Take I-95 North to Exit 373 (Route A1A East). Go East<br />

11.2 miles to Amelia Island Parkway, (turn right at 1st<br />

traffic light, just over Intracoastal Waterway Bridge).<br />

Go 3.4 miles to traffic light and turn right. Go .7 miles<br />

to the round-about and follow Route A1A South. Go<br />

2.7 miles to resort entrance on the left, (third roundabout<br />

Take I-95 to Exit 358A. Go East on Route 105 (North<br />

PROGRAM OBJECTIVES:<br />

Physicians will receive current information on diverse medical subjects pertinent to patient care. Subject<br />

material was chosen based on assessed needs, current research and future trends. This educational<br />

program was developed by family physicians for family physicians<br />

ACCREDITATION AND CREDIT<br />

AMA:<br />

The Georgia Academy of Family Physicians is<br />

accredited by the Medical Association of Georgia<br />

to provide continuing medical education for<br />

physicians.<br />

The Georgia Academy of Family Physicians<br />

designates this live activity for a maximum of 18<br />

AMA PRA Category 1 Credits. Physicians should<br />

claim only the credit commensurate with the<br />

extent of their participation in the activity<br />

AAFP:<br />

Application for CME credit has been filed with<br />

the American Academy of Family Physicians.<br />

Determination of credit is pending.<br />

AOA:<br />

This program has been reviewed and it is<br />

acceptable for up to 18 credits in Category<br />

2-A and up to 6 credits in Category 2-B by the<br />

American Osteopathic Association.<br />

FACULTY DISCLOSURE POLICY:<br />

The <strong>GAFP</strong> complies with the policies and<br />

procedures of the Accreditation Council for<br />

Continuing Medical Education.<br />

speaker and the results will be presented prior to<br />

EDUCATIONAL OBJECTIVES:<br />

This activity was developed for family physicians,<br />

and may be appropriate for other primary care<br />

physicians, midlevel clinicians, office staff, and<br />

other healthcare professionals interested in<br />

improving patient care.<br />

Upon completion of the activity, participants<br />

should be able to 1) identify family medicine<br />

practices that are relevant to the well-being<br />

of their patient population, 2) recognize and<br />

describe current diseases, and offer proper<br />

diagnosis, management and treatment options to<br />

patients and their families, and 3) apply evidencebased<br />

treatments to effectively manage patient<br />

care.<br />
