36 <strong>Partner</strong> <strong>Migration</strong><br />
All <strong>Partner</strong> visa applicants must also provide the following:<br />
• completed application form 47SP Application for migration to Australia by a partner and form 40SP<br />
Sponsorship for a partner to migrate to Australia (completed by your sponsor), (which are available<br />
from the department’s website www.immi.gov.au/allforms/ to be downloaded as PDF fi les, printed<br />
out and fi lled in, or obtained in hard copy from an <strong>of</strong>fi ce <strong>of</strong> the department or Australian mission and<br />
fi lled in);<br />
• if appointing a migration agent or exempt agent or authorised recipient, a completed form 956<br />
Advice by a migration agent/exempt person <strong>of</strong> providing immigration assistance;<br />
• if appointing an authorised recipient that is not a migration agent/exempt person, a completed<br />
form 956A Appointment or withdrawal <strong>of</strong> an authorised recipient;<br />
• 4 (or 2 if applying in Australia) recent passport-size photographs <strong>of</strong> yourself and 2 passport-size<br />
photographs <strong>of</strong> your sponsor (see page 5);<br />
• certifi ed copies <strong>of</strong> your passport or travel documents (see page 27 for information on certifi ed<br />
copies);<br />
• pro<strong>of</strong> <strong>of</strong> identity (see page 38);<br />
• evidence that your relationship is genuine and continuing (see page 39);<br />
• pro<strong>of</strong> that your sponsor is an Australian citizen, Australian permanent resident or eligible New<br />
Zealand citizen who is aged 18 years or over (see page 39);<br />
• statutory declarations from 2 people who are Australian citizens or Australian permanent residents<br />
and who support your claim that the relationship is genuine and continuing (see page 27 for<br />
information on statutory declarations);<br />
• if previously married, certifi ed copy <strong>of</strong> divorce certifi cate or death certifi cate for each previous<br />
marriage; and<br />
• if appropriate, completed health and character checks (see pages 19–21).<br />
You must pay the Visa Application Charge (see page 26).<br />
For each dependent child or other dependent family member included in your application, you<br />
must provide:<br />
• for each dependant aged 18 years or over, a form 47A Details <strong>of</strong> child or other dependent family<br />
member aged 18 years or over (available from the department’s website www.immi.gov.au/allforms/,<br />
from any <strong>of</strong>fi ce <strong>of</strong> the department or Australian mission) completed and signed by you, your partner<br />
and the dependant concerned;<br />
• 4 (or 2 if applying in Australia) recent passport-size photographs;<br />
• certifi ed copies <strong>of</strong> passports or travel documents;<br />
• certifi ed copies <strong>of</strong> birth certifi cates or the family book showing names <strong>of</strong> both parents;<br />
• custody documents or a statutory declaration from the child’s other parent giving permission for him/<br />
her to migrate;<br />
• evidence that the signature on the statutory declaration is the signature <strong>of</strong> that parent, or if your<br />
previous partner died, certifi ed copy <strong>of</strong> their death certifi cate;<br />
• for other dependent relatives, evidence that they have been dependent on you for at least the last<br />
12 months;<br />
• if appropriate, completed health and character checks (see pages 19–21); and<br />
• if the dependant is aged under 18 years, an Australian National Police Check or overseas police<br />
certifi cate(s) from the sponsor (see pages 16–17).<br />
