REPORT<br />

As always, the past 12 months has been busy within College,<br />

with significant progress made on many initiatives old and new,<br />

and undoubtedly even more change in the medico-political<br />

landscape externally.<br />

in April the Council of Australian Governments announced a four hour<br />

National Access Target as part of health reforms for Australian ED.<br />

The New Zealand government had implemented a six hour shorter stays in<br />

ED target in July 2009. in response to these initiatives, the College statement<br />

on time-based targets was developed with wide consultation, and is available<br />

on our website.<br />

leading up to the Australian federal election a very welcome pre-election<br />

announcement by Prime Minister Gillard and Health Minister Roxon<br />

allocated funding to provide additional emergency medicine training places,<br />

to support supervision time for specialists in 129 emergency departments<br />

across the country, plus an additional 10 places to provide training<br />

supervision in the private sector, funding for emergency medicine education<br />

officers to assist in implementation of College’s educational programs,<br />

and for College to administer additional training places, and to develop<br />

educational resources to support the roll-out of non-specialist training<br />

programs across the country.A meeting with representatives from the<br />

Department of Health and Ageing will occur shortly to progress these items.<br />

A planning day was held late in 2009 and considerable progress has been made<br />

across the broad range of priorities identified for further development.<br />

A full report of planning day outcomes and the progress since was included in<br />

the september edition ofYD, available on the College website and i encourage<br />

all Fellows and Trainees to read this, if you have not already done so.<br />

i will mention just a few of the many activities continuing within College<br />

during the year.With regard to Trainees, significant changes have occurred<br />

related to the training program with respect to the delivery of, and progress<br />

through, the primary exam, and changes to requirements for the research<br />

component.The Trainee Committee Chair now has voting rights on additional<br />

committees, and Council is considering this question for itself as well.<br />

Within Council committees and working groups there is increasing engagement<br />

with other Colleges to produce joint policies and position statements, and<br />

more focus on external collaborations in disaster management, and public<br />

health.There is ongoing work on Fellow support resources as well.The Board<br />

of Education has successfully implemented and bedded down the significant<br />

governance changes of some time ago, and has overseen many initiatives to<br />

further improve our training program content and structure. As part of the<br />

continued evolution and fine-tuning of College’s educational activities, the<br />

Training and Assessment ReviewWorking Group will be submitting a report<br />

to Council in November which will provide advice to Council on required<br />

changes to the training program. College has also benefitted from the work<br />

of the Educational Advisory Group this year.Administratively, i am sure all have<br />

welcomed the move to electronic means of communication rather than paper,<br />

which now extends to almost all correspondence.<br />

i reported last year that work was continuing on development of educational<br />

programs for non-specialist doctors practising emergency medicine. i am now<br />

able to report that the pilot of the Emergency Medicine Certificate six month<br />

course has commenced in a number of EDs in Australia.A successful Course<br />

in Clinical supervision for Fellow supervisors for the pilot was recently held<br />

at College, and it is planned to conduct this course<br />

regionally in future.The pilot will be evaluated by<br />

external experts, and feedback incorporated into<br />

future courses.Work to finalise the Diploma course<br />

is underway, with the involvement of key stakeholders<br />

including RACGP and ACRRM.<br />

College contribution to the broader medical<br />

environment continues principally through our<br />

involvement in CPMC and other stakeholder<br />

committees.The health workforce, and the<br />

inextricable link with training, is currently occupying<br />

federal government and other stakeholder groups.<br />

There has been consultation on a national<br />

competency based framework for the health<br />

workforce, and discussion regarding simulated<br />

learning environments for training.<br />

With commencement of national registration in July,<br />

the focus has been on finalising accreditation, and<br />

specialist recognition, arrangements.<br />

i attended the iFEM Board meeting at the very<br />

successful iCEM in singapore this year. unfortunately<br />

i was unable to attend the recent meeting in<br />

las Vegas, where Peter Cameron took the chair as<br />

iFEM President for the first time. ian Knox continues<br />

in the role of Treasurer, with <strong>ACEM</strong> providing the<br />

secretariat through Carol Reardon.<br />

i would like to mention the resignation of Debra<br />

O’Brien as Censor-in-Chief of the College from<br />

November this year, and record my thanks for her<br />

unstinting hard work, attention to detail, and drive to<br />

have the best training program possible, as evidenced<br />

by her wish for College to not only fully meet, but<br />

stay ahead of AMC requirements. Debra will have<br />

held this position for the past three years, during<br />

a time of significant change in the structure of the<br />

then Board of Censors to the Board of Education,<br />

and has overseen the transition of the Board and its<br />

committees tirelessly and enthusiastically. Deb will<br />

continue to contribute in her role as Examiner, and<br />

i know all join with me in thanking her, and wishing<br />

her the best for her future endeavours.<br />

Once again, i am very thankful for the hard work and<br />

dedication of our College staff under the leadership<br />

of Jenny Freeman.The smooth conduct of all College<br />

processes, and their ability to incorporate new<br />

initiatives and changes continues to be impressive.<br />

it is an honour to serve the College as President,<br />

and i thank you all for your support.<br />

sECRETARY’s REPORT<br />

Anthony Lawler<br />

As to be expected in an Australian election year, especially with<br />

an election that seemed to start some months before polling<br />

day and continued some weeks thereafter, health featured<br />

prominently in the public discussion space.Appropriately,<br />

emergency medicine had its place firmly at the forefront at<br />

these discussions.<br />

Following on from the success of the access block summit in september<br />

2008, there was much focus federally on initiatives intended to address<br />

resourcing and access issues in Australia’s emergency departments.<br />

The Rudd Hospital Reform process shone the spotlight on the adoption<br />

nationally of the ‘4 hour target’.While this has been variably taken up over<br />

the country, and while each Fellow and trainee will have his or her own view<br />

on it, the College has been active in clarifying its view on the initiative,<br />

and has made clear that time-based indicators should feature as a part of<br />

a suite of system-wide indicators related to access. in addition, any move<br />

to a time-based indicator should be applied only when clinically appropriate<br />

on a patient-by-patient basis.The development of the College’s statement<br />

on this issue was a good example of the capacity of <strong>ACEM</strong> to reflect a broad<br />

range of views, with discussions involving Council, Faculties, various College<br />

committees including the Trainee Committee, and Fellows leading local<br />

implementation projects.<br />

The College’s statement can be found on the College’s easily navigable<br />

website – as can all of the College’s policies, publications, statements and<br />

governance information.<br />

This year has also seen a number of innovations in the provision of<br />

emergency training, many of which will be addressed elsewhere in this report.<br />

There was no Winter symposium this year, owing to the iCEM being held<br />

in singapore.The 2009 <strong>Annual</strong> scientific Meeting, held in Melbourne in<br />

November, was an outstanding success, with over 800 registrations.<br />

it explored several initiatives, including the provision of the meeting handbook<br />

electronically and the purchase of carbon credits to offset the impact of the<br />

meeting. Congratulations to the convenors and organising committee for<br />

such a successful endeavour.<br />

in order to integrate more effectively the functions of the faculties and<br />

‘head office’, and more importantly to provide a support resource for faculty<br />

officials when issues of national relevance impact locally, the role of Honorary<br />

secretary will be expanded to encompass the function of a Faculty liaison<br />

Officer. it is envisaged that this will complement the provision by the College<br />

of administrative support for Faculty Board activities.<br />

As a result of the continued growth of the College, and due to some<br />

resignations, we have been pleased to welcome the following new staff<br />

members during the year: Helen Kaminski, Jane Macaulay, suzi Taleska, Hydie<br />

Pallier, Freda ligaras, Katherine Ebbs, sarah McArthur, and Carol Reardon.<br />

Jackie King, Myra Karantzas, Helen Cooper and sarah smith have gone on to<br />

other things and we thank them for their contribution. On behalf of Council<br />

i would like to acknowledge the commitment of all staff members of the<br />

College and of the Fellows, trainees and committees that that they support.<br />

i would also like to note the resignation of<br />

Debra O’Brien as Censor-in-Chief of the College<br />

which will take effect from November this year.<br />

We wish her the best and extend our thanks for<br />

her massive contribution.<br />

The work of the College would grind to a halt<br />

without the dedicated contribution of Fellows<br />

and trainees across Australasia. individuals who all<br />

have full enough plates with their own clinical load,<br />

administrative, research or teaching work, and study<br />

give further of themselves to assist in ensuring the<br />

highest quality training, robust clinical standards and<br />

sound system reform.<br />

i would like to end this report with a heartfelt note<br />

of thanks to all Fellows and trainees who give of their<br />

own time, in many capacities, to the advancement of<br />

emergency medicine in Australasia through the<br />

activities of the College.<strong>ACEM</strong> has a role as a training<br />

and standards body, but is also seen as the preeminent<br />

body advocating the provision of emergency<br />

care in Australia and New Zealand.<br />

TREAsuRER’s REPORT<br />

i am pleased to report this year that the operating result shows a<br />

strong surplus of $1,597,851 and net assets have now increased<br />

to $12,555,826 ($10,680,936 in 2009). it is pleasing to see the<br />

College in such a sound financial position.<br />

The surplus is up from $956,798 in 2009 and reflects the fact that the year<br />

has largely been one of consolidation following the very significant increase<br />

in staff, the finalisation of the new premises and the effect of the Global<br />

Financial Crisis on the previous year.<br />

in reviewing the income and Expenditure Account there are increases in<br />

costs associated with the rise in activity within the Council and Board of<br />

Education Committees and conducting of courses, which was to be<br />

expected. However, savings can be seen in other areas such as printing,<br />

postage and stationery due to the introduction of electronic mailing.<br />

Venues to cater for the increased size of the Primary Examination were<br />

sourced early for this year and as a result significant savings were made in<br />

that area. Also, it was noted that there had been some difficulty with the<br />

allocation of exam expenses in general during the somewhat turbulent staff<br />

changes in the last financial year, and it would appear that some of the costs<br />

associated with the Fellowship Examination were allocated to the Primary<br />

Exam codes which distorted the 2009 figures.<br />

The iT requirements of the College have been reviewed recently and a report<br />

will be delivered to Council in November. it is expected that additional funds<br />

will be expended in this area in the coming year. Work has commenced in<br />

this area with a full upgrade to the server and associated hardware being<br />

recently completed. Additional significant expenditure over the next financial<br />

year is not expected.<br />

The College is clearly in a very sound position which will allow future<br />

development without financial impediment.<br />

My thanks to Mandy strates, for her diligent work as Bookkeeper and<br />

to the staff at Headquarters for their support.<br />

Debra O’Brien<br />

it has been a very busy year and i would like to start my report<br />

by acknowledging the wonderful work and support of members<br />

of the Board of Education and its committees.<br />

The BOE and each of the Committees has had a very productive year and<br />

information about the work completed and work underway within each of<br />

the committees is contained in the Board of Education committee reports<br />

that follow my report.<br />

At the strategic Planning Workshop in November last year, it was agreed<br />

that the main focus for <strong>2010</strong>-2011, would be to review the existing training<br />

and assessment program to better identify learning objectives for trainees<br />

as they move through their years of training, and to adjust the methods of<br />

assessment where necessary.The aim is to enhance the current curriculum,<br />

highlight potential gaps, achieve optimal clinical competence and, it is<br />

anticipated, build on the existing syllabus.<br />

To assist in this a Training and Assessment Review Working Group (TARWG)<br />

was established early this year and they are well underway with their task.<br />

The first full report of the TARWG will come to the Board of Education for<br />

consideration at the November Board of Education meeting.<br />

Also at the planning workshop, the recommendations from the AMC<br />

accreditation were again reviewed, with the remaining requirements being<br />

identified for action.The majority of the recommendations had either been<br />

actioned or are currently underway and a number of those remaining form<br />

part of the training program review.<br />

A further initiative was to develop an Educational Advisory Group.<br />

Appointed in February this year, the EAG, whose members have educational<br />

qualifications or a particular interest in education, and or an interest in iT,<br />

has been formed to provide advice to the Board and to the Director of<br />

Education on matters under consideration.The group’s first task has been<br />

to provide advice to the TARWG. The College is fortunate to have many<br />

Fellows with a strong interest in education and they represent a very<br />

valuable resource.<br />

Director of Education<br />

The Director of Education,Alana Killen, has been co-ordinating the Training<br />

and Assessment Review Project and my thanks to her for all she has done<br />

in this area.Amongst other tasks,Alana has involvement in the delivery of<br />

the DEMT Course, held an MCQ writing and review workshop in March<br />

and a further workshop on standard setting for representatives of the exam<br />

committees is planned.<br />

Online Training Assessment<br />

The major addition to the OTA during the past year was the addition of<br />

the Trainee Term Feedback form.This form provides feedback from the<br />

trainee on their term and is completed prior to completion of the iTA.<br />

At the completion of each year, de-identified information will be fed back<br />

to the DEMT and the director of each department, along with de-identified<br />

comparative data in regard to hospital performance.Trainees will also be<br />

providing feedback on their non ED terms.<br />

DEMT Course<br />

The Basic and Advanced DEMT Courses have been<br />

run during the past year.The faculty who deliver<br />

the courses are again to be congratulated on the<br />

consistently high quality of the courses and the<br />

energy and enthusiasm with which the material<br />

is delivered.We all know a little more about our<br />

personality type thanks to Bob Dunn’s Myers-Briggs<br />

sessions.The DEMTs have a very important role in<br />

the College and it is pleasing to be able to provide<br />

some support to them through these initiatives.<br />

Trainee Committee<br />

The Trainee Committee, through the Chair, continues<br />

to provide valuable input to the Board meetings and<br />

to the Committees on which they are represented.<br />

My thanks to shane George for his excellent input<br />

as Chair this year.<br />

it is indeed a vast number of Fellows involved in the delivery of all the elements<br />

that make up the training program.As a College we are so fortunate to have<br />

their collective dedication and skill enabling the provision of such high quality<br />

training, education and assessment.<br />

i would like to pay particular thanks to James Taylor and to ian Rogers.<br />

James retired as Censor for Victoria in November last year, completing three<br />

years as Censor and six years as Deputy Censor. On his retirement<br />

as Censor, James was also the Chair of the Overseas Credentials Committee<br />

– a role he has continued to hold during this year. He continues to hold<br />

positions on the Accreditation Committee and the Adjudicator Panel.<br />

His contribution to the Board over so many years and his unfailing support<br />

for the trainees and their welfare as Censor and Deputy Censor was<br />

exemplary.We are pleased to have his continued input as Chair of the OCC.<br />

We are pleased to welcome Ruth Hew to the position of Censor for Victoria.<br />

ian resigned his position as Censor for Western Australia and Chair of the<br />

Examiners Committee in March and continues as a Peer support Examiner<br />

and a member of the Adjudicator Panel. ian has held many positions over<br />

many years including Censor, Chair of Accreditation, Chair of the Fellowship<br />

Examination Committee and member of the Court of Examiners.To all these<br />

roles he brought his attention to detail, his clear thinking and his enthusiasm<br />

and energy. His contribution to the Board over so many years is much<br />

appreciated and highly valued.<br />

We are pleased to welcome Yuresh Naidoo to the position of Censor for<br />

Western Australia and Di King as Chair of the Examiners Committee.<br />

Finally, many thanks go to Jenny Freeman and the many members of staff<br />

who support the work of the Board and its Committees. Their patience and<br />

diligence make it all possible.The expansion of the headquarters at Jeffcott<br />

street, has meant that we have been able to bring on board additional staff<br />

to help keep pace with the rapid growth of our College. The extraordinary<br />

knowledge of the longstanding staff members is so vital and is now being shared<br />

and passed on to new staff. We are indebted to Jenny’s extremely capable and<br />

dedicated stewardship through this period of great change.<br />

This will be my last report as Censor-in-<br />

Chief as i have decided to step down from<br />

the position at the end of this year.<br />

it has been an honour to serve in this<br />

role for the past three years and to<br />

contribute to the Board of Censors/<br />

Board of Education for 10 years. i feel very<br />

fortunate to have been able to work with<br />

the many Fellows who give so generously<br />

of their time and expertise to the<br />

specialist training activities of the College<br />

and i look forward to continuing my<br />

involvement on the Court of Examiners.<br />

COMMiTTEE REPORTs<br />

Accreditation<br />

Committee<br />

This past year has seen the Accreditation<br />

Committee continue to undertake work<br />

to streamline the accreditation process<br />

for emergency department and special<br />

skills terms.<br />

With respect to the structure of the<br />

committee, we welcomed Robert Day and<br />

David Ward as new members and James<br />

Taylor took on the role of Deputy Chair.<br />

The committee, with the aid of Regional<br />

Censors, Deputy Censors, DEMTs<br />

and Fellows, completed 20 emergency<br />

department inspections and 18 special<br />

skills term inspections in the last<br />

12 months.There are currently 113<br />

emergency departments accredited for<br />

training and 51 special skills terms<br />

available for trainees across Australasia.<br />

The committee would like to thank all<br />

Fellows who have provided their time to<br />

serve in accreditation inspection teams.<br />

From an administrative perspective, the<br />

committee has taken over from Credentials<br />

the role of assessing all Category C<br />

special skills term applications.This will<br />

allow a more consistent approach across<br />

all categories of special skills terms. New<br />

criteria have also been generated with<br />

The creation of Rural Health special skills<br />

terms and the inclusion of CiCM accredited<br />

Basic Training intensive Care units to satisfy<br />

the critical care component of training, have<br />

provided additional training opportunities<br />

for trainees.<br />

College office for her considerable efforts<br />

James Collier, Chair<br />

Credentials Committee<br />

<strong>2010</strong> has been another busy year for the<br />

Credentials Committee.The change in<br />

the <strong>ACEM</strong> Board of Education structure<br />

to form the Credentials Committee has<br />

proved most fruitful. Rather than being<br />

time pressured and only able to consider<br />

urgent matters, the committee has<br />

been able to examine its practices and<br />

undertake some strategic planning.<br />

The main thrust of this planning has been<br />

to develop procedures and policies to<br />

allow the committee to make rulings in<br />

a transparent and consistent fashion.<br />

Two major advances in this area have been<br />

the introduction of clear leave guidelines for<br />

trainees and the introduction of a process<br />

for managing poorly performing trainees.<br />

Previously, trainees who were performing<br />

at below expected levels could be handled<br />

in a variety of manners.A clear, published<br />

guideline now exists to inform all <strong>ACEM</strong><br />

members and ensure trainees are supported<br />

and managed in a consistent fashion.<br />

<strong>2010</strong> has seen consolidation of the OTA<br />

program.The on-line process has been<br />

embraced by the majority of trainees and<br />

DEMTs.This has allowed a streamlining of<br />

credentials processes and contemporaneous<br />

updating of trainee records.<br />

As Chair, i am indebted to liv Cameron<br />

and the rest of the <strong>ACEM</strong> secretariat for<br />

their tireless efforts. liv’s attention to<br />

detail and her vast corporate knowledge<br />

of <strong>ACEM</strong> processes are invaluable to the<br />

smooth running of the committee.<br />

Examiner Committee<br />

in March <strong>2010</strong>, ian Rogers resigned as<br />

Chair but has continued on the Court<br />

of Examiners as a Peer support Examiner.<br />

i would like to thank ian for his valuable<br />

contribution in setting up the Examiner<br />

Committee (EC).<br />

The EC continues to develop into its new<br />

role.Three new members were appointed<br />

in July – Bob Dunn, Bernard Foley and<br />

Jenny Brookes and these members will<br />

participate in our meeting in October.<br />

The annual appointment of new examiners<br />

continues, with orientation occurring at<br />

the first primary exam of the year.<br />

The appointment process for <strong>2010</strong> has not<br />

yet been completed but interest from the<br />

Fellows continues to be strong. Examiners<br />

appointed to the senior Court in <strong>2010</strong><br />

were Peter Aitken, Bernard Foley,<br />

Wayne Hazell, Debbie leach,<br />

lindsay Murray and Kim Yates.<br />

High numbers of examiners continue<br />

to be required for both primary and<br />

fellowship exams and the EC aims to<br />

maintain required examiner numbers,<br />

provide ongoing opportunity for Fellows<br />

to participate as examiners, and maintain<br />

adequate experience and mentoring<br />

capability within the Court.<br />

The EC this year has been focussed on<br />

strengthening the feedback to trainees<br />

with the dissemination of a Feedback<br />

Package to Examiners, Censors and<br />

DEMTs which provides ‘best practice’<br />

practical advice in providing feedback<br />

specific to the examination process for<br />

trainees.This has been very well received.<br />

Work is also underway to develop a<br />

system to improve the consistency and<br />

transparency of the fellowship written<br />

paper marking for the sAQs and the VAQs.<br />

The Fellowship Examination Committee<br />

(FEC) has continued its efforts to<br />

set exit examinations that assess a<br />

broad range of relevant and important<br />

curriculum topics reflecting contemporary<br />

emergency medicine practice. increasingly,<br />

our subcommittees have adopted the<br />

<strong>ACEM</strong> Moodle platform in their work

in preparing the written sections, which<br />

shows promise for facilitating question<br />

development with enhanced security.<br />

Further work has progressed the standard<br />

setting and quality of the MCQ database,<br />

with one result of this being a move to<br />

four distracter items in 2011.<br />

The clinical examinations ran smoothly<br />

in Brisbane and Canberra in 2009/<strong>2010</strong><br />

and were excellently coordinated at all<br />

sites involved. My thanks go to all site<br />

coordinators and their teams.<br />

During <strong>2010</strong>, a much improved process for<br />

feedback to unsuccessful candidates has<br />

been introduced, which we anticipate will<br />

aid these trainees in subsequent attempts.<br />

Finally, i wish to thank the hard working<br />

Chairs and members of each of the FEC<br />

subcommittees, the Court of Examiners<br />

and especially Virginia Cunsolo, the <strong>ACEM</strong><br />

Education Officer who coordinates the<br />

Maintenance of<br />

Professional Standards<br />

Committee (MOPS)<br />

2009/<strong>2010</strong> has been an interesting year<br />

for the MOPs Committee, but <strong>2010</strong>/2011<br />

promises to be much more interesting.<br />

The committee’s activities this year<br />

have focused around the usual role of<br />

management of the MOPs program,<br />

monitoring compliance with the program<br />

and performing audits of Fellows’<br />

MOPs returns.<br />

We are pleased to report that Fellows<br />

are maintaining a compliance rate with the<br />

program that is effectively 96.8% (including<br />

Fellows who have currently been approved<br />

for exemption from the program).<br />

We would like to note quite clearly that<br />

compliance has become a requirement<br />

for ongoing specialist registration with the<br />

new Medical Board of Australia, and that<br />

there are ongoing issues to be addressed

with respect to how the MBA and the<br />

College will interact on CPD issues.<br />

The ACME monitoring subcommittee, in<br />

conjunction with the MOPs Committee, is<br />

supervising a revision of the ACME course,<br />

taking into account feedback from all<br />

Fellows who have undertaken the course to<br />

date. see the subcommittee’s report, below.<br />

Skills Maintenance Subcommittee<br />

The subcommittee has been hard at work<br />

providing the spot Diagnosis and Case<br />

scenario questions that are delivered<br />

on the MOPs site each quarter and are<br />

grateful for Fellows’ feedback on these<br />

questions. in line with the Board of<br />

Education’s recent decision to move to<br />

a four answer format for all MCQs, from<br />

January sD and Cs skills Maintenance will<br />

use a four answer format.<br />

i would like to take this opportunity to<br />

thank Dee Reynolds and Hydie Pallier<br />

for doing most of the work for us,<br />

Alana Killen for her support and input,<br />

Jaycen Cruickshank and stuart Dilley for<br />

their work as Chairs of the subcommittees<br />

ACME Monitoring<br />

Subcommittee<br />

since its inception in 2006, over<br />

200 F<strong>ACEM</strong>s and senior registrars have<br />

participated in an ACME Participants’<br />

Course.Approximately 50 fellows have also<br />

completed the ACME Facilitators’ Course.<br />

Course demand was strong during the<br />

2009/<strong>2010</strong> year and most courses were<br />

fully subscribed. While some of this<br />

demand came from senior registrars,<br />

the majority of participants were<br />

F<strong>ACEM</strong>s. some courses were oversubscribed,<br />

resulting in a need to waitlist<br />

prospective participants or refer<br />

them to alternative centres. interest in<br />

conducting ACME courses in New Zealand<br />

and Western Australia is strong with a<br />

number participants coming from these<br />

regions. Recent ACME courses saw the<br />

participation of senior registrars alongside<br />

Fellows for the first time. All agreed<br />

that this was a positive experience and<br />

feedback from participants continues to<br />

be very good in general terms.<br />

subcommittee members are in the final<br />

stages of reviewing and editing the content<br />

and structure of the course based on<br />

their experiences, participant feedback<br />

and evolving medical knowledge. it is<br />

envisaged that these changes will be<br />

implemented in 2011.<br />

i would like to acknowledge Tim Gray<br />

and other subcommittee members for<br />

their work prior to my involvement with<br />

the subcommittee and for their<br />

continued assistance.<br />

Overseas Credentials<br />

Committee<br />

The Overseas Credentials Committee<br />

(OCC) has been a separate committee<br />

of the BOE for 12 months.The majority<br />

of the processes have been streamlined<br />

to allow decisions and recommendations<br />

to be made online.We aim to have all<br />

applicants interviewed and reviewed<br />

within three months.<br />

To ensure reliability and accountability<br />

of OCC recommendations, our interview<br />

panels regularly include a Fellow from the<br />

applicant’s intended state or country.<br />

in addition ‘cross Tasman’ representatives<br />

have been included to ensure<br />

comparability between Australian<br />

and New Zealand panels.<br />

The OCC has revised the structured<br />

Reference to set the benchmark as<br />

‘substantially equivalant with a newly<br />

qualified Australasian Fellow’. it has also<br />

established guidelines regarding suitability<br />

of specialists in Training (siT) and Area of<br />

Need (AoN) positions. in essence, only<br />

applicants from countries with recognised<br />

training programs, which are members<br />

of the international Federation for<br />

Emergency Medicine (see iFEM website)<br />

will be considered for these positions.<br />

The OCC has continued to expand<br />

its activity and has overseen a record<br />

number of applicants.Twenty-two new<br />

OTs applications have been received to<br />

date and we expect to have processed<br />

approximately thirty by the end of <strong>2010</strong>,<br />

plus siT and AoN applications.<br />

The OCC would like to thank the<br />

members of the interview panels for<br />

their support. in addition we recognize<br />

the efforts of those Fellows who have<br />

provided structured References during<br />

the year.<br />

i would like to congratulate Peter Ritchie<br />

on his appointment as Deputy Chair of<br />

the OCC, and thank Corinne Millane for<br />

her valuable assistance in supporting the<br />

Primary Examination<br />

Committee<br />

<strong>2010</strong> has been a challenging and busy year<br />

for the Primary Examination Committee<br />

(PEC). At the annual workshop in February,<br />

the changes to the syllabus brought about<br />

by the curriculum review in 2009 were<br />

consolidated by extensive revision of the<br />

MCQ examination bank. Members of the<br />

PEC attended an MCQ writing workshop,<br />

run by Alana Killen at the College, aimed<br />

at increasing the level of expertise in<br />

question setting.As a result, we have seen<br />

an increased emphasis on making the basic

sciences accessible to trainees with clinically<br />

framed questions where appropriate.<br />

The growing numbers of candidates<br />

presenting at the primary examination<br />

(over 450 at the latest <strong>2010</strong>/2 exam) has<br />

been recognised as creating unsustainable<br />

logistical difficulties. Plans have been<br />

developed to administer the MCQ<br />

component of the exam as a regionalbased<br />

on-line examination, with selective<br />

invitation to the subsequent Viva for<br />

potential pass candidates.The first<br />

examination of 2011 will see these<br />

changes implemented.<br />

The PEC contributed a submission to<br />

the Training and Assessment Review<br />

Working Group and looks forward to<br />

the outcome of this review.We have<br />

developed closer links with the Fellowship<br />

Examination Committee, streamlining<br />

processes where applicable, and have<br />

participated in the Examiner Committee<br />

and Board of Education. late <strong>2010</strong> will<br />

see PEC involvement in a standard setting<br />

workshop at the College.<br />

On a personal note, i will be relinquishing<br />

the Chair of this committee at the end<br />

of the year and wish to thank all the<br />

committee members, especially the PEC<br />

Executive, for their hard work. i have no<br />

doubt that under the leadership of David<br />

lightfoot the committee will continue to<br />

develop in innovative ways. My final words<br />

must be to thank the College officers<br />

and in particular Dee Reynolds, a pillar of<br />

strength and wisdom in PEC matters, with<br />

whom it has been a pleasure to work over<br />

This has been another dynamic year<br />

for the TRC.The alternative pathway to<br />

satisfy the College’s research requirement<br />

has been successfully established with<br />

approximately 25% of trainees choosing<br />

this option and 27 subjects from<br />

11 universities accredited thus far.<br />

A meeting of adjudicators will be held in<br />

November aimed at further improving<br />

the standard of adjudication of TRPs.<br />

An important part of this will be orienting<br />

adjudicators to the minimum criteria for<br />

a TRP - the standard that all TRPs will be<br />

judged against from January 1 2011.<br />

There has been a 50% increase in the<br />

number of trainees who will complete<br />

the research requirement this year and<br />

as a result of this increased workload,<br />

Kirsty MacDowell has had to hand on<br />

her role as the College officer for the<br />

TRC to concentrate fully on assisting<br />

the Accreditation Committee. On behalf<br />

of the TRC, i would like to thank Kirsty<br />

very much for her enthusiasm, good<br />

humour and hard work for the TRC since<br />

its inception a few years ago.We are<br />

fortunate, however, to be able to welcome<br />

Freda lisgaras as the new College officer<br />

for the TRC and are sure she will also be<br />

a valuable asset to the TRC.<br />

Finally i would like to thank all adjudicators<br />

for their important work in helping the<br />

development of research skills in our<br />

Fellows, and the members of the TRC<br />

for their considerable and intelligent<br />

commitment to the working of the TRC.<br />

Trainee Committee<br />

The past year has once again seen a very<br />

busy Trainee Committee.We have had<br />

increased involvement with all relevant<br />

committees of the College, and have<br />

welcomed the proactive involvement<br />

of trainees in all relevant areas of<br />

College business.<br />

Trainees have embraced the training and<br />

assessment review process with a large<br />

number of submissions to the review from<br />

trainees, as well as the group submission<br />

from the Trainee Committee.As a group<br />

we look forward to the outcomes of this<br />

process and the resultant improvements in<br />

the emergency medicine training program.<br />

The planned 4-hour national access target<br />

continues to worry trainees with the<br />

potential for adverse effects on training<br />

and patient care if not implemented for<br />

the right reasons and with the engagement<br />

of the profession.The Trainee Committee<br />

has been involved in the formulation of the<br />

College position on the targets and looks<br />

forward to further involvement as the<br />

targets are implemented and evaluated.<br />

The Trainee Committee would like to<br />

sincerely thank Virginia Cunsolo for<br />

the support, guidance and time she has<br />

provided the committee as our college<br />

officer over the years.<br />

i will step down from the position of<br />

Trainee Committee Chair in November<br />

this year. i would like to thank all the<br />

members of the committee for their work<br />

and support over my term as chair and<br />

wish Andrew Perry all the best as he takes<br />

on the role.Thanks also to the Board of<br />

Joint Training<br />

Committee for<br />

Paediatric Emergency<br />

Medicine<br />

<strong>2010</strong> has been a year of consolidation<br />

for the JTC-PEM as the <strong>ACEM</strong>-RACP<br />

partnership continues to grow and<br />

strengthen. Paediatric emergency medicine<br />

is now viewed as a serious career choice<br />

among emergency trainees.<br />

Trainee numbers have remained relatively<br />

static throughout <strong>2010</strong>. Currently, there<br />

are approximately 50-60 trainees training<br />

in paediatric EM under the JTC. Of these,<br />

60% are RACP stream and 40% are<br />

<strong>ACEM</strong> stream.<br />

The recent year has seen the <strong>ACEM</strong> and<br />

RACP streams grow closer together.<br />

<strong>ACEM</strong> has adopted a similar policy<br />

to RACP, which will now allow post<br />

F<strong>ACEM</strong> training in paediatric emergency<br />

medicine under the JTC.To align with<br />

their <strong>ACEM</strong> stream counterparts, RACP<br />

stream trainees have also been granted<br />

permission to satisfy their formal project<br />

requirements via an alternative pathway<br />

(ie. university short courses, etc.).<br />

RACP has undertaken a major review<br />

of advanced training for all of its subspecialty<br />

programs with the results<br />

recommending some significant changes<br />

for advanced trainees, including the<br />

introduction of formative assessments.<br />

These changes will be introduced in<br />

2011.This has meant a busy time for the<br />

JTC, as the entire paediatric emergency<br />

medicine curriculum required revision to<br />

accommodate for the proposed changes<br />

in assessment.This process is nearing<br />

completion and it is hoped that the<br />

curriculum will be released shortly.<br />

<strong>2010</strong> has seen the chairing of the<br />

committee revert to the RACP<br />

representatives. Peter Barnett will chair<br />

the committee until April 2012.<br />

Constitution and<br />

Regulations Committee<br />

Following the revision of the<br />

Constitution of the College overseen<br />

by my predecessor,Anthony lawler,<br />

the Constitution and Regulations<br />

Committee has commenced an extensive<br />

review of the College’s Regulations.<br />

several amendments to the Regulations<br />

have already been endorsed by Council at<br />

its most recent meeting in July <strong>2010</strong>, but<br />

there is still a significant amount of work<br />

to be done.<br />

in addition, given the dynamic nature of<br />

the College and the need to regularly<br />

review the Regulations, processes have<br />

been put in place to ensure that we are able<br />

to promptly respond to the needs of the<br />

College and incorporate decisions of the<br />

various committees into the Constitution<br />

and Regulations in a timely fashion.<br />

A needs analysis is being undertaken,<br />

particularly with regard to faculty<br />

regulations, special interest group<br />

regulations, the development of<br />

regulations relating to subcommittee<br />

structure and tenure of the Board<br />

of Education and the review of<br />

communication regulations in light of<br />

the move to electronic communication.<br />

The committee is also currently reviewing<br />

its Terms of Reference and this will be<br />

brought before Council.<br />

Private Practice<br />

Committee<br />

The Private Practice Committee (PPC) has<br />

three main agenda items on its workstream:<br />

Role Delineation for Private EDs<br />

With standards Committee revising<br />

the role delineation document, it was<br />

agreed that there would be separate role<br />

delineation for private EDs, recognising<br />

the fact that private EDs operate under<br />

different organisational conditions.<br />

A draft document has been developed<br />

and is undergoing further refinement, prior<br />

to presenting to the standards Committee<br />

and Council.<br />

Private ED involvement in National<br />

Termset Project<br />

As some F<strong>ACEM</strong>s may be aware, the<br />

Department of Health has developed<br />

a standardised national termset for<br />

presenting problems and diagnosis.The<br />

termset is to be pilot tested in a few<br />

public EDs around the country prior<br />

to being rolled out. it was felt that the<br />

termset should also be piloted in at least<br />

one private ED. However, there was<br />

little enthusiasm from private hospital<br />

CiOs, nor funding to allow this to occur.<br />

The PPC will continue to watch the<br />

development of the termset.<br />

CMBS Items & Descriptors<br />

The recent Medicare schedule has revised<br />

the CMBs descriptors. it appears that this<br />

has occurred without consultation. PPC<br />

intend to review this in more detail and<br />

prepare a response for the HiC.<br />

The issue of inadequate CMBs rebates<br />

is also on the agenda. However, with<br />

the current government there is little<br />

sympathy towards a review of these rates.<br />

Yusuf Nagree, Chair<br />

Public Health<br />

Committee<br />

The Public Health Committee (PHC) has<br />

continued work on policies and statements<br />

relevant to emergency medicine. Policy<br />

statements currently under review and<br />

submitted to standards are: statement on<br />

Early Access to Defibrillation; Policy on<br />

Early Access to Defibrillation for Cardiac<br />

Arrest in Healthcare settings; Policy<br />

on immunisation in EDs;The Policy on<br />

Domestic and Family Violence .A draft<br />

policy on heatwave is in the final stages<br />

of development prior to submission<br />

to standards.<br />

The engagement of speakers and<br />

finalisation of topics for the <strong>ACEM</strong> <strong>Annual</strong><br />

scientific Meeting <strong>2010</strong> - ‘Emergency<br />

Medicine and Public Healthcare Planning<br />

and Policy’ is complete, with excellent<br />

local and international speakers presenting<br />

on public health and disaster medicine.<br />

Contact has been made with the Public<br />

Health Association of Australia (PHAA)<br />

regarding the development of joint policies<br />

and statements. Discussions are ongoing.<br />

Both the PHC and DsC made submissions<br />

to the Training and Assessment Review<br />

Working Group (TARWG) to ensure<br />

that public health and disaster will be an<br />

important part of the revised training<br />

program.A submission was also made<br />

to the iFEM curriculum development<br />

committee to include public health, and<br />

this was supported and adopted.<br />

We are hoping to take a more proactive<br />

role in the future by developing web based<br />

public health bundles, which would contain<br />

information and links for Fellows to access.<br />

The first bundle we are working on is the<br />

heatwave information.<br />

Disaster Subcommittee<br />

The Disaster subcommittee (DsC) has<br />

had an active year with many tasks either<br />

completed or continuing to progress.<br />

The Disaster Working Group’s paper<br />

‘ED surge Capacity: Recommendations of<br />

the Australasian surge strategy Working<br />

Group’ has been published by Academic<br />

Emergency Medicine. There has been a<br />

positive reception to the paper and the<br />

group is looking into the possibility of<br />

development of a surge Action Card.<br />

Contact has been made with iFEM about<br />

the development of linkages between<br />

Disaster Committees in various member<br />

nations to aid future collaboration and<br />

sharing of learning and experience.

A Disaster Medicine session has been<br />

organised at the <strong>2010</strong> <strong>ACEM</strong> <strong>Annual</strong><br />

scientific Meeting in Canberra, with<br />

the generous support of the organising<br />

committee.This promises to be both<br />

interesting and informative.<br />

Members of the subcommittee have been<br />

part of an NHMRC funded review of the<br />

impact of H1N1 on Australian emergency<br />

departments.A number of other research<br />

activities, in early stages of development,<br />

are underway.<br />

The subcommittee has reviewed a<br />

number of College Policies, including:<br />

‘Hazard Material Responses Plan: staff<br />

Participation’;‘Disaster Medical services’<br />

and ‘infectious Disease and Biohazard<br />

Exposure in the ED’.<br />

The subcommittee has also drafted a<br />

College position statement on manmade<br />

climate change and potential for disasters<br />

for consideration by Council, and<br />

supported in principle a proposal from<br />

the Chair of the ANZCA Disaster<br />

Response Executive to explore the<br />

possible establishment of a frozen blood<br />

and blood products capability in Australia.<br />

Bruce Bartley has been appointed as Deputy<br />

Chair of the Disaster subcommittee.<br />

Peter Aitken, Chair<br />

Rural and Regional<br />

Committee<br />

The work of the Rural and Regional<br />

Committee continues and we acknowledge<br />

the contribution of the previous Chair,<br />

liz Mowatt.We have met by teleconference<br />

and will be assembling from far<br />

and wide for a face-to-face meeting at the<br />

College in October.<br />

During the year, the main point of<br />

discussion has been the implementation<br />

by <strong>ACEM</strong> of a Certificate in Emergency<br />

Medicine for non-specialists, which has<br />

the support of the committee.This should<br />

help consolidate <strong>ACEM</strong> as the leading<br />

provider of non-specialist training in<br />

emergency medicine across Australia<br />

and New Zealand.<br />

The issue of encouraging and supporting<br />

trainees to spend time in a rural or regional<br />

emergency department remains a priority<br />

for the committee as a way of ensuring<br />

that a broad spectrum of experience is<br />

available to <strong>ACEM</strong> trainees.The casemix and<br />

experience available in these departments<br />

will be quite different to that available to<br />

trainees in larger metropolitan emergency<br />

departments.Work is underway with the<br />

BOE to look at ways of supporting trainees<br />

spending training time in rural and regional<br />

emergency departments.The various<br />

regional health departments all have<br />

different views on how best to encourage<br />

trainees from all specialities to work in<br />

rural and regional hospitals, and Di Badcock<br />

has been working with the Department of<br />

Health inVictoria in this area.<br />

The new federal program: Rural Health<br />

Continuing Education (RHCE) is now<br />

underway with expressions of interest<br />

submitted to the program.The committee<br />

has participated in the planning for this<br />

program on behalf of the College, and<br />

looks forward to support being allocated<br />

to some of the proposed programs.<br />

The annual <strong>ACEM</strong> Fellow information<br />

sheet now includes the option for<br />

Fellows to identify themselves as a rural<br />

or regional practitioner and it is hoped<br />

this will allow more Fellows to become<br />

involved in the work of the committee.<br />

We will also be looking at other ways of<br />

continuing to promote rural and regional<br />

emergency medicine through the <strong>ACEM</strong><br />

website and the trainee newsletter.<br />

Recruiting and retaining a rural workforce<br />

remains a big challenge which has been<br />

made more challenging following the<br />

introduction of the new APHRA process<br />

mid year! Hopefully, the system will ‘bed<br />

down’ quickly and we can all realise the<br />

benefits of a national registration system.<br />

Scientific Committee<br />

Perhaps two of the committee’s most<br />

significant achievements this year have<br />

related to College endorsement of<br />

externally generated documents. The<br />

National stroke Foundation Guidelines<br />

<strong>2010</strong> and ANZCA’s Procedural sedation<br />

Policy are both now co-badged with<br />

<strong>ACEM</strong>. This came about after extensive<br />

consultation and discussion and i feel<br />

the views of F<strong>ACEM</strong>s are now well<br />

represented in those areas that relate to<br />

our practice and had generated, at times,<br />

significant controversy.<br />

A large, ongoing body of work relates<br />

to oversight of the College’s scientific<br />

Meetings, representation on external<br />

bodies and review of a wide range of<br />

documents.<br />

The Committee is also undertaking a<br />

review of the guidelines for the conduct<br />

of scientific Meetings. This is partly a<br />

routine review of an ageing document,<br />

but also aims to address recent points of<br />

discussion. in particular, the relationships<br />

with industry, keynote speakers, meeting<br />

profits and conference organisers are<br />

being addressed to reflect the current<br />

views of Fellows.<br />

Once again, i would like to thank the<br />

Committee members for their invaluable<br />

time and contribution to another very<br />

Emergency Medicine Clinical<br />

Trials Group Subcommittee<br />

Paul Middleton took over the chair of<br />

the EMCTG at the April <strong>2010</strong> meeting,<br />

following ratification at the March <strong>2010</strong><br />

Council meeting.<br />

Members had previously emphasised<br />

that the original purpose behind the<br />

establishment of the EMCTG, as a<br />

subcommittee of the scientific Committee,<br />

was to set up collaborative clinical<br />

research in emergency medicine under<br />

the auspices of the College and to<br />

promote research in emergency medicine.<br />

The EMCTG membership comprises<br />

researchers with a wide breadth of<br />

experience in the design, execution and<br />

processes involved in clinical trial research,<br />

and as such can provide expert input into<br />

the development of other people’s ideas,<br />

and can provide a process for seeking<br />

College endorsement for clinical trials.<br />

The EMCTG has agreed that the priorities<br />

of the subcommittee should therefore be<br />

education, promotion and organisation.<br />

The focus will be to foster research and<br />

provide the training necessary to set<br />

up research projects, particularly the<br />

development of large, collaborative projects.<br />

The EMCTG terms of reference do not<br />

require all projects to be conducted by<br />

subcommittee members, therefore it is<br />

planned to encourage research and to<br />

train others to conduct projects. it is also<br />

planned to seek allocated time on the<br />

agenda of each scientific meeting to provide<br />

seminars for College members on clinical<br />

trial methodologies.<br />

The EMCTG is currently considering a<br />

multi-centre intervention trial targeting<br />

pain management.<br />

Paul Middleton, Chair<br />

Standards Committee<br />

The standards Committee has been active<br />

throughout <strong>2010</strong>. Following on from<br />

the ground work in 2009, the committee<br />

has had an extensive workload with<br />

reviews of existing policies, statements<br />

and guidelines according to the program<br />

developed in 2009. The year has seen a<br />

number of personnel changes at College<br />

headquarters, which has slowed some<br />

of the workload completion. Now that<br />

this situation has stabilised, a number of<br />

documents have been reviewed, updated<br />

and finalised.A complete list of the<br />

updated policies, guidelines and statements<br />

can be found on the College website.<br />

Committee members can access agendas<br />

and minutes on the website.<br />

New policies, statements and guidelines<br />

in development address areas such as the<br />

roles of Nurse Practitioners in emergency<br />

departments and implications of proposed<br />

new performance indicators following<br />

the new COAG agreement.A complete<br />

revision and restructure of the Emergency<br />

Departments Role Delineation statement<br />

has been undertaken.<br />

To a large extent, the committee and<br />

subcommittees have achieved the program<br />

set in 2009. subcommittees have been<br />

working tirelessly to complete their<br />

workload for this year. Please see the<br />

individual subcommittee reports for<br />

progress during <strong>2010</strong>.<br />

Policies, guidelines and statements are<br />

reviewed on a five year cycle. in addition<br />

to this, it is intended to review the full list<br />

of documents next year with regard to<br />

the numbering, identification and<br />

tracking systems to further enhance<br />

document control.<br />

As Chair, i would like to thank the<br />

members of the standards Committee<br />

for some outstanding work this year<br />

and their contributions to the workload<br />

for <strong>2010</strong>. Additionally, the Chairs and<br />

members of the subcommittees are to be<br />

congratulated on their achievements and<br />

work throughout the year. Their work<br />

and contributions are invaluable to the<br />

success of the standards Committee.<br />

Emergency Department<br />

Ultrasound Subcommittee<br />

The ED ultrasound subcommittee<br />

continues to work to improve ultrasound<br />

education and credentialing.The<br />

international conference on point of<br />

care ultrasound,WiNFOCus 2009, was<br />

held in sydney in september as a satellite<br />

conference to the ‘World Federation<br />

for ultrasound in Medicine and Biology’<br />

international meeting. Most of the<br />

members of the organising committee are<br />

members of <strong>ACEM</strong>, and it was pleasing to<br />

see the demonstration of the high level of<br />

expertise in emergency ultrasound that<br />

exists within the College standing equal<br />

to international experts.The residual<br />

profit from the conference will be used to<br />

create the <strong>ACEM</strong> WiNFOCus ultrasound<br />

Bursary, to support emergency ultrasound<br />

research and development.Additionally,<br />

in recognition of this success and the<br />

growing role of ultrasound for non-imaging<br />

specialists, the Australasian society for<br />

ultrasound in Medicine is considering<br />

hosting regular sections or satellite<br />

meetings for clinicians during its annual<br />

scientific meeting.<br />

Work has continued with Alana Killen,<br />

the College Director of Education, to<br />

create a web based education resource<br />

for emergency ultrasound. special thanks<br />

are extended to Justin Bowra, who has<br />

contributed much of this material.<br />

A survey of the current state of<br />

emergency ultrasound in Australasia has<br />

recently been published by Nagaraj, Chu<br />

and Dinh in Emergency Medicine Australasia,

with a covering editorial. This confirms<br />

the growing use of emergency ultrasound<br />

but also reveals the variable standards of<br />

credentialing that are applied within EDs,<br />

often less than College recommendations.<br />

The subcommittee will continue to work<br />

on this area, to ensure that the benefits of<br />

ultrasound are realised while minimising<br />

the risks inherent in the utilisation of a<br />

new skill by those who are inexperienced.<br />

The growing number of accredited<br />

courses and experienced and credentialed<br />

practitioners is positive, and should allow<br />

those who are still gaining experience<br />

to be properly supervised and assessed<br />

to gain accreditation in keeping with the<br />

College standards.<br />

in response to increasing requests,<br />

the subcommittee has begun to look<br />

at standards for bedside echo in the<br />

periarrest/arrest setting.This is being<br />

undertaken in concert with other relevant<br />

colleges and societies, with the aim of<br />

creating a uniform standard for Australasia.<br />

i would like to thank all of the members<br />

of the subcommittee and our support<br />

staff for their time and efforts during the<br />

past year in assisting this still new area of<br />

practice to continue to develop.<br />

Hospital Overcrowding<br />

Subcommittee<br />

This was the inaugural year for this<br />

subcommittee of the standards Committee.<br />

it was formed from the access block<br />

special interest group due to the clear<br />

need for a subcommittee to advocate on<br />

issues around emergency department<br />

overcrowding. During the first year of its<br />

existence it underwent a name change<br />

to ‘Hospital Overcrowding’ due to the<br />

need to clarify the issue and also because<br />

‘overcrowding’ identifies the hospital<br />

system-wide problem - not just the<br />

ED problem.The subcommittee has been<br />

involved in replying to a number of articles<br />

in medical journals on overcrowding,<br />

as well as providing vital input into the<br />

<strong>ACEM</strong> and AMA position statements on<br />

the 4 hour rule.The subcommittee seeks<br />

to achieve a more widespread consensus<br />

on <strong>ACEM</strong> public pronouncements and<br />

advocacy regarding overcrowding and<br />

potential solutions. it is keen to hear from<br />

Fellows and to be a clearing house for<br />

information on research, solutions and<br />

issues raised by this, the most pressing issue<br />

facing our College. Clarity in statements<br />

in the media on causes of and myths<br />

regarding overcrowding and related issues,<br />

and the implementation of time based<br />

rules are areas of ongoing import for the<br />

subcommittee.The need for a co-ordinated<br />

advocacy on the Australian 4 hour rule and<br />

the New Zealand 6 hour rule will be vital<br />

in the coming year as results from these<br />

experiments in system change become<br />

available. Exciting times ahead!<br />

Pre-Hospital Care<br />

Subcommittee<br />

<strong>2010</strong> has been yet another year where<br />

we have seen <strong>ACEM</strong> Fellows and trainees<br />

actively engaged in pre-hospital care<br />

with excellent outcomes. in concert with<br />

our colleagues in the other critical care<br />

specialties, this expertise continues to<br />

shape and lead this area of practice.<br />

The past year has seen important<br />

advancements in the area of governance.<br />

This has included the revision of the<br />

document ‘Minimum standards for<br />

intrahospital Transport of Critically<br />

ill Patients’, a process coordinated by<br />

ANZCA, as well as the <strong>ACEM</strong> initiated<br />

revision of the key document ‘Minimum<br />

standards for interhospital Transport of<br />

Critically ill Patients’. By the time this<br />

report goes to print, we should also<br />

have the final draft of suggested KPis<br />

for prehospital activities.This will be the<br />

first time KPis have been drafted on an<br />

Australasian basis.<br />

With the ongoing pressures occurring<br />

within EDs across Australasia at the<br />

moment, the year ahead will be an<br />

interesting one.The pressure to move<br />

patients out of ED within specified time<br />

frames has flow on effects, both within the<br />

hospital and also to pre-hospital providers.<br />

We will watch with interest and become<br />

actively engaged in those areas where we<br />

can make a positive contribution.<br />

My thanks to all current and past<br />

members, who work tirelessly in a field<br />

that is mostly out of the limelight but is<br />

none the less a vital facet of our specialty.<br />

Quality Management<br />

Subcommittee<br />

There have been a number of initiatives<br />

undertaken by the subcommittee during <strong>2010</strong>.<br />

Yusuf Nagree, Carmel Crock and<br />

Kim Hansen have been part of a joint<br />

working party with the Royal Australian<br />

and New Zealand College of Radiologists<br />

(RANZCR) which has recently completed<br />

guidelines for imaging in emergency<br />

medicine.These guidelines will be available<br />

soon on the College website and i am sure<br />

that you will join with me in congratulating<br />

the members of both Colleges for<br />

developing such comprehensive and<br />

sensible guidelines for the imaging of<br />

emergency department patients.<br />

spain and myself have also been involved<br />

in discussions with Royal College of<br />

Pathologists of Australasia (RCPA) in<br />

developing appropriate guidelines for<br />

the ordering of pathology tests in EDs.<br />

early in October and are hopeful that we<br />

will be able to circulate some guidelines<br />

for appropriate test ordering in the next<br />

few months.<br />

Carmel Crock has also been involved<br />

with the Australian Patient safety<br />

Foundation (APsF) in an initiative to<br />

develop an Australasian incident reporting<br />

system for emergency medicine.To date<br />

we have been unable to secure funding for<br />

this proposal, but remain hopeful that the<br />

federal government will be convinced<br />

this is a worthwhile project to support.<br />

Fellows may be interested to know<br />

that the APsF is sponsor of ‘The<br />

Australasian Conference on Error in<br />

Medical imaging: Making imaging safer’,<br />

11-12 November <strong>2010</strong> at the swiss<br />

Grand Bondi, sydney. Carmel Crock is the<br />

College representative on the organising<br />

committee and this conference will have<br />

content which will be of interest to<br />

College Fellows and trainees.<br />

The subcommittee is also currently<br />

working on the development of<br />

Australasian Emergency Department<br />

Key Performance indicators which should<br />

be available in draft form for Fellows to<br />

comment on in early 2011.<br />

A policy on ‘Handover in the ED’ has<br />

been recently finalised and will be sent to<br />

the standards Committee for ratification<br />

before going to Council for final approval.<br />

The subcommittee has also been asked to<br />

comment on a number of proposals from<br />

the Australian Committee on safety and<br />

Quality in Healthcare (ACsQHC) as well as<br />

the latest Australian Council on Healthcare<br />

standards (ACHs) Clinical indicators.<br />

As i will be stepping down as Chair of the<br />

subcommittee at the end of <strong>2010</strong>, i would<br />

like to take this opportunity to thank all<br />

the members of the subcommittee as well<br />

as the College administrative officers for<br />

their hard work and cooperation over the<br />

last two years. Carmel Crock has kindly<br />

agreed to take over as Chair and i am sure<br />

she will do an excellent job in this role.<br />

Trauma Subcommittee<br />

The Trauma subcommittee wishes to<br />

thank Andrew Pearce for his valuable<br />

contribution as the previous Chair.<br />

Andrew continues to provide guidance<br />

through his ongoing involvement in<br />

the subcommittee.<br />

<strong>ACEM</strong> and subcommittee members are<br />

working towards forging stronger links<br />

with the Royal Australasian College of<br />

surgeons (RACs) to enhance the care of<br />

trauma related presentations, which we<br />

hope will also improve communication and<br />

teamwork for surgical patients in general.<br />

Tony Joseph, a past Chair of the Trauma<br />

subcommittee is the <strong>ACEM</strong> representative<br />

on the RACs Trauma Committee. Tony<br />

will also coordinate subcommittee input<br />

to development of the Rural Health<br />

Continuing Education (RHCE) Rural<br />

Trauma project. Through this project,<br />

RACs proposes to develop an increased<br />

range and variety of trauma workshops for<br />

rural surgeons and emergency<br />

medicine specialists.<br />

Other subcommittee members are<br />

participating in the trauma verification<br />

process as part of the RACs Trauma<br />

Verification Group.This comprehensive<br />

review process encompasses all the<br />

contingencies involved in trauma care,<br />

including significant resources which are<br />

not generally readily available, and assists<br />

the institution involved to ‘benchmark’ its<br />

trauma services. This process may also<br />

provide a benchmark for other ancillary<br />

services. Andrew Pearce, Gordian Fulde<br />

and Colin Myers generously give their<br />

time to this group.<br />

A ‘Trauma Tool’ has been developed by<br />

the subcommittee, which will identify<br />

and act as a guide to key areas in trauma<br />

management. intended as an aid to<br />

advanced trainees, the document is<br />

currently being reviewed in line with<br />

the College curriculum.<br />

We encourage participation in trauma<br />

team simulation training: multi-disciplinary,<br />

scenario based, often videotaped, followed<br />

by a debrief. The training is targeted to<br />

the level of expertise available locally<br />

and focuses on communication, decision<br />

making and working as team.The ultimate<br />

aim is to make the training affordable<br />

and relevant and to develop a bank of<br />

scenarios/multiple choice questions for<br />

Fellows and trainees to use.<br />

As the provision of trauma care is a key<br />

role and function of our specialty,<br />

we would like to encourage all Fellows<br />

to work with us in providing leadership<br />

and excellence in care. We value your<br />

feedback and ideas and look forward<br />

to hearing from you.<br />

Workforce Subcommittee<br />

The subcommittee has continued to<br />

monitor the response to the ‘Guideline<br />

for Constructing an Emergency Medicine<br />

Medical Workforce’.A literature review<br />

has been carried out to identify new<br />

evidence to add to the document.<br />

Additionally, the longitudinal workforce<br />

study has been progressing and a<br />

workforce survey is nearing completion.<br />

Results of this survey will provide an<br />

understanding of the present state and<br />

construction of the ED workforce across<br />

Australasia. it is hoped that elements of<br />

the survey will form the basis of a future<br />

yearly survey to track the emergency<br />

medicine workforce.<br />

Australian Capital<br />

Territory<br />

it has been a fairly busy year for the<br />

ACT Faculty. in October, the fellowship<br />

examination was held for the first time in<br />

Canberra. it worked well and thanks go<br />

to the whole faculty for helping out, and<br />

especially to sam scanlan and lisa Bell<br />

who coordinated the two sites.<br />

Most of the past year has been spent<br />

preparing to host the <strong>Annual</strong> scientific<br />

Meeting in November <strong>2010</strong>.Again, this has<br />

been an ‘all-in’ effort by faculty members and<br />

we are looking forward to a great meeting.<br />

The faculty’s training program also<br />

continues to perform well, with success in<br />

both primary and fellowship examinations<br />

during the year.We are also beginning to<br />

see some growth in interest amongst JMOs<br />

in pursuing emergency medicine as a career,<br />

which we hope will translate into both new<br />

Fellows and trainees in the future.<br />

Both hospitals have seen some major<br />

issues. For Calvary, it has been the<br />

proposal to sell the public hospital to the<br />

ACT Government.Whilst this initially<br />

appeared to have widespread support,<br />

the offer was eventually withdrawn.<br />

Negotiations have continued throughout<br />

the year, though without a resolution.<br />

We hope a resolution will be reached<br />

soon.Work has begun on the<br />

redevelopment of the combined iCu/<br />

CCu at Calvary, and major redevelopment<br />

works are commencing at TCH, preparing<br />

for the new Women’s and Children’s and<br />

Mental Health buildings.<br />

issues for TCH have included a recent<br />

rapid growth in attendances after a period<br />

of relative stability, and the creation of two<br />

new units: the Mental Health Assessment<br />

unit at the back of the emergency<br />

department, and the opening of the nurserun<br />

Walk-in Centre.As expected, the latter<br />

has had no impact on ED attendances.<br />

Thanks again to all the faculty members<br />

for their work and ongoing support of<br />

the activities of the College.<br />

Andrew Singer,<br />

Faculty Chair and Councillor<br />

New South Wales<br />

There has been continued activity in<br />

relation to the Garling Recommendations.<br />

The NsW Department of Health<br />

states that they have met the<br />

timelines of approximately 60% of the<br />

recommendations, although this has not<br />

included any of the recommendations<br />

that will have significant impact and<br />

therefore be more difficult to implement.<br />

Currently, focus is on ‘recognition of<br />

the deteriorating patient’. standardised<br />

observation charts, including colour coding<br />

of abnormal vital signs as triggers for<br />

escalating assessment and management<br />

(‘between the flags’) are being developed,<br />

along with a range of other projects.<br />

Meetings between the NsW Director-<br />

General of Health and the committee of<br />

NsW Faculty Chairs (<strong>ACEM</strong> and other<br />

colleges) have continued. The Faculty<br />

Chairs remain keen to provide clinical<br />

feedback and input into the NsW<br />

implementation plan for the National<br />

Health Reform agenda.<br />

Considerable concerns have been raised<br />

by Fellows at the potential nationwide<br />

implementation of a four hour rule for<br />

emergency departments.The Faculty<br />

Chairs contributed to extensive<br />

discussions with College Executive about<br />

this issue during the formation of the<br />

College Position statement on national<br />

time based emergency access targets.<br />

Despite a number of communications with<br />

the Department of Health about influenza,<br />

there has been little progress on this issue.<br />

The NsW DOH plan for managing H1N1<br />

influenza, released in June, was a high<br />

level public health document with little<br />

operational information.The plan gave no<br />

indication that EDs would not be used as<br />

the point of assessment of these patients.<br />

The NsW Faculty Chair has again raised<br />

with the NsW Chief Health Officer, the<br />

need for a definitive plan to prevent these<br />

patients being assessed in EDs, which may<br />

potentially compromise ED care.<br />

Overall, there have been no major adverse<br />

effects from the introduction of the NsW<br />

Trauma Plan. information from both<br />

RPAH and Westmead indicates that the<br />

increased urgent trauma surgical load,<br />

and subsequent surgical and iCu bed<br />

utilisation, is impacting on semi-elective<br />

and elective surgical cases. Discussion<br />

continues regarding the impact of this plan,<br />

particularly on Western sydney.<br />

shalini Arunanthy continues to represent<br />

the faculty on the institute of Medical<br />

Education and Training Emergency<br />

Medicine Training implementation Group<br />

(EMTiG), which is being co-chaired<br />

by Jon Hayman (F<strong>ACEM</strong>). standard<br />

selection criteria for EM trainee positions<br />

and unaccredited positions has been<br />

distributed.The implementation of<br />

network based recruitment and ‘lifetime<br />

of training’ contracts has been deferred<br />

to next year.<br />

The tragic and potentially preventable<br />

death of Vanessa Anderson in a NsW<br />

hospital, after the teenager was struck<br />

on the head with a golf ball, prompted<br />

Jillian skinner, the shadow Minister for<br />

Health to introduce a Private Member’s<br />

Bill to NsW Parliament in June <strong>2010</strong>.<br />

The Bill sought to mandate paediatric<br />

review of all children (

New Zealand<br />

The year has been a busy one for the<br />

NZ Faculty Board. There have been three<br />

teleconference meetings, held 2-3 weeks<br />

following the College Council and one<br />

face-to-face meeting held in conjunction<br />

with the Regional ED Meeting in October<br />

last year.<br />

Currently New Zealand has 140 F<strong>ACEM</strong>s<br />

and 180 trainees.<br />

Faculty Courses and Meetings<br />

A ninth pre Fellowship exam course will<br />

be held in Auckland in early November,<br />

convened again by louise Finnel.<br />

As previously reported, these workshops<br />

continue to be very popular and successful.<br />

This year’s course is again fully subscribed,<br />

with a total of 24 registrations and a<br />

number on the waiting list.<br />

A very successful and enjoyable Regional<br />

ED meeting, hosted by Central Region<br />

Emergency Care Coordinating Team<br />

(ECCT) was held in Taupo in October<br />

2009, which was attended by the Minister<br />

of Health. Another meeting is planned for<br />

October <strong>2010</strong>, with the theme ‘EDs, their<br />

place in the community and the 6 hour<br />

target’. The conference is aimed at<br />

providing a forum where we can explore<br />

aspects of where we fit in the bigger<br />

network of acute care providers regionally<br />

and nationally.<br />

New Zealand has been asked to host<br />

the 2011 Winter symposium which will<br />

be held in Queenstown and organised by<br />

Fellows in Christchurch.<br />

New Zealand ‘Shorter Stays in ED’<br />

Ministry Target<br />

This year has seen twelve months of the<br />

6 hour ED length of stay target.The New<br />

Zealand emergency medicine community<br />

has been generally supportive of this<br />

initiative which has Mike Ardagh as the<br />

national champion. if shorter stays in EDs<br />

is a measure of success of the project,<br />

the initiative has been a resounding<br />

success, as the national average has risen<br />

to 87% compliance – with patients being<br />

admitted, discharged or transferred<br />

from the ED within six hours of arrival.<br />

The compliance rate for the final<br />

quarter ranges from 74% to 100% and<br />

each District Health Board (DHB) has<br />

been visited by the Ministry monitoring<br />

group to encourage improvement, share<br />

initiatives and check on reliability of<br />

data.The emphasis of the New Zealand<br />

initiative is on quality of care rather<br />

than speed of care, so much of the<br />

improvement has been in bed management<br />

and inpatient responsiveness to admission.<br />

Council of Medical Colleges<br />

The faculty is a member of the Council<br />

of Medical Colleges (CMC) and meetings,<br />

held every three months, are attended by<br />

the faculty chair or representative. in the<br />

past, CMC has been a useful networking<br />

body but not particularly effective in<br />

influencing change. However, CMC is<br />

now looking to position itself in a more<br />

prominent role, which could realise the<br />

true potential of the representatives of all<br />

the medical colleges to influence change.<br />

Emergency Department IT Systems –<br />

National Health IT Board<br />

The Ministerial Review Group last year<br />

called for ‘a higher level of strategic<br />

leadership and ownership…from clinicians,<br />

managers and governors of iT projects’.<br />

it stated that ‘There is a tendency to<br />

look for iT solutions without getting<br />

the fundamentals right, like looking at<br />

standardising clinical process and assessing<br />

readiness for changes in behaviour<br />

necessary to make the systems work<br />

effectively’. A Clinical leadership Group<br />

(ClG) was established late last year<br />

in recognition of the fact that clinical<br />

leadership was required at a national level<br />

for ‘transfer of care’.The group was made<br />

up of nominations from colleges<br />

and professional bodies including emergency<br />

medicine. With the creation of the National<br />

Health Board, and specifically the National<br />

Health iT Board, the role of the ClG was<br />

extended to provide clinical leadership input<br />

to the National Health iT Plan. The work of<br />

the emergency medicine iT advisory group<br />

has been to develop national standards<br />

for iT support of EM practice. The iT<br />

Board plans to curtail fragmentation of iT<br />

development within DHBs and take the<br />

initiative with regard to standardisation of<br />

practice and iT support. The problem has<br />

been that, over the last 15-20, years each<br />

DHB has purchased their own systems and<br />

each has different platforms and interfacing<br />

problems. The National Health iT Board<br />

have engaged with the Faculty to establish a<br />

common view of iT solutions for emergency<br />

departments and Peter Freeman has been<br />

approached to be the project lead for<br />

this work.<br />

Access Block Point Prevalence Study<br />

This study was undertaken by the<br />

emergency research teams at Auckland<br />

City Hospital and North shore Hospital<br />

by Peter Jones and sarah Olsen, with<br />

the support of the NZ Faculty. it was<br />

identical to the studies carried out in<br />

Australia by Drew Richardson on behalf<br />

of <strong>ACEM</strong>. The project was approved in<br />

New Zealand by the Multi-Region Ethics<br />

Committee as observational research.<br />

The information gathered will assist in<br />

determining the extent of access block<br />

and ED overcrowding in New Zealand<br />

and the ability of EDs to cope with a<br />

major incident. Data was collected at a<br />

single ‘point in time’ from all New Zealand<br />

EDs. The survey was a great success,<br />

with a 100% return rate. The Faculty had<br />

been advised that the emergency research<br />

team at Auckland City Hospital had been<br />

successful in the Health Research Council’s<br />

funding round for their research proposal<br />

‘implementing Performance improvement;

the six hour time target policy’. This is<br />

the first time the Health Research Council<br />

has funded emergency department based<br />

research in New Zealand and will provide<br />

an opportunity to help improve the quality<br />

of care provided in emergency departments.<br />

i would like to thank all members of the<br />

Faculty Board for their ongoing support,<br />

enthusiasm and contribution during the<br />

year and wish John Bonning every success<br />

as the new Chair of Faculty.<br />

i would particularly like to thank<br />

Denise Tringham, who is the NZ <strong>ACEM</strong><br />

administrator, for her untiring support and<br />

contribution to the activities of the faculty.<br />

Peter Freeman, Chair<br />

Northern Territory<br />

The NT Faculty met informally (not<br />

minuted) four times in <strong>2010</strong> with<br />

representatives from Darwin and Alice<br />

springs. Most discussions are at present<br />

at an operational level, but the key<br />

strategic discussions, actions and outcomes<br />

have focused on the following areas:<br />

NT Medical Officers Pay Agreement<br />

Conclusion of this agreement is presently<br />

running behind all other states and<br />

territories except Tasmania.This is an<br />

important factor limiting recruitment and<br />

retention of specialists and hampering<br />

development of EM in the NT. F<strong>ACEM</strong> led<br />

negotiation on specialist pay is now close<br />

to agreement.<br />

Alcohol legislation in the NT<br />

The NT has the highest per capita rate of<br />

alcohol consumption in Australia, highest<br />

alcohol related health costs per capita<br />

(both trauma and chronic illness) and<br />

highest per capita rate of alcohol related<br />

Police costs. Cross college lobbying by NT<br />

branches of <strong>ACEM</strong>, RACP, RACs,AFPHM<br />

to the Minister of Health, in concert with<br />

the Health Ministers Advisory Council<br />

(which has F<strong>ACEM</strong> representation) and<br />

the NT News who have spotlighted<br />

the issue, has resulted in new alcohol<br />

legislation for drinkers causing harm and<br />

increased awareness of the next potential<br />

legislative steps amongst policy makers.<br />

Inaccurate reporting of NT ED data<br />

to the Commonwealth<br />

A letter to Minister Roxon and CE DHF,<br />

has resulted in an apology from Minister<br />

Roxon and re-enforcement of DHF policy<br />

regarding release of data.<br />

We will work to formal and minuted<br />

meetings in 2011<br />

Didier Palmer, Chair<br />

Queensland<br />

Queensland Faculty continues to be very<br />

energetic in providing a forum for Fellows<br />

to communicate and discuss the issues of<br />

the moment.The monthly faculty meetings<br />

are accessible by both teleconference<br />

and videoconference and despite the<br />

challenges of finding time, provide a great<br />

opportunity for Fellows to be involved.<br />

The key issues of the moment in Queensland:<br />

The Generalist in Emergency Medicine<br />

program<br />

Being administered by the Australian<br />

College for Rural and Remote Medicine<br />

(ACRRM), the program has progressed<br />

to the stage of reviewing credentials for<br />

accreditation of the first cohort of GEM<br />

doctors in Queensland. it is fair to say<br />

there are mixed opinions from F<strong>ACEM</strong>s<br />

around the state regarding this. At this<br />

point in time, the faculty position is that<br />

<strong>ACEM</strong> provides an excellent environment<br />

for training and credentialing of emergency<br />

medicine specialists capable of providing<br />

specialist services in emergency<br />

departments, regardless of size or location.<br />

Whilst <strong>ACEM</strong> has not formally endorsed<br />

the GEM curriculum or program, F<strong>ACEM</strong>s<br />

continue to be involved at various levels<br />

in a collaborative fashion.The Qld Faculty<br />

strongly supports the need to upskill,<br />

standardise and recognise the rural and<br />

regional sMO workforce, but it does not<br />

support the need for alternative pathways<br />

for specialist training.<br />

Qld DEMT group formalised<br />

James Collier and Philip Richardson<br />

have been very busy strengthening<br />

communication and coordination of the<br />

DEMTs around the state. Formal meetings<br />

are scheduled to address training issues at<br />

a local level and the faculty is working with<br />

the Queensland Health Medical Workforce<br />

Advisory unit to promote a greater<br />

understanding of training opportunities<br />

and deficits around the state.<br />

Autumn Symposium<br />

Held in Brisbane on May 21-22, the<br />

symposium was attended by over<br />

110 Fellows andTrainees. An excellent<br />

program was very well received and the<br />

quality of 4.10 presentations was extremely<br />

satisfying.The Deputy Premier and Health<br />

Minister attended to present QEMRF<br />

research grants to Queensland F<strong>ACEM</strong>s<br />

and trainees and the symposium was a great<br />

opportunity to showcase the cohesive and<br />

collaborative network of the faculty.<br />

QHealth collaboration<br />

The faculty continues to have a strong<br />

positive contribution to Queensland<br />

Health initiatives and is an active member<br />

of the state ED Network, the Queensland<br />

Council of Medical specialist Colleges<br />

and the National Health Reform External<br />

stakeholders Panel.This provides the<br />

faculty with a fantastic opportunity to<br />

promote the <strong>ACEM</strong> agenda at multiple<br />

levels within Queensland Health.<br />

Queensland Emergency Medicine<br />

Research Foundation<br />

QEMRF continues to progress very<br />

strongly with over $4 million in grant<br />

applications for the upcoming round.<br />

The quality of grant applications has<br />

exponentially increased, indicating the

obvious growth in research capability in<br />

Queensland.The foundation handed out<br />

its first capacity building grants to fund<br />

research personnel within departments.<br />

special thanks to Chris May and Niall<br />

small, who represent Queensland<br />

enthusiastically at Council.<br />

David Rosengren, Chair<br />

South Australia<br />

<strong>2010</strong> began welcoming Judy Cornish to<br />

the position of <strong>ACEM</strong> Councillor for sA.<br />

simon Chu was also welcomed as the<br />

Deputy Censor. in addition to simon,<br />

James smyth also joined the Faculty Board,<br />

which now includes representation from<br />

all <strong>ACEM</strong> accredited sites within the<br />

Adelaide Health service. Di King, Bob<br />

Dunn,Tony Eliseo and Hendrika Meyer<br />

complete the local board.We continue<br />

to have bi monthly meetings.Andrew<br />

Perry – our state trainee representative –<br />

also attends these meetings and provides<br />

feedback to and from the trainee group.<br />

His attendance has provided a further<br />

means of communication between groups,<br />

which is always welcome and i would like<br />

to thank Andrew for the hard work and<br />

enthusiasm that he puts into his role.<br />

The ‘4 hour rule’, not surprisingly,<br />

dominated many faculty meetings<br />

during the year. As with our colleagues<br />

across the nation, we are keeping a close<br />

watch on developments both locally and<br />

federally. At a state level, we are engaging<br />

with our Minister of Health to discuss<br />

implementation issues and the concerns<br />

that faculty members have. it is hoped<br />

that this dialogue will continue, to ensure<br />

the best outcomes for all involved, but<br />

particularly for our patients and the quality<br />

of care provided to them.The faculty is also<br />

represented on the state emergency access<br />

taskforce.This is in addition to the existing<br />

DEM representation within this group.<br />

Representatives of the sA faculties of<br />

<strong>ACEM</strong>, CENA and ACAP combined to<br />

host EMsA <strong>2010</strong> in Adelaide.<br />

This combined emergency health<br />

professional conference was well<br />

supported by all disciplines over its two<br />

day duration. in keeping with a trend<br />

seen at <strong>ACEM</strong> meetings recently, a variety<br />

of practical workshops were run with<br />

trainees and specialist MOPs in mind.<br />

Feedback from these was generally positive<br />

and the inclusion of the workshops was<br />

welcomed by the conference delegates.<br />

i would like to thank all those who<br />

participated in some way to the success of<br />

the event. i would particularly like to thank<br />

our colleagues from interstate who came<br />

to present at the conference and to assess<br />

the 4.10 presentations.<br />

Faculty again hosted a booth at the ‘Big<br />

Careers in Medicine’ expo held at the<br />

Adelaide Entertainment Centre.This is an all<br />

day expo held by the state Department of<br />

Health showcasing careers in all specialties.<br />

Junior medical officers and students are<br />

encouraged to attend.Although the <strong>ACEM</strong><br />

booth always attracts visitors, we felt that<br />

we had a lot more interest in emergency<br />

medicine careers than in previous years.<br />

it will be interesting to see whether this<br />

translates into an increase in job applications<br />

and College membership in this state during<br />

the next year.<br />

Hendrika Meyer, Chair<br />

Tasmania<br />

The Tasmania Faculty has had a ground<br />

breaking year in planning and establishing<br />

the Tasmania Emergency Care Clinical<br />

Network.The inaugural ECN development<br />

workshop was held in May <strong>2010</strong> with<br />

representation from multiple stakeholders<br />

across the state.Terms of reference are<br />

drafted and a steering committee is being<br />

formed.We are currently recruiting for an<br />

experienced F<strong>ACEM</strong> clinical leader and it<br />

is anticipated that this will be filled by the<br />

time this report goes to print.This is<br />

obviously a fantastic endeavour for<br />

emergency medicine in Tasmania and we<br />

look forward to more to come from the<br />

workings of this new clinical network.<br />

Current technology allowed the Tasmania<br />

Faculty to organise monthly Emergency<br />

Medicine Grand Rounds via videoconference.These<br />

are beamed across all<br />

accredited emergency departments in<br />

Tasmania, as well as some other sites.<br />

This has been seen as a great opportunity<br />

to get the departments together on a<br />

monthly basis and share some high-end<br />

topics in emergency medicine.We have<br />

a rotating roster for the presenting<br />

hospital of the Royal Hobart Hospital,<br />

the launceston General Hospital, and<br />

the North West Regional Hospital.These<br />

Grand Rounds have resulted in the<br />

largest gatherings of Tasmania F<strong>ACEM</strong>s<br />

and trainees outside formal conferences<br />

and we are excited to see this continue.<br />

We would strongly encourage other state<br />

faculties to give this a go.<br />

Emergency departments in Tasmania<br />

have encountered some of the typical<br />

recruiting issues problematic in many<br />

areas of Australia.There are F<strong>ACEM</strong> level<br />

vacancies in almost all public hospitals.<br />

As is everyone else, we are supervising a<br />

larger quota of interns. it is hoped this<br />

will result in some light at the end of the<br />

tunnel when a number of them become<br />

trainees of the College.<br />

Among other things, the Tasmania faculty<br />

has also had ongoing involvement with<br />

quality assurance, feedback on the 4<br />

hour rule, research opportunities and<br />

the regular TasFAsT introduction to<br />

Emergency ultrasound Workshops.<br />

Hobart is set to host the <strong>ACEM</strong> <strong>Annual</strong><br />

scientific Meeting in 2012.This will come

Brian Doyle, Chair<br />

Victoria<br />

The last year has seen a number of<br />

interesting events and challenges for the<br />

faculty in Victoria.<br />

The most significant of these was the<br />

staging of the <strong>Annual</strong> scientific Meeting<br />

held in Melbourne in November of 2009.<br />

This was an extremely well attended event,<br />

with a great program and a conference<br />

dinner that will be remembered for<br />

years to come. it also set a benchmark in<br />

being the ‘greenest’ emergency medicine<br />

conference held to date.Again, thanks<br />

and congratulations go to Di Egerton-<br />

Warburton and the organising committee<br />

for their great efforts.We are all looking<br />

forward to seeing what the team in the<br />

ACT can do.<br />

There was also a changing-of-the-guard at<br />

board level.After two years of dedicated<br />

and passionate service, David Eddey stepped<br />

down as the Faculty Chair. Mary Buchanan<br />

handed over theTreasurer’s position to<br />

EmmaWest,Thomas Chan was superseded<br />

by Julia Fisher as secretary and Fergus Kerr<br />

was newly elected as a board member.<br />

Attendance at the faculty meetings<br />

fluctuates, but the dedication of those<br />

who attend in contributing to the<br />

evolution and expansion of emergency<br />

medicine in Victoria is impressive.<br />

The main issue on the agenda has been<br />

the National Access Target ( the ‘4 hour<br />

rule’) but many other issues have stirred<br />

debate, including workforce levels,<br />

relationships with Ambulance Victoria and<br />

the emergence of the Victorian Emergency<br />

Physicians Association (VEPA).<br />

The faculty also reconvened the directors<br />

meeting.All directors of accredited<br />

departments were invited to attend and<br />

this promises to be a positive initiative.<br />

The Victorian scientific Meeting was<br />

held at southern Health on election<br />

day - the day we couldn’t decide who<br />

was going to manage the public hospital<br />

system for the next three years.This<br />

was an excellent meeting and will be the<br />

stepping stone for a bigger event next<br />

year.The faculty is discussing the merits<br />

of taking the pressure off individual health<br />

services in hosting this event and moving<br />

to a centralised meeting with a diverse<br />

organising committee.<br />

The main issue we are facing in Victoria is<br />

the decision by the Federal Government<br />

to introduce the 4-hour national target.<br />

Debate continues as to its pros and<br />

cons, but we remain hopeful that with a<br />

proper understanding of the problems<br />

that contribute to access block and the<br />

injection of funds into the right areas by<br />

the Department of Health, this may be a<br />

positive change.We are asking all of our<br />

Fellows to contribute to this debate and<br />

our Directors to come up with models of<br />

care that we feel are most appropriate for<br />

the Victorian health system. Meetings with<br />

the Health Minister have, so far,<br />

been positive.Watch this space …<br />

Simon Judkins, Chair<br />

Western Australia<br />

The WA Faculty has been active in a<br />

number of areas, including: Healthy Roads,<br />

WA Donation and Transplant Advisory<br />

Committees and WA Careers Expo -<br />

attended by medical students and junior<br />

medical staff.<br />

We held a very successful annual seminar<br />

in May <strong>2010</strong>. This was run by Royal Perth<br />

Hospital and had over 80 registrants.<br />

Our thanks go to Michelle Johnston<br />

for organising this. Next year’s seminar<br />

is being coordinated by swan District<br />

Hospital using the facilities of<br />

Notre Dame university.<br />

A significant number of metropolitan based<br />

Fellows are providing clinical services to<br />

regional hospitals on a fly-in, fly-out basis.<br />

Our Fellows have been very positively<br />

received by these hospitals.There remains<br />

considerable difficulty attracting and<br />

retaining Fellows to live and work in<br />

regional centres, particularly Bunbury.<br />

The faculty position on the 4 hour rule<br />

is one of qualified engagement.A number<br />

of our Fellows are actively engaged in<br />

the implementation of these processes.<br />

We have emphasised the importance of<br />

adequate resourcing behind these changes.<br />

Appropriate quality measures have been<br />

implemented to monitor the changes to<br />

our services.To date, the teaching hospitals<br />

are rarely meeting the target compliance<br />

goals, although a number of Fellows report<br />

significant reduction in access block and<br />

improved working conditions in the<br />

departments.The metropolitan general<br />

hospitals are currently in the early stages<br />

of implementation of these new processes.<br />

it is too early to say how effective these<br />

changes have been.<br />

We are very pleased to acknowledge the<br />

election of David Mountain to President<br />

of the WA Branch of the AMA. David<br />

has contributed extensively to the<br />

development and promotion of<br />

emergency medicine in this state.<br />

EMERGENCY MEDICINE<br />

AuSTRALASIA<br />

The high point of the year was the release of the first impact<br />

Factor (iF) for the journal in mid-June, which surpassed our<br />

expectations at an impressive 0.901.<br />

This iF ranks Emergency Medicine Australasia (EMA) at 11th out of the<br />

19 journals in the 2009 ISI Journal Citation <strong>Report</strong>s®: Emergency Medicine<br />

Category produced by Thomson Reuters. in fact we were close to the median<br />

Emergency Medicine Category iF of 0.916, between scores that ranged from<br />

4.232 (Annals of Emergency Medicine) to 0.075 (Notarzt), with the Emergency<br />

Medicine Journal (EMJ) sitting in 6th spot at 1.477 (yet whose iF was 0.801 as<br />

recently as 2005).Thus 0.901 is an outstanding result, and a truly impressive<br />

international benchmark to rate the quality of our own journal’s scientific<br />

content.<br />

if anyone wonders how this iF figure of 0.901 was reached, it was calculated<br />

from the numerator of 137 citations in 2009 to the denominator of source<br />

articles of which there 152 items published in total in 2007 and 2008<br />

(84 and 68 in respective years). Clearly this does not tell you which actual<br />

papers were cited, or that most papers are not cited at all whereas others were<br />

cited several times! However you wish to interpret this iF on a personal level,<br />

Emergency Medicine Australasia’s inaugural impact Factor is a fine achievement,<br />

and a proud testimony to a first-class editorial and publishing process that we<br />

should all be proud of – from the journal Editorial staff themselves, who are<br />

now spread across 19 different sections, to the Editorial Board, peer reviewers<br />

and Wiley-Blackwell staff. i can assure you, the readers, that you are accessing a<br />

scientifically solid and internationally reputable journal.<br />

An important change that took place in early August was to launch a new<br />

web-based platform Wiley Online Library that now offers integrated access to<br />

more than 4 million articles from 1,500 journals, 9,000 books, and hundreds<br />

of reference works and databases, and replaces Wiley InterScience. Wiley Online<br />

Library provides a clear and simple interface with easier intuitive navigation,<br />

enhanced discoverability options and with an expanded range of functionalities<br />

and personalisation options.All Wiley Interscience content has been migrated<br />

across, along with all online licences for current and perpetual access rights,<br />

so that there should have been no problems for <strong>ACEM</strong> and AsEM subscription<br />

members and others in continuing to utilise content.<br />

The results of the EMA Readership survey became available compiled from<br />

a sorry total of just 61 respondents (including 48 from Australia, 8 from New<br />

Zealand and 22 trainees). lack of sample size validity aside, 69% respondents<br />

still prefer to read the journal in print, and of the content the most highly<br />

ranked for interest included reviews, original research, trauma, toxicology and<br />

paediatric emergency medicine (no real surprises in that list). importantly the<br />

most influential general reasons that shaped readers’ decision which journal<br />

to submit a paper to included speed and quality of peer review, followed<br />

by speed of first publication. it is worth noting then that EMA is able to<br />

immediately publish original research and review papers online in Early View,<br />

then in the next paper issue. i apologise personally to those thankfully few<br />

individuals (you know who you are) who clearly fell outside the acceptable<br />

article turn-around time, and suffered long and frustrating waits – sadly in<br />

reality a problem with every journal.<br />

Challenging all of us who run EMA, the three worst things about the journal<br />

gave ample scope for catharsis with comments about the low quality of<br />

articles, often too long, that fail to pass the ‘so what test’; review papers that<br />

are waffly and not more rigorously evidence-based<br />

complete with number needed to treat/harm etc<br />

(NNT/NNH); and one respondent who simply stated<br />

“Only 3!”. On a slightly more creative front, the three<br />

things that should be changed about the journal<br />

brought comments that mirrored those above,<br />

but also included more emphasis on educational<br />

articles, reflective essays from high-profile emergency<br />

medicine practitioners demonstrating leadership<br />

and advocacy for our specialty, and the enlightening<br />

quip “More clinical, less clerical, more innovative”.<br />

The Editorial Executive will take on board all of these<br />

comments and endeavour to bring you what you<br />

want. unfortunately, this still fundamentally depends<br />

on what you submit, so as i have written many times<br />

before, if you don’t like what you read, your moral,<br />

practical and pre-emptive response should be to<br />

send EMA your best articles. if you want to take a<br />

leadership role and advocate for better emergency<br />

medicine care in Australasia, put this in writing in<br />

a punchy editorial or perspective piece, send us a<br />

useful and concise review paper, and or submit your<br />

cutting edge research. start writing – blow your<br />

own trumpet and join in, don’t simply add to the<br />

background cacophony.<br />

Finally we welcomed some new faces in <strong>2010</strong><br />

including Kevin Chu (original research), Diana<br />

Egerton-Warburton (public health), Andis Graudins<br />

(toxicology) and Garry Wilkes (case reports). i would<br />

like to sincerely thank two outstanding outgoing<br />

editors simon Brown and Geoff isbister for their<br />

tireless work over the years that did much to shape<br />

the journal.Thanks also to all of the section Editors,<br />

peer reviewers,Wiley-Blackwell staff and Editorial<br />

Board with new member Eillyne seow (singapore)<br />

for their hard work in <strong>2010</strong>.The journal could not<br />

exist without your help given so generously and<br />

creatively. let’s look forward to 2011 not for ‘more<br />

of the same’ but for something better, that fulfils the<br />

needs of our readers and acts to convert some of<br />

our doubters.<br />

The Morson Taylor Research Award<br />

is made by the Emergency Medicine<br />

for a study entitled A randomised placebo<br />

controlled trial of antiemetic agents in adult<br />

Principal Activities<br />

The principal activity of the College is to promote and advance<br />

the study of the principles and practice of emergency medicine.<br />

There was no significant change in the nature of the College’s<br />

activity during the year.<br />

Review and Result of Operations<br />

The net surplus of the College for this year was $1,597,851<br />

(2009 - $956,798).The overall increase in the net surplus is<br />

67.0%. (2009 – decrease of 56.0%).<br />

The operating income has increased by 9.8% (2009 : 19.5%).<br />

The non-operating income has decreased by 31.0% ($119,972)<br />

largely due to the decrease in investment trust distributions<br />

received. Expenditure has decreased by 9.7% after accounting for<br />

deficits on sale of investments of $110,394 (2009 - $416,724).<br />

The financial year results have been prepared on an accrual<br />

accounting basis and are therefore fully comparable.<br />

The College’s Net Assets were $12,555,826 as at 30 June <strong>2010</strong><br />

(2009 - $10,680,936).<br />

Dividend<br />

The College’s Memorandum of Association prohibits the payment<br />

of a dividend.<br />

Likely Developments<br />

The College will continue to pursue its aim of promotion and<br />

advancement of the study of the principles and practice of<br />

emergency medicine.<br />

Significant Changes in the State of Affairs<br />

No significant changes in the College’s state of affairs occurred<br />

during the financial year.<br />

Events Subsequent to Balance Date<br />

No matters or circumstances have arisen since the end of the<br />

financial year and the date of this report, which have significantly<br />

affected or are likely, in the opinion of the Councillors,<br />

to significantly affect:<br />

(i) the operations of the College;<br />

(ii) the results of those operations; or<br />

(iii) the state of affairs of the College in subsequent<br />

financial years.<br />

Environmental Regulation and Performance<br />

The College is not subject to any particular or significant<br />

environmental regulation.

The College holds insurance for the Councillors’ liabilities.<br />

corporation with a member of the Council or with a firm of<br />

which a Councillor is a member or with a company in which<br />

a Councillor has a substantial financial interest.<br />

Court Proceedings<br />

No person has applied for leave of the Court to bring<br />

proceedings on behalf of the College or intervene in any<br />

proceedings to which the College is a party for the purpose of<br />

taking responsibility on behalf of the College for all or any part<br />

of those proceedings.The College was not a party to any such<br />

proceedings during the year.<br />

1. Statement of Significant Accounting<br />

Policies<br />

The financial statements are for The Australasian College for<br />

Emergency Medicine as an individual entity.The Australasian<br />

College for Emergency Medicine is limited by guarantee,<br />

incorporated and domiciled in Australia.<br />

Basis of Preparation<br />

The financial statements are general purpose financial statements<br />

that have been prepared in accordance with Australian Accounting<br />

standards (including Australian Accounting interpretations) and the<br />

Corporations Act 2001.<br />

Australian Accounting standards set out accounting policies that<br />

the AAsB has concluded would result in the financial statements<br />

containing relevant and reliable information about transactions,<br />

events and conditions. Material accounting policies adopted in the<br />

preparation of these financial statements are presented below.<br />

They have been consistently applied unless otherwise stated.<br />

The financial statements have been prepared on an accruals basis<br />

and are based on historical costs modified, where applicable,<br />

by the measurement at fair value of selected non-current assets,<br />

financial assets and financial liabilities.<br />

Accounting Policies<br />

Income Tax<br />

The Australian Taxation Office has advised that income of the<br />

College is exempt from the payment of income tax under<br />

section 50-5 of the Income Tax Assessment Act 1997, as amended.<br />

Property, Plant and Equipment<br />

Each class of property, plant and equipment is carried at cost or<br />

fair values as indicated, less, where applicable, any accumulated<br />

depreciation and impairment losses.<br />

Property<br />

Freehold land and buildings are not revalued to an amount above<br />

their recoverable amount, and where carrying values exceed this<br />

recoverable amount assets are written down.<br />

increases in the carrying amount arising on revaluation of land<br />

and buildings are recognised in other comprehensive income<br />

and accumulated in the revaluation surplus in accumulated funds.<br />

Revaluation decreases that offset previous increases of the same<br />

class of assets shall be recognised in other comprehensive income<br />

under the heading of revaluation surplus.All other decreases are<br />

charged to the statement of comprehensive income.<br />

Plant and equipment<br />

Plant and equipment are measured on the cost basis less<br />

depreciation and impairment losses.<br />

The carrying amount of plant and equipment is reviewed annually<br />

by Councillors to ensure it is not in excess of the recoverable<br />

amount from these assets.The recoverable amount is assessed on<br />

the basis of the expected net cash flows that will be received from<br />

the assets’ employment and subsequent disposal.The expected<br />

net cash flows have been discounted to their present values in<br />

determining recoverable amounts.<br />

The cost of fixed assets constructed within the College includes<br />

the cost of materials, direct labour, borrowing costs and an<br />

appropriate proportion of fixed and variable overheads.<br />

subsequent costs are included in the asset’s carrying amount or<br />

recognised as a separate asset, as appropriate, only when it is<br />

probable that future economic benefits associated with the item<br />

will flow to the College and the cost of the item can be measured<br />

reliably.All other repairs and maintenance are charged to the<br />

statement of comprehensive income during the financial period<br />

in which they are incurred.<br />

Depreciation<br />

The depreciable amount of all fixed assets including buildings,<br />

but excluding freehold land, is depreciated on a straight line<br />

or diminishing value basis over their useful life to the College<br />

commencing from the time the asset is held ready for use.<br />

The depreciation rates used for each class of depreciable<br />

assets are:<br />

The assets’ residual values and useful lives are reviewed, and<br />

adjusted if appropriate, at the end of each reporting period.<br />

An asset’s carrying amount is written down immediately to its<br />

recoverable amount if the asset’s carrying amount is greater than<br />

its estimated recoverable amount.<br />

Gains and losses on disposals are determined by comparing<br />

proceeds with the carrying amount. These gains or losses are<br />

included in the statement of comprehensive income. When revalued<br />

assets are sold, amounts included in the revaluation reserve relating<br />

to that asset are transferred to retained surpluses.<br />

Financial Instruments<br />

Initial recognition and measurement<br />

Financial assets and financial liabilities are recognised when the<br />

College becomes a party to the contractual provisions to the<br />

instrument. For financial assets, this is equivalent to the date<br />

the College commits itself to either purchase or sell the asset.<br />

Financial instruments are initially measured at fair value plus<br />

transaction costs except where the instrument is classified<br />

‘at fair value through surplus or deficit’ in which case the<br />

transaction costs are expensed to surplus or deficit immediately.

Classification and subsequent measurement<br />

Financial instruments are subsequently measured at either fair<br />

value, amortised cost using the effective interest rate method or<br />

cost. Fair value represents the amount for which an asset could<br />

be exchanged or a liability settled, between knowledgeable, willing<br />

parties.Where available, quoted prices in an active market are<br />

used to determine fair value.<br />

(i) Financial assets at fair value through surplus and deficit<br />

A financial asset is classified in this category if acquired<br />

principally for the purpose of selling in the short term or if<br />

so designated by management and within the requirements<br />

of AAsB 139: Financial instruments: Recognition and<br />

Measurement. Derivatives are also categorised as held for<br />

trading unless they are designated as hedges. such assets are<br />

subsequently measured at fair value with changes in carrying<br />

value being included in surplus or deficit.<br />

(ii) Loans and receivables<br />

loans and receivables are non-derivative financial assets with<br />

fixed or determinable payments that are not quoted in an<br />

active market and are subsequently measured at amortised<br />

cost using the effective interest rate method.<br />

(iii) Held-to-maturity investments<br />

These investments have fixed maturities, and it is the College’s<br />

intention to hold these investments to maturity.Any heldto-maturity<br />

investments held by the College are stated at<br />

amortised cost using the effective interest rate method.<br />

(iv) Available-for-sale financial assets<br />

Available-for-sale financial assets are non-derivative financial<br />

assets not included in the above categories.Available-for-sale<br />

financial assets are reflected at fair value.<br />

(v) Financial liabilities<br />

Non-derivative financial liabilities are subsequently measured<br />

at amortised cost.<br />

Fair Value<br />

Fair value is determined based on current bid prices for all quoted<br />

investments.Valuation techniques are applied to determine fair<br />

value for all unlisted securities.<br />

Impairment of Assets<br />

At the end of each reporting period, the College reviews the<br />

carrying values of its tangible and intangible assets to determine<br />

whether there is any indication that those assets have been<br />

impaired. if such an indication exists, the recoverable amount of<br />

the asset, being the higher of the asset’s fair value less costs to sell<br />

and value in use, is compared to the asset’s carrying value.<br />

Any excess of the asset’s carrying value over its recoverable<br />

amount is expensed to the statement of comprehensive income.<br />

impairment testing is performed annually for goodwill and<br />

intangible assets with indefinite lives.<br />

Where it is not possible to estimate the recoverable amount of an<br />

individual asset, the College estimates the recoverable amount of<br />

the cash-generating unit to which the asset belongs.<br />

Where an impairment loss on a revalued asset is identified, this is<br />

debited against the revaluation surplus in respect of the same class<br />

of asset to the extent that the impairment loss does not exceed<br />

the amount in the revaluation surplus for that same class of asset.<br />

Intangibles<br />

Web-site costs are initially recognised on the cost basis.<br />

After initial recognition, web-site costs are carried at their cost,<br />

less any accumulated amortisation over their estimated useful life<br />

or 5 years, and any accumulated impairment losses.<br />

Employee Benefits<br />

Provision is made for the College’s liability for employee benefits<br />

arising from services rendered by employees to the end of the<br />

reporting period. Employee benefits that are expected to be<br />

settled within one year have been measured at the amounts<br />

expected to be paid when the liability is settled, plus related<br />

on-costs. Employee benefits payable later than one year have<br />

been measured at the present value of the estimated future cash<br />

outflows to be made for those benefits.<br />

Contributions are made to employee superannuation funds and<br />

are charged as expenses when incurred.All employees are entitled<br />

to varying levels of benefits on retirement, disability or death.<br />

The superannuation plans or equivalent provide accumulated<br />

Provisions<br />

Provisions are recognised when the College has a legal or<br />

the best estimate of the amounts required to settle the obligation<br />

at reporting date.<br />

Cash and Cash Equivalents<br />

Cash and cash equivalents include cash on hand, deposits held<br />

at-call with banks, other short-term highly liquid investments with<br />

original maturities of six months or less, and bank overdrafts. Bank<br />

overdrafts are shown within short-term borrowings in current<br />

liabilities on the statement of financial position.

Revenue<br />

Revenue from the sale of goods is recognised when the goods<br />

are delivered to customers.<br />

interest revenue is recognised on a proportional basis taking<br />

into account the interest rates applicable to the financial assets.<br />

Subscriptions and Exam Fees<br />

subscriptions and Exam Fees have been included on an accruals<br />

basis which is consistent with the accounting policy adopted in<br />

the previous year.The revenue is recognised when the service<br />

has been provided.<br />

Revenue from the providing of all other services is recognised<br />

when the service is provided.<br />

Borrowing Costs<br />

Borrowing costs directly attributable to the acquisition,<br />

construction or production of assets necessarily take a substantial<br />

period of time to prepare for their intended use or sale, are<br />

added to the cost of those assets, until such time as the assets are<br />

substantially ready for their intended use of sale.<br />

All other borrowing costs are recognised in income in the period<br />

in which they are incurred.<br />

Goods and Services Tax (GST)<br />

Revenues, expenses and assets are recognised net of the amount<br />

of GsT, except where the amount of GsT incurred is not<br />

recoverable from the Tax Office. in these circumstances, the GsT<br />

is recognised as part of the cost of acquisition of an asset or as<br />

part of an item of the expense. Receivables and payables in the<br />

balance sheet are shown inclusive of GsT.<br />

Cash flows are presented in the statement of cash flows on<br />

a gross basis, except for the GsT component of investing and<br />

financing activities, which are disclosed as operating cash flows.<br />

Comparative Figures<br />

When required by Accounting standards, comparative figures<br />

have been adjusted to conform to changes in presentation for<br />

the current financial year.<br />

Trade and Other Payables<br />

Trade and other payables represent the liability outstanding at the<br />

end of the reporting period for goods and services rendered by<br />

the company during the reporting period which remain unpaid.<br />

The balance is recognised as a current liability with the amounts<br />

normally paid within 30 days of recognition of the liability.<br />

Critical Accounting Estimates and Judgements<br />

The Councillors evaluate estimates and judgements incorporated<br />

into the financial statements based on historical knowledge and<br />

best available current information. Estimates assume a reasonable<br />

expectation of future events and are based on current trends and<br />

economic data, obtained both externally and within the College.<br />

Adoption of New and Revised Accounting Standards<br />

During the current year, the College adopted all of the new and<br />

revised Australian Accounting standards and interpretations<br />

applicable to its operations which became mandatory.<br />

The adoption of these standards has impacted the recognition,<br />

measurements and disclosure of certain transactions.The following<br />

is an explanation of the impact the adoption of these standards<br />

and interpretations has had on the financial statements of the<br />

College.<br />

Terminology changes – The revised version of AAsB 101 contains<br />

a number of terminology changes, including the amendment of the<br />

names of primary financial statements.<br />

<strong>Report</strong>ing changes in accumulated funds – The revised AAsB<br />

101 requires all non-Fellow changes in accumulated funds to be<br />

presented in the statement of comprehensive income.<br />

Statement of comprehensive income – The revised AAsB<br />

101 requires all income and expenses to be presented in either<br />

one statement, the statement of comprehensive income, or two<br />

separate statements, a separate income statement and a statement<br />

of comprehensive income.The previous version of AAsB 101<br />

required only the presentation of a single income statement.<br />

The College’s financial statements now contain a statement of<br />

comprehensive income.<br />

Other comprehensive income – The revised version of AAsB<br />

101 introduces the concept of ‘other comprehensive income’<br />

which comprises of income and expenses that are not recognised<br />

in surplus or deficit as required by other Australian Accounting<br />

standards. items of other comprehensive income are to be<br />

disclosed in the statement of comprehensive income.<br />

New Accounting Standards for Application in<br />

Future Periods<br />

The AAsB has issued new and amended accounting standards and<br />

interpretations that have mandatory application dates for future<br />

reporting periods.The College has decided against early adoption<br />

of these standards.A discussion of those requirements and their<br />

impact on the College follows:<br />

4. Net Fair Values<br />

The fair values of financial assets and financial liabilities are<br />

presented in the above interest rate risk tables and can be<br />

compared to their carrying values as presented therein as per the<br />

statement of financial position. Fair values are those amounts for<br />

<strong>Report</strong> on the Financial <strong>Report</strong><br />

We have audited the accompanying financial statements of<br />

The Australasian College for Emergency Medicine, which<br />

comprises the statement of financial position as at 30 June <strong>2010</strong>,<br />

and the statement of comprehensive income, statement of changes<br />

in accumulated funds and statement of cash flows for the year<br />

ended on that date, a summary of significant accounting policies<br />

and other explanatory notes and the Councillors’ declaration.<br />

Councillors’ Responsibility for<br />

the Financial <strong>Report</strong><br />

The Councillors of the College are responsible for the<br />

preparation and fair presentation of the financial statements in<br />

accordance with Australian Accounting standards (including the<br />

Australian Accounting interpretations) and the Corporations Act<br />

2001.This responsibility includes designing, implementing and<br />

maintaining internal controls relevant to the preparation and fair<br />

presentation of the financial statements that are free from material<br />

misstatement, whether due to fraud or error; selecting and<br />

applying appropriate accounting policies; and making accounting<br />

estimates that are reasonable in the circumstances.<br />

Auditors’ Responsibility<br />

Our responsibility is to express an opinion on the financial<br />

statements based on our audit.We conducted our audit in<br />

accordance with Australian Auditing standards.These Auditing<br />

standards require that we comply with relevant ethical<br />

requirements relating to audit engagements and plan and perform<br />

the audit to obtain reasonable assurance whether the financial<br />

statements are free from material misstatement.<br />

An audit involves performing procedures to obtain audit evidence<br />

about the amounts and disclosures in the financial statements.<br />

The procedures selected depend on the auditors’ judgment,<br />

including the assessment of the risks of material misstatement<br />

of the financial statements, whether due to fraud or error.<br />

in making those risk assessments, the auditors consider internal<br />

control relevant to the entity’s preparation and fair presentation<br />

of the financial statements in order to design audit procedures that<br />

are appropriate in the circumstances, but not for the purpose of<br />

expressing an opinion on the effectiveness of the entity’s internal<br />

control.An audit also includes evaluating the appropriateness of<br />

accounting policies used and the reasonableness of accounting<br />

estimates made by the Councillors, as well as evaluating the overall<br />

presentation of the financial statements.<br />

We believe that the audit evidence we have obtained is sufficient<br />

and appropriate to provide a basis for our audit opinion.<br />

Independence<br />

in conducting our audit, we have complied with the independence<br />

requirements of the Corporations Act 2001.We confirm that the<br />

independence declaration required by the Corporations Act 2001,<br />

provided to the Councillors of The Australasian College for<br />

Emergency Medicine on 5 October <strong>2010</strong>, would be in the same<br />

terms if provided to the Councillors as at the date of this auditor’s<br />

report.<br />

Auditors’ Opinion<br />

in our opinion, the financial statements presents fairly, in all<br />

material respects, the financial position of The Australasian<br />

College for Emergency Medicine as of 30 June <strong>2010</strong>, and its<br />

financial performance and cash flows for the year then ended<br />

in accordance with the Corporations Act 2001 and the<br />

Australian Accounting standards (including Australian<br />

On the basis of information provided by the College, we have compiled in accordance with APs 9<br />

“statement on Compilation of Financial <strong>Report</strong>s” the additional financial statement of the client<br />

for the year ended 30 June <strong>2010</strong> as set out on page 60.<br />

The extent to which Accounting standards and uiG Consensus Views have or have not been<br />

adopted in the preparation of the general purpose financial statement is set out in Note 1.<br />

The College is solely responsible for the information contained in the general purpose financial<br />

statement and has determined that the accounting policies used are consistent with the financial<br />

reporting requirements of the College’s constitution and are appropriate to meet the needs of<br />

the College for the purpose of management.<br />

The additional financial statement has been prepared from the books and records of the College<br />

which have been subjected to the auditing procedures applied in our statutory audit of the College<br />

for the year ended 30 June <strong>2010</strong>. it will be appreciated that our statutory audit did not cover all<br />

details of the additional financial statement. Accordingly, we do not express an opinion on such<br />

financial statement and no warranty of accuracy or reliability is given.<br />

To the extent permitted by law, we do not accept liability for any loss or damage, which any person,<br />

other than the College, may suffer arising from any negligence on our part. No person should rely<br />

on the additional financial statement without having an audit or review conducted.<br />

The additional financial statement was prepared for the benefit of the College and purpose identified<br />

above. We do not accept responsibility to any other person for the contents of the additional<br />

financial statement.<br />
