A Helping Hand by Seung-Hee Rhee, DDS, FAGD<br />

It is Saturday, July 10,<br />

2010. The place is <strong>New</strong><br />

Orleans, Louisiana. It<br />

is 6:00 in the morning and<br />

already it is sweltering.<br />

The sun is barely up but<br />

you can already feel the<br />

heavy moisture in the air<br />

and the beads <strong>of</strong> sweat<br />

ready to race down your back. It is going to be<br />

another hot day.<br />

In this unfavorable condition, a race is about to<br />

start. It’s the AGD Foundation 5k Fun Run/Walk<br />

(that’s about 3.3 miles to you and me). It is an<br />

annual charity event where participants enjoy a run,<br />

a jog or a walk while supporting the<br />

AGD Foundation in its mission to advance the oral<br />

health <strong>of</strong> the public and support the efforts <strong>of</strong> general<br />

dentists. About 150 people – runners, volunteers,<br />

supporters – gathered not too far from the<br />

banks <strong>of</strong> the mighty Mississippi River for this<br />

event. You see the young and old, fit and not-s<strong>of</strong>it,<br />

smiling faces all around. Some are anxious to<br />

start. Others are ready to enjoy a leisurely run at<br />

their own pace. And they are <strong>of</strong>f! It is a double<br />

loop run which means they will run along the banks<br />

<strong>of</strong> Mississippi, make a turn, run back towards the<br />

finishing line, and repeat. As predicted, the conditions<br />

are grueling. After the first loop, all the runners<br />

are covered in sweat. Everyone grabs a cup <strong>of</strong><br />

water from the watering stand and also gets cheers<br />

and encouragements as a bonus. But in 13 minutes<br />

and 46 seconds, we have our first winner.<br />

Predictably, it is one <strong>of</strong> our military personnel. Dr.<br />

Loren Masuoka <strong>of</strong> Army AGD takes first place.<br />

The first female to cross, Dr. Chirsty Kirk, also<br />

takes third place overall at 14.42.<br />

Then something wonderful happened. Many<br />

who already finished the race, instead <strong>of</strong> resting<br />

or replenishing, went back into the race despite<br />

the horrid conditions. Some young men and<br />

women went back to help their friends sprint to<br />

the finish to make better time, yelling encouragements<br />

like drill sergeants. Some went back to run<br />

alongside their family and friends. Others went<br />

back to lend their support, both physically and<br />

mentally, to those who may not be as fast as they<br />

once used to be. A smile spread across a struggling,<br />

elderly gentleman’s face as a young colleague<br />

came alongside to run the last mile with<br />

him. As the two men ran together, their steps<br />

seem lighter than it was just a few moments<br />

before. Things are always a little easier with a<br />

friend by your side. And one by one, they all<br />

cross the finish line.<br />

Flash back a few days. The place is a large<br />

ballroom at Hilton Riverside Hotel that has been<br />

converted to host the House <strong>of</strong> Delegates for the<br />

AGD. The HOD is in full swing and a delegate<br />

from NY is nervously standing in line at one <strong>of</strong><br />

the microphones for a chance to speak. Although<br />

it is not her first time at the HOD, it is the first<br />

time she got up the nerve to speak in front <strong>of</strong> over<br />

250 representatives <strong>of</strong> the AGD. Not many people<br />

knew but she was terrified <strong>of</strong> public speaking.<br />

She almost lost courage and went back to her<br />

seat. But then she remembered the previous day,<br />

as she sat nervously at the head table as a first<br />

time member <strong>of</strong> the Reference Committee,<br />

addressing the issues at hand with hundred pairs<br />

<strong>of</strong> eyes staring at her. How as her name was<br />

introduced, her colleagues from NY gave a loud<br />

cheer in support, letting her know that there are<br />

friends out there. She also remembered an advice<br />

given to her by Jay Orlik<strong>of</strong>f, Past President <strong>of</strong><br />

NYSAGD. When she confided in him about her<br />

fear <strong>of</strong> public speaking, he told her what Jerry<br />

Seinfeld said on the subject. According to<br />

Seinfeld, more people are afraid <strong>of</strong> public speaking<br />

than they are <strong>of</strong> dying (that did not help her).<br />

So it is more terrifying to deliver the eulogy than<br />

to be the subject <strong>of</strong> the eulogy (that made her<br />

laugh). Jay advised her to practice and also to<br />

select a friend in the audience on the left side and<br />

a friend on the right side and speak directly to<br />

them. He said that it was the same advice given<br />

to him by Howard Glazer, Past President <strong>of</strong><br />

AGD/NYSAGD. He also told her that if she is<br />

ever scared to speak at a meeting, he would be<br />

honored if she chose him to be the one she looked at.<br />

As she remembered these things, she smiled<br />

and stepped up to the microphone and had her say<br />

in front <strong>of</strong> 250 people. Despite her nervousness,<br />

she noted that many people clapped in support <strong>of</strong><br />

what she had to say and that yes, there were<br />

indeed many friends among the crowd.<br />

My mom used to tell me that if our hands<br />

were as hard working as our eyes, we would get<br />

more things done. She is right – our eyes do see<br />

everything but our hands don’t do everything that<br />

they should. But it is the hands that move the<br />

world. It is the hands that lend support to a friend<br />

in need. It is the hands that wipe away tears and<br />

fix scraped knees on a child. It is the hands that<br />

clapped and typed an email that lent courage for<br />

a dentist to get over her fear <strong>of</strong> public speaking.<br />

It is the hands <strong>of</strong> Dr. Fabiola Milord that helped<br />

relieve so much pain and suffering in Haiti as you<br />

will read in this issue <strong>of</strong> <strong>GP</strong>. As dentists, we are<br />

frequently thanked by our patients for helping<br />

them. But too <strong>of</strong>ten, we in turn forget to thank<br />

those who help us in our every day, to make what<br />

we do possible and lend us a helping hand, even<br />

without us asking them to. They are our families,<br />

our loved ones, our hard working staff. They are<br />

our dedicated members <strong>of</strong> the NYSAGD. Let us<br />

remember to take the time to say “thank you” to<br />

all those wonderful helping hands that guide and<br />

support us each and every day. And Jay, thanks<br />

- it worked!<br />

“<br />

I’ll huff and I’ll puff and I’ll blow this<br />

house down!”<br />

Each <strong>of</strong> us, on our own, has little hope <strong>of</strong><br />

duplicating this feat by the big, bad wolf. Come<br />

to think <strong>of</strong> it, if the big, bad wolf were around<br />

today, he would probably need to apply for a<br />

demolition permit, submit architectural drawings,<br />

conduct environmental impact studies, and hire union workers.<br />

Can one person knock down a brick wall, no matter how badly it is<br />

constructed, all by himself? The dents on my forehead prove how<br />

futile this can be, but that doesn’t stop me from trying. It is true that<br />

one person can make a difference. But that difference tends to be a<br />

direct result <strong>of</strong> how that one person inspires others.<br />

How many obstacles do dentists face in the pursuit <strong>of</strong> their pr<strong>of</strong>ession?<br />

Not only do we face our daily barriers, but there are others<br />

inside and outside the pr<strong>of</strong>ession who continually heap additional<br />

obstacles in our way. There are insurance companies, legislators,<br />

lawyers, dental manufacturers, and a multitude <strong>of</strong> others who want a<br />

piece <strong>of</strong> us and regularly test our limits. All these groups have a great<br />

deal more power and money to wield than any one <strong>of</strong> us. If we all<br />

sat in our <strong>of</strong>fices and ignored anything not immediately relevant, we<br />

would get slammed. If you stick your head in the sand like an<br />

ostrich, you stand a real good chance <strong>of</strong> getting “goosed”. All too<br />

<strong>of</strong>ten people think, “If I don’t do it, somebody else will”. Guess<br />

what? That phrase should read, “If I don’t do it, nobody else will”.<br />

This is just the way things are. Sometimes we get lucky and others<br />

pick up the slack. But unfortunately, it doesn’t happen every time.<br />

The one ace-in-the-hole that we have is cohesiveness. I am<br />

happy to say that the NYSAGD and the NYSDA are developing a<br />

fine working relationship. I was honored to have the President<br />

and the Executive Director <strong>of</strong> NYSDA, Drs. Robert Doherty and<br />

Mark Feldman, attend the NYSAGD Installation Dinner. In turn,<br />

I attended the NYSDA House <strong>of</strong> Delegates and Installation<br />

Dinner. Both were great events that have fostered cooperative<br />

relationship between our two organizations.<br />

Membership in a pr<strong>of</strong>essional association should be examined<br />

for both short and long term gains by the individual dentist. I view<br />

the AGD as “A Player”, as Howard Farran might say. If you don’t,<br />

let’s review the following:<br />

You know that the NYSAGD and the AGD <strong>of</strong>fer numerous CE<br />

courses <strong>of</strong> high quality and low cost. For example, there is the<br />

Hazy, Hot, and Humid!<br />

That’s what it was like in <strong>New</strong> Orleans this<br />

summer for the AGD Annual Meeting. But it<br />

was also HOT with excitement in the <strong>New</strong><br />

Orleans Convention Center! There was such a<br />

multitude <strong>of</strong> courses, many <strong>of</strong> which were<br />

hands-on participation courses, that if you<br />

couldn’t find something that you were interested<br />

in, you probably weren’t looking.<br />

This was the first time that the AGD Annual Meeting and<br />

Exhibits was held without the governance occurring at the same<br />

time. The House <strong>of</strong> Delegates finished its business early Thursday<br />

wonderful selection <strong>of</strong> CE courses put together by the NYSAGD<br />

CE Committee. There is also the AGD Annual Meeting which was<br />

held in <strong>New</strong> Orleans this past July (San Diego next year) where<br />

there are high caliber hands-on courses and speakers. The AGD<br />

website also has an extensive CE database that lists courses from<br />

all over the US at any given day and also list free courses - just for<br />

our members. The AGD can also keep track <strong>of</strong> your CE credits in<br />

a transcript that can be used as pro<strong>of</strong> <strong>of</strong> CE requirement for licensure<br />

and in the event <strong>of</strong> an audit. The NYSAGD is happy to say<br />

that after long negotiations, the NYS Dental Board chose to recognize<br />

our transcript as acceptable documentation <strong>of</strong> CE credits.<br />

The AGD also worked diligently to develop a practice management<br />

service called “AGD Patient Alerts”. It is a free service that<br />

ties-in with your current practice management s<strong>of</strong>tware to allow you<br />

to send e-mails, text messages, electronic newsletters (easy to set-up<br />

and customize), and other reminders to patients. In addition, there is<br />

a consumer friendly portal called “KnowYourTeeth.com” with its<br />

wide range <strong>of</strong> pr<strong>of</strong>essionally produced patient fact sheets, a place for<br />

consumers to ask dental questions, and a useful Widget. What’s a<br />

Widget? You will just have to visit to find out.<br />

In addition, the AGD is heavily involved in Advocacy for you<br />

and your patients. A number <strong>of</strong> different states are evaluating<br />

and/or approving the DHAT model (Dental Health Aid<br />

Technician) or the ADH model (Advance Dental Hygienist). Your<br />

awareness <strong>of</strong> this issue is critically important. Fact is, the ADA is<br />

currently testing the DHAT model and the Connecticut Dental<br />

Association just released a statement saying that they support the<br />

testing <strong>of</strong> the DHAT model. It is critical that the AGD make the<br />

opposing views known. The AGD usually supports the ADA policies<br />

except where differences exist on how a general dentist may<br />

be affected. If you are not familiar with the issues that the AGD<br />

has advocated in the past or the current issues we are addressing<br />

on your behalf, please take a moment to know what is going on in<br />

your pr<strong>of</strong>ession. It is in your best interest.<br />

There is so much more, but I leave it to you to look further. If<br />

you have read this far, perhaps you might go further. If not, perhaps<br />

you might be willing to make a leap <strong>of</strong> faith and take what I<br />

have said at face value. Just know that you are important to the<br />

pr<strong>of</strong>ession - how it is today and how it will be tomorrow. Don’t<br />

underestimate what you gain by being a member <strong>of</strong> the AGD. If<br />

you have been affected by the economy, as many <strong>of</strong> us have been,<br />

and are making hard financial choices, AGD membership should<br />

not be one <strong>of</strong> the items on your list <strong>of</strong> “unnecessary expenses”.<br />

Remember – one person can make a difference.<br />

afternoon. By Thursday evening, the Exhibit Hall was open for<br />

business and the first full day <strong>of</strong> dental courses had come to a close.<br />

In addition, this was the first time AGD sponsored an Outreach<br />

Program to help those in need <strong>of</strong> dental services. It was an incredible<br />

success. Those who were involved came back exhausted but<br />

exhilarated by the good that they had done for so many.<br />

The Saturday Night event this year was held at Mardi Gras World<br />

and it was a spectacular event. The setting was an indoor antebellumlike<br />

plantation with a star filled sky that looked as if we were outdoors.<br />

The addition <strong>of</strong> Mardi Gras floats, the wonderful food and<br />

music made this event a wonderful ending to a fabulous week.<br />

As always, I invite you to become more active and be a part<br />

<strong>of</strong> the <strong>Academy</strong>. Take advantage <strong>of</strong> the many benefits and services<br />

available to you as a member <strong>of</strong> the AGD. Check out the CE<br />

Database and the AGD Career Center. They are but two <strong>of</strong> the<br />

many benefits available to you as a member <strong>of</strong> the <strong>Academy</strong>.<br />

At the AGD Annual Meeting 2010 in<br />

<strong>New</strong> Orleans this past July, it was my<br />

honor and privilege to receive the AGD<br />

2010 Membershp Award for the <strong>New</strong> <strong>York</strong><br />

<strong>State</strong> <strong>Academy</strong> <strong>of</strong> <strong>General</strong> <strong>Dentistry</strong>. The<br />

Award was presented by AGD President, Dr.<br />

David Halpern, DMD, FAGD.<br />

I was also proud to be a part in receiving<br />

the 2010 AGD Advocacy Award along with Dr.<br />

Janice Pliszczak, Dr. Ira Levine, Dr. Nathan Hershkowitz and<br />

other delegates from NYS. On a personal note, I was also proud<br />

<strong>of</strong> myself on receiving the Award as one <strong>of</strong> the Top Recruiters in<br />

the Nation for AGD in 2009- 2010. Dr. Judy Ann Taylor was also<br />

honored with the Third Place in Grassroots Member Recruiter<br />

Category. All in all, <strong>New</strong> <strong>York</strong> <strong>State</strong> <strong>Academy</strong> <strong>of</strong> <strong>General</strong><br />

<strong>Dentistry</strong> reaffirmed its status as a voice <strong>of</strong> <strong>General</strong> <strong>Dentistry</strong>.<br />

This is a time to celebrate our accomplishments. All this happened<br />

because <strong>of</strong> the wonderful members <strong>of</strong> the Executive<br />

Council, Board <strong>of</strong> Trustee, and <strong>New</strong> <strong>York</strong> <strong>State</strong> AGD staff by their<br />

dedication, volunteerism, and commitment to do better service to<br />

our public, our pr<strong>of</strong>ession and our beloved <strong>Academy</strong>.<br />

I commend our leaders’ unwavering commitment to our mutual<br />

goal <strong>of</strong> “Advancing the value and excellence <strong>of</strong> <strong>General</strong><br />

<strong>Dentistry</strong>”. Throughout my leadership position in NYSAGD, I<br />

How was your 4th <strong>of</strong> July holiday?<br />

Normally the July 4th holiday is spent<br />

with family and friends at a barbeque, at the<br />

beach, or at a park. I spent my July 4th holiday<br />

in <strong>New</strong> Orleans, with my wife, prior to<br />

the AGD annual meeting.<br />

On Saturday, we took a city bus tour and<br />

on Sunday, we went to brunch with our friends at Commander’s<br />

Palace, one <strong>of</strong> <strong>New</strong> Orleans’ finer restaurants. Then we took a<br />

walking tour <strong>of</strong> the Garden District, the neighborhood where the<br />

restaurant was located, and the homes were magnificent.<br />

Now you may be wondering why I am telling you this. Well,<br />

that weekend was a prelude to the week spent in <strong>New</strong> Orleans at<br />

the AGD Annual Meeting. The first three days <strong>of</strong> the meeting<br />

were spent in governance, such as the Regional Directors Meeting<br />

and the House <strong>of</strong> Delegates. Business is conducted during these<br />

meeting but also in the hallways and in the evening at the receptions<br />

and the dinners. And let me emphasize the dinners - the<br />

restaurants in <strong>New</strong> Orleans are terrific.<br />

met people with exceptional talents and dedicated leaders who<br />

devote their precious free time for the sake <strong>of</strong> pr<strong>of</strong>essional excellence<br />

such as Dr. Stanley Markovits, Dr. Stephen Akseizer, Dr.<br />

Kay Oen, Dr. Lawrence Bailey, Dr. Nathan Hershkowitz, and Dr.<br />

Janice Pliszczak. Their encouragement and support always<br />

inspired me.<br />

As a leader <strong>of</strong> the organization, my philosophy has always<br />

been “Members First”. Members are the heart <strong>of</strong> the organization<br />

and leaders should hear the voice <strong>of</strong> its members and reach out to<br />

non-members as well.<br />

I thank the Nominating Committee who gave me the opportunity<br />

to serve the NYSAGD over the years. My special thanks go<br />

to my friend and Past President <strong>of</strong> NYSAGD, Dr. Nathan<br />

Hershkowitz, for his support. Under his Chairmanship <strong>of</strong> the<br />

Nominating Committee, I was chosen to be the President-Elect <strong>of</strong><br />

NYSAGD. I also want to thank the other members <strong>of</strong> the<br />

Nominating Committee for their support and for giving me the<br />

opportunity to serve the <strong>New</strong> <strong>York</strong> <strong>State</strong> <strong>Academy</strong> <strong>of</strong> <strong>General</strong><br />

<strong>Dentistry</strong>.<br />

Finally, I congratulate all the members <strong>of</strong> the BOT, the<br />

Executive Council and the NYSAGD Staff for receiving 2010<br />

AGD Awards in Advocacy, Membership, and Honorable Mention<br />

in CE Award <strong>of</strong> Excellence, for their selfless service and for dedicating<br />

their priceless time to bring such honor and distinction to<br />

all the AGD dentists <strong>of</strong> <strong>New</strong> <strong>York</strong> <strong>State</strong>.<br />

After the conclusion <strong>of</strong> the governance sessions, I stayed in<br />

<strong>New</strong> Orleans to take some continuing education courses. Over the<br />

years, I have satisfied my CE requirements for NY and the AGD<br />

by taking CE courses given by the AGD and the NYSAGD. There<br />

are many CE organizations in the dental market now as opposed<br />

to years ago and I have taken courses not given by an AGD group.<br />

But I always try to take an AGD, PACE approved provider course<br />

so that the credits will go towards my MAGD, just as it did for my<br />

FAGD.<br />

Next year, the AGD Annual Meeting will be held in San Diego<br />

prior to the July 4th holiday. It will be June 26 - 28, 2011. San<br />

Diego is one <strong>of</strong> the nicest places on this planet. The weather is<br />

always beautiful. You may think it’s too early to think about next<br />

year. You may be right. But put the dates on your calendar and<br />

think about it in early spring 2011.<br />

I hope to see you in San Diego. Maybe we’ll sit next to each<br />

other at one <strong>of</strong> the courses. Or maybe I’ll see you at a NYSAGD<br />

continuing education course. And thank you for your membership<br />

in the AGD and your continued support <strong>of</strong> this fine organization.<br />

I have been given the honor <strong>of</strong> presenting my partner, mentor, and father, Dr. Stanley<br />

Markovits, DDS, MAGD, with this year’s NYSAGD Meritorious Service Award. Though<br />

it was about four years ago, it seems like yesterday that I was standing before this academy<br />

presenting dad with the NYSAGD’s Life Time Achievement Award. Having<br />

received a Life Time Achievement Award, we all thought he’d take the hint and slow<br />

down. Well that didn’t happen. Though he did stop clinical practice over four years<br />

ago, we, his family and those <strong>of</strong> you who know him, know that we NEVER use the<br />

“R” word (retirement) when describing his current status. Why would we? The<br />

fact is he hasn’t. He continues his work as a teacher, lecturer, writer, community<br />

organizer, and <strong>of</strong> course as consigliere to many AGD councils and its members.<br />

The NYSAGD’s Meritorious Service Award recognizes a member <strong>of</strong> the den-<br />

tal community who has consistently<br />

served the NYSAGD by working<br />

to enhance the image <strong>of</strong> general<br />

dentistry through their leadership<br />

within the <strong>Academy</strong> and through<br />

areas <strong>of</strong> civic involvement.<br />

Stan has been a NYSAGD board<br />

member since 1962 when he was<br />

appointed by then NYSAGD president,<br />

Dr. George Kudler, who founded the<br />

NYSAGD. Even in his executive role as past<br />

president <strong>of</strong> both the state and national organization,<br />

he never lost sight <strong>of</strong> this academy’s mission which is to foster, support,<br />

and reward the continuing education <strong>of</strong> the general dentist and by doing so<br />

improve the quality <strong>of</strong> care we bring to our patients. Together with my mother,<br />

Audrey, they have served this pr<strong>of</strong>ession and this academy for more than<br />

half a century with humility, dignity, honesty, and compassion.<br />

He is a Fellow <strong>of</strong> the American College <strong>of</strong> <strong>Dentistry</strong>, the International<br />

College <strong>of</strong> <strong>Dentistry</strong>, the International <strong>Academy</strong> <strong>of</strong> <strong>Dentistry</strong>, the <strong>Academy</strong> <strong>of</strong><br />

Continuing Education, and <strong>of</strong> course, the AGD in which he also earned his<br />

Mastership <strong>of</strong> which I know he is most proud. The NYSAGD and AGD had<br />

previously recognized him with “The Distinguished Service Award”, “The<br />

Meritorious Service Award”, and “The Thaddeus V. Weclew Award”.<br />

Stan is active in local civic organizational work as well. A member <strong>of</strong> the<br />

White Plains Conservation Board, President <strong>of</strong> the North Street Civic<br />

Association, and VP <strong>of</strong> the Glen Brook Homeowners Association, he continues<br />

to contribute to his community in ways that may not affect the dental pr<strong>of</strong>ession<br />

directly but serve as an example to others what this academy and its<br />

members stand for.<br />

It is with great honor and love that I present on behalf <strong>of</strong> the <strong>New</strong> <strong>York</strong><br />

<strong>State</strong> <strong>Academy</strong> <strong>of</strong> <strong>General</strong> <strong>Dentistry</strong>, this year’s Meritorious Service Award to<br />

Lifelong Learning and Service Recognition (LLSR) is given to those Masters<br />

Advocacy in <strong>New</strong> <strong>York</strong> <strong>State</strong><br />

The <strong>Academy</strong> <strong>of</strong> <strong>General</strong> <strong>Dentistry</strong> (AGD) makes sure that general dentists<br />

can speak up when it matters the most. That means monitoring the issues<br />

and relaying the facts so that our members can unite their voices on legislative<br />

and regulatory activities that affect their practices. Read on to find<br />

out what’s happening in <strong>New</strong> <strong>York</strong> <strong>State</strong>.<br />

<strong>New</strong> Bills in <strong>New</strong> <strong>York</strong><br />

On April 13, 2010, the <strong>New</strong> <strong>York</strong> legislature introduced two bills that, if passed,<br />

will affect the dental team. The first, AB 10638, authorizes a limited permit to<br />

hygienists to practice dental hygiene for a period <strong>of</strong> one year, although it may<br />

be renewed for an additional year. The bill requires that hygienists practice<br />

only under the personal supervision <strong>of</strong> a licensed dentist. The second bill, AB<br />

10649, would require quality standards for dental prosthetic devices and for<br />

disclosure <strong>of</strong> the manufacturing location <strong>of</strong> such devices.<br />

Although Align Technology Inc. announced in April that it has dropped the<br />

patient case start quota from its 2010 Invisalign Pr<strong>of</strong>iciency Requirements,<br />

the <strong>New</strong> <strong>York</strong> legislature is not ending its fight against manufacture-enforced<br />

quotas. On April 26, 2010, SB 7614 was introduced to prohibit manufacturers<br />

from enforcing patient quotas on dentists for selected dental treatments.<br />

Preventing Early Childhood Tooth Decay<br />

A bill recently introduced in <strong>New</strong> <strong>York</strong> could help dentists in the fight against<br />

tooth decay in young patients. SB 8014, introduced on May 28, 2010,<br />

requires manufacturers or importers <strong>of</strong> baby bottles and sippy cups to provide<br />

a label to warn about the potential for early childhood tooth decay from<br />

such products.<br />

Hope for Haiti - Making a Difference One Tooth at a Time<br />

Interview with Fabiola Milord, DDS, FAGD<br />

On January 12, 2010 at 4:53 p.m., a catastrophic 7.0 magnitude earthquake struck the small Caribbean nation <strong>of</strong> Haiti on the island<br />

<strong>of</strong> Hispaniola. The epicenter <strong>of</strong> the earthquake was near the town <strong>of</strong> Léogâne, which is approximately 16 miles west <strong>of</strong> Port-au-Prince,<br />

the capital <strong>of</strong> Haiti. The earthquake and its subsequent aftershocks (at least 52 measuring 4.5 or greater in the 12 days following the<br />

earthquake), completely devastated Port-au-Prince and areas surrounding the epicenter, affecting an estimated 3 million people.<br />

Recorded as the 6th deadliest earthquake, its casualties include 230,000 dead and over 300,000 injured. An estimated 250,000 homes<br />

and 30,000 commercial buildings were also destroyed.<br />

Humanitarian aid poured in from all over the world in the aftermath. But with severe damages in communications, transportation,<br />

and electrical systems from the earthquake, the rescue and relief efforts are severely hindered. Tens <strong>of</strong> thousands had to be buried in<br />

mass graves and over a million people were left homeless and without resources. Rescue efforts, medical care, sanitation, and supplies<br />

for the millions remain a priority.<br />

Many from around the globe answered Haiti’s call for help. One <strong>of</strong> our own, Dr. Fabiola Milord, a grass-roots member <strong>of</strong> the<br />

NYSAGD, who practices in Freeport, Long Island, NY heard the call and gave a helping hand. This past June, Dr. Milord was a member<br />

<strong>of</strong> a medical mission that went down to Haiti. <strong>GP</strong> had a chance to talk to Dr. Milord after her mission.<br />

<strong>GP</strong>: Dr. Milord, thank you for taking the time to speak to <strong>GP</strong>.<br />

Could you tell us some background information regarding your<br />

mission to Haiti?<br />

FM: Thank you for giving me the opportunity. Our mission<br />

was from June 18 – June 26, 2010. It was a medical mission<br />

to Port-au-Prince, Haiti with NOAH (National Organization for<br />

the Advancement <strong>of</strong> Haitians) NY’s Stanley Barbot Medical<br />

Mission. Our team members included a pediatrician from<br />

Howard University in Washington DC, five students from<br />

Howard University Medical School, four public health pr<strong>of</strong>essionals,<br />

a photographer/videographer, and me as the lone dentist<br />

<strong>of</strong> the group. Another group EMPACT/DRNW (Disaster<br />

Response Northwest) from Tacoma, Washington joined us later<br />

at one <strong>of</strong> the mission sites.<br />

<strong>GP</strong>: Could you tell us a little about yourself?<br />

FM: Both my parents were Haitian (now deceased) but I was<br />

an international level. I am an active member <strong>of</strong> organized<br />

dentistry – ADA, NYSDA, and the Long Island <strong>Academy</strong> <strong>of</strong><br />

Odontology. I am also a Fellow <strong>of</strong> the <strong>Academy</strong> <strong>of</strong> <strong>General</strong>


<strong>Dentistry</strong> and sit on the Board <strong>of</strong> Directors for Nassau County<br />

Dental Society.<br />

<strong>GP</strong>: Those are some impressive credentials! So what influenced<br />

your decision to go on a mission to Haiti? Did you need<br />

to take any special precautions medically prior to your mission?<br />

FM: Through the Stanley Barbot Medical Mission and various<br />

other organizations, I’ve participated in over twenty medical<br />

missions to countries such as Haiti, Jamaica, South Africa,<br />

Ghana, Guyana, St. Vincent’s, St. Lucia, and the Lower Ninth<br />

Ward in <strong>New</strong> Orleans after Hurricane Katrina. Since both <strong>of</strong><br />

my parents were Haitian and I still have family members there,<br />

it only seemed natural for me to contribute in any way I could.<br />

All <strong>of</strong> my vaccines were up-to-date since I have been on many<br />

previous medical missions. I did take a bottle <strong>of</strong> Noroxin<br />

antibiotics previously prescribed to me to stop diarrhea. I also<br />

took a box <strong>of</strong> Imodium as a preventive measure. Thankfully, I<br />

did not have to use any <strong>of</strong> them.<br />

<strong>GP</strong>: What was your first impression <strong>of</strong> the devastation in<br />

Haiti?<br />

FM: From the air, as the plane was descending, we noticed<br />

myriads <strong>of</strong> blue dots on the landscape which turned out to be<br />

tarps that were used to make up the tents for the tent cities that<br />

were now peoples’ homes after the devastating January 12th<br />

earthquake.<br />

We landed on Aeoport Toussaint L’Ouverture in Port-au-Prince<br />

and the damage from the earthquake was immediately evident.<br />

The airport itself was closed (deemed unsafe due to structural<br />

damage) so a shuttle bus took us to a hangar where we were to<br />

clear customs and retrieve our luggage. Needless to say, since<br />

this was a hangar, there was no ventilation and the air was stiflingly<br />

hot. Proper documentations were presented to Haitian<br />

customs agents by the Medical Director <strong>of</strong> the mission, Henri<br />

R. Paul, MD, allowing us to enter the country with Lidocaine<br />

which we brought to perform dental procedures. We had about<br />

30 boxes <strong>of</strong> Lidocaine – 10 were purchased and the rest were<br />

donated by Dorcas Medical Mission and a dentist from SUNY<br />

Downstate Medical Center. All other medications had been<br />

shipped prior to our arrival and were waiting for us at the site<br />

where we were going to work.<br />

<strong>GP</strong>: How were your accommodations during the mission?<br />

FM: All the hotels were fully booked so we stayed in a rented<br />

home which was welcoming and comfortable. We were fortunate.<br />

The group <strong>of</strong> EMTs and nurses that joined us from<br />

Tacoma was unable to get accommodations and had to sleep in<br />

tents at the field hospital where we worked.<br />

<strong>GP</strong>: Could you tell us about your mission?<br />

FM: We arrived on Friday, June 18 so the weekend was spent<br />

unpacking, organizing, setting up equipment, supplies, and laying<br />

out instruments in anticipation <strong>of</strong> Monday’s busy work<br />

schedule. This was all crucial to saving time. We were set to<br />

work at two sites. The primary site was AIMER-HAITI<br />

Hospital which is a privately run facility set up at an abandoned<br />

amusement park converted into a field hospital after the<br />

earthquake. The secondary site was Eliazare Germain Hospital<br />

- a state run hospital in a section <strong>of</strong> Haiti known as Petionville.<br />

The field hospital was staffed by a local physician. I was to be<br />

the first dentist to see patients there.<br />

We visited AIMER-HAITI Hospital on Saturday and were<br />

greeted and given a tour by the hospital’s administrator. We<br />

were shown the waiting areas, the tent where general surgeries<br />

take place, where patients are seen on an outpatient basis, and<br />

the pharmacy. Patients have no out <strong>of</strong> pocket expense. She<br />

thanked us for coming and as a small token <strong>of</strong> appreciation for<br />

our services; she told us that we would be served a hot lunch<br />

every day.<br />

<strong>GP</strong>: What was on the menu?<br />

FM: Stewed peas, rice, and chicken. It was delicious.<br />

<strong>GP</strong>: Please continue.<br />

FM: I asked the administrator where I would be working. She<br />

<strong>of</strong>fered me space under a large open hut (no walls or doors). I<br />

was assigned a couple <strong>of</strong> local volunteers to help me with set<br />

up, registration, and sterilization. We unpacked bags and organized<br />

ourselves for Monday. The set up would stay as is until<br />

then. We were assured that there was 24-hour security at the<br />

hospital and that all we had to do was cover everything. We<br />

blanketed the table with a huge tarp and I left knowing that I<br />

would be ready to go come Monday morning.<br />

On Sunday, Dr. Paul, the pediatrician, and I set out to see<br />

where we would be working at the government run hospital in<br />

Petionville. I was hopeful because I was told that the hospital<br />

was a three-story building and had a dental clinic. Upon our<br />

arrival, we were graciously met by the hospital’s medical<br />

director, Dr. Fantale, who gave us a tour <strong>of</strong> the facility. The<br />

pediatrician consulted an in-house patient (an infant being<br />

treated for malnutrition). The director <strong>of</strong> our mission, Dr.<br />

The Palais Nacional and most government buildings in downtown Port-au-<br />

Prince were destroyed.<br />

Paul, consulted another patient whose<br />

problem was jaundice <strong>of</strong> unknown origin.<br />

After the appropriate tests were<br />

ordered (and generously paid for by<br />

him), we went back downstairs to take<br />

a look at the dental clinic which is<br />

staffed by one dentist. Unfortunately,<br />

the clinic was locked and the medical<br />

director did not have the key. As it<br />

was Sunday, the appropriate staff was<br />

not there to make the clinic accessible<br />

to us. Since the attending dentist was<br />

<strong>of</strong>f on Wednesdays, I promised the<br />

medical director I would return that<br />

day to donate my services. I was<br />

assured that there was a dental unit and<br />

a chair in the clinic. Not knowing how<br />

well equipped it was, I made a mental<br />

note to pack as much as I could with<br />

what I had brought so that I would not be in short supply when<br />

I began seeing patients.<br />

That afternoon, we were given a tour <strong>of</strong> downtown Port-au-<br />

Prince where the devastation from the earthquake was most<br />

evident. To date, the reality <strong>of</strong> how powerful nature’s forces<br />

can be were apparent. We drove amongst buildings and structures<br />

that were ripped and torn apart. Some were tipped over<br />

so perilously, a stiff wind would easily bring them down.<br />

Government buildings and churches were not spared. The<br />

most difficult part to observe was the numerous tent cities that<br />

had sprung up in and around the capital. As we understood it,<br />

the people needed help and in their minds what better place to<br />

set up a tent than around a government building no matter how<br />

destroyed it was?<br />

<strong>GP</strong>: How was the working environment?<br />

FM: At the AIMER-HAITI Hospital, I was basically treating<br />

patients in a large outdoor hut. It was very windy and it rained<br />

every day (No, the hut did not leak!). I treated patients who<br />

were seated in a metal folding chair –I was standing, hunched<br />

over them. Yes, I worked like this all day! There were no x-<br />

Haiti. I was told the reservoir would<br />

arrive in approximately 3 weeks but<br />

that would have come after I was<br />

gone.<br />

At Eliazare Germain Hospital, I was<br />

really looking forward to the luxury <strong>of</strong><br />

a dental chair and a pr<strong>of</strong>essionally<br />

trained dental assistant. But unfortunately,<br />

the dental unit was inoperable<br />

so mostly extractions were done there<br />

as well.<br />

<strong>GP</strong>: Sounds like the conditions were<br />

pretty harsh. Please describe your<br />

days at the mission – how many<br />

patients did you see and what type <strong>of</strong><br />

procedures were you able to perform?


FM: The sun gets hot, even in<br />

early mornings. So we started<br />

seeing patients at 7:30 a.m. I<br />

don’t know when the patients<br />

started to line up but lines<br />

were already long even before<br />

we opened. The announcement<br />

went out that there would<br />

be a dentist providing services.<br />

Patients came with various<br />

requests from cleanings and<br />

restorative treatment to extractions.<br />

Most <strong>of</strong> them spoke<br />

French-Creole so I was able to<br />

communicate with them directly.<br />

The majority <strong>of</strong> the days<br />

were spent doing extractions.<br />

All the treatments were rendered<br />

under universal precautions<br />

as well as with the use <strong>of</strong><br />

a head light. Needless to say,<br />

doing extractions 8 to 10<br />

hours a day standing in the<br />

same position can take a physical<br />

and mental toll on you. I<br />

did not keep count as to how<br />

many patients I saw but I<br />

believe it was an average <strong>of</strong><br />

30 patients a day. One patient<br />

blends into another and after a<br />

while fatigue starts to play<br />

tricks with your mind.<br />

All the members <strong>of</strong> the team,<br />

as well as the local volunteers,<br />

were frantically involved in<br />

their particular tasks.<br />

Unfortunately, we did not have<br />

time to interact with patients<br />

on a personal level, let alone<br />

each other. As the only dentist<br />

there, I worked by myself. By<br />

the end <strong>of</strong> each day, we were<br />

all stiff, exhausted, and in pain,<br />

just looking to put up our<br />

swollen feet.<br />

On the last day <strong>of</strong> the mission,<br />

the patients have lined up<br />

extremely early to be seen. The waiting area was filled to<br />

capacity and we were not sure if we would be able to see<br />

everyone. As usual, with diligence, patience, blood, and<br />

sweat, we got through the day.<br />

<strong>GP</strong>: Was anyone turned away?<br />

FM: No one was turned away. I saw everyone.<br />

<strong>GP</strong>: What are your concerns and hopes for the future <strong>of</strong><br />

Haiti?<br />

FM: On our way to the airport on our last day in Haiti, we<br />

passed by one <strong>of</strong> the many tent cities strewn all across Port-au-<br />

Prince since the earthquake. This lifestyle has become a distressing<br />

way <strong>of</strong> life for many. It rained every day we were<br />

there. How much shelter can a tent provide? Right before we<br />

<strong>of</strong> problems still facing the families<br />

there.<br />

My hopes are that after this year’s<br />

presidential elections in Haiti,<br />

there will be new leadership compelled<br />

to do something to help<br />

Haiti and her people.<br />

<strong>GP</strong>: Do you plan on going on<br />

another mission to Haiti? If so,<br />

how can we help?<br />

FM: Another mission to Haiti is<br />

being planned for February and<br />

June 2011 even as we speak.<br />

There’s so much work to be done.<br />

I know I have to go back and finish<br />

what I started. Hopefully, by<br />

then, Haiti will be in a better position<br />

than what I left behind.<br />

Every drop in the bucket matters.<br />

As far as help is concerned, my<br />

hope is that on the next medical<br />

mission, I will not be the only<br />

dentist present. Hopefully someone<br />

reading this article will be<br />

motivated to leave their comfort<br />

zone and help out a fellow human<br />

being in need thousands <strong>of</strong> miles<br />

away. I’m optimistic that by the<br />

next medical mission, I won’t be<br />

the only dentist volunteering services.<br />

Unfortunately, the following scenario<br />

plays out year after year throughout the<br />

entire dental community. It goes like this:<br />

Years ago, you decided it was time to bring<br />

an associate into your practice. You interviewed<br />

candidates until you met the<br />

“ideal” associate for your practice. Of<br />

course, you didn’t know whether or not<br />

“things would work out”, so you both<br />

decided it would be best to work together<br />

for a while without a formal contract. The<br />

cost <strong>of</strong> a contract seemed unnecessary at<br />

this point and anyway, you are both pr<strong>of</strong>essionals,<br />

what is there to worry about?<br />

Everyone really liked this young doctor and<br />

bringing in the new associate immediately<br />

served to be a perfect boost to your practice.<br />

The associate was quite comfortable with<br />

the working arrangement. After all, he did<br />

not have to go into debt to get a practice<br />

started (like you once did). If the practice<br />

failed to make money, he did not lose any <strong>of</strong><br />

his personal funds (like you could). No risk,<br />

no pressure and no practice management<br />

responsibility made this arrangement a perfect<br />

situation for the associate. And since<br />

you still owned your practice and you were<br />

still “in control”, you were comfortable with<br />

the arrangement as well.<br />

Things could not have been better. You<br />

began to take a little more time <strong>of</strong>f without<br />

losing money because <strong>of</strong> the pr<strong>of</strong>its you<br />

were making <strong>of</strong>f <strong>of</strong> the associate’s production.<br />

Your associate was getting busier and<br />

the practice was steadily growing. <strong>New</strong><br />

patient flow was increasing. The associate<br />

was attracting patients from his generation<br />

so younger patients were beginning to<br />

come back into your practice. Your existing<br />

patients really liked the associate a lot<br />

and, for the most part, did not seem to<br />

mind that you were not in the practice as<br />

much as you used to be. It didn’t bother<br />

you when you realized that you and your<br />

staff were referring to your associate as<br />

your partner, even though he really was not<br />

a partner and you still “owned” the practice<br />

and were “in control”.<br />

As more time went by and the associate’s<br />

speed improved, you began to take even<br />

more time <strong>of</strong>f. You took up golf and also<br />

began to hunt and fish again. Appearing to<br />

have unending energy, your associate easily<br />

picked up the potential production drop-<br />

THE PRACTICE GIVE-AWAY!<br />

<strong>of</strong>f that would have been created by your<br />

decrease in clinical time.<br />

Enjoying more free time and seeing the<br />

practice continue to grow, you were even<br />

more convinced that the associate was the<br />

perfect addition to the practice. You were<br />

very pleased with his attitude, his work<br />

habits, his desire to learn, his overall treatment<br />

<strong>of</strong> the patients and his great relationship<br />

with the staff and the patients. You<br />

became increasingly sure that the associate<br />

was the right buyer for your practice. In<br />

fact, it was even more than that. You were<br />

out-and-out proud <strong>of</strong> your ability to select<br />

such a great “heir apparent”.<br />

For whatever reason (typically because <strong>of</strong><br />

informal past chats with your associate), one<br />

day you decided that the timing was right to<br />

have a discussion with your associate about<br />

having a formal agreement drawn up that<br />

would clarify your working relationship.<br />

However, you were surprised to discover<br />

that the associate was not enthusiastic about<br />

the idea. He told you he felt that everything<br />

was running smoothly and he loved it there,<br />

so why bother with an expensive contract to<br />

say what they both knew was destined to<br />

happen anyway? He reassured you by<br />

telling you that the two <strong>of</strong> you were on the<br />

exact same page about him buying the practice<br />

when you were ready to sell. You were<br />

reasonably happy with this verbal commitment<br />

and since everything was running<br />

smoothly, you soon forgot about a written<br />

contract. After all, why rock the boat? Your<br />

clinical time was down and your income<br />

was up; you still “owned” the practice and<br />

you were “in control”.<br />

What you did not realize, however, is that<br />

you may have still own the equipment and<br />

perhaps the building, but your associate<br />

was gaining ownership over your practice!<br />

The value <strong>of</strong> a practice is in the relationship<br />

between the doctor and the patients,<br />

not the equipment and facility. When<br />

patients see two doctors working together,<br />

they generally perceive the new doctor as<br />

a “partner.” The associate is usually<br />

younger than the practice owner is, so<br />

patients think this new doctor will ultimately<br />

take over their treatment when the<br />

current owner retires. This perception is<br />

probably even promoted by the associate<br />

and perhaps even the staff as a means <strong>of</strong><br />

preparing the patients for your eventual<br />

retirement.<br />

Eventually you are sincerely considering<br />

retirement and the time finally comes to<br />

talk seriously to the associate about purchasing<br />

your practice. You reveal to the<br />

associate what you think a fair price and<br />

terms should be. At this point in the relationship<br />

you have a very rude awakening.<br />

The associate tells you that he may be<br />

interested in buying your practice, but only<br />

on his terms and at his price. He goes on to<br />

explain that he really would only be buying<br />

your practice out <strong>of</strong> “the goodness <strong>of</strong><br />

his heart” anyway since he does not have<br />

to buy your practice because <strong>of</strong> the following<br />

reasons:<br />

He already has a relationship with all his<br />

patients and does not have to buy that part<br />

<strong>of</strong> the practice (you can’t sell the associate<br />

his own patient goodwill). These patients<br />

don’t really know you and will follow the<br />

associate to a new <strong>of</strong>fice.<br />

He knows that if you should quit practicing,<br />

many <strong>of</strong> your patients will transfer to<br />

the associate if he opens a new <strong>of</strong>fice nearby.<br />

He can simply send a notice to all the<br />

patients <strong>of</strong> the practice, not just his own<br />

(don’t be naive - the associate already has<br />

a list <strong>of</strong> all the patients by now).<br />

He realizes that it is very unlikely that anyone<br />

else will buy your practice because<br />

they will fear that the associate will move<br />

down the street and take most <strong>of</strong> the<br />

patients with him (and probably most <strong>of</strong><br />

the staff as well). The fact is your practice<br />

has very little, if any value, to anyone<br />

other than the associate.<br />

He either knows or at least has a good idea<br />

that the staff will follow him. They need<br />

job security. They know you would like to<br />

retire. They also know that if the associate<br />

leaves, many patients will also leave.<br />

Either your retirement would force them to<br />

seek employment elsewhere. A severe loss<br />

<strong>of</strong> patients would also suggest lay<strong>of</strong>fs. The<br />

future is uncertain. If the associate leaves,<br />

they know they would have long-term job<br />

security and a constant paycheck.<br />

He realizes he is a prime position to “negotiate”<br />

a great deal for himself. He will feel<br />

that he can set the practice price and terms<br />

HoW To TREAT DENTAL CARIES DISEASE<br />

I recently attended a lecture given by<br />

Dr. V. Kim Kutsch, founder and CEO <strong>of</strong><br />

the Oral BioTech Corporation, manufacturer<br />

<strong>of</strong> CariFree caries control solutions.<br />

His concept is that caries develop from<br />

bi<strong>of</strong>ilm on teeth. Dr. Kutsch has developed<br />

a full line <strong>of</strong> products (patent pending)<br />

designed to reduce acidity in the oral<br />

cavity, thereby selecting against the<br />

microorganisms in the bi<strong>of</strong>ilm that flourish<br />

in an acidic environment.<br />

At the heart <strong>of</strong> the treatment protocol is<br />

a hand-held device, the CariScreen, which<br />

measures the adenosine tri-phosphate (ATP)<br />

by bioluminescence, expressed as a<br />

Relative Light Unit (RLU) as an indicator<br />

<strong>of</strong> caries susceptibility. This device requires<br />

that the dentist swab the lingual surface <strong>of</strong><br />

the un-brushed lower anterior teeth only,<br />

avoiding the gingiva. The dentist then activates<br />

the enzyme-filled swab, shakes it for<br />

ten seconds, then inserts the swab into the<br />

RLU meter to obtain a reading <strong>of</strong> the<br />

patient’s caries susceptibility number.<br />

The treatment protocol is to reduce the<br />

susceptibility number, beginning with the<br />

use <strong>of</strong> an Oral Neutralizing Gel applied with<br />

a toothbrush. This product raises the pH to<br />

basic levels and is completely non-abrasive.<br />

It combines the benefits <strong>of</strong> xylitol with<br />

CariFree’s patent-pending pH+ technology.<br />

[It cannot legally be called “toothpaste”<br />

because the FDA monograph for dentifrice<br />

is acidic and abrasive.] This pH neutralizing<br />

gel is basic and non-abrasive.<br />

For severe cases, a pH neutralizing gel<br />

called CariFree pHluorigel is recommended.<br />

It combines the anti-caries benefit <strong>of</strong><br />

5,000 ppm <strong>of</strong> sodium fluoride with a therapeutic<br />

dose <strong>of</strong> xylitol. This product contains<br />

the most powerful anti-caries therapeutics<br />

for an all-in-one, prescription<br />

strength, cavity-fighting, powerhouse. It<br />

may be used in a custom tray appliance for<br />

increased exposure and effectiveness. This<br />

product is similar to others presently available<br />

commercially but without their acidity<br />

and/or abrasiveness.<br />

The patient is also instructed to rinse<br />

twice daily for the first month with CariFree<br />

Treatment Rinse, designed to treat the cariogenic<br />

bi<strong>of</strong>ilm in plaque to reduce the overpopulation<br />

<strong>of</strong> cariogenic bacteria and to neutralize<br />

the oral pH. This product comes in<br />

two bottles with a small cup for the patient<br />

to mix 5 cc from each bottle. This product<br />

contains sodium hypochlorite, which kills<br />

the microorganisms in the bi<strong>of</strong>ilm, and<br />

tastes like swimming pool water.<br />

After the first month, the patient is recommended<br />

to use CariFree Maintenance<br />

Rinse, which does not require mixing.<br />

This product contains xylitol and 0.05%<br />

sodium fluoride to neutralize the acidic<br />

oral pH. This rinse is available in citrus<br />

and mint flavors.<br />

The CariFree line also includes a xylitol-containing<br />

spray called CariFree Boost<br />

Mouth Spray. This spray changes the oral<br />

pH to basic. It is to be used after meals,<br />

with two or three sprays into the mouth. It<br />

is especially effective with patients suffering<br />

from xerostomia.<br />

Xerostomia is a symptom caused by<br />

over 3000 medications (list available on<br />

CariFree website). The CariFree product<br />

line includes Oral Neutralizer Gum, containing<br />

xylitol and pH+ technology (patent<br />

pending), to be used up to five times daily<br />

in order to stimulate salivary flow, which<br />

increases pH and promotes a healthy bi<strong>of</strong>ilm.<br />

Dentists might consider this as an<br />

alternative to prescribing Pilocarpine,<br />

which may have possible drug interactions<br />

with the patient’s other medications.<br />

However, the use <strong>of</strong> the gum avoids these<br />

potential medical complications.<br />

The concept <strong>of</strong> caries disease treatment<br />

requires that the dentist diagnose the major<br />

contributing factor that is responsible for<br />

one’s caries. The dentist determines patient’s<br />

caries susceptibility by completing a Caries<br />

Risk Assessment Form. This is known as<br />

CAMBRA — Caries Management By Risk<br />

Assessment. There are two forms available –<br />

one for toddlers to age six and another for<br />

older children and adults.<br />

One <strong>of</strong> the primary risk factors for caries<br />

is sipping sugary liquids throughout the day.<br />

For example, when an athlete drinks an<br />

“energy boosting” beverage or when a student<br />

sips soda all day long. The acidic condition<br />

that results promotes cariogenic bacteria<br />

and leads to net mineral loss from the<br />

enamel. It is recommended that dentists<br />

apply CariFree fluoride varnish to the enamel<br />

with periodic frequency, depending on the<br />

degree <strong>of</strong> caries susceptibility. CariFree fluoride<br />

varnish is a mixture <strong>of</strong> 5% sodium fluoride<br />

in a resin carrier, dispensed in singleuse<br />

dose applications. This same enamel<br />

varnish also effectively treats patients with<br />

cervical/root sensitivity or decay.<br />

The CariFree Company also provides a<br />

complete line <strong>of</strong> products specifically<br />

designed for toddlers and pre-schoolers<br />

under the age <strong>of</strong> 5 called CariFree Xyli-<br />

Tots Littles Kit.<br />

The kit includes the following:<br />

(1) A box <strong>of</strong> Xyli-Tots Oral Wipes - Makes<br />

it easy to clean on the go while creating a<br />

healthy oral environment with xylitol and<br />

elevated pH+ technology. These are available<br />

in flavors <strong>of</strong> “Grape Giggles” or<br />

“Watermelon Wiggles.”<br />

(2) Flavored Xyli-Tots Mouth Spray - It is<br />

xylitol-rich, neutralizes the mouth after<br />

eating, and is safe to swallow.<br />

(3) Xyli-Tots Lollies - Permit the toddler to<br />

lick away decay with this 0.5 gram xylitol<br />

lollipop, available in assorted flavors.<br />

(4) Xyli-Tots Tooth Gel (toothpaste) - The<br />

ultimate product for toddlers. It is grape<br />

flavored and combines the proven benefits<br />

<strong>of</strong> xylitol with the neutralizing pH+ technology.<br />

It contains no fluoride and is<br />

therefore safe to swallow.<br />

The CariFree Xyli-Tots Juniors Kit contains<br />

all <strong>of</strong> the above products except the<br />

wipes but does contain Xyli-Tots Anti-cavity<br />

Rinse. Designed to promote healthy bio-film<br />

development and limit the growth <strong>of</strong> decaycausing<br />

bacteria, it is available in Grape<br />

Giggles and Watermelon Wiggles. It contains<br />

3 grams <strong>of</strong> xylitol per swish and 0.05%<br />

neutral sodium fluoride, with the pH+ technology<br />

to neutralize cavity-causing acids.<br />

A word <strong>of</strong> caution - Xylitol is dangerous<br />

if ingested by animals/pets.<br />

Dr. Kutsch reported in his lecture that<br />

an ongoing, large-scale study in<br />

Queensland, Australia is expected to produce<br />

results supporting the effectiveness <strong>of</strong><br />

the CariFree treatment protocol for the<br />

detection, treatment, and prevention <strong>of</strong><br />

dental caries. Dentists must think <strong>of</strong> caries<br />

as a treatable disease. The CariFree<br />

Company provides the armamentarium for<br />

the dentist to do so.<br />

(which will most likely be much, much<br />

less than the value you had in mind) and<br />

you will have no choice but to accept.<br />

You can’t believe what is happening. You<br />

are thinking how ungrateful and conniving<br />

the associate is. But realistically, you<br />

must understand the associate’s perspective.<br />

The associate feels that you have<br />

made a fortune all these years <strong>of</strong>f <strong>of</strong> his<br />

labor. All the associate’s friends, family<br />

and advisors have told him that he should<br />

leave you and open a new <strong>of</strong>fice. They tell<br />

him you are being selfish and greedy for<br />

trying to sell him the practice instead <strong>of</strong><br />

just turning the practice over to him after<br />

all the service he gave you. In the associate’s<br />

mind, he is not conniving or ungrateful;<br />

he really believes he is right!<br />

You are totally devastated, feeling<br />

betrayed and absolutely reeling from the<br />

eye-opening loss <strong>of</strong> hundreds <strong>of</strong> thousands<br />

<strong>of</strong> dollars in practice equity that you<br />

were counting on to retire. If you did a<br />

good job saving for retirement, maybe<br />

you overcome this. If not, you may be<br />

forced to practice forever and rebuild a<br />

practice at a time in your life when retirement<br />

was foremost on your mind.<br />

As previously stated, this scenario happens<br />

year after year. It happens to unsuspecting<br />

doctors as well as those who were<br />

made aware <strong>of</strong> the risks beforehand. But<br />

for some reason, no one believes it will<br />

happen to him or her. Would you like a<br />

better scenario for you and your practice?<br />

There is a good way to bring on your “heir<br />

apparent.”<br />

Fellowship and Mastership: The Road to<br />

Excellence<br />

The current economy has impacted the pr<strong>of</strong>ession<br />

<strong>of</strong> dentistry and dentists around the country are<br />

struggling to make end meets. A good way to stay<br />

competitive is to keep current on the latest techniques<br />

and procedures that have streamlined many<br />

<strong>of</strong> the old ways dentistry has been practiced.<br />

The <strong>Academy</strong> has long been a good source to help<br />

our members achieve the highest level <strong>of</strong> education<br />

so that even through the tough times, they would be able to find innovative<br />

ways to keep things moving along.<br />

One <strong>of</strong> the greatest assets the <strong>Academy</strong> <strong>of</strong>fers its members are the<br />

Fellowship and Mastership Awards. These awards are pr<strong>of</strong>essional designations<br />

within the AGD that reflects a general dentist’s ongoing commitment<br />

to provide quality care through continuing education. Here is a<br />

brief overview <strong>of</strong> each designation.<br />

Fellowship Requirements<br />

[1] Current AGD membership for three continuous years by December<br />

31 <strong>of</strong> the year <strong>of</strong> the convocation for which application is made, to begin<br />

no earlier than the month <strong>of</strong> dental school graduation.<br />

[2] A dental license that has not been suspended or revoked within the last<br />

five years and is not currently under probation, suspension, or revocation.<br />

[3] 500 hours <strong>of</strong> approved continuing education.<br />

[4] Passing the Fellowship Exam.<br />

[5] Attendance at an AGD convocation ceremony, held during an AGD<br />

annual meeting, to receive the award. Successful candidates are allowed<br />

three years following approval to complete this requirement.<br />

Mastership Requirements<br />

[1] Current AGD member ship in good standing.<br />

[2] A dental license that has not been suspended or revoked within the<br />

[3] Fellowship in the <strong>Academy</strong> <strong>of</strong> <strong>General</strong> <strong>Dentistry</strong>.<br />

[4] 1100 hours <strong>of</strong> FAGD/MAGD approved continuing education. At least<br />

400 hours must be in participation courses and at least 600 hours must be<br />

in specific disciplines. Participation hours can be earned at any time during<br />

membership with an implementation date <strong>of</strong> January 1, 2007.<br />

[5] Attendance at an AGD convocation ceremony, held during an AGD<br />

annual meeting, to receive the award. Successful candidates are allowed<br />

three years following approval to complete this requirement.<br />

Mastertrack<br />

<strong>New</strong> <strong>York</strong> <strong>State</strong> is proud to be one the AGD Constituents to <strong>of</strong>fer our<br />

members the opportunity to participate in the Mastertrack program. The<br />

program is a series <strong>of</strong> lectures, participations, and/or on-site/in-<strong>of</strong>fice<br />

protocol courses. A protocol course is a combination <strong>of</strong> on-site and in<strong>of</strong>fice<br />

courses. They are typically hands-on work with models, devices,<br />

and patients. The program concludes with a presentation <strong>of</strong> case studies<br />

by each participant to his/her peers. Every year since 2002, our local<br />

Mastertrack Committee sought to find the best clinicians around the<br />

country to bring to <strong>New</strong> <strong>York</strong> to ensure that the attendees get the most upto-date<br />

information on a particular subject.<br />
