ear the end of last year, the Government of <strong>India</strong><br />

Ntook a call on f<strong>in</strong>ish<strong>in</strong>g the regulations on E-<br />

<strong>Pharmacy</strong>. Yet, till now, the Rs 3500 crores<br />

<strong>in</strong>dustry, which is expected to rise to Rs. 25000 <strong>in</strong> the<br />

next three-four years, presently lies with no regulations.<br />

This absence of regulation has proliferated the number<br />

of e-pharmacy stores <strong>in</strong> the country. Further, the advent<br />

of technology and penetration of the <strong>in</strong>ternet has added<br />

to the electronic market, where most of the users are<br />

sitt<strong>in</strong>g with smartphones and tablets <strong>in</strong> their laps and<br />

enjoy<strong>in</strong>g onl<strong>in</strong>e medic<strong>in</strong>e. <strong>The</strong> E-<strong>Pharmacy</strong> <strong>in</strong>dustry,<br />

without regulations, is explod<strong>in</strong>g and the drug <strong>in</strong>dustry<br />

that calls for a sharp precision and safety concerns, thus<br />

puts forth the question-Will E-pharmacy be a<br />

dangerous trend <strong>in</strong> future if not regulated properly?<br />

<strong>The</strong> Desideratum<br />

of Regulations <strong>in</strong><br />

E-<strong>Pharmacy</strong> Industry<br />

E-<strong>Pharmacy</strong> comes under the Drugs and Cosmetics Act,<br />

1940 and Information Technology Act, 2000. However,<br />

neither of them <strong>in</strong>terprets the state of onl<strong>in</strong>e and offl<strong>in</strong>e<br />

regulations. E-<strong>Pharmacy</strong> stores, thus, may not stand by<br />

these and can sidestep them. Ultimately, govern<strong>in</strong>g<br />

authorities are f<strong>in</strong>d<strong>in</strong>g it challeng<strong>in</strong>g to control or<br />

monitor the sell<strong>in</strong>g of drugs via the <strong>in</strong>ternet due to the<br />

absence of available guidel<strong>in</strong>es. Medic<strong>in</strong>e sold without<br />

regulation can contribute to a drug epidemic, drug<br />

abuse, habit-form<strong>in</strong>g, misutilization, and persons be<strong>in</strong>g<br />

addicted to drugs. Most particularly, there is no physical<br />

chemist to safeguard the prescription’s correctness.<br />

Additionally, the drugs are be<strong>in</strong>g traded on the image of<br />

prescription, rather than the prescription itself. <strong>The</strong><br />

similar names of drugs can put dilemma <strong>in</strong>to the m<strong>in</strong>ds<br />

of users as well as sellers. Moreover, without an<br />

authenticated prescription, more problems will emerge.<br />

Proper guidel<strong>in</strong>es should be drawn up and notified to<br />

monitor the number of medic<strong>in</strong>e sold onl<strong>in</strong>e.<br />

Implement<strong>in</strong>g the regulations will make this new<br />

<strong>Pharmacy</strong> more accountable.<br />

E-<strong>Pharmacy</strong> surely br<strong>in</strong>gs advantages with it such as<br />

money and time sav<strong>in</strong>g, cost-effectiveness, possibility of<br />

24/7 access, <strong>in</strong>creased convenience, easy medic<strong>in</strong>es<br />

availability, an easy cost comparison of medic<strong>in</strong>es,<br />

<strong>in</strong>creased consumer <strong>in</strong>formation, and <strong>in</strong>formation<br />

exchange, privacy, and fast distribution. However, it<br />

exhibits some drawbacks too. For the betterment of the<br />

humank<strong>in</strong>d, the regulatory authorities and the<br />

Government of <strong>India</strong> must draw the regulations for<br />

E-<strong>Pharmacy</strong> keep<strong>in</strong>g <strong>in</strong> m<strong>in</strong>d the modern pharmacy<br />

system and Pharmacists alongside car<strong>in</strong>g for the safety<br />

Dr.D.Y.Patil Vidyapeeth, Pune. Born<br />

<strong>in</strong> 1957, <strong>in</strong> a village <strong>in</strong> Sangli district.<br />

He obta<strong>in</strong>ed his B.Sc. degree<br />

secur<strong>in</strong>g first class with Dist<strong>in</strong>ction<br />

(1974) from Shivaji University,<br />

Kolhapur. He obta<strong>in</strong>ed M.Sc.<br />

(Geology) with first class <strong>in</strong> 1980<br />

from the Pune University and later<br />

acquired his Ph.D. degree <strong>in</strong><br />

Geology, <strong>in</strong> 1985, from the Pune<br />

University. He was recipient of the<br />

Young Scientist Award <strong>in</strong>stituted by <strong>India</strong>n Science<br />

Congress Association Ranchi <strong>in</strong> 1984 session for his<br />

research work. For post-doctoral research, he received<br />

the prestigious British Council Fellowship for two<br />

consecutive terms <strong>in</strong> 1985 and 1987 for carry<strong>in</strong>g out<br />

research <strong>in</strong> U.K. Despite hail<strong>in</strong>g from rural based family,<br />

Dr Pawar has risen rapidly <strong>in</strong> the academic circles<br />

through sheer perseverance and patience. He has<br />

contributed immensely to the fields of teach<strong>in</strong>g and<br />

research, especially <strong>in</strong> terms of his specializations <strong>in</strong><br />

With<strong>in</strong> the span of a decade, the college has achieved a<br />

ABOUT THE PRESIDENT<br />

Mark<strong>in</strong>g the foundation of a new era of<br />

education, late S. Narasa Raju<br />

established the Oxford Educational<br />

Institutions <strong>in</strong> the year 1974 under the<br />

aegis of Children’s Education Society. He<br />

started his professional journey as a<br />

schoolteacher and later emerged as one<br />

of the em<strong>in</strong>ent educationists of the<br />

country. S<strong>in</strong>ce then, <strong>The</strong> Oxford<br />

Educational Institutions, exist<strong>in</strong>g for over<br />

four decades <strong>in</strong> the education arena,<br />

have evolved as an <strong>in</strong>spiration for<br />

knowledge explorers across the world.<br />

This immense growth has been the direct<br />

result of the leadership and guidance of<br />

the S Narasa Raju and S. N. V. L<br />

Narasimha Raju. It is a liv<strong>in</strong>g testimony of<br />

S Narasa Raju's <strong>in</strong>domitable will,s<strong>in</strong>glem<strong>in</strong>ded<br />

dedication, and an unrelent<strong>in</strong>g<br />

Achievement Column<br />

<strong>The</strong> <strong>in</strong>stitution is proud of its achievements and provides<br />

facilities for faculty and student research activities. <strong>The</strong><br />

Institute motivates the students to devote their time to<br />

extra-curricular activities and enhance their <strong>in</strong>nate talents.<br />

As far as notable achievement go, there are two ongo<strong>in</strong>g<br />

faculty research works <strong>in</strong> progress with the f<strong>in</strong>ancial<br />

support of Rs.9.5 Lakhs from Rajiv Gandhi University of<br />

Health Sciences. <strong>The</strong> college claims that the <strong>in</strong>stitution's<br />

students have achieved university Gold medals and<br />

<strong>in</strong>dividual subject ranks for 2014-2018 academic year .<br />

Students have successfully obta<strong>in</strong>ed first, second, and fifth<br />

ranks <strong>in</strong> B.Pharm exam from all over Karnataka.<br />

Similarly, first, sixth and tenth ranks <strong>in</strong> Pham D [PB]<br />

exam were also scored from <strong>The</strong> Oxford College of<br />

<strong>Pharmacy</strong>. Additionally, Rajiv Gandhi University of<br />

Health Sciences announced 75 <strong>in</strong>dividual subject ranks,<br />

which is a proud moment for the college.<br />

<strong>The</strong> college regularly motivates students and teachers to<br />

attend national and <strong>in</strong>ternational level conferences and<br />

workshops. Consequently, this enables them to understand<br />

pharmacy field. <strong>The</strong> college emphasizes more on research<br />

activities and presentation of posters and publications <strong>in</strong><br />

Society, Pune, established the AISSMS<br />

College of <strong>Pharmacy</strong> <strong>in</strong> 1996. <strong>The</strong><br />

predeterm<strong>in</strong>ed goal dur<strong>in</strong>g<br />

establishment was of becom<strong>in</strong>g a<br />

center of excellence for pharmaceutical<br />

education and research. <strong>The</strong> college<br />

THE IMPORTANCE OF<br />

“<strong>The</strong> good physician treats the disease; the great physician<br />

treats the patient who has the disease”- William Osler<br />

he healthcare <strong>in</strong>dustry has come a long way and has<br />

Tevolved through several factors. <strong>The</strong>se factors<br />

<strong>in</strong>clude changes <strong>in</strong> the practice of medic<strong>in</strong>e and<br />

technological advancements. Among those, one is chang<strong>in</strong>g<br />

its facet <strong>in</strong> the healthcare <strong>in</strong>dustry. It is patient-oriented<br />

care, also known as patient-centered care. <strong>The</strong> idea of such<br />

oriented care came back <strong>in</strong> 2007. It got widespread <strong>in</strong> no<br />

time because of its positive results <strong>in</strong> deliver<strong>in</strong>g<br />

personalized care to the patients.<br />

In patient-centered care, healthcare providers focus on<br />

keep<strong>in</strong>g the patient at the center of the treatment. <strong>The</strong>y<br />

place coord<strong>in</strong>ated care and improve access, quality,<br />

prevention and safety of the patients. In simple words, such<br />

care focuses on patient’s needs, giv<strong>in</strong>g them the help they<br />

need, when they need, and the way they need. People have<br />

outcome. In the same way, the healthcare system must work<br />

by tak<strong>in</strong>g <strong>in</strong>to account these methods. And, their care must<br />

Emotional Support and Elim<strong>in</strong>at<strong>in</strong>g Fear and Anxiety<br />

Anxieties can severely affect the patients, their illness and<br />

can create discomfort for the family members. Healthcare<br />

providers must pay attention to the anxiety of patients over<br />

physical status, treatment, and prognosis. <strong>The</strong>y should also<br />

pay attention to the patient’s anxieties developed over the<br />

f<strong>in</strong>ancial aspects of the treatments.<br />

Family and Friends Involvement<br />

Here, family and friends’ role is to provide support and<br />

comfort to improve the patient experience. On the opposite,<br />

the healthcare provider’s role is to arrange accommodation<br />

for the family and friends and <strong>in</strong>volve them <strong>in</strong> the shared<br />

decision-mak<strong>in</strong>g process. <strong>The</strong>y should provide support to<br />

the family members and ask them to do the same for the<br />

patient. Although patient-centered, the hospitals should also<br />

care for the family and friend’s needs.<br />

Cont<strong>in</strong>uity and Transition<br />

After the patient’s discharge, they should be able to care for<br />

themselves. <strong>The</strong>ir role here is to understand <strong>in</strong>formation<br />

about after-discharge self-care such as medication, physical<br />

limitations etc. Healthcare providers should coord<strong>in</strong>ate after<br />

the treatment and provide <strong>in</strong>formation on access to cl<strong>in</strong>ic,<br />

social, physical and f<strong>in</strong>ancial support on a cont<strong>in</strong>uity basis.<br />

Access to Care<br />

<strong>The</strong> patients should feel that all the needs they require, after<br />

the treatment, would be provided to them when needed.<br />

Healthcare provider’s role here is to help discharged<br />

patients with easy access to cl<strong>in</strong>ics and physician’s offices.<br />

As mentioned earlier, patient-centered care, focus on the<br />

health outcomes of patients with their preferences. Hence,<br />

they themselves should ask the healthcare provider the right<br />

questions. A healthy treatment requires cont<strong>in</strong>ues<br />

communication with the patients. By such, both healthcare<br />

provider and receiver can make the best possible decision.<br />

<strong>The</strong>y will also be aware of the possible outcomes. What<br />

will be the side effects of this treatment? Is it possible<br />

cancer will reappear later? What is the f<strong>in</strong>ancial support<br />

from your hospital for this disease? Answers to such<br />

questions will break the communication barriers between<br />

the healthcare provider, its receiver, and family. Also, with a<br />

current on-go<strong>in</strong>g wave of digitization, patients can access<br />

healthcare options on their computer, cell phones etc. This<br />

contributes to hav<strong>in</strong>g more participation com<strong>in</strong>g from a<br />

well-<strong>in</strong>formed patient.<br />

Outcome that Matters<br />

Every patient wishes for a good outcome. <strong>The</strong>y can achieve<br />

such by identify<strong>in</strong>g which treatment options work best for<br />

them. <strong>The</strong>y should also study the effectiveness of these<br />

treatments. <strong>The</strong> role of caretakers and physicians is to<br />

provide the patient with the necessary <strong>in</strong>fo to take better<br />

healthcare decisions with the patient’s preference.<br />

Additionally, this will put the care receivers <strong>in</strong> front and<br />

center of the treatment and will answer all their questions.<br />

<strong>The</strong> idea of patient-centered care is made for the welfare of<br />

the patient’s need and requirement. Thus, it is important to<br />

focus on patients and people’s preferences, which can<br />

change over the course of time. <strong>The</strong> challenge here is the<br />

and above all the government budget allocation for health is meagre compared to the poor <strong>in</strong> the country.<br />

Poor have more health care needs and unfortunately no access. Insecurity and illiteracy are important reasons<br />

for population explosion. Population growth needs to be curbed. Freebie services should be curtailed for<br />

more than two issues <strong>in</strong> a family and awareness of preventive measures such as birth control should be<br />

effectively implemented.<br />

Basic services such as birth control, medical term<strong>in</strong>ation of pregnancy, delivery, neonatal care and paediatric<br />

health should be easily and universally available. Regular preventive measures such as immunisations,<br />

nutritional guidance, physical activity (can be through education of yoga), hygiene practices (personal,<br />

community and environmental) should be made mandatory and easily accessible and <strong>in</strong>culcated as essential<br />

practices. Regular health assessment activities such as screen<strong>in</strong>g for cancers, anaemia, mental disorders,<br />

metabolic diseases should be accessible through simple tools and re<strong>in</strong>forced with awareness about these to<br />

nurture a healthy society.<br />

Health care facilities should have def<strong>in</strong>ed roles and limitations well identified. Primary health facilities<br />

(PHCs) should be run by paramedics and the care be limited to first aid and attend<strong>in</strong>g to basic emergency<br />

Universal standardised protocols for quality care are a necessity and these should be regularly evolved,<br />

dissem<strong>in</strong>ated and implemented. Much of the government programs are well studied and drafted but seriously<br />

fail <strong>in</strong> appropriate implementation. Accountability should be sought from everyone concerned about simple<br />

mandates such as fumigation od dis<strong>in</strong>fectants, clean<strong>in</strong>g of trench, clean public lavatories, garbage disposal,<br />

pollution control etc: <strong>The</strong> national programs should not only be limited to the government facilities but the<br />

<strong>in</strong>formation should be made easily accessible to private practitioners as 85 % of the medical care is provided<br />

<strong>in</strong> the private sector.<br />

Doctors should be well paid to compensate for the rigours they undergo to learn the subject. <strong>The</strong> skills of the<br />

doctor should be appreciated and well compensated. Much of the skilled doctors are mov<strong>in</strong>g to corporate<br />

sector due to better work<strong>in</strong>g conditions and revenue generation. Medicos also choose to leave the country for<br />

better research avenues and facilities for children education and access to super speciality education and<br />

services. This needs to be made available on our country to avoid skilled community dra<strong>in</strong>. Paramedical<br />

services should be improvised through skills tra<strong>in</strong><strong>in</strong>g and larger accessibility.<br />

<strong>The</strong> school curricula should be revised and much stress upon basic health <strong>in</strong>formation and knowledge be<br />

imparted. Serious lack of awareness about nutrition and physical activity has already put the country on the<br />

edge to blow up <strong>in</strong>to a metabolic diseases volcano and these can have far reach<strong>in</strong>g implications for the<br />

genetic programm<strong>in</strong>g of the generations to come. We as a country have our own special health needs and<br />

<strong>The</strong> doctors and health care providers should be protected and respected and attacks on the doctors and<br />

medical personnel dealt with seriously. Such attacks are a result of ignorance and refusal of acceptance of the<br />

Studies reveal that these both parties confirm that there are<br />

significant barriers that exist between them and both<br />

struggles with these. Studies also reveal that they both offer<br />

solutions <strong>in</strong> the form of improved technology and more<br />

face-to-face <strong>in</strong>teraction.<br />

Most of the time, the language of pharmacists and nurses<br />

gets <strong>in</strong> the way and builds obstacles. As previously<br />

mentioned, both these parties concentrate on serv<strong>in</strong>g the<br />

needs of the patients; however, the fundamental differences<br />

<strong>in</strong> operations and focus can affect the communication<br />

between them. A solution can be implemented as<br />

pharmacists shadow<strong>in</strong>g the workflow of the nurses and<br />

spend<strong>in</strong>g a day <strong>in</strong> the nurse’s shoes and vice versa. Further<br />

understand<strong>in</strong>g will assist <strong>in</strong> tear<strong>in</strong>g down the barriers and<br />

will take both parties <strong>in</strong> the same language. Another block<br />

is the shortage of face-to-face cooperation between these<br />

two. In acute-care sett<strong>in</strong>g, the teams of pharmacists and<br />

nurses operate on separate floors. As the result, nurses often<br />

are not acqua<strong>in</strong>ted with the workflow of the pharmacy<br />

department. Hence, lack of shared knowledge of workflow<br />

or lack of communication <strong>in</strong>fluence each other. As each<br />

party’s expectations may not be the same, it results <strong>in</strong> no<br />

rapport between the teams and absence of communication<br />

between these team players and ultimately the discipl<strong>in</strong>es<br />

struggles to communicate efficiently.<br />

Effects on the Patient Care<br />

Communication challenges with<strong>in</strong> the healthcare<br />

departments can generate mistrust <strong>in</strong> patients and delay <strong>in</strong><br />

care. S<strong>in</strong>ce patient experience is directly comparable to the<br />

potential revenue flow <strong>in</strong>to the hospitals, therefore, work<strong>in</strong>g<br />

on the team relationships can be the bottom l<strong>in</strong>e. All the<br />

departments play a direct role <strong>in</strong> the patient’s care and<br />

<strong>in</strong>adequate communication can set up barriers between the<br />

discipl<strong>in</strong>es. If not managed well, it can affect a patient’s<br />

health severely. For ex<strong>amp</strong>le, if a nurse calls a replacement<br />

medication via the electronic medical record, the<br />

pharmacists send the medic<strong>in</strong>e but it does not reach the<br />

nurse for an hour or two. Time passes and when the nurse<br />

confronts the pharmacists, they say that the medication dose<br />

was verified and sent. Ultimately, the patient’s treatment is<br />

delayed as they do not receive medication on time. This is<br />

just a light situation, <strong>in</strong> the case of surg<strong>in</strong>g hospital cases, a<br />

delay of an hour or two can be severe for the patient’s life.<br />

Facilities with a multidiscipl<strong>in</strong>ary approach to patient care<br />

are equipped with the f<strong>in</strong>est tools required to tackle the<br />

communication challenges. Outside of the patient<br />

communication challenges, healthcare leaders should also<br />

note the culture of the hospitals as it also plays a prom<strong>in</strong>ent<br />

role <strong>in</strong> staff and patient satisfaction.<br />

Improv<strong>in</strong>g Communication and Collaboration<br />

Align<strong>in</strong>g the objectives of both pharmacist’s and nurse’s<br />

team can enhance the conversation. Sometimes, the<br />

pharmacists should be united with<strong>in</strong> the teams. This<br />

strengthens the relationship between the pharmacists,<br />

nurses, and other team members alongside identify<strong>in</strong>g<br />

potential issues and their solutions. Likewise, face-to-face<br />

<strong>in</strong>teraction can assist <strong>in</strong> improv<strong>in</strong>g communication and<br />

association. A weak po<strong>in</strong>t between the pharmacists and<br />

nurses is the missed <strong>in</strong>formation on medication preparation<br />

and its distribution. To dim<strong>in</strong>ish this, they can set a monthly<br />

or quarterly meet<strong>in</strong>g on each floor to troubleshoot recurr<strong>in</strong>g<br />

issues and updat<strong>in</strong>g the staff on new equipment, new<br />

medication, and upcom<strong>in</strong>g <strong>in</strong>-service tra<strong>in</strong><strong>in</strong>g. <strong>The</strong> goal is<br />

to encourage an open discussion between the two parties so<br />

that nurse’s emergency should be well understood with the<br />

balance of pharmacist’s need for patient safety and quality<br />

assurance.<br />

Another measure is to promote the merger of location of<br />

both the departments. Most hospitals follow the traditional<br />

practice of separat<strong>in</strong>g the pharmacy on a different floor,<br />

away from the care. Decentraliz<strong>in</strong>g some portion can also<br />

carry out the needed <strong>in</strong>teraction between these two<br />

departments and aid <strong>in</strong> reduc<strong>in</strong>g delay <strong>in</strong> medication<br />

delivery. Incorporat<strong>in</strong>g new technologies like medication<br />

track<strong>in</strong>g tools can also be productive and will br<strong>in</strong>g<br />

improvements <strong>in</strong> workflow of pharmacists, nurses, and the<br />

REFORMS IN INDIA<br />

With a population of more than 1.3 billion people,<br />

<strong>India</strong> is currently a home to a fifth of the<br />

world’s population. Although <strong>India</strong>’s<br />

population growth rate has been show<strong>in</strong>g a decreas<strong>in</strong>g trend<br />

s<strong>in</strong>ce more than three decades, population is still <strong>in</strong>creas<strong>in</strong>g<br />

by 1.2% annually. <strong>The</strong> UN predicts that <strong>India</strong> will be<br />

world’s most populous country by 2024 and by 2030 <strong>India</strong><br />

will become the first political entity to reach the 1.5 billion<br />

people mark! <strong>India</strong> is world’s sixth largest economy by<br />

GDP (nom<strong>in</strong>al) and the third largest by purchas<strong>in</strong>g power<br />

parity, and yet it spends only about 1.2% of its GDP on<br />

health sector – among the lowest <strong>in</strong> the world.<br />

<strong>The</strong> constitution of <strong>India</strong> guarantees free healthcare for all<br />

its citizens, and yet private healthcare sector has been the<br />

primary source of health care for 70% and 63% of<br />

households <strong>in</strong> urban and rural areas of <strong>India</strong> respectively.<br />

58% of <strong>India</strong>’s hospitals are run by private sector and a<br />

whopp<strong>in</strong>g 81% of doctors serve <strong>in</strong> private healthcare sector.<br />

On the other hand public healthcare sector has catered to<br />

only 18% of total outpatient care and 44% of total <strong>in</strong>patient<br />

care. Clearly the <strong>India</strong>n government has failed by far to<br />

provide the people their right to free quality health care.<br />

<strong>The</strong> fact that 57% of households have cited poor quality of<br />

care offered at government hospitals as the major reason for<br />

preferr<strong>in</strong>g private sector over public sector is a major<br />

testimonial to the negligent attitude of the government<br />

towards healthcare reforms. Currently the healthcare system<br />

of <strong>India</strong> cannot completely mitigate the needs of <strong>India</strong>n<br />

population. Because the <strong>India</strong>n government spends only<br />

1.2% of its GDP on the health sector, out-of-pocket<br />

payments make up 75% of the total expenditure on health<br />

care. 40% of the people that are hospitalized are either<br />

pushed <strong>in</strong>to lifelong debts or below the poverty l<strong>in</strong>e! About<br />

23% of patients do not have sufficient money to afford<br />

medical treatment where as 63% of the patients lack regular<br />

access to necessary medications. In simpler terms, this<br />

means that the government has been deny<strong>in</strong>g a major<br />

proportion of our population an access to health care<br />

services. <strong>The</strong>se facts and figures make us contemplate about<br />

the healthcare policies and reforms <strong>in</strong>troduced by the<br />

government and question whether their implementation was<br />

<strong>in</strong> synergy with the true spirit of their formulation.<br />

In 2005, the government launched National Rural Health<br />

Mission (NRHM) with an aim to address the longstand<strong>in</strong>g<br />

issue of lack of professionals <strong>in</strong> rural area. <strong>The</strong> goals of<br />

NRHM were to provide effective healthcare to rural people.<br />

For this a compulsory rural service for young doctors was<br />

<strong>in</strong>stituted <strong>in</strong> the hopes that they will rema<strong>in</strong> <strong>in</strong> rural areas.<br />

In the Twelfth National Plan, <strong>India</strong>n government decided to<br />

expand NRHM to the entire country and called it National<br />

Health Mission (NHM). In 2013, National Urban Health<br />

Mission (NUHM) was approved as a sub-mission of NHM.<br />

NUHM aims to have one Urban Public Health Centre for<br />

each population of 50,000 and aims to fix current facilities<br />

and create new ones. Improv<strong>in</strong>g sanitation and dr<strong>in</strong>k<strong>in</strong>g<br />

water and reduction of out-of-pocket expenditure for<br />

treatment are among other aims of NUHM.<br />

In 2011, a report that was compiled by High Level Expert<br />

Group (HLEG), which was constituted by the Plann<strong>in</strong>g<br />

Commission of <strong>India</strong>, gave an elaborate f<strong>in</strong>ancial plan and<br />

framework for Universal Health Coverage (UHC). <strong>The</strong><br />

HLEG def<strong>in</strong>es UHC as “ensur<strong>in</strong>g equitable access for all<br />

<strong>India</strong>n citizens, resident <strong>in</strong> any part of the country,<br />

regardless of <strong>in</strong>come, social status, gender, caste or religion,<br />

to affordable, accountable, appropriate health services of<br />

assured quality (promotive, preventive, curative, and<br />

rehabilitative), as well as public health services address<strong>in</strong>g<br />

the wider determ<strong>in</strong>ants of health delivered to <strong>in</strong>dividuals<br />

and populations, with the government be<strong>in</strong>g guarantor and<br />

enabler, although not necessarily the only provider, of<br />

health and related services.” Follow<strong>in</strong>g were some of the<br />

key recommendations of the HLEG:<br />

1. Development of a national health package which would<br />

offer essential services as a part of citizen entitlement;<br />

2. No fee of any k<strong>in</strong>d to be charged for healthcare services<br />

under UHC;<br />

3. General taxation to be used as pr<strong>in</strong>cipal source of health<br />

f<strong>in</strong>anc<strong>in</strong>g;<br />

4. District hospital networks to be strengthened as care<br />

delivery and tra<strong>in</strong><strong>in</strong>g networks; and<br />

5. Effective contract<strong>in</strong>g guidel<strong>in</strong>es with checks and<br />

balances for care provision to the formal private sector.<br />

However due to some slacken<strong>in</strong>g <strong>in</strong> <strong>India</strong>’s economic<br />

growth, UHC was shelved. It was only <strong>in</strong> 2018 that the<br />

course of UHC was reset when the government<br />

‘Aayushman Bharat’ <strong>in</strong> the national budget. This <strong>in</strong>itiative<br />

has two components, the first of which has been named<br />

National Health Protection Scheme (NHPS). <strong>The</strong><br />

government has allocated a fund of INR 20 billion. <strong>The</strong><br />

central government will bear 60% of the costs of NHPS and<br />

the rema<strong>in</strong><strong>in</strong>g 40% will be contributed by each state. Under<br />

NHPS the government plans to provide coverage up to<br />

about INR 5 lakhs per family per year for secondary and<br />

tertiary care hospitalization. Second component consists of<br />

upgradation of 150000 subcentres <strong>in</strong>to Health and Wellness<br />

Centres (HWCs) across the country. <strong>The</strong>se HWCs will offer<br />

free basic cl<strong>in</strong>ical and preventive services <strong>in</strong>clud<strong>in</strong>g simple<br />

diagnostic tests and provide essential generic drugs to all.<br />

Upon implementation of these aforementioned schemes it is<br />

obvious that the vacancies will be on the rise. <strong>The</strong> WHO<br />

prescribes ideal doctor population ratio of 1:<strong>10</strong>00. <strong>India</strong> has<br />

a doctor population ratio of 0.62:<strong>10</strong>00, far less than the<br />

recommended figure. To overcome this dearth of<br />

physicians, the government has an ambitious plan of one<br />

medical college <strong>in</strong> each district of <strong>India</strong>. Even though the<br />

government is a long way from achiev<strong>in</strong>g this herculean<br />

task, the progress of government for the same is<br />

commendable.<br />

<strong>The</strong> profession of pharmacy has been an <strong>in</strong>tegral part of the<br />

healthcare systems of many countries s<strong>in</strong>ce decades, and<br />

yet it was not until recently that a curriculum for Cl<strong>in</strong>ical<br />

Pharmacists was <strong>in</strong>itiated <strong>in</strong> <strong>India</strong>. Till then <strong>India</strong> was<br />

essentially miss<strong>in</strong>g on an entire segment of cl<strong>in</strong>ical<br />

pharmacy services <strong>in</strong> its healthcare system, considered to be<br />

crucial for the provision of quality healthcare. In 2008, a six<br />

years <strong>in</strong>tegrated post-graduation course for the qualification<br />

PharmD (Doctor of <strong>Pharmacy</strong>) was <strong>in</strong>stituted by the<br />

<strong>Pharmacy</strong> Council of <strong>India</strong>. With <strong>in</strong>itiation of PharmD<br />

course, <strong>India</strong>n healthcare sector will now see a rise <strong>in</strong> the<br />

number of cl<strong>in</strong>ical pharmacists. Be<strong>in</strong>g tra<strong>in</strong>ed to provide<br />

services like comprehensive patient counsell<strong>in</strong>g, detection<br />

and management of drug related problems and adverse<br />

events, therapeutic drug monitor<strong>in</strong>g and provision of cost<br />

effective therapy, these cl<strong>in</strong>ical pharmacists will add a<br />

previously non-existent segment of cl<strong>in</strong>ical pharmacy<br />

services <strong>in</strong>to the <strong>India</strong>n healthcare segment.<br />

In conclusion, it can be said that although the government<br />

has now <strong>in</strong>itiated ambitious healthcare reforms, it still needs<br />

to ensure their success by meticulous plann<strong>in</strong>g and robust<br />

implementation. <strong>The</strong> government needs to do more to<br />

utilize other healthcare professionals like physicians from<br />

Ayurveda, Homeopathy, Siddha and Unani backgrounds,<br />

Pharmacists and Nurses, but at the same time ensure that<br />

the quality of healthcare services is not compromised.<br />

Surely HWC-led comprehensive primary health care and<br />

NHPS will <strong>in</strong>crease access to health services. For these<br />

schemes to succeed, the healthcare works force needs to be<br />

scaled up <strong>in</strong> both, number and quality. On other hand,<br />

regulatory systems also need to be strengthened to improve<br />

quality and accountability by keep<strong>in</strong>g str<strong>in</strong>gent checks on<br />

the healthcare sector. Although <strong>India</strong> has committed to<br />

UHC, it may take years to achieve it <strong>in</strong> true sense.<br />

A diligent personality Kunwar<br />

Shekhar Vijendra is the<br />

Co-founder and Chancellor of<br />

Shobhit University, <strong>India</strong>. He is a<br />

prom<strong>in</strong>ent social entrepreneur based <strong>in</strong><br />

New Delhi, who carries leadership<br />

roles <strong>in</strong> many organizations. He is a<br />

persistent advocate of the effective<br />

<strong>in</strong>itiatives taken by education<br />

fraternities for the poor, secular values,<br />

crisis management through diplomatic<br />

and peaceful ways, globalized systems<br />

of learn<strong>in</strong>g, and peaceful co-existence.<br />
