Welcome
We’ve created this booklet to help you prepare for your
new NDIS plan. For those who are new to the NDIS, here’s
a quick guide as to where this booklet fits into your overall
NDIS journey.
Why choose
Independence Australia
Step 1
Determine your
NDIS eligibility*
Step 2
Apply for
support*
If you’re eligible and receive
an Access Decision letter,
the next step is creating
your NDIS plan.
This is where this booklet
will come in handy for you!
Step 4
Use your
plan
Step 3
Create your plan
Your plan is a written agreement worked out
with you. Everyone’s plan is different. Use
this booklet to consider all the things that
you want or need to include within your plan
and share that with the person assisting you
with creating your plan. Depending on your
situation that could be an NDIA Plan
Manager, a Local Area Coordinator
(LAC) or an Early Childhood Early
Intervention (ECEI) Partner.
Independence Australia is a social enterprise providing choices and services for people living
with a disability or other personal need. We have been supporting people across Victoria since
1957 to regain and retain their independence within a supportive community. As of 2021, these
services have also been available for people living in South Western Sydney, with our team
bringing more than 60 years of expert knowledge supporting people living with a disability.
Why choose Independence
Australia?
Working with our qualified, nationally
registered and specifically trained
staff who provide support and care
will give you the opportunity to
create a program that you want,
tailored to your needs.
Customer flexibility and choice is
essential, and we have made this a
focus in the delivery of our services
for customers in Sydney.
Who do you provide
support for?
We provide support and access
to services for people located in
South Western Sydney or anywhere
in Victoria, who are participants of
NDIS and other funding bodies.
Independence Australia provides
services for people seeking a
minimum 5 hours of support per
week, with a minimum of 2 hours
per shift as a requirement.
How we help is up to you...
Our support workers are available
to visit your home in the mornings,
afternoons or evenings to help you
prepare or wind down for the day.
Monday to Sunday, 7 days
per week.
We provide support across the
following categories:
• Complex Care
• Personal Care
• Domestic Assistance
• Community Access
• Meal Preparation
• Sleepovers
• Psychology and Counseling*
• Support Groups*
• Health and Wellbeing
Programs*
We will work with you to develop
and implement a support plan that
complements your preferences and
clinical support needs. Our friendly
and experienced support workers
will provide comfort and care to help
you through your daily tasks.
We promise to...
• Listen and understand
what support you require,
as it changes over time
• Deliver your program
with experienced support
workers who are nationally
accredited, and police
checked
• Be there for you from 2
hours to 24 hours a day,
every day as you choose
• Meet your needs – we
currently conduct more than
5,000 customer visits in
Victoria each week and are
now bringing this successful
model to New South Wales.
We are also one of Australia’s
largest suppliers of healthcare
products and can assist you to
access healthcare solutions.
What my
life looks like
everyday
Your living situation
Give your planner some information about your living situation. Let us know about where you currently
live, the people (and animals) that you live with and note if you would like to make changes to your home
or live somewhere different.
When you most need assistance
Think about how you navigate your day and list the times that you most need assistance.
Your regular or occasional
activities and needs
Add a little more information about what your day looks like and consider the times and situations
in which you may require additional support. You may wish to add details about your needs such as
showering and getting dressed, toileting, managing continence, eating, transferring between bed, chairs
and wheelchairs or taking medication.
The people who support you
Are there some key people in your life who provide you with additional support or who are simply
important to you? These could include friends, family, neighbours, workmates, people in your community.
List their names and the support they provide here.
