General Information TRAVEL At the time of this printing, the following travel requirements were accurate. Please check with your travel agent or airline for up-to-date information. Participants entering Mexico from the United States or Canada will need a valid passport. No visa is required. Travelers will be asked to complete a tourist card (FMT form) at their point of entry. Those coming from other countries will need valid passports. Minors traveling with one parent will need a notarized letter of permission from the other parent. For up-to-date travel information visit www.travel.state.gov. MITT (cancun@mittmexico.com) is the recommended transportation from the airport to the hotel. A special group rate will be provided. This information is posted on the Mayo Clinic website (http://www.mayo.edu/cme/cardiovascular-diseases-2013R011). Travel arrangements are the sole responsibility of the individual registrant. To secure your room at the Marriott CasaMagna Resort, call and state that you will be attending the Cardiology at Cancún conference. The hotel room block is scheduled to be held until Monday, February 4, 2013, but may be sold out before this date. It is recommended to arrange your reservation early. All reservation requests will require a one (1) night advance payment that will be refundable only if the reservation is canceled at least 14 days prior to the arrival date. Porterage (bellmen) gratuities are $5.00 per person; housekeeping gratuities are $4.00 per room per day. These will be posted to individual rooms unless notified otherwise. Registrants are asked to guarantee two nights’ room rental plus tax with a major credit card or check. Health Club Facilities – Complimentary use of the CasaMagna Health Club facilities has been arranged for our group. Early departure fees will be charged by the hotel if an attendee checks out of the hotel

Because these are private tours for the Cardiology participants and their guests, a minimum of 30 people will be required. • All other tours available in Cancun and Riviera Maya on their regular schedules. • Restaurant reservations.
