Section 1 – Introduction<br />

This directory provides a list of <strong>AmeriChoice</strong> Personal <strong>Care</strong> Plus plan providers. To get detailed<br />

information about your health care coverage, please see your Evidence of Coverage.<br />

You will have to choose one of our plan providers that are listed in this directory to be your<br />

<strong>Primary</strong> <strong>Care</strong> {Physician/Provider} (PCP). The term “PCP” will be used throughout this<br />

directory. Generally, you must get your health care coverage from your PCP, a licensed medical<br />

doctor (MD) or doctor of osteopathy (DO) or certain other licensed medical practitioner who,<br />

within the scope of practice and in accordance with State certification/licensure requirements,<br />

standards, and practices, is responsible for providing all required primary care services to<br />

enrollees, including periodic examinations, preventive health care and counseling,<br />

immunizations, diagnosis and treatment of illness or injury, coordination of overall medical care,<br />

record maintenance, and initiation of referrals to specialty providers described in this contract<br />

and the Benefits Package, and for maintaining continuity of patient care. A PCP shall include<br />

general/family practitioners, pediatricians, internists, and may include specialist physicians,<br />

physician assistants, Certified Nurse Midwives (CNMs) or Certified Nurse Practitioners/Clinical<br />

Nurse Specialists (CNPs/CNSs), provided that the practitioner is able and willing to carry out all<br />

PCP responsibilities in accordance with these contract provisions and licensure requirements.<br />

The “plan providers” listed in this directory have agreed to provide you with your health care<br />

coverage. You may go to any of our plan providers listed in this directory; referrals are not<br />

required. If you have been going to one plan provider, you are not required to continue going to<br />

that same provider. In some cases, you may get covered services from non-plan providers. Non<br />

par providers must obtain prior authorization. In limited cases, such as emergency care, urgently<br />

needed care when our network is not available, or out of service area dialysis, you may get<br />

covered services from non-plan (out-of-network) providers without obtaining prior authorization.<br />

If an out-of-network provider sends you a bill that you think we should pay for, do not pay the<br />

bill. Please contact Member Services or send the bill to us for payment.<br />
