AmeriChoice Personal Care Plus® (HMO) is offered by the AmeriChoice of New Jersey, Inc., a New Jersey
licensed HMO with a Medicare Advantage contract and a Medicare approved Part D sponsor. The federal
government has approved AmeriChoice Personal Care Plus as a Special Needs Plan for people with Medicare
(Parts A and B) and Medicaid. All beneficiaries who live in the service area may apply. You must continue to
pay your Medicare Part B premium if not otherwise paid for by Medicaid. The Medicare health plan’s contract
with CMS is renewed annually. Availability of coverage beyond the end of the current contract year is not
guaranteed. This document is available in alternative formats. Eligible beneficiaries can enroll at any time.
