Welcome to<br />

the <strong>Grange</strong> medical centre, Kinsley medical centre<br />

and Greenview medical centre<br />

This is a brief guide to our practice, which we hope you will use as a source of<br />

reference.<br />

■ PRActIce HIStoRY<br />

<strong>The</strong> practice has been in existence for over 100 years and was carried out from three<br />

surgeries in <strong>Hemsworth</strong>. <strong>The</strong> practice buildings were wooden huts until 1967 when a<br />

purpose-built surgery was built in <strong>Highfield</strong> <strong>Road</strong>,<strong>Hemsworth</strong>.We soon outgrew this building<br />

and moved to our current site in 1980. <strong>The</strong> surgery was extended in 1993 to provide<br />

additional patient care facilities,a larger waiting room and more office space.In March<br />

1996 our new therapy unit was opened, providing dedicated facilities for physiotherapy,<br />

including exercise equipment, heat therapy and hydrotherapy. <strong>The</strong> new two storey<br />

building also accommodates our practice and community nurses,and provides facilities<br />

for staff training and seminars.<br />

In May 2001, a new purpose-built surgery was completed at Kinsley, providing much<br />

needed extra space and state-of-the-art facilities.On 1 August 2003,the <strong>Grange</strong> <strong>Medical</strong><br />

<strong>Centre</strong> and <strong>Highfield</strong> <strong>Road</strong> Surgery merged as one practice.<br />

On 1 July 2006 <strong>The</strong> <strong>Grange</strong> <strong>Medical</strong> <strong>Centre</strong> merged with the Greenview <strong>Medical</strong> <strong>Centre</strong><br />

at Upton.<br />

<strong>The</strong>re is access and toilet facilities for the disabled at all surgeries. <strong>The</strong>re is also a lift at<br />

<strong>The</strong> <strong>Grange</strong> and Kinsley <strong>Medical</strong> <strong>Centre</strong> to enable disabled patients access to the first<br />

floor area.<br />

■ PRActIce AReA<br />

<strong>The</strong> practice accepts new patients from the following areas: <strong>Hemsworth</strong>,Kinsley,Fitzwilliam,<br />

South Kirkby,South Hiendley,Upton or South Elmsall.Please refer to the map on the back<br />

cover. To register as a patient please ask at reception.<br />

Our practice charter leaflet sets out the standards and quality of service which we try<br />

Do you feel vulnerable, threatened or intimidated? Are you afraid to walk past groups who<br />

are gathered in the streets or afraid in your own home? If so read on: In today’s society of<br />

crime, drug abuse and violence, ordinary law abiding people have more fear than ever of<br />

going out. Violence and sex crimes are on the increase, whilst women feel more vulnerable<br />

and are increasingly aware that they need to take extra precautions against possible attack<br />

by being strong and positive whilst out on their own, or in their own home.<br />

Self defence is devoted to increasing your level of awareness and to provide simple<br />

preventative advice to assist you in avoiding or defusing potential situations which could<br />

allow an attack to be committed against yourself.<br />

Personal safety is not about the ability to fight, it is about having an awareness of situations<br />

which may suddenly ignite into violence and how to prevent or avoid these. You can do<br />

this by looking and feeling confident. By looking and acting confident you are less likely<br />

to become a victim of a criminal, who would usually choose weaker and easier targets. <strong>The</strong><br />

law states that you can use reasonable force to ‘defend’ yourself. Self defence teaches you<br />

awareness, builds confidence and self-esteem, and demonstrates how to protect yourself<br />

against violence.<br />

<strong>The</strong>re are many various options available, classes are held locally and great fun for all the<br />

family. Taking part also provides healthy exercise. So contact your local centre and find<br />

Feet bear the brunt of our busy and hectic<br />

lifestyles – so good care of our feet is vital to<br />

ensure pain-free, lifetime usage.<br />

Chiropodists and podiatrists specialise<br />

in assessing, diagnosing and treating<br />

abnormalities and diseases of the lower<br />

limb. <strong>The</strong>y are involved with the total care<br />

of the feet. Proper care and attention of the<br />

feet is as important at an early age to ensure<br />

proper growth as it is in later life to keep feet<br />

functioning well.<br />

Chiropodists and podiatrists are trained to<br />

treat a wide range of foot problems and<br />

provide professional advice on proper foot<br />

care to patients of all ages and from all walks<br />

of life. <strong>The</strong>y work closely with other medical<br />

practitioners in providing health care. Take<br />

■ NuRSe PRActItIoNeR/mINoR IllNeSS NuRSe<br />

<strong>The</strong> nurse practitioner is available for consultations.This is a nurse trained to an advanced<br />

level, who can advise about minor ailments and common health problems. She can<br />

provide information and an explanation about any aspect of health, and support you<br />

through any lifestyle changes you wish to make to improve your health.<br />

<strong>The</strong> minor illness nurse is a nurse practising in an extended role. She sees patients in the<br />

surgery alongside the doctors providing a service in areas such as minor illnesses and<br />

health promotion and is also able to advise on general health issues.<br />

■ PRActIce NuRSeS<br />

Our practice nurses provide health education and health screening as<br />

well as assisting in minor surgery and consultant clinics.<strong>The</strong>y also undertake<br />

routine procedures such as ear syringing,travel vaccinations,blood sampling<br />

and wound dressings in the daily treatment room sessions.<br />

Practice nurses hold clinics to include management of chronic diseases,<br />

eg, diabetes, asthma, coronary heart disease and chronic obstructive<br />

pulmonary disease.<br />

■ commuNItY NuRSeS<br />

Our health visitors help with the care of pre-school children and supporting families.Health<br />

visitors run well-baby clinics and assessment clinics at the surgeries and are available to<br />

advise on the health of your child. Health visitors recognise pressures in today's society<br />

and can listen, advise and, if necessary, direct you to more specialised help. Health<br />

visitors have a public health role, and provide health education, run a family planning<br />

clinic and a youth advisory clinic.<br />

■ PuBlIc HeAltH NuRSe<br />

Our public health nurse works with all members of our team to provide accessible health<br />

care to all our patients.<br />

<strong>The</strong> role is part clinical, part research. <strong>The</strong> clinical element is for patients and includes<br />

coronary heart disease prevention, sexual health and health screening. <strong>The</strong> research<br />

looks at the population we serve and aims to develop appropriate health care services,<br />

like delivery of health education to patients, visual displays and learning sessions, and<br />

one-to-one consultations.<br />

<strong>The</strong> public health nurse will deal with any topics raised by the media.<br />

■ ADmINIStRAtIoN AND RecePtIoN StAFF<br />

<strong>The</strong> staff are led by one manager who is responsible for the day-to-day<br />

running of the surgery. As a patient your first point of contact will probably be<br />

the receptionist. She will be pleased to deal with any queries you may have,<br />

or will direct you to the most appropriate member of staff to help you.<br />

not be able to see the doctor of your choice.<br />

If you are unable to keep your appointment, please tell us as soon as possible so that<br />

someone else can be offered that time.<br />

■ Home VISItS<br />

If you can get to the surgery, please try and do so. Home visits are for patients<br />

who are too ill to get to the surgery and not for those who find it inconvenient.<br />

Please ring the surgery before 11.30am to ask for a visit that day, so that the<br />

doctors may plan their rounds.<br />

■ emeRGeNcY cAllS<br />

A receptionist will take your call during the times that the surgery is open.<br />

Outside surgery times your call will automatically be diverted to West Yorkshire<br />

Urgent Care Services.<br />

Also, <strong>The</strong> White Rose Surgery provides a walk-in-centre which is open between 9.00am<br />

and 10.00pm, seven days a week.<br />

■ RePeAt PReScRIPtIoNS<br />

If you take certain medicines regularly it may be possible to have them<br />

authorised on a repeat basis. <strong>The</strong> right hand side of your computer<br />

prescription is a request form to be kept for the purpose of requesting<br />

repeat prescriptions.<br />

You should always use the computer slip to ensure that you receive the correct<br />

medication.<br />

When you use your computer slip to request a repeat prescription you can expect the<br />

prescription to be ready for collection in 48 hours.<br />

If you do not use the computer slip, you should expect to wait at least 72 hours before<br />

you can collect the repeat prescription. Repeat prescriptions can only be collected<br />

between 9.00am - 6.00pm at <strong>The</strong> <strong>Grange</strong> <strong>Medical</strong> <strong>Centre</strong> and whenever the surgery is<br />

open at Kinsley <strong>Medical</strong> <strong>Centre</strong>.Telephone prescriptions at <strong>The</strong> <strong>Grange</strong> <strong>Medical</strong> <strong>Centre</strong><br />

are taken between 10.00am - 4.00pm.<br />

After a number of “repeats” it will be necessary for you to see the doctor to review your<br />

medication.<br />

■ PRIVAte ceRtIFIcAteS, INSuRANce FoRmS etc<br />

Some private services such as private medical certificates,insurance forms,<br />

passport photographs etc must be paid for by the patient.A list of charges<br />

is available on request at administration.You should expect to have to wait<br />

at least ten working days for the doctor to complete insurance forms and<br />

similar documents.<br />

■ cHANGe oF ADDReSS AND otHeR DetAIlS<br />

In order to ensure that we provide an efficient, effective service, we need<br />

to have your personal details up to date, so do please keep us informed<br />

of any changes of name, address or telephone numbers.<strong>The</strong>se details will<br />

be kept on the practice computer, but will not be accessible to anyone<br />

unauthorised by the practice.<br />

■ HYGIeNe<br />

<strong>The</strong> surgery is cleaned daily by our team of cleaners. It is essential to maintain a<br />

high standard of hygiene on the premises and your assistance with this would be<br />

appreciated.<br />

Smoking is not allowed at either <strong>The</strong> <strong>Grange</strong> <strong>Medical</strong> <strong>Centre</strong> or Kinsley <strong>Medical</strong><br />

<strong>Centre</strong>.<br />

■ SuGGeStIoNS AND comPlAINtS<br />

We are continually looking for ways to improve the service we give and<br />

welcome your suggestions and involvement. If you have any suggestions<br />

or constructive criticism to make,please ask at reception for a suggestions<br />

card which, when completed, can either be handed in at reception or<br />

placed in the suggestions box in the waiting room. All suggestions will be<br />

given serious consideration and if a response has been requested this will be made<br />

within two weeks.<br />

If you are in any way unhappy with the service you have received from us, please let<br />

us know by asking to speak to the complaints officer. If you prefer you may put your<br />

concerns in writing, either to the complaints officer or to one of the doctors. Please ask<br />

at reception if you require any assistance.<br />

■ FReeDom oF INFoRmAtIoN – PuBlIcAtIoN ScHeme<br />

<strong>The</strong> Freedom of Information Act 2000 obliges the practice to produce a Publication<br />

Scheme. A Publication Scheme is a guide to the ‘classes’ of information the practice<br />

intends to routinely make available. This scheme is available from reception.<br />

■ DAtA PRotectIoN<br />

As part of the Data Protection Act 1998 all information held on the premises is treated<br />

with the highest of confidentiality by all our staff.<br />

■ ReGISteRING AS A NeW PAtIeNt<br />

To register as a new patient please call at the reception desk and request a new<br />

patient registration form. After completion return it to the reception desk and please<br />

allow 48 hours for approval. After registration is approved you will be asked to make an<br />

appointment with a nurse for a new patient check. This entails a brief medical history<br />

and BP check.<br />

■ otHeR SeRVIceS<br />

<strong>The</strong> following is a guide to the other services which we provide for our<br />

patients. Most clinics are by appointment only. Please ask at reception for<br />

details of clinic times and appointment booking.<br />

Well Person<br />

<strong>The</strong> well person clinic is for both men and women and is run by the practice nurse as a<br />

health check.Clinics are held at both surgeries.You can discuss your health and lifestyle<br />

and advice is available on staying fit and well. It is recommended that you have such<br />

a check every three years. All new patients are invited to a well person clinic. <strong>The</strong> nurse<br />

can also advise on giving up smoking.<br />

menopause<br />

Information and advice on the menopause can be obtained from any of the doctors or<br />

the nurse practitioner.<strong>The</strong> health visitor also holds a menopause clinic at Kinsley <strong>Medical</strong><br />

<strong>Centre</strong> on Friday mornings.<br />

Welfare Advice<br />

This service is for people who are having difficulty completing forms to claim benefits. It<br />

is available between 10.30am and 12 noon on Tuesday - no appointment required. For<br />

further details please ask at reception.<br />

Asthma, Diabetes And Hypertension management<br />

Special clinics are arranged for the monitoring of patients with asthma, diabetes or<br />

hypertension.<br />

contraception clinic<br />

<strong>The</strong> surgery provides a contraception and sexual health clinic (CASH) twice a week,one<br />

at Kinsley and one at <strong>Hemsworth</strong>.<br />

cervical Smears<br />

It is normal to have a smear at least every three years and these are carried out by the<br />

doctors and nurses. <strong>The</strong> practice operates a call and recall system to remind ladies of<br />

when their smear is due.<br />

travel clinic<br />

<strong>The</strong> practice nurse can provide advice on and arrange travel vaccinations. She will also<br />

advise on sensible precautions to take to stay healthy when travelling abroad. If you<br />

think you may need travel vaccinations, please enquire at least six weeks in advance of<br />

travel.<br />

Family Planning<br />

<strong>The</strong> public health nurse provides family planning services. Please ask at reception for<br />

details.<br />

Antenatal<br />

Antenatal clinics are held at both the <strong>Grange</strong> and Kinsley <strong>Medical</strong> <strong>Centre</strong>s.Please enquire<br />

at reception for clinic times. <strong>The</strong> clinics are run by the community midwives.<br />

child Health<br />

A full programme to immunise and monitor the development of children<br />

from birth upwards is run by the practice in conjunction with the health<br />

visitors.<br />

<strong>The</strong> well baby clinics are run on a walk-in basis.<br />

Please ask at reception for full details of this service and to register children under the<br />

age of five for child health surveillance.<br />

Youth Advisory clinic<br />

<strong>The</strong> health visitor runs a weekly clinic at Kinsley <strong>Medical</strong> <strong>Centre</strong> on Wednesdays from<br />

4.00 - 5.00pm.<strong>The</strong> clinic is for teenagers and young adults seeking advice or information<br />

on a range of health matters, including HIV and AIDS, substance abuse, safe sex and<br />

healthy eating.<br />

minor Surgery<br />

Certain minor operations can be performed in the surgery by the doctors.An appointment<br />

will be arranged for you at reception after an initial consultation.<br />

Specialist clinics<br />

Your doctor may refer you to a consultant or therapist for assessment or<br />

treatment. For certain conditions we are able to offer appointments with<br />

consultants or therapists at <strong>The</strong> <strong>Grange</strong> <strong>Medical</strong> <strong>Centre</strong>, rather than the more<br />

usual referral which would involve a journey to hospital. <strong>The</strong>se services include<br />

physiotherapy, chiropody (also provided at Kinsley <strong>Medical</strong> <strong>Centre</strong>) and x-ray.<br />

Your doctor will discuss this with you if you need to be referred.<br />

■ ZeRo toleRANce<br />

<strong>The</strong> practice operates a Zero Tolerance policy regarding abusive/aggressive behaviour<br />

towards staff and other patients. Measures will be taken to have offenders removed<br />

from our practice list.<br />

DO stop smoking<br />

DON’T start smoking and advise your children not to start<br />

DO cut down on excess calories where possible<br />

DON’T eat foods with a high fat content<br />

DO eat foods with a high fibre content<br />

DON’T forget to keep immunisations up to date<br />

DO exercise regularly<br />

DON’T let stress and anxiety build up - learn about relaxation techniques<br />

DO remember we are here to help you stay healthy. Our practice nurses are able to<br />

offer advice and help you in becoming healthy and maintaining a healthier lifestyle, eg<br />

dietary advice, help with stopping smoking, immunisations and well person checks.<br />

■ FIRSt AID<br />

Burns And Scalds<br />

Do place the burnt area under cold running water for at least 10 minutes.<br />

or give a short squeeze on the tummy.<br />

Do several sharp squeezes in an adult, standing behind them and holding them in a<br />

Nosebleeds<br />

Sit in a chair leaning forwards with your mouth open and pinch the nose just below the<br />

bone for 10 minutes. Avoid hot drinks and hot food for 24 hours.<br />

If symptoms persist consult your doctor.<br />

Sprains, Strains And torn muscles<br />

Remember RICE (Rest, Ice, Compression, Elevation).<br />

Rest - especially if a weight bearing part such as the knee or ankle.<br />

Ice - immediately, less useful as the hours pass. A bag of frozen peas moulds well to the<br />

injured part.<br />

Compression - a firm (not too tight) supporting bandage.<br />

Elevation - especially for leg injuries.<br />

When the injury is over 24 hours old, warmth is better than cold. As the injury improves,<br />

gentle, non-weight bearing, loosening exercises will help. Allow full recovery before<br />

gradually returning to normal activities.<br />

Sunburn<br />

Everyone should avoid sunburn and, if prone to sunburn, wear loose fitting clothing in<br />

the sun - this is particularly important in relation to babies and children. If sunbathing,<br />

always use a sunscreen of the correct sun protection factor, re-applying at least every<br />

two hours.Be particularly careful to pay attention to sensitive areas like lips,nose,eyelids,<br />

shoulders, nipples, ears and legs. Avoid sunbathing when the sun is particularly intense<br />

eg late morning, early afternoon.<br />

Tan gradually - don’t rush it and never stay in the sun until your skin goes red.<br />

Insect Bites And Stings<br />

Antihistamine tablets may be obtained from the chemist without prescription and will<br />

usually relieve most symptoms. Calamine lotion or hydrocortisone cream may also<br />

help. Note: bee stings should be scraped away rather than “plucked” in order to avoid<br />

squeezing the contents of the venom sac into the wound.<br />

the child With A temperature<br />

We are aware of how worrying it can be to have a sick child. If you are concerned<br />

about your child we will always see them the same day at the surgery. We do ask that<br />

you bring your child to the surgery rather than requesting a home visit. If wrapped up<br />

well a child will come to no harm being brought to the surgery and can usually be seen<br />

sooner.Your co-operation in this matter is greatly appreciated.<br />

It is always wise to keep a supply of children’s paracetamol (Calpol or Disprol) at home.<br />

Paracetamol reduces a child’s temperature. Sometimes feverish children may not be<br />

able to keep the medicine down and it is useful to have some paracetamol suppositories<br />

(Alvedon) available; do ask your doctor about this. In most minor illnesses in childhood<br />

this is the only treatment required. However, if you are worried about the child or if the<br />

child fails to improve in two or three days, it is worth bringing them to the surgery for a<br />

check.<br />
