Registration Information<br />
Bring the following documents to either Registration Center in Hammond or Amite.<br />
1. Student’s original birth certificate<br />
2. Photo ID of parent/guardian<br />
3. Student’s social security card, if available<br />
4. 2 Proofs of residency showing the physical 911 address (examples: utility bills, property tax statement, lease<br />
agreement).<br />
5. Up-to-date immunization record<br />
6. Withdrawal documents from previous school<br />
7. IEP or IAP, if applicable<br />
8. Copy of court order or custody agreement if parents are separated or divorced<br />
9. Copy of the court decree in the case of a student living with a legal guardian.
