Letter from the Editor<br />

EDITORIAL<br />

Ihad been looking forward to writing this introduction to the issue of<br />

the <strong>EPUAP</strong> <strong>Review</strong> that is distributed at the time of our Annual Open<br />

Meeting. There is always something pleasing about being able to welcome<br />

<strong>EPUAP</strong> members and those (hopefully) soon to be members to the<br />

conference. However, this year sees <strong>EPUAP</strong> approach the Tampere conference<br />

in sombre mood. The recent death of Gerry Bennett, our first Recorder<br />

has been a great shock to the <strong>EPUAP</strong> ‘family’ members. Gerry’s contribution<br />

to the development of <strong>EPUAP</strong> was immense – as our first Recorder he<br />

was responsible to the scientific programme at the seminal <strong>EPUAP</strong> conferences.<br />

As his successor in the post of Recorder, I was personally grateful for<br />

his advice and support during the Budapest conference last year. However,<br />

Gerry did much more than organise the conference programme, this issue<br />

of the <strong>EPUAP</strong> <strong>Review</strong> contains the draft text of a new <strong>EPUAP</strong> guideline on the<br />

role of nutrition in pressure ulcer prevention and treatment. This project<br />

was instigated by Gerry Bennett last year during the Budapest conference<br />

and he was responsible for bringing together the team that worked on the<br />

guideline text. I hope that the material we developed reflects Gerry’s enthusiasm<br />

for understanding pressure ulceration, while all members of the guideline<br />

group would like to thank him for his comments upon the various drafts<br />

we produced. The <strong>EPUAP</strong> has lost one of its strongest supporters, we will<br />

miss him for Gerry helped to build <strong>EPUAP</strong> into today’s vibrant organization.<br />

Looking ahead from this sombre time, we must continue to develop <strong>EPUAP</strong><br />

and strengthen its influence. A growing, respected and influential <strong>EPUAP</strong><br />

would indeed be a fitting tribute.<br />

This issue of the <strong>EPUAP</strong> <strong>Review</strong> also contains the first section from the<br />

recent pressure ulcer guidelines published in the Netherlands last year. This<br />

is the first part of an on-going series of articles that set out the content of this<br />

important evidence-based guideline. <strong>EPUAP</strong> became aware of this guideline<br />

through our members from the Netherlands, it would be excellent if <strong>EPUAP</strong><br />

could develop a library of pressure ulcer guidelines from across Europe.<br />

Could I ask members to send the <strong>EPUAP</strong> Business Office copies of national<br />

pressure ulcer guidelines or links to web sites where the documents could be<br />

accessed.<br />

from the President<br />

Dear Readers,<br />

THIS is my last contribution as the President of <strong>EPUAP</strong> and I would<br />

like to begin by remembering our colleague, Professor Gerry Bennett.<br />

Gerry will be remembered by <strong>EPUAP</strong> both for his accomplishments<br />

as a professional and for his sterling qualities as a person. He was an outstanding<br />

physician and an effective teacher who enriched all of us with his<br />

activity in the field of pressure ulcers. Even though I was not a close friend or<br />

even a close colleague, I, like many others admired him greatly. Since the<br />

foundation of <strong>EPUAP</strong> he expressed an eclectic interest in the progression of<br />

the society, with his most recent contribution being the suggestion to develop<br />

the nutrition guidelines which will be presented at our next annual<br />

meeting in Tampere. In my interactions with Gerry he had a delightful, subtle<br />

sense of humour that was expressed in the playful way in which he related<br />

to those with whom he was familiar. The memory of Professor Gerry Bennett<br />

will be held forever in inestimable esteem by each of us who were privileged<br />

to know him.<br />

The success and improvement of the <strong>EPUAP</strong> <strong>Review</strong> depends on the material<br />

available to the production team, and this availability depends not<br />

only on our trustees, but also on our many members. During my two years of<br />

presidency a large number of contributions were submitted and many of<br />

these were from different countries, reflecting the international readership<br />

and widespread interest in pressure ulcer prevention and management. My<br />

personal thanks go to Mike Clark for his invaluable hard work and continued<br />

support of the <strong>Review</strong>. Thanks also to the Business Office in Oxford, Dr<br />

and Mrs Cherry and Jane Green, who actively strive to keep improving the<br />

<strong>EPUAP</strong> <strong>Review</strong>.<br />

I look forward to working with the <strong>EPUAP</strong> in the future; helping to improve<br />

future activities and also in making the <strong>EPUAP</strong> <strong>Review</strong> a valuable read<br />

for all interested in research, prevention and management of pressure ulcers.<br />

NEWS FROM <strong>EPUAP</strong><br />

THE annual meeting of the <strong>EPUAP</strong> this year in<br />

Tampere, Finland is, as in the past, the major activity<br />

of our organization. It is appropriate that we are<br />

holding it in Finland this year, especially in view of the support<br />

that our Finnish members have given the <strong>EPUAP</strong> since<br />

its inception, and I would particularly like to thank Helvi<br />

Hietanen for her energetic contribution as a leader of the<br />

local organizing committee.<br />

The meeting is organized by the Business Office with<br />

the input of the scientific committee chaired by Michael<br />

Clark. This year we have had more than eighty-five excellent<br />

abstracts received for presentation on pressure ulcers.<br />

In the programme there are slots for twenty-eight oral presentations;<br />

the number was limited in order that the majority<br />

of delegates could attend the main sessions. There will<br />

be more than fifty poster presentations with the authors<br />

presenting at special breakfast and tea sessions near the<br />

posters in order to allow them to discuss their work with<br />

attendees. The posters are a major feature of the <strong>EPUAP</strong><br />

Annual Meeting and they contribute a wealth of knowledge<br />

on pressure ulcers as well as allowing time for in-depth discussion<br />

between the authors and delegates.<br />

Following the success of the satellite symposia at our<br />

meeting last year in Budapest, this year (on Thursday morning)<br />

we have four sponsored symposia which have been arranged<br />

by ConvaTec, Johnson & Johnson, KCI and Smith<br />

& Nephew. Two of these will take place before coffee and<br />

two before lunch, before the opening of the main meeting.<br />

We hope that you will support these excellent sessions in<br />

which the subjects are all related to pressure ulcer prevention<br />

and management, with well known and knowledgeable<br />

from the Business Office<br />

speakers in each of the symposia’s programmes. Details of<br />

the symposia are in the programme abstract booklet.<br />

If you are a paid-up member of the <strong>EPUAP</strong> I would encourage<br />

you to attend this year’s annual general meeting<br />

on Friday, at lunch time, as a number of administrative<br />

changes need to be ratified, particularly those concerning<br />

becoming Trustees of the <strong>EPUAP</strong>.<br />

You will find an invitation in this issue of the <strong>Review</strong> to<br />

attend a celebration of Gerry Bennett’s life and work, which<br />

is to be held in London on 21 September 2003. Please note<br />

that if you wish to attend, please contact the Warden’s Office<br />

at the address on the invitation.<br />

Next year our annual meeting will be held in Paris at<br />

the Palais des Congres during the World Union of Wound<br />

Healing Societies meeting 8–13 July 2004. <strong>EPUAP</strong> is a cohosting<br />

society of this meeting and is part of the programme<br />

planning committee with regard to pressure ulcers.<br />

<strong>EPUAP</strong> GUIDELINES<br />

On the Role of Nutrition in <strong>Pressure</strong> <strong>Ulcer</strong> Prevention and Management<br />

THIS issue of the <strong>EPUAP</strong> <strong>Review</strong> reproduces the draft<br />

text of a new <strong>EPUAP</strong> guideline covering the role of<br />

nutrition in pressure ulcer prevention and management.<br />

This guideline was initiated by the late Professor Gerry<br />

Bennett during the <strong>EPUAP</strong> Open Meeting held in Budapest<br />

last year. Since September 2002, the guideline development<br />

group (members listed below) met in Amsterdam<br />

in February 2003 with subsequent revisions to the draft text<br />

made via e-mail. This draft guideline will be presented for<br />

discussion during the <strong>EPUAP</strong> Open Meeting to be held in<br />

Tampere. All comments received during this presentation<br />

will be considered by the guideline development group and<br />

the text revised where appropriate. If you have any comments<br />

regarding the content of this draft guideline please<br />

send them to the <strong>EPUAP</strong> Business Office by the end of<br />

October 2003. We have also reproduced the list of references<br />

Guideline on nutrition in pressure ulcer<br />

prevention and treatment.<br />

Clinical Need for this Guideline<br />

<strong>Pressure</strong> ulcers are the result of a complex interplay between<br />

myriad extrinsic and intrinsic risk factors – excessive<br />

mechanical loading, immobility, incontinence, advanced<br />

age among many others. While the consequences of immobility<br />

are often viewed as the key predisposing factors in<br />

prompting the development of a pressure ulcer, it is often<br />

assumed that there is also a direct causal relationship between<br />

nutrition and pressure ulcer development. The scientific<br />

basis for this assumption is unclear with as yet no<br />

sound studies linking impaired nutrition and an increased<br />

incidence of pressure ulcers. However, it is possible that<br />

impaired nutrition may influence tissue vulnerability to<br />

extrinsic factors such as pressure. It is important to note<br />

that only a few risk factors can be influenced by our actions<br />

– tissue loading and nutrition being two key issues we can<br />

address. The perceived importance of malnutrition in pressure<br />

ulcer development and management is briefly considered<br />

within existing <strong>EPUAP</strong> guidelines; for example:<br />

• a full risk assessment in patients to include: General<br />

skin condition, skin assessment, mobility, moistness<br />

and incontinence, nutrition and pain,<br />

• following assessment nutritionally compromised<br />

individuals should have a plan of appropriate support<br />

and/or supplementation that meets individual needs<br />

and is consistent with overall goals of therapy,<br />

• ensure adequate dietary intake to prevent malnutrition<br />

to the extent that this is compatible with the<br />

individual’s wishes or condition.<br />

The purpose of this guideline is to expand upon the<br />

references to malnutrition within existing <strong>EPUAP</strong> guidelines<br />

and provide clinicians with specific guidance upon<br />

nutritional screening and assessment and following assessment,<br />

appropriate intervention. It is intended that the<br />

guidelines be appropriate for all care settings although it is<br />

recognized that the access to specific tools such as weighing<br />

scales and personnel such as dieticians may be limited<br />

in some sectors.<br />

The recommendations offered in this guideline have<br />

been graded using the following systems:<br />

Source of Evidence that underpins the recommendation<br />

I Evidence from systematic review or meta-analysis of<br />

randomised controlled trials or at least one<br />

randomised controlled trial.<br />

II Evidence from at least one controlled trial without<br />

randomisation or at least one other type of quasiexperimental<br />

study.<br />

III Evidence from non-experimental descriptive studies,<br />

such as comparative studies, correlation studies and<br />

case-control studies.<br />

IV Evidence from expert committee reports or opinions<br />

and/or clinical experience of respected authorities.<br />

Recommendation Grading<br />

A Directly based on category I evidence.<br />

B Directly based on category II evidence or extrapolated<br />

recommendation from category I evidence.<br />

C Directly based on category III evidence or extrapolated<br />

recommendation from category I or II evidence.<br />

D Directly based on category IV evidence or extrapolated<br />

recommendation from category I, II or III<br />

evidence.<br />

Both grading systems were adapted from Eccles M, Mason J<br />

(2001). How to develop cost-conscious guidelines. Health<br />

Technology Assessment 5: 8.<br />

Structure of the Guideline<br />

The recommendations of this guideline are considered to<br />

apply to both the prevention and management of pressure<br />

ulcers. Where guidance relates solely to pressure ulcer treatment<br />

this will be highlighted in the text. It should also be<br />

noted that the <strong>EPUAP</strong> considers all recommendations to<br />

be equally valid regardless of the grade of evidence upon<br />

which they are based. In the following recommendations<br />

where a source and level of evidence is not explicit, the<br />

recommendation should be considered as a level IV, D recommendation.<br />

Screening and Assessment of Nutritional Status<br />

Screening and assessment of an individual’s nutritional status<br />

can be performed using a number of measures ranging<br />

from tools such as the Subjective Global Assessment (Detsky<br />

et al., 1987) to relatively simple measures of height and<br />

weight (combined as Body Mass Index). However, some<br />

measurements (height, laboratory tests, skin fold thickness)<br />

may not be readily available in all care settings. Undesired<br />

weight loss (>10% of normal body weight in the past six<br />

months, or >5% in the past month) may provide an indication<br />

of malnutrition although where possible reasons for<br />

this unintentional weight loss should be explored with the<br />

individual patient.<br />

Accurate measurement of body weight and height, and<br />

hence Body Mass Index, may be problematical in many settings<br />

through lack of available equipment or challenges in<br />

measuring body length among some patient groups. While<br />

BMI has been found to be less valid in some patient groups,<br />

such as children and the very elderly, due to an altered/<br />

different fat/lean body mass ratio.<br />

Nutritional assessment may also include nutritional in-<br />

take over the past 1, 3 or 7 days; this information may be<br />

gathered using 24-hour recall, self or carer reported food<br />

intake records or through the involvement of a dietician,<br />

where available. It is important to consider why the intake<br />

of food and fluids is at the reported level.<br />

Biochemical measurements such as serum albumin,<br />

hemoglobin and potassium may be helpful when considering<br />

the nutritional status of the ill although these indicators<br />

may provide more information upon chronic, rather<br />

than acute depletion of specific nutrients. In general it is<br />

unlikely that biochemical measurements will provide more<br />

information than other indicators such as undesired weight<br />

loss although a number of studies cite an association between<br />

albumin and pressure ulcers.<br />

The use of nutritional screening or assessment tools<br />

appears to be becoming more prevalent in managing patients<br />

at risk of/with pressure ulcers. These tools require to<br />

be validated and reliable, and like general risk assessment<br />

tools should not replace clinical judgement. However, the<br />

use of validated nutritional assessment tools may help to<br />

foster attention upon the need to consider nutrition when<br />

assessing vulnerability to pressure ulcer development.<br />

Nutritional status should be re-assessed regularly following<br />

an individualized assessment plan which includes an<br />

evaluation date. The frequency of assessment should be<br />

based upon the condition of the individual and should occur<br />

following specific events such as surgery and any development<br />

of infections or other catabolic processes likely to<br />

stress the nutritional status of the individual.<br />

While looking at the individual patient the clinical judgement<br />

of appropriately trained health professionals may provide<br />

sound evaluations of probable nutritional status, it<br />

should be acknowledged that excess of body weight may<br />

mask nutritional deficiencies – for example morbidly obese<br />

individuals may still be malnourished.<br />

Nutritional Intervention<br />

Where an assessment or screening of nutritional status indicates<br />

that malnutrition may be present, nutritional intervention<br />

should be considered. The primary goal of nutritional<br />

intervention is generally to correct protein-energy<br />

malnutrition ideally through oral feeding. When considering<br />

any limitations on normal food and fluid intake, consider<br />

the local environment such as ease of access to food,<br />

social and functional issues along with the texture of the<br />

diet. Changes in these aspects may encourage or facilitate<br />

increased oral intake. Overall the goal should be to consider<br />

the quality and energy-density of the food intake rather<br />

than its quantity. Considering fluid intake quantity is equally<br />

important as quality.<br />

Where enhanced normal feeding is not possible, protein-energy<br />

rich oral supplements may be considered (Recommendation<br />

1, B; Benati et al., 2001, Bourdel-Marchasson<br />

et al., 2000, Breslow et al., 1993, Chernoff et al., 1990, Delmi<br />

et al., 1990). The value of vitamin and trace element supplementation<br />

in pressure ulcer prevention is unclear (Recommendation<br />

1, B; Taylor et al., 1974, ter Riet et al., 1995).<br />

Where normal feeding and oral supplementation fail<br />

to resolve apparent malnutrition then other routes (for<br />

example tube-feeding) may be undertaken although the<br />

risks associated with these interventions should be considered.<br />

While the amount of supplementation required by individuals<br />

will vary, general guidance can be offered where<br />

an individual may require a minimum of 30–35 kcal per kg<br />

body weight per day, with 1 to 1.5 g/kg/day protein required<br />

and 1ml per kcal per day of fluid intake.<br />

Specific guidance on energy expenditure may be provided<br />

through the use of standard equations such as the<br />

Harris-Benedict or Schofield formulae although it is recommended<br />

that advice on their use and interpretation be<br />

sought from a dietician (where available) or the multidisciplinary<br />

care team.<br />

The success of nutritional intervention should be reviewed<br />

within the on-going regular nutritional assessments<br />

and may be indicated by outcomes such as increased weight<br />

or improved functional ability and/or enhanced healthrelated<br />

quality of life. Successful nutritional intervention<br />

may also be marked by a reduced incidence of new pressure<br />

ulcers and the healing of established pressure ulcers.<br />

Regular evaluation of the effects of nutritional interventions<br />

is required but it should be borne in mind that where<br />

individuals are malnourished the effects of feeding and/or<br />

supplementation may not be immediately apparent, probably<br />

because there first needs to be a restoration of already<br />

depleted reserves.<br />

Where patients have established pressure ulcers then a<br />

similar strategy of nutritional intervention should normally<br />

be considered (normal feeding, then oral supplements and<br />

finally tube-feeding) although the demands may be greater.<br />

There are a number of observations upon the role of nutritional<br />

deficiencies and pressure ulcer healing that can be<br />

extracted from controlled trials – protein and calorie supplementation,<br />

along with the use of arginine, vitamins and<br />

trace elements with antioxidant effects appear to have a<br />

positive effect on healing (Recommendation 1, B; Benati<br />

et al., 2001, Bourdel-Marchasson et al., 2000, Breslow et al.,<br />

1993, Chernoff et al., 1990, Delmi et al., 1990). The evidence<br />

for the value of ascorbic acid supplementation is equivocal<br />

(Recommendation 1, B; Taylor et al., 1974, ter Riet et al.,<br />

1995), and the evidence for zinc supplementation is weak<br />

(Recommendation 1, B; Norris 1971).<br />

Specific issues may need to be resolved if normal feeding<br />

is to be enhanced – for example control of wound odour,<br />

altered body image, pain associated with the pressure ulcer<br />

and loss of self-esteem because these issues can reduce nutritional<br />

intake.<br />

Where individual patients present with severe pressure<br />

ulcers (Grades 3 and 4) then the multidisciplinary team<br />

should consider their basal energy expenditure and pay<br />

particular attention to the increased fluid loss through such<br />

wounds.<br />

The nutritional requirements of specific groups may be<br />

different from those outlined in these guidelines, for example<br />

the spinal cord injured.<br />

Nutritional assessment and intervention should of<br />

course be combined with all other appropriate interventions<br />

including pressure management.<br />

These guidelines have not addressed several specific issues<br />

– nutritional assessment and intervention in neonates<br />

and paediatrics, the role of parenteral nutrition and specific<br />

needs of individual patient groups such as the immunosuppressed,<br />

those with cancers, orthopaedic, trauma and<br />

surgical patients and those who have experienced burns.<br />

Pharmacological interventions such as the use of anabolic<br />

steroids also are not included.<br />

In all of the preceding recommendations regarding nutritional<br />

assessment and supplementation all decisions<br />

should be taken with regard to patient choice and in light<br />

of the overall goals of treatment.<br />

Education<br />

There is a requirement for all staff (including but not limited<br />

to health professionals, untrained staff, catering and<br />

porters) to be aware of the importance of nutrition and to<br />

understand their role in improving the nutritional status of<br />

patients. This education will range from the performance<br />

of nutritional screening and assessment, the preparation<br />

of attractive, appetizing meals and the delivery and presentation<br />

of meals dependent upon the needs of individual<br />

staff members. There is a need to establish a nutritional<br />

culture within healthcare prompting the appropriate availability<br />

and presentation of meals through to continuity of<br />

nutritional care across departments and care settings.<br />

Summary of Recommendations<br />

The <strong>EPUAP</strong> recommends that as a minimum, assessment<br />

of nutritional status should include regular weighing of<br />

patients, skin assessment, documentation of food and fluid<br />

intake. Additional procedures including anthropometric<br />

measurements and laboratory tests may also be performed<br />

although these may best be viewed as more advanced assessment<br />

techniques. Nutritional intervention should focus<br />

upon improving the individual’s intake of food and fluids –<br />

through consideration of the quality of what is offered along<br />

EUROPEAN PRESSURE ULCER ADVISORY PANEL<br />

from the Netherlands<br />

A NEW EVIDENCE-BASED PRACTICE GUIDELINE<br />

ON PRESSURE ULCER PREVENTION AND MANAGEMENT<br />

IN 2002 new guidelines on pressure ulcer prevention<br />

and management were released in the Netherlands.<br />

These evidence-based guidelines built upon the literature<br />

review conducted during the development of the<br />

United States Agency for Health Care Policy and Research<br />

(AHCPR) pressure ulcer guidelines issued in the early 1990s.<br />

As such the new Dutch guidelines offer new insights into<br />

the management of pressure ulcer prevention and management<br />

given their foundation upon new research studies<br />

and an awareness of literature published in languages other<br />

than English. The <strong>European</strong> <strong>Pressure</strong> <strong>Ulcer</strong> <strong>Advisory</strong> <strong>Panel</strong><br />

would like to acknowledge CBO, Dutch Institute for Quality<br />

in Health Care (Kwaliteitsinstituut voor de Gezondheidszorg)<br />

for their kind permission to present an edited version<br />

of these new guidelines within the <strong>EPUAP</strong> <strong>Review</strong>. In<br />

this issue we cover the methodological issues related to the<br />

development of the new guidelines.<br />

Why was the new guideline developed?<br />

Recent years have seen greater focus upon the problem of<br />

pressure ulcers in the Netherlands at a political and economic<br />

level. For example political initiatives have included<br />

a report on pressure ulceration to the Secretary of Health<br />

and the Health Council. 1 This report was followed by a written<br />

reply to the report’s conclusions submitted by the Health<br />

Minister to the Dutch House of Commons. In parallel with<br />

these political issues there has been a growth in the availability<br />

of national epidemiological data 2 along with a<br />

stronger awareness of the economic impact of pressure ulcers.<br />

3–18 In the Netherlands approximately 13% of all patients<br />

in teaching hospitals have pressure ulcers, the prevalence<br />

then rises in home care (17%), general hospitals<br />

(23%) and to about 30% in nursing homes 2 while the annual<br />

spend on pressure ulcer care is about 600 million Euros<br />

with a total population of 16 million. 16–17 These developments<br />

have not occurred in a vacuum; Dutch demography<br />

has changed over the past ten years with greater numbers<br />

of elderly people, while the scientific basis for pressure ulcer<br />

prevention and management has also advanced. Taken<br />

together there was perceived to be a need to revise earlier<br />

consensus texts developed in the Netherlands during the<br />

mid 1980s, and to update the US AHCPR guidelines that<br />

date from 1992. This update of the guideline was commissioned<br />

by the CBO (Dutch quality organization for health<br />

care) with all relevant interest groups represented in the<br />

working group established to direct the guideline update.<br />

What is the purpose of the new guideline?<br />

Essentially the guideline must be appropriate for use in daily<br />

medical, nursing and paramedical practice giving sound recommendations<br />

and practical advice. All patient groups and<br />

care settings are to be covered by the guideline’s recommendations<br />

– both those with an elevated risk of developing<br />

pressure ulcers and those with established pressure ulcers.<br />

One important consideration was that the guideline<br />

should seek to facilitate the co-ordination of care both within<br />

and between institutions.<br />

Definition of pressure ulceration used<br />

throughout the guideline<br />

In this guideline, ‘pressure ulcer’ should be considered to<br />

refer to any tissue damage caused by mechanical loading<br />

(pressure, shear and friction alone or in combination).<br />

Developing the guideline<br />

In 2000 a Working Group was formed to lead the development<br />

of the new guideline (See Appendix 1 for composition<br />

of the group) formed of society representatives and<br />

active researchers. In particular this latter group comprised<br />

of post-doctoral fellows drawn from the Netherlands and<br />

Belgium and collectively known as the <strong>European</strong> <strong>Pressure</strong><br />

<strong>Ulcer</strong> Research Interest Group (EPURIG).<br />

The working group first met on 11 May 2000 and separated<br />

the guideline development into two phases – development<br />

of the recommendations, and commentary from<br />

appropriate stake-holders. The EPURIG members were<br />

tasked to identify the relevant literature, review it and form<br />

draft recommendations. The primary role of the scientific<br />

society representatives was to provide comment upon these<br />

draft recommendations. Studies to be included in the review<br />

were to have been published since the AHCPR pressure<br />

ulcer prevention guideline issued in 1992. Initially it<br />

was anticipated that the latest date of publication to be covered<br />

by the review was Summer 2000 but this process extended<br />

to include new publications for a further twelve<br />

months (Summer 2001). Throughout the review process<br />

close co-operation was sought with relevant Dutch steering<br />

committees and <strong>European</strong> and North American pressure<br />

ulcer organizations (<strong>EPUAP</strong> and NPUAP).<br />

During the initial meeting, the chair of the working<br />

group proposed the guideline be divided into five sections<br />

covering:<br />

• risk assessment<br />

• prevention<br />

• pressure ulcer assessment<br />

• treatment and complications<br />

• organizational issues and specific care-setting<br />

situations<br />

This structure was subsequently adopted during the development<br />

of the guideline.<br />

Search strategy<br />

Relevant literature was identified through a combination<br />

of searches of electronic data bases and hand-searching of<br />

reference lists and other available evidence-based guidelines.<br />

The electronic search covered four key data-bases –<br />

the Cochrane Library, CINAHL, EMBASE and MEDLINE.<br />

Each data-base was searched using the following key-words;<br />

pressure ulcer, pressure ulcers, pressure sore, pressure sores<br />

and decubitus. Studies retrieved with the last key-word were<br />

rejected from the review if they referred to the decubitus<br />

position. The search was refined using a variety of study<br />

designs as additional key-words; randomized clinical trials,<br />

cohort studies, retrospective studies, prospective studies,<br />

case control studies and case studies. Eligible studies had<br />

to be published between 1992 until July 2001. Members of<br />

the working group also submitted publications out with<br />

those included on the electronic data-bases while further<br />

publications were identified from reference lists and other<br />

evidence-based guidelines. In total over 400 publications<br />

were considered in this review. Each publication was then<br />

reviewed and graded using the following criteria:<br />

Where publications reported the use of interventions for<br />

either pressure ulcer prevention or treatment;<br />

A1 systematic reviews that included some studies of<br />

A2 level, with consistent results across the individual<br />

studies,<br />

A2 appropriately designed and conducted<br />

randomized controlled trials,<br />

B low quality randomized controlled trials or nonrandomized<br />

comparative cohort studies or<br />

patient-controlled studies,<br />

C non-comparative studies,<br />

D expert opinion.<br />

And in cases where publications reported upon diagnostic<br />

procedures for example risk assessment;<br />

A1 several prospective studies (including some at A2<br />

level) with minimal loss to follow-up within<br />

specified patient groups that reported clinical<br />

outcomes achieved after implementing a specified<br />

diagnostic procedure,<br />

A2 studies comparing the results from a diagnostic<br />

procedure against previously agreed ‘gold standard’<br />

reference procedure, with appropriate<br />

statistical analyses,<br />

B studies comparing the results from a diagnostic<br />

procedure against a reference procedure, may not<br />

have appropriate statistical analyses,<br />

C non comparative studies,<br />

D expert opinion.<br />

Throughout the process of reviewing and grading studies<br />

the working group would like to thank the Belgian <strong>Pressure</strong><br />

<strong>Ulcer</strong> Guideline Committee for the assistance they provided.<br />

Having graded the sources of evidence, the recommendations<br />

in the new guideline were also weighted as follows:<br />

1 Recommendation based on one systematic review<br />

(A1), or at least two independent studies on level A1<br />

or A2,<br />

2 Based on at least two independently executed studies<br />

on level B,<br />

3 Based on one study at level A2 or B, or studies on<br />

level C,<br />

4 Based on expert opinion.<br />

Each recommendation is supported by its scientific basis,<br />

representing a summary of the studies that underpin<br />

the recommendation. The key studies and weighting (level<br />

1 to 4) are illustrated for each of the given recommendations.<br />

Where recommendations were in part based on information<br />

outside the scientific studies (for example patient<br />

preferences, cost, equitable access throughout all care<br />

settings) the non-scientific aspects are listed as ‘other considerations’.<br />

Adopting this procedure for presenting recommendations<br />

was considered to increase the transparency<br />

and implementation of the guideline while also facilitating<br />

discussion within the working group!<br />

Legal implications of the guideline<br />

Clinical guidelines are not laws, but rather offer insights<br />

based on the available evidence that would assist caregivers<br />

to deliver ‘good’ care. These insights are based on the average<br />

or typical patient and when faced with a heterogeneous<br />

patient population it is likely that caregivers may exercise<br />

their professional judgement and decide to deviate from<br />

the guideline. Such deviations should be noted in the patients<br />

medical record.<br />

Updating the guideline<br />

The CBO will update the guideline no later than 2007 although<br />

earlier review may be required dependent upon<br />

advancves in scientific knowledge and clinical practices.<br />
