Community
As we enter our 30th year of service to the Cape
Cod community, we are pleased to present the 2010
Annual Report of Hospice & Palliative Care of Cape
Cod. The report anchors on a poignant story of
the profound personal impact of our “Open Door”
philosophy, illustrating why it is so central to our
mission. We are deeply grateful to the many donors
throughout our community whose generous support
makes it possible for us to maintain this approach to
providing hospice care.
2010 was truly a watershed year for the organization,
building on our three decades of service to the Cape
Cod community and positioning us for a strong
future. Our Board of Directors completed a major
strategic plan, concluding that we must become a
larger regional provider in order to ensure H&PCCC’s
long-term health and ability to provide critical new
services to our Cape and Island communities. Doing
so fulfills our mission to increase access to hospice
to all who are eligible. It also prudently leverages our
administrative overhead to be a more efficient and
sustainable organization.
Towards this end, we were fortunate to be presented
with the opportunity to acquire Hospice of Boston &
Greater Brockton, a small non-profit provider with a
comparable thirty-year history. The acquisition, which
closed in June, has enabled us to bring our “Open
Door” philosophy of care to a contiguous service area
from Provincetown to Boston, including Barnstable,
Dukes, Plymouth, Bristol, Suffolk and Norfolk
Counties.
We also completed the process of affiliating with
Alzheimer’s Services of Cape Cod and the Islands,
enabling us to collaborate in reaching an expanding
range of patients and caregivers coping with a serious
illness. It’s a first step in the direction of our strategic

goal of providing support to people with chronic
conditions before they are eligible for hospice.
The affiliation is structured to allow economies of
scale for services, with each organization retaining
its individual identity, mission, corporate name,
fundraising, 501(c)(3) designation, staff and Board
of Directors.
We also continued to grow our hospice services in
Dukes County through our Island Hospice team,
working in partnership with Hospice of Martha’s
Vineyard and the Vineyard Nursing Association.
The collective impact resulted in serving more than
1,000 patients and families and employment of 50
new staff.
We are now the third largest hospice provider
in Massachusetts and the fastest growing. Our
fundraising efforts in 2010 attracted $1.7 million
for patient care and support. This is a great
accomplishment in any year, but given the recent
economic climate, we welcome it as a strong vote of
confidence from our community. None of this would
be possible without the ongoing support of so many
faithful and new donors from across the community
spectrum. Together we look forward to meeting
future challenges and opportunities. We believe we
are positioned to meet the demands of providing
high quality service that improves quality of life and
reduces cost – and that is exactly what the next
thirty years of health care, and Hospice & Palliative
Care of Cape Cod, will be all about.

Impact: The Positive Difference You Made in 2010
Because of donors like you:
n Three young sisters (all in elementary school) found a safe place to laugh again following
the death of their dad, by attending our specialized Kids Grieve Too! (KGT) workshops.
n With our commitment to our Open Door philosophy, we provided symptom-relieving
treatments, such as blood transfusions, radiation, dialysis, chemotherapy, and IV
therapy to all our patients in need of this essential care, totaling nearly $800,000.
n We expanded our services to children. In addition to providing grief and loss counseling
through KGT, we also provide direct medical care for children with life-limiting illness.
n We provided more than $100,000 in free care for those who do not have insurance
coverage.
n A teenage girl, on the brink of suicide from grief over her mother’s death, found help in
our KGT program and is now an honor roll student. She is one of over 200 at-risk youth
cared for in 2010 as part of our work with the Cape-wide suicide prevention effort.

“Open Door” Made Possible

ELAINE ANdErSON’S hOSPICE ExPErIENCE
is a compelling example of the personal impact of
an organizational philosophy that provides hope
for many patients and families. Anderson needed
to continue receiving chemotherapy and radiation
treatments to relieve her cancer symptoms. But
she and her family also needed the expert care and
support of our hospice team. Because of HPCCC’s
Open Door philosophy, Anderson did not have to
choose between the two. She is just one of the more
than 2,500 patients served since HPCCC initiated its
“Open Door” approach three years ago. For many
of those, “Open Door” is — as HPCCC CEO David
Rehm describes it — “a game changer”.
In 2010, your donations helped us care for Elaine
Anderson and many others, providing essential
treatments and improving quality of life.
“Open Door” is how we describe our philosophy
of care. Medicare has only two requirements for
hospice eligibility:
1. Physician judgment of life expectancy of
6 months or less if the illness runs its normal
course
2. Patient desire for hospice services
Beyond that, each hospice is allowed to establish
and practice its own philosophy of care. Over
the years, many hospices have superimposed
other criteria for admission which HPCCC
believes can create obstacles to hospice access.
In order to maximize usage, we created and
market what we have coined as our “Open Door”
philosophy, which specifically adheres only to
those two Medicare requirements.
As a result of this approach, we are reaching and
caring for many more eligible patients. If they
can benefit from special treatments, they can
continue to receive them while in our care. We
admit anyone eligible regardless of the complexity
or intensity of their care needs. There is no
need to give anything up. They can be receiving
treatments such as chemotherapy, radiation,
blood transfusions, IV antibiotics or dialysis —
and other treatments. And, they don’t have to
have a caregiver or sign a DNR (Do Not
Resuscitate) order. Many people don’t realize
this is possible with hospice. And even more
surprising to most is the fact that the care,
medications and durable medical equipment
related to the hospice diagnosis are covered
by insurance.

The benefit of this “Open Door” philosophy was
strikingly obvious two years ago when 87-year-old
Sandwich resident Elaine Anderson and her son Joel
first discussed Elaine’s serious cancer diagnosis
with Dr. Peter Hopewood, an oncology surgeon at
Falmouth Hospital.
Joel recalled recently, “When Dr. Hopewood first
mentioned hospice, it took us both by surprise. But
the surprise was short-lived: he obviously knew it
would be a great way for us to get a lot of support.
We both knew the prognosis — that this wasn’t going
to go away. I didn’t have a clue as to how to look
ahead to deal with it. Then Dr. Hopewood presented
his hospice description — and it was a light in the
darkness.”
What Dr. Hopewood told the Andersons that day
brought them comfort and relief. “I tell families —
you’re getting exhausted — you need help for
your family,” Hopewood says. “Hospice is just an
enhancement that will make things easier. Your
medications will be covered by hospice. That’s big —
often families are relieved to hear that. We don’t know
exactly how long we have — I say let’s just call and
see what they can do to help.”
Like many in their situation, neither Elaine nor Joel
Anderson had a previous understanding of what
hospice was, beyond the fact that it had something
to do with end of life. Dr. Hopewood’s description
put them at ease. The HPCCC admission nurse
came to Elaine’s house, explained everything and
answered all their questions. By the end of that day,
all arrangements were in place.
Joel knew that no matter what, Elaine’s fondest hope
was to be able to stay in her own home with her
family’s beautiful artwork and her beloved cat Mittens.
Joel says, “I am so grateful she was able to do that
because of Hospice & Palliative Care of Cape Cod.
It never would have been possible without them.”

As her illness progressed, Elaine continued to be
treated by Dr. Hopewood, as well as Dr. Victor
Aviles, founder of Hematology Oncology Specialists
of Cape Cod in North Falmouth, and Dr. Daniel
Canaday, a radiation oncologist at Cape Cod
Hospital. They coordinated closely with the hospice
medical team, continuing to treat Elaine with
chemotherapy and radiation in order to relieve the
pain of her tumors.

Dr. Aviles says, “In general, oncology practice today
involves many limits – financial, insurance-related,
regulatory, bureaucratic; pressures are always
mounting. New discoveries and advancing science
create more treatment options, but fewer treatments
may actually become available to patients because
of current medical economics. When I think about
HPCCC’s Open Door, I think about limits being
removed for the first time in years. This program can
liberate clinicians’ minds so they can be creative
and flexible in applying multidisciplinary palliation to
actively reduce suffering. Elaine had almost every
form of cancer treatment applied to help her feel as
comfortable and pain-free as possible.”

During the almost two years she was in HPCCC’s
care, Elaine Anderson made extensive use of
the full range of services available to her through
the Interdisciplinary Team, which addresses and
coordinates physical, spiritual and emotional needs,
delivered through a wide range of specialists,
including nurses, home health aides, volunteers,
social workers and chaplains. She drew on the
expertise of all of them, but made an especially close
connection with her volunteer, Anne Johnson.
Being in their shared presence, it was easy to
immediately pick up on the special sisterly bond
between the two. They bantered with great delight.
Anne was scrupulous about her own attention to her
charge. As Elaine exclaimed, “She’s always thinking
of me — she comes every week, she’s never missed
once!”

Anne drove her to appointments and errands,
chemotherapy, and out to lunch. They even went
wig shopping together, laughing throughout. Elaine
was surprised by the wig’s price tag, saying “I’m not
spending all this money - I’m not going to be around
long enough to get my money’s worth!” Ann quickly
retorted “You don’t know that!” She recalls, “Anyone
seeing us would never have believed we were a
hospice patient and her volunteer. We had so much
fun no matter what we did.” When Anne told Elaine
how much of a gift she was to her, Elaine came back
laughing with “Oh dear, I think you got the booby
prize!”
But Anne also had to balance her desire to help.
“Elaine was one of the most ferociously independent
women I’ve ever met. She would often turn down my
help, saying she was saving me for when she really
needed me — I had to respect that. It was great to
see her care for herself and allow us to do things for
her. It’s a trust that developed.” Elaine appreciated
Anne’s giving her space too. “When I said I can do
it, she would let me.”
While in HPCCC’s care, Elaine experienced two
different palliative procedures that necessitated her to
be cared for in our McCarthy Care Center, our shortterm
inpatient care facility in Sandwich, designed
to provide patient stabilization and symptom
management. Elaine loved the place, saying “It’s so
wonderful; the people are all just as kind as can be.
There’s a warmth of love there, giving you a feeling
of security. They are there to take care of your every
need — they bring so much compassion, support
and laughter.”

And indeed, sitting with her in the McCarthy dining
room one sunny afternoon in January, one could
see this in action. Staff members were constantly
coming in to give her a hug, giggle over her latest
story or bask in her warm good humor. One of them,
McCarthy Center social worker Susan Fencer-Cote,
helped Joel coordinate medical arrangements. “I
had no other resources,” Joel said. “I don’t live on
the Cape, had no contacts or network there, and no
siblings to help. There’s a whole lot of trust going on
here, right from the beginning. I was totally confident
that they were looking out for her. I don’t know what I
would have done without them.”
Elaine summed it all up best herself. “Hospice people
are the best – God has given them to me. To know
they are there any time is a big reassurance. I just
wish I’d known about hospice sooner. I want to shout
about it from the mountain top! If I can help just one
person, I will feel like I did something. They’ve kept
me going happily. I haven’t given up — hospice
gives me hope. With hospice, you are living — the
words are in my heart. You took away all my fear and
brought me comfort.”
And throughout it all, Anne Johnson considers it
a deep inspiration to have witnessed Elaine’s zest
for life and her willingness to enjoy every moment,
teaching Anne to remember just how precious Now
can be.
Elaine Anderson passed away peacefully at the
McCarthy Care Center on February 19th, a testimony
to the ability of the Open Door philosophy to provide
hospice care that truly enables the best possible
quality of life.
