a letter From the Chair oF the FrienDs
Committee oF the U-m DePartment oF Urology
Dear Friends:
harsh economic times have
impacted philanthropic support
for the University of michigan
Department of Urology. there
have been challenges to fundraising,
both economically and
practically, but the Friends’
Committee continues to meet
those challenges with the cooperation
and support of the
doctors and staff of the Department
of Urology, particularly Dr.
David Bloom, Chair of the
Department.
our Par golf outing had an excellent turnout at the U-m
golf Course under the leadership of gary marley and his
dedicated committee. We expect an even more successful
outing in 2009 with the date moved to monday, June
15th. Please mark your calendars.
our very successful michigan Fantasy Football Camp
encountered practical challenges in 2008, which included
the unavailability of the stadium and locker rooms due to
construction at “the Big house.” While we continue to
enjoy the support of the michigan athletic Department
and the michigan football coaches and staff, it was
decided that a diluted camp, short on both facilities and
time, would not offer the type of experience we created in
prior camps. therefore, the 2008 Camp was cancelled.
the annual men’s event held in september at Flemings
steak house in Birmingham, michigan, was a success.
132 supporters attended the dinner which was preceded
by a presentation and Q&a session by members of the
Department of Urology. significant networking took
place throughout the evening.
through the support and efforts of Friends Committee
member hamilton Chang, we took our message
“on the road” to meet enthusiastic supporters in the
“Chicagoland” area. We believe that these types of
opportunities through the support of the Friends
2
Committee help to widen our philanthropic donor base
in distant communities due to individuals with a history
of support for the University of michigan.
lois stulberg, another member of the Friends Committee,
has been involved for many years supporting research into
ways of battling kidney cancer. through her continuing
efforts, the U-m Kidney Cancer symposium was presented
for the first time with successful attendance and results.
the Chang lecture on art and medicine featuring Dr.
mysoon rizk on “healing Power: art, artist, and Disease
through history” was presented in the summer to a packed
house composed of Department of Urology faculty and
staff as well as Chang family supporters and friends.
Dr. James montie heads the Prostate Cancer survivorship
Clinic. Daniela Wittmann, lmsW, Cst, Department of
Urology sexual health Coordinator, organized our first
survivorship retreat to address the issues surrounding
sexuality for prostate cancer survivors and their
partners. From attendees’ feedback, the retreat was a
success and was presented again on april 21st, 2009.
as you can see, the economic downturn did not obliterate
our fund raising activities or the level of enthusiasm
of the Committee. $67,578.00 was raised through
the Friends Committee to support the Department of
Urology while philanthropic activities of the Department
as a whole raised over $5 million through individual
gifts, foundations and corporate support. 2009 will
continue to present challenges. We believe that with
your continuing support and encouragement, our goal
of continuing philanthropic support for the U-m health
system Department of Urology, can be sustained.
With best regards,
David Bloom
a WorD From oUr Chairman
Dear Friends:
the many exciting vistas ahead of
us in science, medicine, and in
medical science are tempered by
immediate concerns of the world
economy in addition to the issues
of global politics and climate
change lurking not so far away.
yet our fundamental missions of
teaching the next generation,
deploying state-of-the-art medical
care, and improving that care
through discovery must and will
continue. the Department of
Urology is fulfilling these three
missions well and anticipates doing
so even better in the future.
this residency season all eight U-m medical students
who applied for urology positions matched with their top
choices at the very best of our peer institutions. given
that the national match rate for urology was only 68%
our placement of all eight U-m students is extraordinary.
this was the first year for us to take four residents,
having gained approval to increase our allotment from
three per year. this year we will have one U-m student,
one student from Johns hopkins, one from Boston
University, and one from University of Pittsburgh.
our health system is transforming our outpatient
facilities to our newly organized Faculty group Practice,
meaning that clinical departments will manage
outpatient activities, ending an era in which they were
managed by the hospital. our department now functions
at 14 clinical locations and i expect that as health care
reformats nationally we will see larger systems such as
ours getting even larger and busier.
John Wei became our associate Chair for research
encompassing our health services research Division
under Brent hollenbeck and our established laboratory
research Division under Jill macoska. the health
services group with the support of the Dow Foundation
recently moved to an offsite commercial property called
the michigan house. Jill macoska has a newly obtained
ro1 grant in her teeter laboratory. evan Keller obtained
a renewal of his Po1 grant for $8 million to continue his
long-term study of prostate cancer bone metastases.
mark Day directs our george o’Brien Urology research
Center Program that focuses on genitourinary
development and urologic disease. he studies how genes
are regulated, the phenomenon called epigenesis, and he
is a stalwart teacher of young PhDs doing urologic
research. monica liebert studies urothelial stem cells in
both normal and cancerous bladders and kidneys. John
Park continues to investigate the pathophysiology of
urinary tract obstruction, a major clinical problem in
pediatric urology. Will roberts and his histotripsy lab
are pursing the use of cavitational ultrasound energy for
noninvasive destruction of benign or malignant tumors.
according to federal data from 2006, michigan leads the
country in urological research with a total funding level
of nearly $20 million. this is distributed throughout our
university in the work of our joint faculty members as
well as our core faculty. Ken Pienta directs the nihfunded
sPore grant for prostate cancer and was recently
named associate Dean for research. arul Chinnaiyan
(named howard hughes investigator in 2007) is director
of the michigan Center for translational Pathology.
Chinnaiyan’s laboratory is focused on genomic, proteomic,
metabolomics and bioinformatic approaches to
understand cancer biology and to discover biomarkers.
Kathy Cooney is the Principal investigator of the U-m
Prostate Cancer genetics Project. the gary smith
laboratory continues to make ground-breaking progress
towards successful oocyte cryo-preservation and nanofluidics
platforms for successful sperm sorting.
the generous gifts of our benefactors have primed the
pump of urologic research here at U-m. We hope to stay
in the top ranks in years to come.
We expect challenging economic times in the months
ahead but intend that our educational efforts, clinical
practice, and discovery will continue full steam ahead.
Best wishes.
over 60 golfers including Dr. John Park, the Chengyang
Chang Professor of Pediatric Urology and
his wife, helen. the event raised $4,000 bringing
the cumulative total to over $24,000 and donor
participation was much higher than previous
years. the auction included fantastic prizes such
as a U-m tailgate Package including U-m Football
Club and many other fabulous items.
We owe a great deal of thanks to Vicky Button,
Christopher’s mom, and her good friends, Kim
and tom mcDonald, mary nicoletti and their
team for supporting our pediatric research in
honor of her late son. Chris was 20 years old when
he was diagnosed with stage four rhabdosarcoma.
his tumor was located on the prostate and was
so large that it encompassed his bladder as well.
Chris was treated at the University of michigan to
surgically remove the tumor followed by chemo
and radiation therapy.
Chris endured two separate surgeries. the first
was an attempt to save the bladder and reconstruct
it after removing the tumor. Unfortunately, the
cancer returned and Chris was faced with having
to remove the bladder entirely. he took the chemo
and radiation therapy in stride, but the idea of
having to remove his bladder was emotionally
traumatic. this aspect of the disease and treatment
was Chris’s biggest hurdle and the most difficult
to bear.
Chris fought very hard to stay positive and beat
the cancer. sadly, Chris passed away June 13th
2004, four years after his diagnosis. as stated
by Vicky “i donate to this Department because
Christopher was a very giving person who
loved kids and helping people. When he went
through his treatments the only thing that was
catastrophic was losing his bladder. it was his
hardest adjustment. he felt he would not be able to
lead a normal life because of that. in spite of this,
with a lot of support from family and friends he
adjusted. however, for children having to go long
term through life it is a hardship.” she continues
with “i as his mother was at first shocked that
there was not a better medical solution. getting
to know Dr. Park, i think he is truly trying to help
children and families with a better quality of life
all around, medically and emotionally. this is a
very worthy cause that will help not only children
but everyone in the long run.”
ConTInUED
last year, robotic-assisted radical prostatectomy was the
most frequent treatment performed for the management of
localized prostate cancer in the United states. this trend
is true at the University of michigan where over 80%
of prostate cancer surgeries are performed robotically.
however, unlike many other robotic prostatectomy
programs, our care does not end with the procedure.
through research in health-related quality of life, we closely
follow the outcomes of our patient following surgery and
strive to improve upon our excellent results in both treating
the cancer and returning patients to their pre-surgery
urinary and sexual function. the recognized importance
of quality of life as well as curing the cancer has lead us to
develop one of the nation’s few survivorship programs for
prostate cancer which focuses on the functional recovery
of our patients following surgery. this effort is part of
our multidisciplinary prostate cancer care through the
University of michigan Comprehensive Cancer Center
where we work in conjunction with medical oncology and
radiation oncology to provide holistic care to our patients
with prostate cancer. in an effort to expand options for the
management of prostate cancer, we currently are expanding
our cryotherapy program for prostate cancer treatment,
another minimally invasive option. in addition, we have
developed an active surveillance program which allows
physicians and prostate cancer patients to work together to
decide when the appropriate time is to intervene.
our use of robotic surgery has extended to the treatment
of kidney cancer. Currently, we are one of the most
experienced health centers with laparoscopic partial
nephrectomy through the pioneering techniques of
Dr. J. stuart Wolf, Jr. Partial nephrectomy offers similar
cancer control as removal of the entire kidney but leaves
healthy kidney behind to preserve a patient’s kidney
function. in an effort to treat larger tumors with minimally
invasive approaches, we are currently performing
robotic assisted partial nephrectomy which allows patients
to undergo the surgery through several small punctures
instead of the large flank incision had been the post
standard of care. this results in a faster recovery for the
patient. in addition to treating larger tumors with robotic
approaches, we offer other minimally invasive approaches
which destroy the tumor without requiring removal based
on the size and location of the cancer in the kidney. these
approaches can be performed laparoscopically or percutaneously
using radiofrequency ablation or cryotherapy.

Finally, based on extensive research performed by our
colleagues in the health services research Division of the
Department of Urology, we have expanded the use of renal
biopsy and surveillance identifying patients that either
have benign tumors that do not require active treatment
or slow growing tumors that can be observed until active
treatment is indicated. We continue to also offer treatment
for advanced renal cell carcinoma in conjunction with
the genitourinary Program at the University of michigan
Comprehensive Cancer Center. in many circumstances
surgical removal of the cancerous kidney can be performed
laparoscopically allowing equivalent cancer control with
shorter patient recovery. this allows selected patients to
undergo other treatments sooner than would occur following
open surgery.
Building on one of the largest experiences with radical
cystectomy (bladder removal for bladder cancer) combined
with orthotopic urinary diversion (new bladder creation
using small intestine); we are expanding our use of
robotic surgery to the treatment of patients with bladder
cancer. robotic cystectomy may provide lower blood loss,
quicker recovery and better preservation of function than
traditional open radical cystectomy. as part of this effort,
we have joined an international collaboration of academic
institutions to evaluate outcomes for patients undergoing
this procedure. in addition, several ongoing clinical trials
are accruing patients to determine whether modification in
surgical technique can improve urinary and sexual function
following radical cystectomy in men. our multidisciplinary
team is able to offer treatment for the entire spectrum of
bladder cancer from superficial disease to patients with
metastases focusing not only on cancer treatment but also
Chad was welcomed into this world in
Flint, michigan, on February 2, 1970,
by his parents, his brother, todd,
and sister, susan. his toddler years
were spent playing with and chasing
after his siblings and dog, mitzie, as
normal kids do. By 1973, our family
had moved to Canton, michigan but
1975 brought changes.
the family status changed to a single, working mom with three
children when in July we received Chad’s diagnosis of cancer of the
prostrate. he was only 5 years old at the time. the diagnosis meant
surgery to remove his bladder, radiation treatments as well as three
different types of chemotherapy. the ensuing years brought numerous
doctor visits, lab work, X-rays, along with many hospitalizations
(both regularly scheduled as well as emergency admissions) at C. s.
mott and eventually the new U-m hospital.
all was not doom and gloom though. he graduated from high
school with high honors as well as U-m Dearborn and enjoyed
fraternity life (or should i say the parties) and especially his 1988
5.0 mustang. he worked at radio shack and liked all the electronic
gadgets and even built his own computer.
over the years, complications of all the surgeries, radiation and
chemotherapy took a toll on his body. sadly, we lost him January
31, 1997 … just two days before his 27th birthday.
that, thanks in large part to the prudence of our financial
management policies and practices; the University is in sound
financial shape.
Our investment, debt, and cash management/liquidity policies
are all structured to withstand significant volatility in the
continue forward with our Fy09 plans and maintain our focus
on excellence in all teaching, research, clinical and auxiliary
operations and initiatives.
although the recent upheavals in the market have had little
direct or immediate impact on us, we must be mindful of the
ramifications resulting from a potential downturn in state,
national and global economies. as we plan for the next fiscal
play out, and develop contingencies as necessary to ensure we
sustain and enhance our commitment to excellence in all our
PeDiatriC Urology
our team of physicians and nurses continues to provide the best care for
the most challenging urologic problems facing children. We strive in both
clinical and basic research endeavors. Pediatric Urology is a urological
and surgical specialty that deals with the diagnosis and treatment of
abnormalities involving the kidneys, ureters, bladder, urethra and the
reproductive organs of children. Pediatric urologists are board certified
urologists who have completed additional training in the evaluation and
treatment of urological problems in children. Urology’s only subspecialty
certification was initiated in 2008 and Drs. John Park, Julian Wan and
David Bloom have earned this new level of credentials. Pediatric urologists
work with children and genitourinary conditions affecting fetuses to older
adolescents on the verge of adulthood. We commonly consult and treat
newborn babies who were diagnosed as fetuses as having an enlarged
kidney (antenatal hydronephrosis) infants with intersex to teenage boys
with varicose veins in the scrotum (varicoceles). the pediatric urology team
deals with children who have voiding dysfunction, neurogenic bladder
(such as spina bifida), undescended testes, hypospadias, intersex, pediatric
urinary tract trauma and vesicourethral reflux. We also have significant
experience with complex disorders such as bladder exstrophy and prune
belly syndrome. the pediatric urology nursing team is exceptional at
managing the conditions and families we see.
We recently relocated our Pediatric research lab to a space where we
investigate the mechanisms of bladder deterioration caused by blockage
along with refining strategies for bladder regeneration. We have actively
engaged other investigators around the country as one of the future
clinical trial sites for tissue-engineered bladder reconstructions. We have
also established the University of michigan Center for Disorders of sex
Development (formerly known as intersex), where we will become the
model system of multi-specialty care in the country for those children born
with stigmatizing congenital birth defects of genitalia and reproductive
organs. Finally, we will soon be embarking on creating a state-of-the-art
Decision aid for families considering complex bladder reconstructions to
improve their quality of life regarding urinary and fecal incontinence.
researChers DisCoVer metaBolite
linKeD to aggressiVe Prostate CanCer
Finding could lead to test to help guide treatment decisions
Dr. Arul Chinnaiyan
researchers from the University of michigan Center
for translational Pathology have identified a panel of
small molecules, or metabolites, that appear to indicate
aggressive prostate cancer. the finding could lead to a
simple test that would help doctors determine which
prostate cancers are slow-growing and which require
immediate, aggressive treatment. results of the study
appear in the Feb. 12 issue of Nature.
“one of the biggest challenges we face in prostate cancer
is determining if the cancer is aggressive. We end up over
treating our patients because physicians don’t know which
tumors will be slow-growing. With this research, we have
identified a potential marker for the aggressive tumors,”
says senior study author Dr. arul Chinnaiyan, director
of the michigan Center for translational Pathology and
s.P. hicks endowed Professor of Pathology, Professor of
Pathology and Urology at the U-m medical school.
14
the researchers looked at 1,126 metabolites across
262 samples of tissue, blood or urine associated with
benign prostate tissue, early stage prostate cancer and
advanced, or metastatic, prostate cancer. they mapped
the alterations in metabolites and identified about 10
that were present more often in prostate cancer than
in the benign cells and were present most often in the
advanced cancer samples.
“When we’re looking at metabolites, we’re looking
several steps beyond genes and proteins. it allows us
to look very deeply at some of the functions of the
cells and the biochemistry that occurs during cancer
development,” says Chinnaiyan, a howard hughes
medical institute investigator.
one metabolite in particular, sarcosine, appeared to
be one of the strongest indicators of advanced disease.
levels of sarcosine, an amino acid, were elevated in 79
percent of the metastatic prostate cancer samples and in
42 percent of the early stage cancer samples. none of the
cancer-free samples had detectable levels of sarcosine.
in the study, sarcosine was a better indicator of
advancing disease than the traditional prostate specific
antigen, or Psa, test that is currently used to monitor
or screen for prostate cancer. sarcosine was detected in
the urine, which has researchers hopeful that a simple
urine test could be used.
in addition, the researchers found that sarcosine is
involved in the same pathways that are linked to cancer
invasiveness. this suggests sarcosine may be a potential
target for future drug development.
“this research gets at characterizing the chemical
complexity of a sample of cancer progression. in
the future, this science will drive how doctors make
treatment recommendations for their patients,” says
study author Dr. Christopher Beecher, professor of
pathology at the U-m medical school.
results are preliminary at this point and will need
years of further testing and development before this
technology would be available for patients.
