THAT COULD SAVE YOUR LIFE<br />

Chances are you know someone<br />

who has had a stroke. An estimated<br />

795,000 people in the United<br />

States have a stroke each year, according<br />

to the Centers for Disease<br />

Control and Prevention.<br />

Despite how common strokes are, there<br />

are a lot of widely believed misconceptions.<br />

Unfortunately, not knowing the facts can put<br />

a person at bigger risk for experiencing a<br />

stroke themselves, or, not being able to help<br />

someone else who may be experiencing a<br />

stroke.<br />

To help separate fact from fiction, the<br />

medical experts at Life Line Screening share<br />

the truth about the top misconceptions<br />

about stroke:<br />

Misconception: Strokes only happen to<br />

older people<br />

Fact: Research found 61 percent of strokes<br />

happen in people over the age of 65. That<br />

means 39 percent of strokes happen to<br />

younger people.<br />

Misconception: Strokes are not a problem<br />

in the United States<br />

Fact: You may only know a few people<br />

who’ve had a stroke in their life, but someone<br />

has a stroke every 40 seconds in the U.S.<br />

Misconception: A stroke will kill you<br />

Fact: Approximately one out of eight<br />

strokes results in death within thirty days.<br />

The other seven instances leave the person<br />

disabled.<br />

Stroke is fatal in about 10 to 20 percent<br />

of cases and, among survivors, it can<br />

cause a host of disabilities, including loss<br />

of mobility, impaired speech, and cognitive<br />

problems.<br />

Misconception: Strokes cannot be<br />

prevented<br />

Fact: Up to 80 percent of strokes<br />

could be stopped before they start.<br />

<strong>Health</strong> screenings are an effective way<br />

to identify and understand risk factors<br />

so they can be properly managed.<br />

Research shows nine out of 10 cardiovascular<br />

doctors support preventive health<br />

screenings for cardiovascular disease (plaque<br />

in the arteries) among patients with key risk<br />

Misconception: Only a doctor can identify<br />

a stroke<br />

Fact: Everyone can and should know the<br />

signs and symptoms of stroke. By taking<br />

quick action, you could save a life.<br />

According to the CDC, the most common<br />

signs of stroke are:<br />

u Sudden numbness or weakness in the<br />

face, arm or leg, especially on one side of<br />

the body.<br />

u Sudden confusion, trouble speaking or<br />

difficulty understanding speech.<br />

u Sudden trouble seeing in one or both<br />

eyes.<br />

u Sudden trouble walking, dizziness, loss of<br />

balance or lack of coordination.<br />

u Sudden severe headache with no known<br />

cause.<br />

If you or someone else has any of these<br />

symptoms, call 911 immediately. Understanding<br />

the facts about stroke helps empower<br />

you to control your own health. Even if risk<br />

factors are present, you can take proactive<br />

measures to help prevent stroke for yourself<br />

and loved ones.<br />

BE OPEN WHEN TALKING<br />

After practicing dermatology for<br />

more than 15 years, I’ve learned<br />

many things from my patients, especially<br />

how important communication<br />

is. Sometimes a patient will present<br />

us with something that we think is not<br />

significant, but they are self conscious about<br />

and it makes them<br />

anxious or avoid<br />

social situations.<br />

As health care<br />

providers, we need<br />

to get a thorough<br />

history on why the<br />

patient is here and<br />

what motivates<br />

them to be here.<br />

It’s not necessary<br />

to spend an entire<br />

office visit doing<br />

a psychological<br />

evaluation, but it<br />

needs to be part of the patient history.<br />

First, we need to validate the patients’ feelings<br />

whether it be fear, anxiety or skepticism<br />

and let them know we take their problem seriously.<br />

Open-minded communication is key to<br />

establishing a successful office visit by letting<br />

the patient know they can ask questions and<br />

be part of the treatment plan. By letting the<br />

patient know there is an open line of communication,<br />

they will feel more comfortable in<br />

disclosing their history to you.<br />

Second, eye contact with the patient is important<br />

and can result in a more honest and<br />

forthcoming statement from the patient. In<br />

addition, by observing the patient’s body language,<br />

one can learn a lot about the patient’s<br />

state of mind — whether they smile, make<br />

eye contact or have visible or no affect. Many<br />

times the patient may appear being rude or<br />

aloof but instead they are depressed.<br />

Third, it’s important to understand our<br />

patients’ fears, hopes and expectations. Lots<br />

of times they have identifiable emotional reactions<br />

such as being anxious or experiencing<br />

anger or depression.<br />

However, it doesn’t always manifest in classic<br />

ways. In order to communicate with patients<br />

successfully and make sure a treatment<br />

plan works, health care providers need to take<br />

the time and assess a patients’ motivations.<br />

Many times, patients fear their skin problems<br />

will get worse, intensify or never go<br />

away, causing them social anxiety or avoiding<br />

social activities.<br />

Sometimes as providers, we do not understand<br />

the motivation of the patient and<br />

assume the patient wants to be there. For<br />

example, a<br />

parent brings<br />

in their teenager<br />

for acne<br />

on the face but<br />

the teen has<br />

no interest in<br />

being there. Or<br />

some patients’<br />

motivations are<br />

that they want<br />

a modest improvement<br />

of<br />

their condition<br />

versus someone<br />

who has an unrealistic expectation.<br />

Finally, a thorough history is a crucial part<br />

of the correct diagnosis and successful treatment<br />

of the patient. Many times a patient<br />

will present to the dermatology office with a<br />

specific problem and answer the health care<br />

provider with one answer that is inaccurate.<br />

An example of this is, when asking a patient<br />

what creams they are applying to their body<br />

and they respond by saying nothing. After<br />

10 minutes of interviewing the patient they<br />

disclose they are using nine different creams<br />

their friends or neighbors gave them.<br />

They defend their answer of stating they<br />

are using nothing by saying, “nothing has<br />

changed.”<br />

Or I may ask what medications they take<br />

and a common response I get is, “a small<br />

white pill in a white bottle with a really long<br />

name.”<br />

In conclusion, by writing down specific<br />

questions before the office visit regarding<br />

your skin condition and a list of previous<br />

medications tried, this can save many office<br />

visits and co-pays by providing the health<br />

care provider with the most accurate information.<br />

Accreditation Matters~Since 2006 Coastal has been nationally accredited by the<br />

Commission on Accreditation of Ambulance Services (CAAS). As one of the first three<br />

Florida agencies to successfully earn this distinction we are proud to have lead the way for<br />

others to follow. Florida now has 15 CAAS accredited agencies that have attained this distinction but that number remains<br />

small in comparison to the total number of ambulance services operating in Florida. Considered the “Gold Standard” for<br />

ambulance services, Coastal is very proud of the achievement which continues to demonstrate our commitment to quality<br />

and our dedication to our community and our healthcare partners.<br />

Quality Assured~At Coastal “Quality” is a benchmark for day-to-day performance. Coastal believes quality must be<br />

measured in many different ways, from patient surveys, on time performance measures, to clinical indicator reviews and<br />

patient care report accuracy just to name a few. Coastal’s franchise agreement with <strong>Brevard</strong> County mandates very specific<br />

performance parameters including one that establishes a minimum of 90% for on time performance. Exceeding that<br />

requirement every year, Coastal’s on time performance has averaged 97% over the past 10 years. Additionally, patients<br />

responding to Coastal’s patient satisfaction survey have consistently rated their transports above 3.8 on a 4.0 matrix<br />

rating scale. Coastal takes performance standards very seriously and closely monitors how that performance meets the<br />

expectations of its patients, healthcare partners and county leaders.<br />

Americans are now living longer<br />

than ever before. In fact,<br />

one of the fastest growing<br />

segments is people over the<br />

age of 85 who will represent<br />

20 percent of the population by the<br />

year 2040. Because we are living<br />

longer, certain conditions specific to<br />

seniors are also on a steady rise. Dehydration,<br />

falls, fractures, cognition<br />

loss and attention deficits are now<br />

becoming more commonplace.<br />

In a recent paper titled “Salt Appetite<br />

Across Generations” presented at<br />

a medical conference in Switzerland,<br />

Israeli researchers from the University<br />

of Haifa indicated that among seniors,<br />

a reduced sense of thirst could<br />

increase the increased risk of serious<br />

dehydration. They also noted that<br />

the appetite for salt does not diminish<br />

with age, and suggested that this<br />

could be used to help sustain hydration<br />

and prevent the dangerous<br />

symptoms that result from dehydration.<br />

Another study published in the<br />

American Journal of Hypertension<br />

identified significant risks to cardiovascular<br />

health and longevity from<br />

consuming any less than 1, or more<br />

than 3 teaspoons of salt per day. Fortunately,<br />

most Americans, including<br />

seniors, when left to their own choice<br />

consume right in the middle of this<br />

range.<br />

Seniors in assisted living centers<br />

can be especially susceptible to the<br />

dangers of low salt diets. In 2013 a<br />

task force of 12 professional medical,<br />

nursing, and nutritional organizations<br />

assembled by the Pioneer Network<br />

published the “New Dining Practice<br />

Standards.” Their report concluded<br />

that low salt diets were contributing<br />

to malnutrition and weight loss<br />

among a significant percentage of<br />

seniors in assisted living facilities.<br />

Low salt diets can also cause seniors<br />

to suffer from mild hyponatremia,<br />

an electrolyte imbalance in the<br />

blood which may not sound bad but<br />

can lead directly to walking impairment,<br />

attention deficits and a much<br />

higher frequency of falls. Several<br />

recent medical papers found a direct<br />

relationship between hyponatremia<br />

and unsteadiness, falls, bone fractures<br />

and attention deficits.<br />

Falls are one of the most serious<br />

problems for the elderly and about<br />

a third of people over 65 fall at least<br />

once every year. Fall-related injuries<br />

in the elderly are associated with<br />

numerous psychological and physical<br />

consequences and are a leading<br />

cause of bone breakage and hip<br />

fractures, which can lead to complications<br />

and permanent disability or<br />

death. Some seniors do need a low<br />

salt diets but many do not, and it<br />

should not be assumed that they all<br />

do or benefit from when in fact the<br />

opposite may be the case.<br />
