7 Ethical consequences<br />

Shoulder arthroplasty needs to be compared with other interventions in well-designed<br />

controlled studies before it is introduced as a clinical routine on a large scale.<br />

However, in individual patients with severe pain or disability it may well be ethical to<br />

offer such surgical intervetion. For detailed analysis see Appendix 5.<br />

Organisation<br />

8a When can this new health technology be put into practice?<br />

Shoulder arthroplasty is today performed at 3 hospitals in VGR, (see 8b).<br />

8b Is this technology used in other hospitals in Western Region of Sweden?<br />

In VGR elective shoulder arthroplasties are performed at the Departments of<br />

Orthopaedics at the <strong>Sahlgrenska</strong> University Hospital Mölndal, the Södra Älvsborg<br />

Hospital, Borås, and the Norra Älvsborgs County Hospital, Uddevalla, Sweden.<br />

8c According to the work group, will there be any consequences of the new health<br />

technology for personnel?<br />

The expected increased use of shoulder arthroplasty will require more resources in<br />

terms of personnel, education, and accessibility to operating theaters, as well as for<br />

equipment and implants.<br />

8d Will there be any consequences for other clinics or supporting functions at the<br />

hospital or in the whole Western Region of Sweden?<br />

The documented increase in prosthetic arthroplasties in patients with primary<br />

shoulder osteoarthritis (see 2b) will cause an increased need of resources in terms of<br />

surgical procedures and in- and outpatient care.<br />

If the ongoing expansion of shoulder arthroplasty will not be reimbursed, the<br />

production of other types of elective orthopaedic surgery including total hip and knee<br />

arthroplasty will have to be reduced.<br />
