Route:
Take I-95 to Exit 358A. Go East on Route 105 (North
Heckscher Drive) and follow the signs to Amelia
Island.
From the North
Take I-95 South to Exit 373, (Route A1A East). Go East

PROGRAM OBJECTIVES:
Physicians will receive current information on diverse medical subjects pertinent to patient care. Subject
material was chosen based on assessed needs, current research and future trends. This educational
program was developed by family physicians for family physicians
ACCREDITATION AND CREDIT
AMA:
The Georgia Academy of Family Physicians is
accredited by the Medical Association of Georgia
to provide continuing medical education for
physicians.
The Georgia Academy of Family Physicians
designates this live activity for a maximum of 18
AMA PRA Category 1 Credits. Physicians should
claim only the credit commensurate with the
extent of their participation in the activity
AAFP:
Application for CME credit has been filed with
the American Academy of Family Physicians.
Determination of credit is pending.
AOA:
This program has been reviewed and it is
acceptable for up to 18 credits in Category
2-A and up to 6 credits in Category 2-B by the
American Osteopathic Association.
FACULTY DISCLOSURE POLICY:
The GAFP complies with the policies and
procedures of the Accreditation Council for
Continuing Medical Education.
While having an interest or an affiliation with
any corporate organization does not necessarily
influence a speaker’s presentation, the relationship
or a lack of a relationship must be disclosed.
Declaration forms were requested from each
speaker and the results will be presented prior to
the activity.
EDUCATIONAL OBJECTIVES:
This activity was developed for family physicians,
and may be appropriate for other primary care
physicians, midlevel clinicians, office staff, and
other healthcare professionals interested in
improving patient care.
Upon completion of the activity, participants
should be able to 1) identify family medicine
practices that are relevant to the well-being
of their patient population, 2) recognize and
describe current diseases, and offer proper
diagnosis, management and treatment options to
patients and their families, and 3) apply evidencebased
treatments to effectively manage patient
care.

Cancellation Policy:
Cancellations must be received in writing by May 11, 2018 to receive a partial refund. The
cancellation request may be emailed to Tenesha Wallace at twallace@gafp.org or faxed to
the GAFP office at (404) 321-7450. Refunds will be made following the meeting, less a $75
administrative fee. Cancellations made after May 11, 2018 will not be eligible for a refund.
