Just looking at the specifications of a<br />

camera doesn’t mean it will take the<br />

kind of pictures wanted. Needs, budget,<br />

and knowledge are important determinates<br />

in deciding the best camera for you.<br />

Here are some terms and related discussion<br />

points about digital cameras.<br />

Megapixel<br />

The first question asked by most dentists<br />

about digital cameras is “What is its meg-<br />

Introduction<br />

The popularity of digital photography is booming. In a recent survey, 47% of dentists<br />

who responded owned a digital camera and 13% planned to purchase in the next 12<br />

months 1 . Ninety percent of those surveyed use their digital pictures for laboratory<br />

communication. Digital photography makes it possible for the immediate taking and<br />

communicating of images between the patient, dental laboratory, general dentist,<br />

and specialist. Pictures, especially for lab communication, are a necessity when trying<br />

to match teeth for color, luster, shape, and character with fabricated crowns and veneers<br />

2,3 . Photographs are much better than hand drawn maps of teeth and, in fact, a digital camera with specialized software can<br />

now give accurate shade prescriptions, reducing subjectivity and the difficulty of shade matching.<br />

Photographs with the case also give an emotional connection between the technician and patient 3,4 . That is, the ceramist is not<br />

just looking at a stone model; also seen are the teeth in the face, the shape of the head, the symmetry of the smile, and what is<br />

exposed in the aesthetic zone. All of these are assets that help the ceramist do their very best work.<br />

things consider because most cameras<br />

today meet or exceed what is needed.<br />

Each “pixel” represents a color point on<br />

the digital camera’s conversion sensor<br />

(CCD or CMOS). One megapixel equals<br />

one million dots of the color red, green,<br />

or blue. It is the mixture of these colors<br />

that makes up all color and the image 5 .<br />

Have you wondered why pictures vary in<br />

file size? In nature we see more greens<br />

and blues than reds. So even though<br />

there are, for example, a million pixels, research<br />

shows there are a higher number<br />

of green and blue pixels than red ones.<br />

So, in this case, the picture of pure green<br />

or blue is around 1780k and the picture<br />

of red is 35% less, at 1162k. The manufacturer’s<br />

picture color is often skewed<br />

toward a particular color (see white balance<br />

below).<br />

Approximately, 1 megapixel is capable of<br />

printing a 4” x 6” sheet without deterioration;<br />

2 megapixels does the same for an<br />

8.5” x 11” sheet. To put it in perspective, most computer<br />

monitors are just smaller than 1 megapixel (1024 x 768 =<br />

786,432 pixels). A 6 megapixel image would be 6 times<br />

as large as your screen and up to 15 megabytes—far too<br />

large for storage, manipulation, or electronic communication.<br />

Also, 2 megapixels that are sharp and clear is better<br />

than 6 megapixels that are blurry!<br />

Cameras have quality settings that also affect image size.<br />

For most uses, simply adjust image size to between 1 to 2<br />

megabytes. The cameras mentioned below have reached<br />

a point where the resolution (around 6 megapixels)<br />

exceeds the needs of most users. Emphasis should be<br />

placed on usability and image quality 5,6 .<br />

White Balance/Color Adjustment<br />

Off the shelf, each camera will have its own color characteristics<br />

and adjustment methods. Usually, custom white<br />

balancing the camera gets color close to accurate. This<br />

procedure is not difficult and will be explained in the<br />

owner’s manual7 . Not all cameras have the white balance<br />

function and, therefore, give you less ability to correct<br />

color. This is true of the Kodak 7490 that has no white balance<br />

feature and gives a magenta look.<br />

Aperature (f stop)<br />

The larger the number, the increased depth of field. So<br />

f22 would be suitable for an anterior picture straight<br />

on where front and back teeth need to be in focus. F5.6<br />

would be for a full-face portrait where a blurry background<br />

is wanted4 .<br />

DIGITAL CAMERAS<br />

There are two classes of cameras—DSLR (Digital Single<br />

Lens Reflex) interchangeable lens cameras and the modi-<br />

fied, lighter weight Point and Shoot cameras.<br />

DSLR Cameras<br />

In DSLR cameras, the most popular brands (in order) are<br />

the Canon Digital Rebel XT (Pic 2), Canon 20D (Pic 3), Fuji<br />

S3 Pro (Pic 4), and Nikon D70.<br />

Lighting<br />

Similarities with these cameras are that they are sophisticated<br />

with capabilities that go beyond dental<br />

photography; they should have a front-mounted ring<br />

flash, point flash, or dual mounted lights, and are usually<br />

manually focused for close up photography. Without a<br />

ring flash, macro pictures will sometimes be washed out8 .<br />

The external flash is metered through the lens by the<br />

cameras mentioned, except the Nikon. On the Nikon, the<br />

power of the flash must be set manually which then may<br />

need readjusting depending on the picture. Controlling<br />

the power and angle of light on the teeth gives different<br />

important information.<br />

Lens<br />

Most owners outfit their camera with a 100mm macro<br />

lens which has the ability to focus up close. Because the<br />

digital camera uses a CCD or CMOS sensor that is of a<br />

different size than 35mm film, a 100mm lens on a 35mm<br />

camera converts to 160mm on a digital camera. This<br />

means a higher magnified lens gives a flattening effect<br />

on pictures. Canon recently released a 60mm macro lens<br />

which equates to 96mm—this is a better choice.<br />

Image Size - Megapixel<br />

All these cameras are around 6 megapixels and have<br />

many adjustments, so image quality will not be a problem.<br />

Settings<br />

The camera should be put on aperture priority (Av) and<br />

f stop set around f22 for close up pictures where good<br />

depth of field is needed. It is recommended to slightly<br />

underexpose pictures to better see tooth color and luster.<br />

A picture can always be slightly brightened without<br />

losing quality but you cannot darken an overexposed<br />

picture and maintain quality. Full-face photos should be<br />

taken at f5.6 for more emphasis on the face 3 . On Canon<br />

cameras the flash can be manually set to the desired<br />

power of light (-1/3 on the 30D).<br />

DSLR cameras, at about 4 pounds, are substantially heavier<br />

than Point and Shoot cameras and can be intimidating<br />

because of the weight and attached apparatus. Most<br />

people prefer a lighter camera if quality is comparable.<br />

DSLR cameras have the following characteristics:<br />

1) Viewing is Through the Lens (TTL)<br />

2) Require external lighting<br />

3) Are manually focused<br />

4) Are heavier<br />

5) Provide greater depth of field<br />

6) Are more expensive<br />

Point and Shoot Digital Cameras<br />

As noted above, don’t let camera size or megapixel drive<br />

your purchasing decision. High quality and DSLR features<br />

can be found in a small package that takes superb pictures.<br />

Some of these cameras have been modified and<br />

some are off the shelf. Some cameras have the capability<br />

to take dental photos and some do not, regardless of the<br />

specifications! These cameras are between 4 and 7 megapixels—plenty<br />

big enough for good quality. Things to<br />

look for are the ability to focus close, color control, proper<br />

lighting, exposure control, depth of field, overall ease of<br />

use, and support. All the following cameras qualify.<br />

The Canon <strong>Dental</strong>Foto, Olympus C5060, and Canon G6<br />

are three good examples in this classification. These cameras<br />

have similar features to DSLR cameras although lighter<br />

weight, but don’t have the range of settings as DSLR or interchangeable<br />

lenses. In addition, viewing is done through<br />

an LCD screen, not through the lens.<br />

The Canon <strong>Dental</strong>Foto (Pic 7) and<br />

The saying, “beauty is in the eye of the beholder”,<br />

is very true in the realm of cosmetics; additionally,<br />

beauty is in the mind of the owner. Beyond<br />

the technical aspects of dentistry, in cosmetics the<br />

psychology and opinions of the patient and the<br />

people in their lives who influence those opinions<br />

are equal, if not more important factors.<br />

The dentist’s technical command of the technique<br />

to create precise margins, the optimal choice of<br />

restorative materials, proper interproximal contour<br />

design, and harmonizing with physiological occlusal<br />

function, all are factors in case longevity and are<br />

the foundation for long term success. The shorter<br />

term challenge of gaining enthusiastic patient acceptance<br />

is in the emotional realm of whether or<br />

not they like the “look”.<br />

Following is a checklist of the more important factors used<br />

to define what makes the “look” of a smile attractive and can<br />

be used as an adjunct when designing a case (Figure 1).<br />

Horizontal plane of occlusion is perpendicular to the long<br />

axis of the face. Most patients have some facial asymmetry<br />

that should be taken into account when determining the<br />

long axis of the face. The best way to make an assessment<br />

of the horizontal plane is with the patient standing straight<br />

with the dentist positioned directly in front of them. A physical<br />

recording of the plane can then be made (e.g. stick bite<br />

registration or facebow). A Fox plane is an excellent adjunct<br />

to highlight the plane.<br />

The slight curve of the maxillary teeth follows the curve of<br />

the lower lip when smiling. This can be quite subtle or occasionally<br />

exaggerated. Edges of the teeth are relatively higher<br />

as the eye moves distally (Figure 2).<br />

There is a proportion and dominance to anterior teeth. The<br />

maxillary central incisor is usually 10.5-11mm from gingival<br />

to incisal edge. If the length-to-width ratio of that tooth can<br />

be kept to 75-80% and the Golden proportion (1.6/1.0/0.6)<br />

of the relative visual width of the central, lateral, and cuspid,<br />

the smile will have a natural harmony.<br />

Incisal embrasures of maxillary teeth progress from small to<br />

large with a distinct and slightly asymmetric form between<br />

lateral and cuspid. This tends to separate teeth and is characteristic<br />

of youth (Figure 3).<br />

Bilateral symmetry exists. Perfect symmetry is actually<br />

undesirable as it looks synthetic. The relative display of the<br />

contralateral tooth tends to mirror the other including incisal<br />

edge. The gingival architecture is as important as tooth<br />

structure (Figure 4).<br />

Aesthetic Dentistry<br />

A more youthful smile displays more teeth, both vertically<br />

and horizontally. It is not uncommon to have close to 100%<br />

display of maxillary anterior in a full smile, especially in<br />

young women. An extension of 1-2mm below the edge of<br />

the rested lip is also often present. Bicuspids tend to fill the<br />

buccal space giving the appearance of breadth (Figure 5).<br />

Surface contours are typically highly reflective and convex,<br />

with vertical flutings to break up the light. This seems to be<br />

true whether the teeth, particularly incisors, are shaped relatively<br />

square, ovoid, round, or triangular. Using the general<br />

outline of the face turned upside-down seems to work. A<br />

more ovoid form works for many females, especially if the<br />

desire is to create a more youthful and softer appearance<br />

(Figure 6).<br />

Shade is light. The societal trend is ‘lighter is better’. Bleaching<br />

is marketed everywhere. If in the design of restorations<br />

the dentist can maintain a natural looking translucency pattern,<br />

as the shades of the teeth become lighter it counters<br />

the tendency to become overly opaque. Hue, chroma, and<br />

value are all factors that play important roles. Use various<br />

lighting to chose the best overall option because patients are<br />

not seen under full spectrum light conditions (Figure 7).<br />

Individual teeth are not monochromatic but exhibit a<br />

gradation of color and opacity from gingival through incisal.<br />

Because of thinning of the enamel toward the gingival edge<br />

of a tooth, more intense colors appear.<br />

Teeth are aligned smoothly into the arch form and have a<br />

harmonious flow from tooth to tooth. As the eye sees the<br />

smile, there are no abrupt changes in the outline of individual<br />

teeth (Figure 8).<br />

This short list of parameters can be helpful in taking a qualitative<br />

subject that is largely emotional for the patient and<br />

As the modern dental practice continues to<br />

evolve, many dentists are aiming toward a<br />

“paperless” practice. I personally think the term<br />

“paperless” is not really accurate as there will<br />

always be paper in the office. However, chartless<br />

is a realistic goal. Many practices no longer have a<br />

physical chart that is moved from room to room.<br />

As data becomes digital, protecting that data is<br />

absolutely critical. Unfortunately, what is seen<br />

in many practices is inadequate or no backup<br />

protocol for crucial data. How would your practice<br />

recover from such a traumatic loss? Why find out?<br />

If you talk to ten people in the IT (Information<br />

Technology) field, you’ll get ten different opinions<br />

on how to do a proper backup. Rather than give<br />

you an explanation of how I design my backup,<br />

here are the important points to consider:<br />

1. There is no “right” or “wrong” way to do a<br />

backup, but for me there are two absolutely<br />

critical parts to the backup design. First, you<br />

need to backup on a frequency that minimizes<br />

the amount of data you would need to<br />

re-enter manually should you need to restore<br />

the data. For most offices, this is once a day,<br />

but for busy, large group practices, more<br />

frequent backups may be indicated. Secondly,<br />

and just as important, the backup must be<br />

taken offsite on a daily basis. Having a great<br />

backup at the office won’t do any good if the<br />

computers are stolen or if the office burns<br />

down.<br />

2. There are many programs designed to backup<br />

data. Windows® has a built-in program<br />

that, while rudimentary, can get the job done.<br />

My favorite is an inexpensive program called<br />

Karen’s Replicator, which can be customized<br />

for backup frequency and time. Certain dental<br />

programs (e.g. Eaglesoft, Dentrix Image)<br />

need special software to back up if you don’t<br />

want to manually “close” the database each<br />

night.<br />

3. Verification of the backup must be made on<br />

Simply restore the data to another computer. Many<br />

dentists install the program on a home computer,<br />

load the backed-up data, and then run the program<br />

at home to check that the data is correct, that the<br />

schedule looks right, that recent transactions are<br />

there, etc. In my opinion, this is the only way to<br />

truly know if the data has been backed up successfully.<br />

4. I’m not a huge fan of tapes. Why? See #2 above.<br />

Few people (if any) have a tape drive on their home<br />

computer so there’s no easy way to verify a tape<br />

backup. Secondly, the server is usually the only<br />

office computer with a tape drive, so if the server<br />

crashes, the tape is useless. Finally, I find most tape<br />

backup software to be confusing and clunky.<br />

5. I do like external USB drives, such as those from<br />

6. I suggest getting two drives with a folder on each<br />

for each day of the week and then swapping the<br />

drives out daily. That way, you will have 10 days’<br />

worth of data spread over two drives which is fine<br />

for almost any office.<br />

7. Having redundant backup is always a great idea.<br />

Options to consider would include backing up to<br />

at least one workstation, a laptop, and/or using an<br />

online backup service. Choose this service wisely, as<br />

you’ll want one that can keep multiple days of data,<br />

allows quick restoration, and protects the data well.<br />

Every office that is computerized should take stock of<br />

their current backup regimen, verify the data, and take<br />

any steps necessary to prevent a data loss disaster.<br />

Predictable Access Strategies<br />

Access is a critical foundation of endodontic<br />

therapy. Optimal cleansing,<br />

shaping, and obturation are only possible<br />

if adequate access has been made.<br />

Access procedures are most effi ciently<br />

carried out when viewed through a<br />

surgical operating microscope (SOM)<br />

(Global Surgical Microscopes, Global<br />

Surgical, St. Louis, MO, USA). Access can<br />

easily be too small, but is rarely made<br />

too large. Restricted access made with<br />

the intention of saving tooth structure<br />

from the belief that the tooth will be<br />

less at risk of vertical fracture is hopeful<br />

and misdirected. Restricted access ultimately<br />

causes defi ciencies in cleaning<br />

and shaping canals.<br />

Using the SOM properly can facilitate only the removal of<br />

tooth structure that is necessary. Access can, and often<br />

does, look quite different from the pictures in textbooks.<br />

Rotations, tipped teeth, crowns and previous restoratives<br />

(among many causes) can influence the size of access<br />

preparations.<br />

Ideal access creates straight-line entry into the coronal<br />

third of roots. Achieving this goal, especially in molar<br />

teeth, requires removal of the cervical dentinal triangle<br />

by the means described below. Straight-line access<br />

implies that a file can be placed vertically into the orifice<br />

of the canal without touching or being flexed by one of<br />

the walls of the access preparation. Unrestricted straightline<br />

access to the orifice allows subsequent access to all<br />

canal thirds. Ideal access and coronal third management<br />

is consistent with greater volumes of irrigation, increased<br />

tactile control, cleaner canals, and fewer iatrogenic issues.<br />

Conceptually, it is helpful to visualize the canal enlargement<br />

as starting from the coronal surface, continuing<br />

through the orifice, coronal and middle thirds, and ending<br />

at the minor constriction. In essence, access is the first<br />

step in cleaning and shaping. Preparation of the apical<br />

third to the correct size and optimal cleanliness has, as<br />

its origin, an access preparation that allows straight-line<br />

insertion of instruments and copious irrigation into the<br />

apical third. An access that is too small can easily engage<br />

file flutes through restrictive dentin with a reduction of<br />

tactile control. Such restrictive forces also increase the<br />

risk of fracture, strip perforation, and canal transportations<br />

of all types.<br />

Ideal access facilitates efficient canal location. The<br />

converse is true. Once located, orifices can be enlarged<br />

efficiently with rotary nickel titanium (RNT) orifice openers.<br />

An orifice that is correctly enlarged allows ideal<br />

tactile control, copious irrigation, and is easier to work<br />

through during obturation and placement of the final<br />

coronal seal. Specifically, if an orifice is opened to a .12<br />

taper in the coronal third, the orifice can then ideally be<br />

filled several mm into the canal from the pulpal floor<br />

with a flowable composite such as PermaFlo (Ultradent,<br />

South Jordan, Utah, USA). Flowable composite used in<br />

this manner can provide an ideal coronal seal especially<br />

when used in combination with a bonded obturation<br />

with a material like RealSeal.* PermaFlo can be used to<br />

fill just the orifice or the entire access preparation to complete<br />

the coronal build up as indicated. Under the SOM,<br />

orifice enlargement is simple, efficient, and predictable<br />

using orifice openers like the K3 Shapers* (*SybronEndo,<br />

Orange, California, USA). K3 shapers are available in three<br />

tapers .12, .10, and .08 with a fixed #25 tip size. The K3<br />

shapers are flexible, durable, and very efficient cutting<br />

instruments.<br />

In access for retreatment procedures, coronal access becomes<br />

even more vitally important. Removal of previous<br />

obturation material as well as metal or other fragments of<br />

all types (from posts to separated instruments) requires<br />

adequate room to visualize the fragment and provide<br />

space into which the fragment can be delivered. These<br />

goals are pursued in tandem with the requirement that<br />

access not only allow such removal but do so in a manner<br />

that does not additionally risk an iatrogenic event.<br />

Consideration should be given to removal of the whole<br />

crown in such a clinical case to; 1) save time, 2) allow<br />

visualization under the crown and see if recurrent decay<br />

or fractures are present, 3) confirm restorability. The<br />

CORONAflex (Kavo, Lake Zurich, Illinois, USA) can make<br />

crown removal simple, rapid, and efficient.<br />

Having the needed instruments to create access easily<br />

and efficiently has obvious value. It is noteworthy that<br />

access is made through a wide variety of materials and<br />

clinical situations (limited openings, rotated and tipped<br />

teeth, through porcelain, metals of all types including<br />

precious, semi precious, and non precious, as well as cast<br />

posts, etc). I use the LA Axxess kit (SybronEndo, Orange,<br />

California, USA) because it comprehensively contains all<br />

the burs needed for access, irrespective of the clinical<br />

indication. The kit also contains side cutting stainless<br />

steel line angle burs (of various sizes) with parabolic safe<br />

ended tips that both straighten up access line angles<br />

and act as orifice openers. Shaping the coronal third with<br />

stainless steel instead of RNT orifice openers can provide<br />

cost savings. The line angle burs of the LA Axxess kit have<br />

the potential to cause strip perforation, similar to a Peezo<br />

reamer, if they are used without regard for the width of<br />

the furcal side of the root wall. The line angle burs and<br />

the K3 shapers are both ideal for removing the cervical<br />

dentinal triangle.<br />

In summary, the access created to make endodontics<br />

possible is vitally important for the quality of the entire<br />

procedure. Excellent visualization should be combined<br />

with knowledge of expected anatomy to locate all canals<br />

and give the clinician a comprehensive operating command<br />

over the canals during their cleansing, shaping,<br />

and obturation.<br />

1. RATIONALE FOR LOWER ANTERIOR<br />

VENEERS<br />

Young adults often show their upper anterior teeth when<br />

they smile and speak but do not usually show their lower<br />

anterior teeth. However, due to the loss of muscle tone<br />

with aging, unavoidable sagging of facial tissues often<br />

causes display of lower anterior teeth in mature adults<br />

when speaking. Many patients are amazed to see the<br />

objectionable color and/or malpositioning of their lower<br />

anterior teeth, and express a desire for improved esthetics.<br />

Exercise to determine if lower anterior teeth are<br />

displayed when speaking:<br />

• Provide patient with a hand held mirror large<br />

enough to show the full face.<br />

• Instruct patient to hold mirror at arm’s length in<br />

front of their face and repeatedly say words with “s”<br />

sounds (i.e. Mississippi or six-thousand-six-hundredsixty-six).<br />

• Remind patient that they smile and show their<br />

upper anterior teeth only occasionally, but in their<br />

everyday speaking they display their lower anterior<br />

teeth continually.<br />

2. TOOTH PREPARATION and SEATING<br />

LOWER ANTERIOR CERAMIC VENEERS<br />

Lower anterior teeth are smaller than upper anterior<br />

teeth and have several millimeters of proximal contact in<br />

the incisal-apical direction. Their small size and the need<br />

for specific ceramic thickness for strength and correct<br />

color development result in tooth veneer preparations<br />

that are often cut through enamel and into dentin in<br />

some locations. To avoid sensitivity, pulpal death, and potential<br />

debonding problems, tooth preparations for lower<br />

anterior veneers require special bonding considerations<br />

and retentive features.<br />

A. Tooth preparations. Three general degrees of tooth<br />

reduction for veneer preps are: 1) No or slight, 2)<br />

Moderate, and 3) Deep (see CRA <strong>News</strong>letter Jan.<br />

’06). Slight to moderate tooth preparation is desired<br />

for lower anterior ceramic veneers, leaving the most<br />

enamel possible on the tooth surface for bonding.<br />

In some cases, very minimal or no tooth reduction<br />

is needed on the facial surface. Lower anterior<br />

veneers serve best when the incisal edge is reduced<br />

1–1.5mm into dentin, leading to a “butt” joint on<br />

the lingual surface of the tooth (see left illustration).<br />

Interproximal margins in the contact areas are<br />

easier to finish (see illustration Page 27).<br />

B. Obtaining adequate retention. A tooth veneer<br />

preparation cut into dentin can compromise<br />

long-term retention of the veneer. When<br />

more than one-half of the tooth preparation<br />

is in dentin, it is advisable to place additional<br />

mechanical retention into the proximal and<br />

incisal dentin on the seating appointment as<br />

shown in illustrations at left. (Also see D below<br />

and CRA <strong>News</strong>letter, Jan. ’06 for further information.)<br />

C. Matrices. Standard mylar matrices (about<br />

0.002 inch or 50um thick) often do not allow<br />

proper seating of small lower anterior veneers,<br />

but force them away from the tooth preparation<br />

in the facial direction. A significantly<br />

thinner matrix, 9um Mylar Matrix (Ho <strong>Dental</strong><br />

Company Inc., 805-898-9748), allows proper<br />

seating of veneers and retains optimal contact<br />

with adjacent teeth, but can be difficult to use<br />

because of its flexibility.

D. Seating. When minimal enamel is present, additional mechanical retention and<br />

special bonding techniques help make procedure more predictable. CRA Evaluators<br />

report clinical success and little to no post-op sensitivity when they use magnification,<br />

seat one veneer at a time, and follow techniques outlined below during<br />

seating appointment.<br />

Etch bonding surface of enamel with<br />

phosphoric acid gel. Wash gel off<br />

quickly and thoroughly to avoid<br />

etching the dentin.<br />

Apply self-etch primer and bond over<br />

entire tooth preparation including<br />

previously etched enamel, and blow<br />

thin with compressed air.<br />

Place veneer resin cement on prepared<br />

tooth and veneer surfaces.<br />

Seat veneer on tooth.<br />

Cure both the bond and cement through<br />

the veneer. Avoid overheating tooth<br />

by directing a gentle stream of<br />

compressed air onto the site during<br />

the cure.<br />

E. Finishing. Minimal interproximal finishing of each veneer is required before adjacent<br />

veneer is seated.<br />

F. Occlusion. Occlusal adjustment after cementation is always necessary. Incisal<br />

surfaces of veneers must be adequately thick and adjusted to exactly occlude with<br />

opposing upper anterior teeth, have proper incisal guidance, and have equal occlusal<br />

load on each veneer.<br />

G. Polishing adjusted ceramic. It is desirable to leave the original glaze surface on<br />

the veneer but is not possible when the incisal edge is adjusted. CRA Evaluators<br />

have found following finishing sequence to produce good esthetics.<br />

1) Smooth using standard composite finishing disks.<br />

2) Treat surface with Dialite intraoral pre-polish “pink” disks (W18DM), followed<br />

by Dialite intraoral fine grit “grey” polishing disks (W18D) (Brasseler USA,<br />

800-841-4522).<br />

3) Final polish using DiaShine intraoral paste with their round 12 mm soft bristle<br />

polishing brush (VH Technologies, 888-628-8300).<br />

3. CRA CONCLUSIONS<br />

Mature adults and some younger patients often have a prominent display of their lower<br />

anterior teeth when they speak. This report outlines the rationale for lower anterior veneers<br />

to improve esthetics and provides clinical techniques to aid successful treatment.<br />

Introduction<br />

Mtwo endodontic instruments (VDW, Germany) are a new<br />

generation of NiTi rotary instruments. The standard set includes<br />

four instruments with variable tip sizes ranging from<br />

#10 to #25 and tapers ranging from .04 to .06.<br />

After this basic sequence that gives to the canal a #25/.06<br />

shape, the system permits two different approaches to root<br />

canal preparation. The first sequence allows clinicians to<br />

achieve enlarged apical diameters that facilitate vertical<br />

condensation of gutta-percha, maintaining a size #25 apical<br />

preparation.<br />

Morphological Features<br />

The coloured ring on the handle identifies the size (according<br />

to ISO standards) and the number of grooved rings<br />

on the handle identifies instrument taper. Instruments are<br />

available in 21mm, 25mm, and 31mm lengths. These instruments<br />

are also produced with an extended cutting portion<br />

of 21mm, as well as the conventional 16mm cutting part,<br />

allowing the instrument to cut in the coronal portion of the<br />

canals on cavity access walls, where dentin interferences are<br />

often located (Fig. 2).<br />

The RA is one of the most effective measures in NiTi rotary<br />

instruments, enhancing cutting efficiency (Fig. 3c). The tip is<br />

non-cutting (Fig. 3d).<br />

The HA is determined by the instrument’s blade pitch—the<br />

larger it is, the more open the HA will be. The HA is variable<br />

in the same instrument, increasing from tip to handle as<br />

does the spiral pitch, while it is constant for smaller files. The<br />

variable HA reduces the tendency of the instrument to be<br />

sucked down into the canal.<br />

The tendency to advance spontaneously in the root canal<br />

for the smaller instrument is necessary to progress in the<br />

canal during the first phase of treatment. The operator<br />

should tend toward a pulling-out movement, holding back<br />

the instrument in rotation, enhancing the characteristic of<br />

removing debris and cutting efficiency.<br />

The Mtwo system is completed with two files specifically<br />

designed for retreatment—Mtwo R 15/.05 and Mtwo R<br />

25/.05—presenting an active tip which allows easy penetration<br />

of obturation material (Fig. 4).<br />

Mtwo NiTi rotary instruments are used at 300 rpm<br />

in a simultaneous technique without any early<br />

coronal enlargement 4 . After a glide path has been<br />

established with a #10 stainless steel K-type file,<br />

each instrument is taken to working length (WL)<br />

with light apical pressure. As soon as the clinician<br />

feels a binding sensation, the instrument should be<br />

pulled away for 1 to 2mm so it can work passively in<br />

a brushing action to selectively remove interferences<br />

and to advance toward the apex. The instruments are<br />

used with a lateral pressing movement to obtain<br />

a circumferential cut and only allowed to rotate at<br />

length for few seconds (Fig. 5, 6).<br />

The operative sequence suggested for these<br />

instruments is a crown-down technique whereby<br />

the apex is reached by every NiTi instrument at<br />

each step. This means that this is a technique from<br />

crown to apex, but it first uses smaller instruments<br />

before using larger ones, as is done in the step-back<br />

technique. The inventor defines this as a “simultaneous<br />

technique,” as the entire length of the canal is<br />

approached at the same time. The instrument must<br />

not be forced in; as soon as the clinician feels a<br />

binding sensation, the instrument must be backed<br />

away for 1 to 2mm so that it can work passively to<br />

create the space necessary to go to the apex (Fig.<br />

7a, 7b). Using the instruments with a lateral pressing<br />

movement (brushing, milling), the tendency to<br />

progress automatically in the canal (a sensation of<br />

being “sucked down”) increases efficiency. The high<br />

flexibility and fatigue resistance 11 of Mtwo instruments<br />

permit the use of this approach in severely<br />

curved root canals with an efficient and safe action<br />

(Fig. 8, 9) 6-11 .<br />

Conclusion<br />

An important consideration regarding the proposal<br />

for a simultaneous approach using NiTi rotary<br />

instruments concerns differing points of view about<br />

the crown-down concept. In the simultaneous<br />

technique, the coronal portion is prepared before<br />

the apical one, using smaller instruments first. In<br />

the NiTi era, the crown-down concept is instead<br />

associated with the use of larger instruments (eg.<br />

tip diameter, taper) for the shaping of the coronal<br />

portion, followed by smaller instruments to advance<br />

toward the apex15 . The use of smaller instruments<br />

first is not in contrast to the crown-down approach,<br />

because it is also a crown-down technique in which<br />

the canal is prepared starting from the coronal<br />

toward the apical portion, even if all instruments<br />

reach the apex.<br />

This new concept facilitates root canal shaping<br />

particularly in the most difficult cases, reducing the<br />

incidence of procedural errors in the first phase of<br />

treatment in which the canal has to be negotiated<br />

with rigid stainless steel files to at least 0.20mm.<br />

The ability to control bleeding can be the cause of failure in<br />

the case of crown and bridge treatments. Bleeding makes<br />

it difficult or even impossible to take a good impression. In<br />

this case, aluminum chloride or ferric sulfate can be used to<br />

achieve hemostasis.<br />

Aluminum chloride, unlike ferric sulfate, functions via<br />

“vaso-constriction” but not in the customary mechanism of<br />

vascular smooth muscle constriction as occurs with epinephrine.<br />

Aluminum chloride causes the collagen fibers in the<br />

tissues around the capillaries to swell. While this occurs with<br />

all mineral astringents, it occurs to a greater degree with<br />

aluminum chloride. The expansion of the collagen around<br />

the capillaries induces pressure, which causes the capillaries<br />

to constrict, thus achieving temporary hemostasis.<br />

The advantages of aluminum chloride include: no coagulum<br />

staining of the hard and/or soft tissues.<br />

ViscoStat ® Clear is a 25% (m/m) aluminum chloride gel with<br />

a viscous and aqueous consistency and a pH of ~1. ViscoStat<br />

Clear is used to create temporary hemostasis of the gingiva<br />

during dental procedures such as, but not limited to, dental<br />

impressions, seating of temporary and permanent restorations,<br />

restorations of cavities, and placement of a rubber dam.<br />

ViscoStat Clear is supplied with the manufacturer’s Dento-<br />

Infusor tip. The brush fibers are kind to tissues, adjustable<br />

for ideal customization, and allow the material to spread<br />

efficiently. ViscoStat Clear causes the collagen in the capillary<br />

ends to swell and closes off the capillaries.<br />

hemostatics, thoroughly wash the region using a firm air/<br />

water spray. Hemostatics, temporary cements, and blood<br />

can compromise the quality of adhesion and polymerization<br />

of resins, which can lead to microleakage under the<br />
