Dr. Jacqueline Delmont, MD, M.B.A., a Venezuelan native, is the Founder, President and Chief Executive Officer of Delmont<br />
Medical Care and Delmont Healthcare Consultants.<br />
Dr. Delmont became Board Certified in Internal Medicine in Venezuela in 1995 and in the United States in 1999. In 2012, she<br />
was selected to participate as a Health IT Fellow in the ONC Health IT Vanguards program. In 2013, Dr. Delmont completed<br />
her master’s degree in Business Administration at Adelphi University.<br />
In 2000, Dr. Delmont founded Delmont Medical Care, a multi-site, multi-specialty, patient-centered medical practice with<br />
20 primary care providers, delivering culturally sensitive, ambulatory, efficient and high-quality medical care to 25,000<br />
underserved populations across Long Island and Queens.<br />
Dr. Delmont’s wide experience makes her an expert on various subjects and as an early adopter of a patient-centered teambased<br />
approach and Electronic Medical Records, she was a pioneer in the field of medical practice transformation and the<br />
Patient Centered Medical Home. As an advocate, Dr. Delmont collaborates with various not-for-profit organizations and<br />
conducts community outreach to create awareness on preventative health, chronic diseases and self-management.<br />
Between 2012 and 2015 Dr. Delmont was the Chief Medical Officer at Beacon Health Partners, a Long Island based Independent<br />
Practice Association and Accountable Care Organization with over 1000 physicians participating in the Medicare<br />
Shared Savings program and commercial payer contracts. As Chief Medical Officer, Dr. Delmont was responsible for care<br />
coordination, disease management, and quality assurance and compliance requirements. In addition, she established and<br />
implemented clinical guidelines and patient-centered medical home transformation.<br />
In July 2019 Dr. Delmont joined SOMOS IPA as the Chief Medical Officer. Together with the leadership team, she is tasked to<br />
develop a modular population health program that can be customized for both full risk and partial risk contracts across the<br />
spectrum of IPAs and lines of business. Dr. Delmont’s experience gives her unique insight into quality healthcare delivery. A<br />
key contribution of Dr. Delmont is bridging the gap between clinical and business elements within the value-based care model.<br />
