what you have seen in school?<br />

5. Examples <strong>of</strong> where you observed co-workers providing service to the <strong>University</strong>, to the community, or to the pr<strong>of</strong>ession. What were<br />

the costs and benefits <strong>of</strong> the service activities you observed?<br />

Use your observations to create and attach the following documents:<br />

1) a 1-2 page assessment <strong>of</strong> the specific knowledge, skills, and competencies you developed during your internship. Include an<br />

assessment <strong>of</strong> the internship site.<br />

2) a 1-2 page discussion <strong>of</strong> how your internship has shaped your career/pr<strong>of</strong>essional goals<br />

3) a one-page assessment written by your supervisor commenting on <strong>of</strong> the specific knowledge, skills, and competencies s/he feels<br />

you gained during the internship.<br />
