Just looking at the specifications of a
camera doesn’t mean it will take the
kind of pictures wanted. Needs, budget,
and knowledge are important determinates
in deciding the best camera for you.
Here are some terms and related discussion
points about digital cameras.
Introduction
The popularity of digital photography is booming. In a recent survey, 47% of dentists
who responded owned a digital camera and 13% planned to purchase in the next 12
months 1 . Ninety percent of those surveyed use their digital pictures for laboratory
communication. Digital photography makes it possible for the immediate taking and
communicating of images between the patient, dental laboratory, general dentist,
and specialist. Pictures, especially for lab communication, are a necessity when trying
to match teeth for color, luster, shape, and character with fabricated crowns and veneers
2,3 . Photographs are much better than hand drawn maps of teeth and, in fact, a digital camera with specialized software can
now give accurate shade prescriptions, reducing subjectivity and the difficulty of shade matching.
Photographs with the case also give an emotional connection between the technician and patient 3,4 . That is, the ceramist is not
just looking at a stone model; also seen are the teeth in the face, the shape of the head, the symmetry of the smile, and what is
exposed in the aesthetic zone. All of these are assets that help the ceramist do their very best work.
apixel?” The reality is this is one of the last
things consider because most cameras
today meet or exceed what is needed.
Each “pixel” represents a color point on
the digital camera’s conversion sensor
(CCD or CMOS). One megapixel equals
one million dots of the color red, green,
or blue. It is the mixture of these colors
that makes up all color and the image 5 .
Have you wondered why pictures vary in
file size? In nature we see more greens
and blues than reds. So even though
there are, for example, a million pixels, research
shows there are a higher number
of green and blue pixels than red ones.
So, in this case, the picture of pure green
or blue is around 1780k and the picture
of red is 35% less, at 1162k. The manufacturer’s
picture color is often skewed
toward a particular color (see white balance
below).
Approximately, 1 megapixel is capable of
printing a 4” x 6” sheet without deterioration;
2 megapixels does the same for an
8.5” x 11” sheet. To put it in perspective, most computer
monitors are just smaller than 1 megapixel (1024 x 768 =
786,432 pixels). A 6 megapixel image would be 6 times
as large as your screen and up to 15 megabytes—far too
large for storage, manipulation, or electronic communication.
Also, 2 megapixels that are sharp and clear is better
than 6 megapixels that are blurry!
Cameras have quality settings that also affect image size.
For most uses, simply adjust image size to between 1 to 2
megabytes. The cameras mentioned below have reached
a point where the resolution (around 6 megapixels)
exceeds the needs of most users. Emphasis should be
placed on usability and image quality 5,6 .
White Balance/Color Adjustment
Off the shelf, each camera will have its own color characteristics
and adjustment methods. Usually, custom white
balancing the camera gets color close to accurate. This
procedure is not difficult and will be explained in the
owner’s manual7 . Not all cameras have the white balance
function and, therefore, give you less ability to correct
color. This is true of the Kodak 7490 that has no white balance
feature and gives a magenta look.
Aperature (f stop)
The larger the number, the increased depth of field. So
f22 would be suitable for an anterior picture straight
on where front and back teeth need to be in focus. F5.6
would be for a full-face portrait where a blurry background
is wanted4 .
DIGITAL CAMERAS
Lens Reflex) interchangeable lens cameras and the modi-
DSLR Cameras
In DSLR cameras, the most popular brands (in order) are
the Canon Digital Rebel XT (Pic 2), Canon 20D (Pic 3), Fuji
S3 Pro (Pic 4), and Nikon D70.
Lighting
Similarities with these cameras are that they are sophisticated
with capabilities that go beyond dental
photography; they should have a front-mounted ring
flash, point flash, or dual mounted lights, and are usually
manually focused for close up photography. Without a
ring flash, macro pictures will sometimes be washed out8 .
The external flash is metered through the lens by the
cameras mentioned, except the Nikon. On the Nikon, the
power of the flash must be set manually which then may
need readjusting depending on the picture. Controlling
the power and angle of light on the teeth gives different
important information.
Lens
Most owners outfit their camera with a 100mm macro
lens which has the ability to focus up close. Because the
digital camera uses a CCD or CMOS sensor that is of a
different size than 35mm film, a 100mm lens on a 35mm
camera converts to 160mm on a digital camera. This
means a higher magnified lens gives a flattening effect
on pictures. Canon recently released a 60mm macro lens
which equates to 96mm—this is a better choice.
Image Size - Megapixel
All these cameras are around 6 megapixels and have
many adjustments, so image quality will not be a problem.
Settings
The camera should be put on aperture priority (Av) and
f stop set around f22 for close up pictures where good
depth of field is needed. It is recommended to slightly
underexpose pictures to better see tooth color and luster.
A picture can always be slightly brightened without
losing quality but you cannot darken an overexposed
picture and maintain quality. Full-face photos should be
taken at f5.6 for more emphasis on the face 3 . On Canon
cameras the flash can be manually set to the desired
power of light (-1/3 on the 30D).
DSLR cameras, at about 4 pounds, are substantially heavier
than Point and Shoot cameras and can be intimidating
because of the weight and attached apparatus. Most
people prefer a lighter camera if quality is comparable.
DSLR cameras have the following characteristics:
1) Viewing is Through the Lens (TTL)
2) Require external lighting
3) Are manually focused
4) Are heavier
5) Provide greater depth of field
For more Product Information, please visit www.dentalHUB.net
6) Are more expensive
Point and Shoot Digital Cameras
As noted above, don’t let camera size or megapixel drive
your purchasing decision. High quality and DSLR features
can be found in a small package that takes superb pictures.
Some of these cameras have been modified and
some are off the shelf. Some cameras have the capability
to take dental photos and some do not, regardless of the
specifications! These cameras are between 4 and 7 megapixels—plenty
big enough for good quality. Things to
look for are the ability to focus close, color control, proper
lighting, exposure control, depth of field, overall ease of
use, and support. All the following cameras qualify.
The Canon DentalFoto, Olympus C5060, and Canon G6
are three good examples in this classification. These cameras
have similar features to DSLR cameras although lighter
weight, but don’t have the range of settings as DSLR or interchangeable
lenses. In addition, viewing is done through
an LCD screen, not through the lens.
Lighting
The Canon DentalFoto (Pic 7) and
Olympus C5060 use onboard lighting
located on the camera body close to
the lens. Light is diffused and eliminates
the need for a ring flash. The
Canon G6 needs either the Canon
MR-14EX ring light or a proprietary
diffuser. Lighting is very good.
set up correctly. The Olympus C5060
uses no additional close up lens and
works best at about 8 inches from
the subject (further away than the
other two models).
Image Size – Megapixel
goes to the Canon DentalFoto
C5060 due to memorized settings
The saying, “beauty is in the eye of the beholder”,
is very true in the realm of cosmetics; additionally,
beauty is in the mind of the owner. Beyond
the technical aspects of dentistry, in cosmetics the
psychology and opinions of the patient and the
people in their lives who influence those opinions
are equal, if not more important factors.
The dentist’s technical command of the technique
to create precise margins, the optimal choice of
restorative materials, proper interproximal contour
design, and harmonizing with physiological occlusal
function, all are factors in case longevity and are
the foundation for long term success. The shorter
term challenge of gaining enthusiastic patient acceptance
is in the emotional realm of whether or
not they like the “look”.

Following is a checklist of the more important factors used
to define what makes the “look” of a smile attractive and can
be used as an adjunct when designing a case (Figure 1).
Horizontal plane of occlusion is perpendicular to the long
axis of the face. Most patients have some facial asymmetry
that should be taken into account when determining the
long axis of the face. The best way to make an assessment
of the horizontal plane is with the patient standing straight
with the dentist positioned directly in front of them. A physical
recording of the plane can then be made (e.g. stick bite
registration or facebow). A Fox plane is an excellent adjunct
to highlight the plane.
The slight curve of the maxillary teeth follows the curve of
the lower lip when smiling. This can be quite subtle or occasionally
exaggerated. Edges of the teeth are relatively higher
as the eye moves distally (Figure 2).
There is a proportion and dominance to anterior teeth. The
maxillary central incisor is usually 10.5-11mm from gingival
to incisal edge. If the length-to-width ratio of that tooth can
be kept to 75-80% and the Golden proportion (1.6/1.0/0.6)
of the relative visual width of the central, lateral, and cuspid,
the smile will have a natural harmony.
Incisal embrasures of maxillary teeth progress from small to
large with a distinct and slightly asymmetric form between
lateral and cuspid. This tends to separate teeth and is characteristic
of youth (Figure 3).
Bilateral symmetry exists. Perfect symmetry is actually
undesirable as it looks synthetic. The relative display of the
contralateral tooth tends to mirror the other including incisal
edge. The gingival architecture is as important as tooth
structure (Figure 4).
Aesthetic Dentistry
A more youthful smile displays more teeth, both vertically
and horizontally. It is not uncommon to have close to 100%
display of maxillary anterior in a full smile, especially in
young women. An extension of 1-2mm below the edge of
the rested lip is also often present. Bicuspids tend to fill the
buccal space giving the appearance of breadth (Figure 5).
Surface contours are typically highly reflective and convex,
with vertical flutings to break up the light. This seems to be
true whether the teeth, particularly incisors, are shaped relatively
square, ovoid, round, or triangular. Using the general
outline of the face turned upside-down seems to work. A
more ovoid form works for many females, especially if the
desire is to create a more youthful and softer appearance
(Figure 6).
Shade is light. The societal trend is ‘lighter is better’. Bleaching
is marketed everywhere. If in the design of restorations
the dentist can maintain a natural looking translucency pattern,
as the shades of the teeth become lighter it counters
the tendency to become overly opaque. Hue, chroma, and
value are all factors that play important roles. Use various
lighting to chose the best overall option because patients are
not seen under full spectrum light conditions (Figure 7).
Individual teeth are not monochromatic but exhibit a
gradation of color and opacity from gingival through incisal.
Because of thinning of the enamel toward the gingival edge
of a tooth, more intense colors appear.
Teeth are aligned smoothly into the arch form and have a
harmonious flow from tooth to tooth. As the eye sees the
smile, there are no abrupt changes in the outline of individual
teeth (Figure 8).
This short list of parameters can be helpful in taking a qualitative
subject that is largely emotional for the patient and
quantifying it to give patients truly attractive smiles. APDN
As the modern dental practice continues to
evolve, many dentists are aiming toward a
“paperless” practice. I personally think the term
“paperless” is not really accurate as there will
always be paper in the office. However, chartless
is a realistic goal. Many practices no longer have a
physical chart that is moved from room to room.
As data becomes digital, protecting that data is
absolutely critical. Unfortunately, what is seen
in many practices is inadequate or no backup
protocol for crucial data. How would your practice
recover from such a traumatic loss? Why find out?
If you talk to ten people in the IT (Information
Technology) field, you’ll get ten different opinions
on how to do a proper backup. Rather than give
you an explanation of how I design my backup,
here are the important points to consider:
1. There is no “right” or “wrong” way to do a
backup, but for me there are two absolutely
critical parts to the backup design. First, you
need to backup on a frequency that minimizes
the amount of data you would need to
re-enter manually should you need to restore
the data. For most offices, this is once a day,
but for busy, large group practices, more
frequent backups may be indicated. Secondly,
and just as important, the backup must be
taken offsite on a daily basis. Having a great
backup at the office won’t do any good if the
computers are stolen or if the office burns
down.
2. There are many programs designed to backup
data. Windows® has a built-in program
that, while rudimentary, can get the job done.
My favorite is an inexpensive program called
Karen’s Replicator, which can be customized
for backup frequency and time. Certain dental
programs (e.g. Eaglesoft, Dentrix Image)
need special software to back up if you don’t
want to manually “close” the database each
night.
3. Verification of the backup must be made on
a regular basis. I recommend at least once
per month, but once a week would be great
if you have the time. How do you do this?
Simply restore the data to another computer. Many
dentists install the program on a home computer,
load the backed-up data, and then run the program
at home to check that the data is correct, that the
schedule looks right, that recent transactions are
there, etc. In my opinion, this is the only way to
truly know if the data has been backed up successfully.
4. I’m not a huge fan of tapes. Why? See #2 above.
Few people (if any) have a tape drive on their home
computer so there’s no easy way to verify a tape
backup. Secondly, the server is usually the only
office computer with a tape drive, so if the server
crashes, the tape is useless. Finally, I find most tape
backup software to be confusing and clunky.
5. I do like external USB drives, such as those from
Maxtor, Western Digital, or Seagate. Because they
Looking for the
latest technology in
dentistry?
We focus on
Dental
Products
are USB they can easily be used at home, are fast,
secure, and have almost unlimited storage.
6. I suggest getting two drives with a folder on each
for each day of the week and then swapping the
drives out daily. That way, you will have 10 days’
worth of data spread over two drives which is fine
for almost any office.
7. Having redundant backup is always a great idea.
Options to consider would include backing up to
at least one workstation, a laptop, and/or using an
online backup service. Choose this service wisely, as
you’ll want one that can keep multiple days of data,
allows quick restoration, and protects the data well.
Every office that is computerized should take stock of
their current backup regimen, verify the data, and take
any steps necessary to prevent a data loss disaster.
Predictable Access Strategies

Richard Mounce DDS
Access is a critical foundation of endodontic
therapy. Optimal cleansing,
shaping, and obturation are only possible
if adequate access has been made.
Access procedures are most effi ciently
carried out when viewed through a
surgical operating microscope (SOM)
(Global Surgical Microscopes, Global
Surgical, St. Louis, MO, USA). Access can
easily be too small, but is rarely made
too large. Restricted access made with
the intention of saving tooth structure
from the belief that the tooth will be
less at risk of vertical fracture is hopeful
and misdirected. Restricted access ultimately
causes defi ciencies in cleaning
and shaping canals.

Dr. Mounce lectures globally and is widely published.
He is a private practice Endodontist in Vancouver,
Washington, USA. Among other appointments, he
is the endodontic consultant for the Belau National
Hospital Dental Clinic in the Republic of Palau. Korror,
Palau (Micronesia). He can be reached at
RichardMounce@mounceendo.com.
Using the SOM properly can facilitate only the removal of
tooth structure that is necessary. Access can, and often
does, look quite different from the pictures in textbooks.
Rotations, tipped teeth, crowns and previous restoratives
(among many causes) can influence the size of access
preparations.
Ideal access creates straight-line entry into the coronal
third of roots. Achieving this goal, especially in molar
teeth, requires removal of the cervical dentinal triangle
by the means described below. Straight-line access
implies that a file can be placed vertically into the orifice
of the canal without touching or being flexed by one of
the walls of the access preparation. Unrestricted straightline
access to the orifice allows subsequent access to all
canal thirds. Ideal access and coronal third management
is consistent with greater volumes of irrigation, increased
tactile control, cleaner canals, and fewer iatrogenic issues.
Conceptually, it is helpful to visualize the canal enlargement
as starting from the coronal surface, continuing
through the orifice, coronal and middle thirds, and ending
at the minor constriction. In essence, access is the first
step in cleaning and shaping. Preparation of the apical
third to the correct size and optimal cleanliness has, as
its origin, an access preparation that allows straight-line
insertion of instruments and copious irrigation into the
apical third. An access that is too small can easily engage
file flutes through restrictive dentin with a reduction of
tactile control. Such restrictive forces also increase the
risk of fracture, strip perforation, and canal transportations
of all types.
Ideal access facilitates efficient canal location. The
converse is true. Once located, orifices can be enlarged
efficiently with rotary nickel titanium (RNT) orifice openers.
An orifice that is correctly enlarged allows ideal
tactile control, copious irrigation, and is easier to work
through during obturation and placement of the final
coronal seal. Specifically, if an orifice is opened to a .12
taper in the coronal third, the orifice can then ideally be
filled several mm into the canal from the pulpal floor
with a flowable composite such as PermaFlo (Ultradent,
South Jordan, Utah, USA). Flowable composite used in
this manner can provide an ideal coronal seal especially
when used in combination with a bonded obturation
with a material like RealSeal.* PermaFlo can be used to
fill just the orifice or the entire access preparation to complete
the coronal build up as indicated. Under the SOM,
orifice enlargement is simple, efficient, and predictable
using orifice openers like the K3 Shapers* (*SybronEndo,
Orange, California, USA). K3 shapers are available in three
tapers .12, .10, and .08 with a fixed #25 tip size. The K3
shapers are flexible, durable, and very efficient cutting
instruments.
In access for retreatment procedures, coronal access becomes
even more vitally important. Removal of previous
obturation material as well as metal or other fragments of
all types (from posts to separated instruments) requires
adequate room to visualize the fragment and provide
space into which the fragment can be delivered. These
goals are pursued in tandem with the requirement that
access not only allow such removal but do so in a manner
that does not additionally risk an iatrogenic event.
Consideration should be given to removal of the whole
crown in such a clinical case to; 1) save time, 2) allow
visualization under the crown and see if recurrent decay
or fractures are present, 3) confirm restorability. The
CORONAflex (Kavo, Lake Zurich, Illinois, USA) can make
crown removal simple, rapid, and efficient.
Having the needed instruments to create access easily
and efficiently has obvious value. It is noteworthy that
access is made through a wide variety of materials and
clinical situations (limited openings, rotated and tipped
teeth, through porcelain, metals of all types including
precious, semi precious, and non precious, as well as cast
posts, etc). I use the LA Axxess kit (SybronEndo, Orange,
California, USA) because it comprehensively contains all
the burs needed for access, irrespective of the clinical
indication. The kit also contains side cutting stainless
steel line angle burs (of various sizes) with parabolic safe
ended tips that both straighten up access line angles
and act as orifice openers. Shaping the coronal third with
stainless steel instead of RNT orifice openers can provide
cost savings. The line angle burs of the LA Axxess kit have
the potential to cause strip perforation, similar to a Peezo
reamer, if they are used without regard for the width of
the furcal side of the root wall. The line angle burs and
the K3 shapers are both ideal for removing the cervical
dentinal triangle.
In summary, the access created to make endodontics
possible is vitally important for the quality of the entire
procedure. Excellent visualization should be combined
with knowledge of expected anatomy to locate all canals
and give the clinician a comprehensive operating command
over the canals during their cleansing, shaping,
and obturation.
1. RATIONALE FOR LOWER ANTERIOR
VENEERS
Young adults often show their upper anterior teeth when
they smile and speak but do not usually show their lower
anterior teeth. However, due to the loss of muscle tone
with aging, unavoidable sagging of facial tissues often
causes display of lower anterior teeth in mature adults
when speaking. Many patients are amazed to see the
objectionable color and/or malpositioning of their lower
anterior teeth, and express a desire for improved esthetics.
Exercise to determine if lower anterior teeth are
displayed when speaking:
• Provide patient with a hand held mirror large
enough to show the full face.
• Instruct patient to hold mirror at arm’s length in
front of their face and repeatedly say words with “s”
sounds (i.e. Mississippi or six-thousand-six-hundredsixty-six).
• Remind patient that they smile and show their
upper anterior teeth only occasionally, but in their
everyday speaking they display their lower anterior
teeth continually.
Treatment options
Upgrading appearance and function of healthy, vital,
discolored or malpositioned lower anterior teeth includes
several options: 1) Bleaching; 2) Bonding; 3) Orthodontics;

4) Orthodontics and bleaching; 5) Bleaching and bonding;
6) Veneers; and 7) Crowns. Options should be discussed
with patient to educate and allow them to choose
the treatment they prefer, and to satisfy legal requirements
for informed consent.
2. TOOTH PREPARATION and SEATING
LOWER ANTERIOR CERAMIC VENEERS
Lower anterior teeth are smaller than upper anterior
teeth and have several millimeters of proximal contact in
the incisal-apical direction. Their small size and the need
for specific ceramic thickness for strength and correct
color development result in tooth veneer preparations
that are often cut through enamel and into dentin in
some locations. To avoid sensitivity, pulpal death, and potential
debonding problems, tooth preparations for lower
anterior veneers require special bonding considerations
and retentive features.
A. Tooth preparations. Three general degrees of tooth
reduction for veneer preps are: 1) No or slight, 2)
Moderate, and 3) Deep (see CRA Newsletter Jan.
’06). Slight to moderate tooth preparation is desired
for lower anterior ceramic veneers, leaving the most
enamel possible on the tooth surface for bonding.
In some cases, very minimal or no tooth reduction
is needed on the facial surface. Lower anterior
veneers serve best when the incisal edge is reduced
1–1.5mm into dentin, leading to a “butt” joint on
the lingual surface of the tooth (see left illustration).
Interproximal margins in the contact areas are
easier to finish (see illustration Page 27).
CRA Report
B. Obtaining adequate retention. A tooth veneer
preparation cut into dentin can compromise
long-term retention of the veneer. When
more than one-half of the tooth preparation
is in dentin, it is advisable to place additional
mechanical retention into the proximal and
incisal dentin on the seating appointment as
shown in illustrations at left. (Also see D below
and CRA Newsletter, Jan. ’06 for further information.)
C. Matrices. Standard mylar matrices (about
0.002 inch or 50um thick) often do not allow
proper seating of small lower anterior veneers,
but force them away from the tooth preparation
in the facial direction. A significantly
thinner matrix, 9um Mylar Matrix (Ho Dental
Company Inc., 805-898-9748), allows proper
seating of veneers and retains optimal contact
with adjacent teeth, but can be difficult to use
because of its flexibility.
CRA Report
D. Seating. When minimal enamel is present, additional mechanical retention and
special bonding techniques help make procedure more predictable. CRA Evaluators
report clinical success and little to no post-op sensitivity when they use magnification,
seat one veneer at a time, and follow techniques outlined below during
seating appointment.
Tooth surface mostly enamel
Etch bonding surface of veneer with
Silanate etched surface of veneer.
Etch bonding surface of enamel with
phosphoric acid gel. Wash gel off
quickly and thoroughly to avoid
removal of dentin smear layer and
etching the dentin.
Apply self-etch primer and bond over
entire tooth preparation including
previously etched enamel, and blow
thin with compressed air.
Place veneer resin cement on prepared
tooth and veneer surfaces.
Seat veneer on tooth.
Cure both the bond and cement through
the veneer. Avoid overheating tooth
by directing a gentle stream of
compressed air onto the site during
the cure.
E. Finishing. Minimal interproximal finishing of each veneer is required before adjacent
veneer is seated.
F. Occlusion. Occlusal adjustment after cementation is always necessary. Incisal
surfaces of veneers must be adequately thick and adjusted to exactly occlude with
opposing upper anterior teeth, have proper incisal guidance, and have equal occlusal
load on each veneer.
G. Polishing adjusted ceramic. It is desirable to leave the original glaze surface on
the veneer but is not possible when the incisal edge is adjusted. CRA Evaluators
have found following finishing sequence to produce good esthetics.
1) Smooth using standard composite finishing disks.
2) Treat surface with Dialite intraoral pre-polish “pink” disks (W18DM), followed
by Dialite intraoral fine grit “grey” polishing disks (W18D) (Brasseler USA,
800-841-4522).
3) Final polish using DiaShine intraoral paste with their round 12 mm soft bristle
polishing brush (VH Technologies, 888-628-8300).
3. CRA CONCLUSIONS
Mature adults and some younger patients often have a prominent display of their lower
anterior teeth when they speak. This report outlines the rationale for lower anterior veneers
to improve esthetics and provides clinical techniques to aid successful treatment.
Introduction
Mtwo endodontic instruments (VDW, Germany) are a new
generation of NiTi rotary instruments. The standard set includes
four instruments with variable tip sizes ranging from
#10 to #25 and tapers ranging from .04 to .06.
After this basic sequence that gives to the canal a #25/.06
shape, the system permits two different approaches to root
canal preparation. The first sequence allows clinicians to
achieve enlarged apical diameters that facilitate vertical
condensation of gutta-percha, maintaining a size #25 apical
preparation.
Morphological Features
The coloured ring on the handle identifies the size (according
to ISO standards) and the number of grooved rings
on the handle identifies instrument taper. Instruments are
available in 21mm, 25mm, and 31mm lengths. These instruments
are also produced with an extended cutting portion
of 21mm, as well as the conventional 16mm cutting part,
allowing the instrument to cut in the coronal portion of the
canals on cavity access walls, where dentin interferences are
often located (Fig. 2).
The RA is one of the most effective measures in NiTi rotary
instruments, enhancing cutting efficiency (Fig. 3c). The tip is
non-cutting (Fig. 3d).
The HA is determined by the instrument’s blade pitch—the
larger it is, the more open the HA will be. The HA is variable
in the same instrument, increasing from tip to handle as
does the spiral pitch, while it is constant for smaller files. The
variable HA reduces the tendency of the instrument to be
sucked down into the canal.
The tendency to advance spontaneously in the root canal
for the smaller instrument is necessary to progress in the
canal during the first phase of treatment. The operator
should tend toward a pulling-out movement, holding back
the instrument in rotation, enhancing the characteristic of
removing debris and cutting efficiency.
The Mtwo system is completed with two files specifically
designed for retreatment—Mtwo R 15/.05 and Mtwo R
25/.05—presenting an active tip which allows easy penetration
of obturation material (Fig. 4).
Mtwo NiTi rotary instruments are used at 300 rpm
in a simultaneous technique without any early
coronal enlargement 4 . After a glide path has been
established with a #10 stainless steel K-type file,
each instrument is taken to working length (WL)
with light apical pressure. As soon as the clinician
feels a binding sensation, the instrument should be
pulled away for 1 to 2mm so it can work passively in
a brushing action to selectively remove interferences
and to advance toward the apex. The instruments are
used with a lateral pressing movement to obtain
a circumferential cut and only allowed to rotate at
length for few seconds (Fig. 5, 6).
The operative sequence suggested for these
instruments is a crown-down technique whereby
the apex is reached by every NiTi instrument at
each step. This means that this is a technique from
crown to apex, but it first uses smaller instruments
before using larger ones, as is done in the step-back
technique. The inventor defines this as a “simultaneous
technique,” as the entire length of the canal is
approached at the same time. The instrument must
not be forced in; as soon as the clinician feels a
binding sensation, the instrument must be backed
away for 1 to 2mm so that it can work passively to
create the space necessary to go to the apex (Fig.
7a, 7b). Using the instruments with a lateral pressing
movement (brushing, milling), the tendency to
progress automatically in the canal (a sensation of
being “sucked down”) increases efficiency. The high
flexibility and fatigue resistance 11 of Mtwo instruments
permit the use of this approach in severely
curved root canals with an efficient and safe action
(Fig. 8, 9) 6-11 .
Conclusion
An important consideration regarding the proposal
for a simultaneous approach using NiTi rotary
instruments concerns differing points of view about
the crown-down concept. In the simultaneous
technique, the coronal portion is prepared before
the apical one, using smaller instruments first. In
the NiTi era, the crown-down concept is instead
associated with the use of larger instruments (eg.
tip diameter, taper) for the shaping of the coronal
portion, followed by smaller instruments to advance
toward the apex15 . The use of smaller instruments
first is not in contrast to the crown-down approach,
because it is also a crown-down technique in which
the canal is prepared starting from the coronal
toward the apical portion, even if all instruments
reach the apex.
This new concept facilitates root canal shaping
particularly in the most difficult cases, reducing the
incidence of procedural errors in the first phase of
treatment in which the canal has to be negotiated
with rigid stainless steel files to at least 0.20mm.
The ability to control bleeding can be the cause of failure in
the case of crown and bridge treatments. Bleeding makes
it difficult or even impossible to take a good impression. In
this case, aluminum chloride or ferric sulfate can be used to
achieve hemostasis.
Aluminum chloride, unlike ferric sulfate, functions via
“vaso-constriction” but not in the customary mechanism of
vascular smooth muscle constriction as occurs with epinephrine.
Aluminum chloride causes the collagen fibers in the
tissues around the capillaries to swell. While this occurs with
all mineral astringents, it occurs to a greater degree with
aluminum chloride. The expansion of the collagen around
the capillaries induces pressure, which causes the capillaries
to constrict, thus achieving temporary hemostasis.
The advantages of aluminum chloride include: no coagulum
