Editorial
A Helping Hand by Seung-Hee Rhee, DDS, FAGD
It is Saturday, July 10,
2010. The place is New
Orleans, Louisiana. It
is 6:00 in the morning and
already it is sweltering.
The sun is barely up but
you can already feel the
heavy moisture in the air
and the beads of sweat
ready to race down your back. It is going to be
another hot day.
In this unfavorable condition, a race is about to
start. It’s the AGD Foundation 5k Fun Run/Walk
(that’s about 3.3 miles to you and me). It is an
annual charity event where participants enjoy a run,
a jog or a walk while supporting the
AGD Foundation in its mission to advance the oral
health of the public and support the efforts of general
dentists. About 150 people – runners, volunteers,
supporters – gathered not too far from the
banks of the mighty Mississippi River for this
event. You see the young and old, fit and not-sofit,
smiling faces all around. Some are anxious to
start. Others are ready to enjoy a leisurely run at
their own pace. And they are off! It is a double
loop run which means they will run along the banks
of Mississippi, make a turn, run back towards the
finishing line, and repeat. As predicted, the conditions
are grueling. After the first loop, all the runners
are covered in sweat. Everyone grabs a cup of
water from the watering stand and also gets cheers
and encouragements as a bonus. But in 13 minutes
and 46 seconds, we have our first winner.
Predictably, it is one of our military personnel. Dr.
Loren Masuoka of Army AGD takes first place.
The first female to cross, Dr. Chirsty Kirk, also
takes third place overall at 14.42.
Then something wonderful happened. Many
who already finished the race, instead of resting
or replenishing, went back into the race despite
the horrid conditions. Some young men and
women went back to help their friends sprint to
the finish to make better time, yelling encouragements
like drill sergeants. Some went back to run
alongside their family and friends. Others went
back to lend their support, both physically and
mentally, to those who may not be as fast as they
once used to be. A smile spread across a struggling,
elderly gentleman’s face as a young colleague
came alongside to run the last mile with
him. As the two men ran together, their steps
seem lighter than it was just a few moments
before. Things are always a little easier with a
friend by your side. And one by one, they all
cross the finish line.
Flash back a few days. The place is a large
ballroom at Hilton Riverside Hotel that has been
converted to host the House of Delegates for the
AGD. The HOD is in full swing and a delegate
from NY is nervously standing in line at one of
the microphones for a chance to speak. Although
it is not her first time at the HOD, it is the first
time she got up the nerve to speak in front of over
250 representatives of the AGD. Not many people
knew but she was terrified of public speaking.
She almost lost courage and went back to her
seat. But then she remembered the previous day,
as she sat nervously at the head table as a first
time member of the Reference Committee,
addressing the issues at hand with hundred pairs
of eyes staring at her. How as her name was
introduced, her colleagues from NY gave a loud
cheer in support, letting her know that there are
friends out there. She also remembered an advice
given to her by Jay Orlikoff, Past President of
NYSAGD. When she confided in him about her
fear of public speaking, he told her what Jerry
Seinfeld said on the subject. According to
Seinfeld, more people are afraid of public speaking
than they are of dying (that did not help her).
So it is more terrifying to deliver the eulogy than
to be the subject of the eulogy (that made her
laugh). Jay advised her to practice and also to
select a friend in the audience on the left side and
a friend on the right side and speak directly to
them. He said that it was the same advice given
to him by Howard Glazer, Past President of
AGD/NYSAGD. He also told her that if she is
ever scared to speak at a meeting, he would be
honored if she chose him to be the one she looked at.
As she remembered these things, she smiled
and stepped up to the microphone and had her say
in front of 250 people. Despite her nervousness,
she noted that many people clapped in support of
what she had to say and that yes, there were
indeed many friends among the crowd.
My mom used to tell me that if our hands
were as hard working as our eyes, we would get
more things done. She is right – our eyes do see
everything but our hands don’t do everything that
they should. But it is the hands that move the
world. It is the hands that lend support to a friend
in need. It is the hands that wipe away tears and
fix scraped knees on a child. It is the hands that
clapped and typed an email that lent courage for
a dentist to get over her fear of public speaking.
It is the hands of Dr. Fabiola Milord that helped
relieve so much pain and suffering in Haiti as you
will read in this issue of GP. As dentists, we are
frequently thanked by our patients for helping
them. But too often, we in turn forget to thank
those who help us in our every day, to make what
we do possible and lend us a helping hand, even
without us asking them to. They are our families,
our loved ones, our hard working staff. They are
our dedicated members of the NYSAGD. Let us
remember to take the time to say “thank you” to
all those wonderful helping hands that guide and
support us each and every day. And Jay, thanks
- it worked!
From the President
by Ira Levine, DDS, FAGD
“
I’ll huff and I’ll puff and I’ll blow this
house down!”
Each of us, on our own, has little hope of
duplicating this feat by the big, bad wolf. Come
to think of it, if the big, bad wolf were around
today, he would probably need to apply for a
demolition permit, submit architectural drawings,
conduct environmental impact studies, and hire union workers.
Can one person knock down a brick wall, no matter how badly it is
constructed, all by himself? The dents on my forehead prove how
futile this can be, but that doesn’t stop me from trying. It is true that
one person can make a difference. But that difference tends to be a
direct result of how that one person inspires others.
How many obstacles do dentists face in the pursuit of their profession?
Not only do we face our daily barriers, but there are others
inside and outside the profession who continually heap additional
obstacles in our way. There are insurance companies, legislators,
lawyers, dental manufacturers, and a multitude of others who want a
piece of us and regularly test our limits. All these groups have a great
deal more power and money to wield than any one of us. If we all
sat in our offices and ignored anything not immediately relevant, we
would get slammed. If you stick your head in the sand like an
ostrich, you stand a real good chance of getting “goosed”. All too
often people think, “If I don’t do it, somebody else will”. Guess
what? That phrase should read, “If I don’t do it, nobody else will”.
This is just the way things are. Sometimes we get lucky and others
pick up the slack. But unfortunately, it doesn’t happen every time.
The one ace-in-the-hole that we have is cohesiveness. I am
happy to say that the NYSAGD and the NYSDA are developing a
fine working relationship. I was honored to have the President
and the Executive Director of NYSDA, Drs. Robert Doherty and
Mark Feldman, attend the NYSAGD Installation Dinner. In turn,
I attended the NYSDA House of Delegates and Installation
Dinner. Both were great events that have fostered cooperative
relationship between our two organizations.
Membership in a professional association should be examined
for both short and long term gains by the individual dentist. I view
the AGD as “A Player”, as Howard Farran might say. If you don’t,
let’s review the following:
You know that the NYSAGD and the AGD offer numerous CE
courses of high quality and low cost. For example, there is the
Hazy, Hot, and Humid!
That’s what it was like in New Orleans this
summer for the AGD Annual Meeting. But it
was also HOT with excitement in the New
Orleans Convention Center! There was such a
multitude of courses, many of which were
hands-on participation courses, that if you
couldn’t find something that you were interested
in, you probably weren’t looking.
This was the first time that the AGD Annual Meeting and
Exhibits was held without the governance occurring at the same
time. The House of Delegates finished its business early Thursday
www.nysagd.org | Fall 2010 | GP 4
wonderful selection of CE courses put together by the NYSAGD
CE Committee. There is also the AGD Annual Meeting which was
held in New Orleans this past July (San Diego next year) where
there are high caliber hands-on courses and speakers. The AGD
website also has an extensive CE database that lists courses from
all over the US at any given day and also list free courses - just for
our members. The AGD can also keep track of your CE credits in
a transcript that can be used as proof of CE requirement for licensure
and in the event of an audit. The NYSAGD is happy to say
that after long negotiations, the NYS Dental Board chose to recognize
our transcript as acceptable documentation of CE credits.
The AGD also worked diligently to develop a practice management
service called “AGD Patient Alerts”. It is a free service that
ties-in with your current practice management software to allow you
to send e-mails, text messages, electronic newsletters (easy to set-up
and customize), and other reminders to patients. In addition, there is
a consumer friendly portal called “KnowYourTeeth.com” with its
wide range of professionally produced patient fact sheets, a place for
consumers to ask dental questions, and a useful Widget. What’s a
Widget? You will just have to visit to find out.
In addition, the AGD is heavily involved in Advocacy for you
and your patients. A number of different states are evaluating
and/or approving the DHAT model (Dental Health Aid
Technician) or the ADH model (Advance Dental Hygienist). Your
awareness of this issue is critically important. Fact is, the ADA is
currently testing the DHAT model and the Connecticut Dental
Association just released a statement saying that they support the
testing of the DHAT model. It is critical that the AGD make the
opposing views known. The AGD usually supports the ADA policies
except where differences exist on how a general dentist may
be affected. If you are not familiar with the issues that the AGD
has advocated in the past or the current issues we are addressing
on your behalf, please take a moment to know what is going on in
your profession. It is in your best interest.
There is so much more, but I leave it to you to look further. If
you have read this far, perhaps you might go further. If not, perhaps
you might be willing to make a leap of faith and take what I
have said at face value. Just know that you are important to the
profession - how it is today and how it will be tomorrow. Don’t
underestimate what you gain by being a member of the AGD. If
you have been affected by the economy, as many of us have been,
and are making hard financial choices, AGD membership should
not be one of the items on your list of “unnecessary expenses”.
Remember – one person can make a difference.
afternoon. By Thursday evening, the Exhibit Hall was open for
business and the first full day of dental courses had come to a close.
In addition, this was the first time AGD sponsored an Outreach
Program to help those in need of dental services. It was an incredible
success. Those who were involved came back exhausted but
exhilarated by the good that they had done for so many.
The Saturday Night event this year was held at Mardi Gras World
and it was a spectacular event. The setting was an indoor antebellumlike
plantation with a star filled sky that looked as if we were outdoors.
The addition of Mardi Gras floats, the wonderful food and
music made this event a wonderful ending to a fabulous week.
As always, I invite you to become more active and be a part
of the Academy. Take advantage of the many benefits and services
available to you as a member of the AGD. Check out the CE
Database and the AGD Career Center. They are but two of the
many benefits available to you as a member of the Academy.
Until next time!
From the President-Elect
by Muhammed Kasem, DDS, MAGD
At the AGD Annual Meeting 2010 in
New Orleans this past July, it was my
honor and privilege to receive the AGD
2010 Membershp Award for the New York
State Academy of General Dentistry. The
Award was presented by AGD President, Dr.
David Halpern, DMD, FAGD.
I was also proud to be a part in receiving
the 2010 AGD Advocacy Award along with Dr.
Janice Pliszczak, Dr. Ira Levine, Dr. Nathan Hershkowitz and
other delegates from NYS. On a personal note, I was also proud
of myself on receiving the Award as one of the Top Recruiters in
the Nation for AGD in 2009- 2010. Dr. Judy Ann Taylor was also
honored with the Third Place in Grassroots Member Recruiter
Category. All in all, New York State Academy of General
Dentistry reaffirmed its status as a voice of General Dentistry.
This is a time to celebrate our accomplishments. All this happened
because of the wonderful members of the Executive
Council, Board of Trustee, and New York State AGD staff by their
dedication, volunteerism, and commitment to do better service to
our public, our profession and our beloved Academy.
I commend our leaders’ unwavering commitment to our mutual
goal of “Advancing the value and excellence of General
Dentistry”. Throughout my leadership position in NYSAGD, I
From the Regional Director
by Abe Dyzenhaus, DDS, FAGD
How was your 4th of July holiday?
Normally the July 4th holiday is spent
with family and friends at a barbeque, at the
beach, or at a park. I spent my July 4th holiday
in New Orleans, with my wife, prior to
the AGD annual meeting.
On Saturday, we took a city bus tour and
on Sunday, we went to brunch with our friends at Commander’s
Palace, one of New Orleans’ finer restaurants. Then we took a
walking tour of the Garden District, the neighborhood where the
restaurant was located, and the homes were magnificent.
Now you may be wondering why I am telling you this. Well,
that weekend was a prelude to the week spent in New Orleans at
the AGD Annual Meeting. The first three days of the meeting
were spent in governance, such as the Regional Directors Meeting
and the House of Delegates. Business is conducted during these
meeting but also in the hallways and in the evening at the receptions
and the dinners. And let me emphasize the dinners - the
restaurants in New Orleans are terrific.
met people with exceptional talents and dedicated leaders who
devote their precious free time for the sake of professional excellence
such as Dr. Stanley Markovits, Dr. Stephen Akseizer, Dr.
Kay Oen, Dr. Lawrence Bailey, Dr. Nathan Hershkowitz, and Dr.
Janice Pliszczak. Their encouragement and support always
inspired me.
As a leader of the organization, my philosophy has always
been “Members First”. Members are the heart of the organization
and leaders should hear the voice of its members and reach out to
non-members as well.
I thank the Nominating Committee who gave me the opportunity
to serve the NYSAGD over the years. My special thanks go
to my friend and Past President of NYSAGD, Dr. Nathan
Hershkowitz, for his support. Under his Chairmanship of the
Nominating Committee, I was chosen to be the President-Elect of
NYSAGD. I also want to thank the other members of the
Nominating Committee for their support and for giving me the
opportunity to serve the New York State Academy of General
Dentistry.
Finally, I congratulate all the members of the BOT, the
Executive Council and the NYSAGD Staff for receiving 2010
AGD Awards in Advocacy, Membership, and Honorable Mention
in CE Award of Excellence, for their selfless service and for dedicating
their priceless time to bring such honor and distinction to
all the AGD dentists of New York State.
Peace be with you.
After the conclusion of the governance sessions, I stayed in
New Orleans to take some continuing education courses. Over the
years, I have satisfied my CE requirements for NY and the AGD
by taking CE courses given by the AGD and the NYSAGD. There
are many CE organizations in the dental market now as opposed
to years ago and I have taken courses not given by an AGD group.
But I always try to take an AGD, PACE approved provider course
so that the credits will go towards my MAGD, just as it did for my
FAGD.
Next year, the AGD Annual Meeting will be held in San Diego
prior to the July 4th holiday. It will be June 26 - 28, 2011. San
Diego is one of the nicest places on this planet. The weather is
always beautiful. You may think it’s too early to think about next
year. You may be right. But put the dates on your calendar and
think about it in early spring 2011.
I hope to see you in San Diego. Maybe we’ll sit next to each
other at one of the courses. Or maybe I’ll see you at a NYSAGD
continuing education course. And thank you for your membership
The 2010 Meritorious Service Award
Dr. Stanley Markovits, DDS, MAGD
Presentation of Meritorious Service Award to Dr. Stanley Markovits, DDS, MAGD by his son,
Dr. Gary Markovits, DDS, FAGD, PLLC.
www.nysagd.org | Fall 2010 | GP 6
I have been given the honor of presenting my partner, mentor, and father, Dr. Stanley
Markovits, DDS, MAGD, with this year’s NYSAGD Meritorious Service Award. Though
it was about four years ago, it seems like yesterday that I was standing before this academy
presenting dad with the NYSAGD’s Life Time Achievement Award. Having
received a Life Time Achievement Award, we all thought he’d take the hint and slow
down. Well that didn’t happen. Though he did stop clinical practice over four years
ago, we, his family and those of you who know him, know that we NEVER use the
“R” word (retirement) when describing his current status. Why would we? The
fact is he hasn’t. He continues his work as a teacher, lecturer, writer, community
organizer, and of course as consigliere to many AGD councils and its members.
The NYSAGD’s Meritorious Service Award recognizes a member of the den-
tal community who has consistently
served the NYSAGD by working
to enhance the image of general
dentistry through their leadership
within the Academy and through
areas of civic involvement.
Stan has been a NYSAGD board
member since 1962 when he was
appointed by then NYSAGD president,
Dr. George Kudler, who founded the
NYSAGD. Even in his executive role as past
president of both the state and national organization,
he never lost sight of this academy’s mission which is to foster, support,
and reward the continuing education of the general dentist and by doing so
improve the quality of care we bring to our patients. Together with my mother,
Audrey, they have served this profession and this academy for more than
half a century with humility, dignity, honesty, and compassion.
He is a Fellow of the American College of Dentistry, the International
College of Dentistry, the International Academy of Dentistry, the Academy of
Continuing Education, and of course, the AGD in which he also earned his
Mastership of which I know he is most proud. The NYSAGD and AGD had
previously recognized him with “The Distinguished Service Award”, “The
Meritorious Service Award”, and “The Thaddeus V. Weclew Award”.
Stan is active in local civic organizational work as well. A member of the
White Plains Conservation Board, President of the North Street Civic
Association, and VP of the Glen Brook Homeowners Association, he continues
to contribute to his community in ways that may not affect the dental profession
directly but serve as an example to others what this academy and its
members stand for.
It is with great honor and love that I present on behalf of the New York
State Academy of General Dentistry, this year’s Meritorious Service Award to
my father, Dr. Stanley Markovits.
“There was never another woman in our
marriage. Just AGD.”
- Mrs. Audrey Markovits,
married 59 years to Dr. Stanley Markovits
Dear Colleagues:
Audrey and I can’t thank you
enough for letting us share the
evening with the installation of our
new officers. The affair was beautifully
orchestrated which wasn’t a
surprise knowing that Steve and
Paula made all the arrangements.
Audrey thanks you for her flowers
and our new globe which is a wonderful
gift. Now we can store our
thirty year old atlas and finally
locate so many new countries and
boundaries that exist now.
I find it embarrassing to receive
honors of this magnitude for taking
part in the Academy’s goals, objectives,
etc. even now in the twilight
years of my life. Thank you all for
letting me continue working with
you to better our patient care and
maintain our continued education at
the quality it needs to be.
Very sincerely,
Audrey and Stanley Markovits

Service Recognition Award
Lifelong Learning and Service Recognition (LLSR) is given to those Masters
of Academy of General Dentistry (MAGD) who wholly embrace the AGD’s
core principles and ideals even after achieving Mastership. They continue their
commitment to lifelong learning, volunteer their services to communities in
need, act as mentors to associates and new dentists, and act as a voice of general
dentistry by participating in organized dentistry. These are the four distinct
steps that need to be accomplished in order to earn the prestigious LLSR.
NYSAGD is proud to recognize this year’s recipient, Dr. Emile Martin, DDS,
New Bills in New York
On April 13, 2010, the New York legislature introduced two bills that, if passed,
will affect the dental team. The first, AB 10638, authorizes a limited permit to
hygienists to practice dental hygiene for a period of one year, although it may
be renewed for an additional year. The bill requires that hygienists practice
only under the personal supervision of a licensed dentist. The second bill, AB
10649, would require quality standards for dental prosthetic devices and for
disclosure of the manufacturing location of such devices.
Although Align Technology Inc. announced in April that it has dropped the
patient case start quota from its 2010 Invisalign Proficiency Requirements,
the New York legislature is not ending its fight against manufacture-enforced
quotas. On April 26, 2010, SB 7614 was introduced to prohibit manufacturers
from enforcing patient quotas on dentists for selected dental treatments.
Preventing Early Childhood Tooth Decay
A bill recently introduced in New York could help dentists in the fight against
tooth decay in young patients. SB 8014, introduced on May 28, 2010,
requires manufacturers or importers of baby bottles and sippy cups to provide
a label to warn about the potential for early childhood tooth decay from
such products.
Hope for Haiti - Making a Difference One Tooth at a Time
Interview with Fabiola Milord, DDS, FAGD
On January 12, 2010 at 4:53 p.m., a catastrophic 7.0 magnitude earthquake struck the small Caribbean nation of Haiti on the island
of Hispaniola. The epicenter of the earthquake was near the town of Léogâne, which is approximately 16 miles west of Port-au-Prince,
the capital of Haiti. The earthquake and its subsequent aftershocks (at least 52 measuring 4.5 or greater in the 12 days following the
earthquake), completely devastated Port-au-Prince and areas surrounding the epicenter, affecting an estimated 3 million people.
Recorded as the 6th deadliest earthquake, its casualties include 230,000 dead and over 300,000 injured. An estimated 250,000 homes
and 30,000 commercial buildings were also destroyed.
Humanitarian aid poured in from all over the world in the aftermath. But with severe damages in communications, transportation,
and electrical systems from the earthquake, the rescue and relief efforts are severely hindered. Tens of thousands had to be buried in
mass graves and over a million people were left homeless and without resources. Rescue efforts, medical care, sanitation, and supplies
for the millions remain a priority.
Many from around the globe answered Haiti’s call for help. One of our own, Dr. Fabiola Milord, a grass-roots member of the
NYSAGD, who practices in Freeport, Long Island, NY heard the call and gave a helping hand. This past June, Dr. Milord was a member
of a medical mission that went down to Haiti. GP had a chance to talk to Dr. Milord after her mission.
GP: Dr. Milord, thank you for taking the time to speak to GP.
Could you tell us some background information regarding your
mission to Haiti?
FM: Thank you for giving me the opportunity. Our mission
was from June 18 – June 26, 2010. It was a medical mission
to Port-au-Prince, Haiti with NOAH (National Organization for
the Advancement of Haitians) NY’s Stanley Barbot Medical
Mission. Our team members included a pediatrician from
Howard University in Washington DC, five students from
Howard University Medical School, four public health professionals,
a photographer/videographer, and me as the lone dentist
of the group. Another group EMPACT/DRNW (Disaster
Response Northwest) from Tacoma, Washington joined us later
at one of the mission sites.
GP: Could you tell us a little about yourself?
FM: Both my parents were Haitian (now deceased) but I was
born in the Republic of Congo in Africa. I lived in NY since I
was a month old. I am a product of the NYC public school system
– I attended P.S. 9, I.S. 44, and Washington Irving High
School. I received my Bachelor of Science degree from St.
John’s University, followed by a Doctor of Dental Surgery
from New York University College of Dentistry. I completed
Currently, I am the Dental Director for Catholic Charities
Health Systems, Inc., Diocese of Rockville Centre on Long
dentistry – ADA, NYSDA, and the Long Island Academy of
Odontology. I am also a Fellow of the Academy of General
Dentistry and sit on the Board of Directors for Nassau County
Dental Society.
GP: Those are some impressive credentials! So what influenced
your decision to go on a mission to Haiti? Did you need
to take any special precautions medically prior to your mission?
FM: Through the Stanley Barbot Medical Mission and various
other organizations, I’ve participated in over twenty medical
missions to countries such as Haiti, Jamaica, South Africa,
Ghana, Guyana, St. Vincent’s, St. Lucia, and the Lower Ninth
Ward in New Orleans after Hurricane Katrina. Since both of
my parents were Haitian and I still have family members there,
it only seemed natural for me to contribute in any way I could.
All of my vaccines were up-to-date since I have been on many
previous medical missions. I did take a bottle of Noroxin
antibiotics previously prescribed to me to stop diarrhea. I also
took a box of Imodium as a preventive measure. Thankfully, I
did not have to use any of them.
GP: What was your first impression of the devastation in
Haiti?
FM: From the air, as the plane was descending, we noticed
myriads of blue dots on the landscape which turned out to be
tarps that were used to make up the tents for the tent cities that
were now peoples’ homes after the devastating January 12th
earthquake.
We landed on Aeoport Toussaint L’Ouverture in Port-au-Prince
and the damage from the earthquake was immediately evident.
The airport itself was closed (deemed unsafe due to structural
damage) so a shuttle bus took us to a hangar where we were to
clear customs and retrieve our luggage. Needless to say, since
this was a hangar, there was no ventilation and the air was stiflingly
hot. Proper documentations were presented to Haitian
customs agents by the Medical Director of the mission, Henri
R. Paul, MD, allowing us to enter the country with Lidocaine
which we brought to perform dental procedures. We had about
30 boxes of Lidocaine – 10 were purchased and the rest were
donated by Dorcas Medical Mission and a dentist from SUNY
Downstate Medical Center. All other medications had been
shipped prior to our arrival and were waiting for us at the site
where we were going to work.
GP: How were your accommodations during the mission?
FM: All the hotels were fully booked so we stayed in a rented
home which was welcoming and comfortable. We were fortunate.
The group of EMTs and nurses that joined us from
Tacoma was unable to get accommodations and had to sleep in
tents at the field hospital where we worked.
GP: Could you tell us about your mission?
FM: We arrived on Friday, June 18 so the weekend was spent
unpacking, organizing, setting up equipment, supplies, and laying
out instruments in anticipation of Monday’s busy work
schedule. This was all crucial to saving time. We were set to
work at two sites. The primary site was AIMER-HAITI
Hospital which is a privately run facility set up at an abandoned
amusement park converted into a field hospital after the
earthquake. The secondary site was Eliazare Germain Hospital
- a state run hospital in a section of Haiti known as Petionville.
The field hospital was staffed by a local physician. I was to be
the first dentist to see patients there.
Organizing and setting up instruments and supplies. This would be my
“office”.
Local volunteers assigned to help.
Buildings destroyed by the earthquake.
We visited AIMER-HAITI Hospital on Saturday and were
greeted and given a tour by the hospital’s administrator. We
were shown the waiting areas, the tent where general surgeries
take place, where patients are seen on an outpatient basis, and
the pharmacy. Patients have no out of pocket expense. She
thanked us for coming and as a small token of appreciation for
our services; she told us that we would be served a hot lunch
every day.
www.nysagd.org | Fall 2010 | GP 17
GP: What was on the menu?
FM: Stewed peas, rice, and chicken. It was delicious.
GP: Please continue.
FM: I asked the administrator where I would be working. She
offered me space under a large open hut (no walls or doors). I
was assigned a couple of local volunteers to help me with set
up, registration, and sterilization. We unpacked bags and organized
ourselves for Monday. The set up would stay as is until
then. We were assured that there was 24-hour security at the
hospital and that all we had to do was cover everything. We
blanketed the table with a huge tarp and I left knowing that I
would be ready to go come Monday morning.
On Sunday, Dr. Paul, the pediatrician, and I set out to see
where we would be working at the government run hospital in
Petionville. I was hopeful because I was told that the hospital
was a three-story building and had a dental clinic. Upon our
arrival, we were graciously met by the hospital’s medical
director, Dr. Fantale, who gave us a tour of the facility. The
pediatrician consulted an in-house patient (an infant being
treated for malnutrition). The director of our mission, Dr.
The Palais Nacional and most government buildings in downtown Port-au-
Prince were destroyed.
Paul, consulted another patient whose
problem was jaundice of unknown origin.
After the appropriate tests were
ordered (and generously paid for by
him), we went back downstairs to take
a look at the dental clinic which is
staffed by one dentist. Unfortunately,
the clinic was locked and the medical
director did not have the key. As it
was Sunday, the appropriate staff was
not there to make the clinic accessible
to us. Since the attending dentist was
off on Wednesdays, I promised the
medical director I would return that
day to donate my services. I was
assured that there was a dental unit and
a chair in the clinic. Not knowing how
well equipped it was, I made a mental
note to pack as much as I could with
www.nysagd.org | Fall 2010 | GP 18
Treating a patient at the AIMER-HAITI Hospital.
what I had brought so that I would not be in short supply when
I began seeing patients.
That afternoon, we were given a tour of downtown Port-au-
Prince where the devastation from the earthquake was most
evident. To date, the reality of how powerful nature’s forces
can be were apparent. We drove amongst buildings and structures
that were ripped and torn apart. Some were tipped over
so perilously, a stiff wind would easily bring them down.
Government buildings and churches were not spared. The
most difficult part to observe was the numerous tent cities that
had sprung up in and around the capital. As we understood it,
the people needed help and in their minds what better place to
set up a tent than around a government building no matter how
destroyed it was?
GP: How was the working environment?
FM: At the AIMER-HAITI Hospital, I was basically treating
patients in a large outdoor hut. It was very windy and it rained
every day (No, the hut did not leak!). I treated patients who
were seated in a metal folding chair –I was standing, hunched
over them. Yes, I worked like this all day! There were no x-
Overcrowding in the capital.
rays and I had problems ordering the
reservoir for the Cavitron while in
Haiti. I was told the reservoir would
arrive in approximately 3 weeks but
that would have come after I was
gone.
At Eliazare Germain Hospital, I was
really looking forward to the luxury of
a dental chair and a professionally
trained dental assistant. But unfortunately,
the dental unit was inoperable
so mostly extractions were done there
as well.
GP: Sounds like the conditions were
pretty harsh. Please describe your
days at the mission – how many
patients did you see and what type of
procedures were you able to perform?
FM: The sun gets hot, even in
early mornings. So we started
seeing patients at 7:30 a.m. I
don’t know when the patients
started to line up but lines
were already long even before
we opened. The announcement
went out that there would
be a dentist providing services.
Patients came with various
requests from cleanings and
restorative treatment to extractions.
Most of them spoke
French-Creole so I was able to
communicate with them directly.
The majority of the days
were spent doing extractions.
All the treatments were rendered
under universal precautions
as well as with the use of
a head light. Needless to say,
doing extractions 8 to 10
hours a day standing in the
same position can take a physical
and mental toll on you. I
did not keep count as to how
many patients I saw but I
believe it was an average of
30 patients a day. One patient
blends into another and after a
while fatigue starts to play
tricks with your mind.
All the members of the team,
as well as the local volunteers,
were frantically involved in
their particular tasks.
Unfortunately, we did not have
time to interact with patients
on a personal level, let alone
each other. As the only dentist
there, I worked by myself. By
the end of each day, we were
all stiff, exhausted, and in pain,
just looking to put up our
swollen feet.
On the last day of the mission,
the patients have lined up
extremely early to be seen. The waiting area was filled to
capacity and we were not sure if we would be able to see
everyone. As usual, with diligence, patience, blood, and
sweat, we got through the day.
GP: Was anyone turned away?
FM: No one was turned away. I saw everyone.
Extraction being performed at Eliazare-Germain Hospital.
GP: What are your concerns and hopes for the future of
Haiti?
FM: On our way to the airport on our last day in Haiti, we
passed by one of the many tent cities strewn all across Port-au-
Prince since the earthquake. This lifestyle has become a distressing
way of life for many. It rained every day we were
there. How much shelter can a tent provide? Right before we
Patients waiting to be seen.
Tent cities have become an all too familiar site in Port-au-Prince, Haiti.
left, the public health professionals
had a chance to speak to the
people who lived in these tents.
The leaks, the mud, and lack of
privacy are but a few in a long list
of problems still facing the families
there.
My hopes are that after this year’s
presidential elections in Haiti,
there will be new leadership compelled
to do something to help
Haiti and her people.
GP: Do you plan on going on
another mission to Haiti? If so,
how can we help?
FM: Another mission to Haiti is
being planned for February and
June 2011 even as we speak.
There’s so much work to be done.
I know I have to go back and finish
what I started. Hopefully, by
then, Haiti will be in a better position
than what I left behind.
Every drop in the bucket matters.
As far as help is concerned, my
hope is that on the next medical
mission, I will not be the only
dentist present. Hopefully someone
reading this article will be
motivated to leave their comfort
zone and help out a fellow human
being in need thousands of miles
away. I’m optimistic that by the
next medical mission, I won’t be
the only dentist volunteering services.
Unfortunately, the following scenario
plays out year after year throughout the
entire dental community. It goes like this:
Years ago, you decided it was time to bring
an associate into your practice. You interviewed
candidates until you met the
“ideal” associate for your practice. Of
course, you didn’t know whether or not
“things would work out”, so you both
decided it would be best to work together
for a while without a formal contract. The
cost of a contract seemed unnecessary at
this point and anyway, you are both professionals,
what is there to worry about?
Everyone really liked this young doctor and
bringing in the new associate immediately
served to be a perfect boost to your practice.
The associate was quite comfortable with
the working arrangement. After all, he did
not have to go into debt to get a practice
started (like you once did). If the practice
failed to make money, he did not lose any of
his personal funds (like you could). No risk,
no pressure and no practice management
responsibility made this arrangement a perfect
situation for the associate. And since
you still owned your practice and you were
still “in control”, you were comfortable with
the arrangement as well.
Things could not have been better. You
began to take a little more time off without
losing money because of the profits you
were making off of the associate’s production.
Your associate was getting busier and
the practice was steadily growing. New
patient flow was increasing. The associate
was attracting patients from his generation
so younger patients were beginning to
come back into your practice. Your existing
patients really liked the associate a lot
and, for the most part, did not seem to
mind that you were not in the practice as
much as you used to be. It didn’t bother
you when you realized that you and your
staff were referring to your associate as
your partner, even though he really was not
a partner and you still “owned” the practice
and were “in control”.
As more time went by and the associate’s
speed improved, you began to take even
more time off. You took up golf and also
began to hunt and fish again. Appearing to
have unending energy, your associate easily
picked up the potential production drop-
THE PRACTICE GIVE-AWAY!
By Berdj Feredjian, DDS, FAGD and Katherine Feredjian
off that would have been created by your
decrease in clinical time.
Enjoying more free time and seeing the
practice continue to grow, you were even
more convinced that the associate was the
perfect addition to the practice. You were
very pleased with his attitude, his work
habits, his desire to learn, his overall treatment
of the patients and his great relationship
with the staff and the patients. You
became increasingly sure that the associate
was the right buyer for your practice. In
fact, it was even more than that. You were
out-and-out proud of your ability to select
such a great “heir apparent”.
For whatever reason (typically because of
informal past chats with your associate), one
day you decided that the timing was right to
have a discussion with your associate about
having a formal agreement drawn up that
would clarify your working relationship.
However, you were surprised to discover
that the associate was not enthusiastic about
the idea. He told you he felt that everything
was running smoothly and he loved it there,
so why bother with an expensive contract to
say what they both knew was destined to
happen anyway? He reassured you by
telling you that the two of you were on the
exact same page about him buying the practice
when you were ready to sell. You were
reasonably happy with this verbal commitment
and since everything was running
smoothly, you soon forgot about a written
contract. After all, why rock the boat? Your
clinical time was down and your income
was up; you still “owned” the practice and
you were “in control”.
What you did not realize, however, is that
you may have still own the equipment and
perhaps the building, but your associate
was gaining ownership over your practice!
The value of a practice is in the relationship
between the doctor and the patients,
not the equipment and facility. When
patients see two doctors working together,
they generally perceive the new doctor as
a “partner.” The associate is usually
younger than the practice owner is, so
patients think this new doctor will ultimately
take over their treatment when the
current owner retires. This perception is
probably even promoted by the associate
and perhaps even the staff as a means of
preparing the patients for your eventual
retirement.
Eventually you are sincerely considering
retirement and the time finally comes to
talk seriously to the associate about purchasing
your practice. You reveal to the
associate what you think a fair price and
terms should be. At this point in the relationship
you have a very rude awakening.
The associate tells you that he may be
interested in buying your practice, but only
on his terms and at his price. He goes on to
explain that he really would only be buying
your practice out of “the goodness of
his heart” anyway since he does not have
to buy your practice because of the following
reasons:
He already has a relationship with all his
patients and does not have to buy that part
of the practice (you can’t sell the associate
his own patient goodwill). These patients
don’t really know you and will follow the
associate to a new office.
He knows that if you should quit practicing,
many of your patients will transfer to
the associate if he opens a new office nearby.
He can simply send a notice to all the
patients of the practice, not just his own
(don’t be naive - the associate already has
a list of all the patients by now).
He realizes that it is very unlikely that anyone
else will buy your practice because
they will fear that the associate will move
down the street and take most of the
patients with him (and probably most of
the staff as well). The fact is your practice
has very little, if any value, to anyone
other than the associate.
He either knows or at least has a good idea
that the staff will follow him. They need
job security. They know you would like to
retire. They also know that if the associate
leaves, many patients will also leave.
Either your retirement would force them to
seek employment elsewhere. A severe loss
of patients would also suggest layoffs. The
future is uncertain. If the associate leaves,
they know they would have long-term job
security and a constant paycheck.
He realizes he is a prime position to “negotiate”
a great deal for himself. He will feel
that he can set the practice price and terms
HoW To TREAT DENTAL CARIES DISEASE
I recently attended a lecture given by
Dr. V. Kim Kutsch, founder and CEO of
the Oral BioTech Corporation, manufacturer
of CariFree caries control solutions.
His concept is that caries develop from
biofilm on teeth. Dr. Kutsch has developed
a full line of products (patent pending)
designed to reduce acidity in the oral
cavity, thereby selecting against the
microorganisms in the biofilm that flourish
in an acidic environment.
At the heart of the treatment protocol is
a hand-held device, the CariScreen, which
measures the adenosine tri-phosphate (ATP)
by bioluminescence, expressed as a
Relative Light Unit (RLU) as an indicator
of caries susceptibility. This device requires
that the dentist swab the lingual surface of
the un-brushed lower anterior teeth only,
avoiding the gingiva. The dentist then activates
the enzyme-filled swab, shakes it for
ten seconds, then inserts the swab into the
RLU meter to obtain a reading of the
patient’s caries susceptibility number.
The treatment protocol is to reduce the
susceptibility number, beginning with the
use of an Oral Neutralizing Gel applied with
a toothbrush. This product raises the pH to
basic levels and is completely non-abrasive.
It combines the benefits of xylitol with
CariFree’s patent-pending pH+ technology.
[It cannot legally be called “toothpaste”
because the FDA monograph for dentifrice
is acidic and abrasive.] This pH neutralizing
gel is basic and non-abrasive.
For severe cases, a pH neutralizing gel
called CariFree pHluorigel is recommended.
It combines the anti-caries benefit of
5,000 ppm of sodium fluoride with a therapeutic
dose of xylitol. This product contains
the most powerful anti-caries therapeutics
for an all-in-one, prescription
strength, cavity-fighting, powerhouse. It
may be used in a custom tray appliance for
increased exposure and effectiveness. This
product is similar to others presently available
commercially but without their acidity
and/or abrasiveness.
The patient is also instructed to rinse
twice daily for the first month with CariFree
Treatment Rinse, designed to treat the cariogenic
biofilm in plaque to reduce the overpopulation
of cariogenic bacteria and to neutralize
the oral pH. This product comes in
two bottles with a small cup for the patient
to mix 5 cc from each bottle. This product
contains sodium hypochlorite, which kills
the microorganisms in the biofilm, and
tastes like swimming pool water.
By Howard J. Kirschner, DDS
After the first month, the patient is recommended
to use CariFree Maintenance
Rinse, which does not require mixing.
This product contains xylitol and 0.05%
sodium fluoride to neutralize the acidic
oral pH. This rinse is available in citrus
and mint flavors.
The CariFree line also includes a xylitol-containing
spray called CariFree Boost
Mouth Spray. This spray changes the oral
pH to basic. It is to be used after meals,
with two or three sprays into the mouth. It
is especially effective with patients suffering
from xerostomia.
Xerostomia is a symptom caused by
over 3000 medications (list available on
CariFree website). The CariFree product
line includes Oral Neutralizer Gum, containing
xylitol and pH+ technology (patent
pending), to be used up to five times daily
in order to stimulate salivary flow, which
increases pH and promotes a healthy biofilm.
Dentists might consider this as an
alternative to prescribing Pilocarpine,
which may have possible drug interactions
with the patient’s other medications.
However, the use of the gum avoids these
potential medical complications.
The concept of caries disease treatment
requires that the dentist diagnose the major
contributing factor that is responsible for
one’s caries. The dentist determines patient’s
caries susceptibility by completing a Caries
Risk Assessment Form. This is known as
CAMBRA — Caries Management By Risk
Assessment. There are two forms available –
one for toddlers to age six and another for
older children and adults.
One of the primary risk factors for caries
is sipping sugary liquids throughout the day.
For example, when an athlete drinks an
“energy boosting” beverage or when a student
sips soda all day long. The acidic condition
that results promotes cariogenic bacteria
and leads to net mineral loss from the
enamel. It is recommended that dentists
apply CariFree fluoride varnish to the enamel
with periodic frequency, depending on the
degree of caries susceptibility. CariFree fluoride
varnish is a mixture of 5% sodium fluoride
in a resin carrier, dispensed in singleuse
dose applications. This same enamel
varnish also effectively treats patients with
cervical/root sensitivity or decay.
The CariFree Company also provides a
complete line of products specifically
designed for toddlers and pre-schoolers
under the age of 5 called CariFree Xyli-
Tots Littles Kit.
The kit includes the following:
(1) A box of Xyli-Tots Oral Wipes - Makes
it easy to clean on the go while creating a
healthy oral environment with xylitol and
elevated pH+ technology. These are available
in flavors of “Grape Giggles” or
“Watermelon Wiggles.”
(2) Flavored Xyli-Tots Mouth Spray - It is
xylitol-rich, neutralizes the mouth after
eating, and is safe to swallow.
(3) Xyli-Tots Lollies - Permit the toddler to
lick away decay with this 0.5 gram xylitol
lollipop, available in assorted flavors.
(4) Xyli-Tots Tooth Gel (toothpaste) - The
ultimate product for toddlers. It is grape
flavored and combines the proven benefits
of xylitol with the neutralizing pH+ technology.
It contains no fluoride and is
therefore safe to swallow.
The CariFree Xyli-Tots Juniors Kit contains
all of the above products except the
wipes but does contain Xyli-Tots Anti-cavity
Rinse. Designed to promote healthy bio-film
development and limit the growth of decaycausing
bacteria, it is available in Grape
Giggles and Watermelon Wiggles. It contains
3 grams of xylitol per swish and 0.05%
neutral sodium fluoride, with the pH+ technology
to neutralize cavity-causing acids.
A word of caution - Xylitol is dangerous
if ingested by animals/pets.
Dr. Kutsch reported in his lecture that
an ongoing, large-scale study in
Queensland, Australia is expected to produce
results supporting the effectiveness of
the CariFree treatment protocol for the
detection, treatment, and prevention of
dental caries. Dentists must think of caries
as a treatable disease. The CariFree
Company provides the armamentarium for
the dentist to do so.
“There is no dentistry better than no dentistry.”
THE PRACTICE GIVE-
AWAY…continued from page 22
(which will most likely be much, much
less than the value you had in mind) and
you will have no choice but to accept.
You can’t believe what is happening. You
are thinking how ungrateful and conniving
the associate is. But realistically, you
must understand the associate’s perspective.
The associate feels that you have
made a fortune all these years off of his
labor. All the associate’s friends, family
and advisors have told him that he should
leave you and open a new office. They tell
him you are being selfish and greedy for
trying to sell him the practice instead of
just turning the practice over to him after
all the service he gave you. In the associate’s
mind, he is not conniving or ungrateful;
he really believes he is right!
You are totally devastated, feeling
betrayed and absolutely reeling from the
eye-opening loss of hundreds of thousands
of dollars in practice equity that you
were counting on to retire. If you did a
good job saving for retirement, maybe
you overcome this. If not, you may be
forced to practice forever and rebuild a
practice at a time in your life when retirement
was foremost on your mind.
As previously stated, this scenario happens
year after year. It happens to unsuspecting
doctors as well as those who were
made aware of the risks beforehand. But
for some reason, no one believes it will
happen to him or her. Would you like a
better scenario for you and your practice?
There is a good way to bring on your “heir
apparent.”
From the Education Manager
Fellowship and Mastership: The Road to
Excellence
The current economy has impacted the profession
of dentistry and dentists around the country are
struggling to make end meets. A good way to stay
competitive is to keep current on the latest techniques
and procedures that have streamlined many
of the old ways dentistry has been practiced.
The Academy has long been a good source to help
our members achieve the highest level of education
so that even through the tough times, they would be able to find innovative
ways to keep things moving along.
One of the greatest assets the Academy offers its members are the
Fellowship and Mastership Awards. These awards are professional designations
within the AGD that reflects a general dentist’s ongoing commitment
to provide quality care through continuing education. Here is a
brief overview of each designation.
Fellowship Requirements
[1] Current AGD membership for three continuous years by December
31 of the year of the convocation for which application is made, to begin
no earlier than the month of dental school graduation.
[2] A dental license that has not been suspended or revoked within the last
five years and is not currently under probation, suspension, or revocation.
[3] 500 hours of approved continuing education.
[4] Passing the Fellowship Exam.
[5] Attendance at an AGD convocation ceremony, held during an AGD
annual meeting, to receive the award. Successful candidates are allowed
three years following approval to complete this requirement.
If you would purchase this as a stand-alone product, you would
have to pay up to $5000.00. But the Academy provides this as
one of the perks of your membership dues. For complete information,
go to www.AGD.org and search for “Patient Alerts”. You
will be glad you did.
Professional Liability Insurance - In today’s world, you know this
is something you can not operate without. Dentist’s Advantage is
now even more competitive when compared to other companies
out in the market. You owe it to yourself to get a quick quote to
see where you stand and you would be surprised that it will actually
pay to switch! You comparison shop all the time - why not
do it for the protection you need. Just go to www.AGD.org and
search for “Dentist’s Advantage”. You will be glad you did.
Career Center - If you are a recent graduate or finishing your residency
program, the Academy offers a great Career Center to help
you jump start to practice. Place your resume on the web for free
and search for open positions around the country. Also, check out
the NYSAGD’s Transition to Practice which you will find on our
web site, www.nysagd.org, giving you great ideas as you move
forward - from job search to marketing and licensing in New York
State. You will be glad, once again, you did.
There are many other benefits just waiting for you to take advantage
of. Don’t wait - go to either the AGD or NYSAGD website
to see how you can save money today!
Mastertrack
New York State is proud to be one the AGD Constituents to offer our
members the opportunity to participate in the Mastertrack program. The
program is a series of lectures, participations, and/or on-site/in-office
protocol courses. A protocol course is a combination of on-site and inoffice
courses. They are typically hands-on work with models, devices,
and patients. The program concludes with a presentation of case studies
by each participant to his/her peers. Every year since 2002, our local
Mastertrack Committee sought to find the best clinicians around the
country to bring to New York to ensure that the attendees get the most upto-date
information on a particular subject.
For additional requirements, please visit the AGD web site at
www.agd.org. For information on our upcoming fall Masters Program,
you can visit our web site at www.nysagd.org.
I hope our members view these awards as valuable assets in their dental
profession. Who wouldn’t want to become a Master in their chosen field
and have the advanced knowledge needed to better serve and deliver?
