We offer three levels of eye care benefits from Superior<br />

1. The Essential <strong>Vision</strong> Plan (Start<strong>in</strong>g at 12.71/month for <strong>in</strong>dividuals)<br />
‣In Network Providers Covered <strong>in</strong> Full<br />
‣Out-of-network Providers cost between $36-$42<br />
‣$125 allowance on s<strong>in</strong>gle vision lenses<br />
‣$120 allowance on contact lenses<br />
‣Does not cover factory scratch coat<strong>in</strong>gs or polycarbonate<br />
lenses<br />
