CDM Study Identifi es Diabetes/Prediabetes<br />

In High Percent <strong>of</strong> At-Risk <strong>Dental</strong> Patients<br />

According to 2011 estimates from the National Diabetes Information<br />

Clearinghouse, nearly 26 million people in the United States have<br />

diabetes, but seven million are unaware <strong>of</strong> their disease. Diabetes,<br />

the sixth major cause <strong>of</strong> death in the United States, can also lead to<br />

blindness, kidney failure, nerve damage, lower-extremity amputations<br />

and an increase <strong>of</strong> risk for heart disease and stroke. Like diabetes,<br />

periodontal disease is a chronic infl ammatory condition. Evidence<br />

suggests that periodontal changes are the fi rst clinical manifestation<br />

<strong>of</strong> diabetes and that periodontitis is a possible negative factor in<br />

glycemic control. This bidirectional relationship was one <strong>of</strong> the<br />

reasons that CDM researchers decided to develop an identifi cation<br />

protocol for high blood sugar levels in dental patients, a study<br />

supported by a grant from Colgate Palmolive. A paper describing<br />

the study was published in the July 2011 Journal <strong>of</strong> <strong>Dental</strong> Research.<br />

Its authors suggest that because many diabetic patients remain<br />

undiagnosed, oral health examinations, basic to the diagnosis <strong>of</strong><br />

periodontal disease, may also play a signifi cant role in detecting<br />

diabetes and prediabetes, <strong>of</strong>fering “an unrealized opportunity for<br />

the identifi cation <strong>of</strong> affected individuals unaware <strong>of</strong> their condition.”<br />

Explaining the dental setting’s potential for diagnosing<br />

diabetes and prediabetes, Dean Ira Lamster,<br />

the paper’s senior author, not only cited periodontal<br />

disease as an early complication <strong>of</strong> diabetes, but<br />

emphasized that about 70 percent <strong>of</strong> U.S. adults<br />

see a dentist annually. He also pointed out that,<br />

”prior research focused on identifi cation strategies<br />

in medical settings,” adding “but oral healthcare<br />

sites have not been evaluated before, nor have<br />

the contributions <strong>of</strong> oral fi ndings ever been<br />

tested prospectively.” (continued on page 18)<br />

Remembering Irwin D. Mandel '45<br />

Family, friends and colleagues gathered at a commemoration on<br />

September 14th to celebrate the life and achievements <strong>of</strong> Pr<strong>of</strong>essor<br />

Emeritus Irwin D. Mandel, who died in May. The roster <strong>of</strong> speakers<br />

included former students, colleagues, friends and family members,<br />

who commented on the enormous influence Dr. Mandel had on<br />

generations <strong>of</strong> students and the major role he played in shifting the<br />

focus <strong>of</strong> dentistry from repair to prevention.<br />

“Irwin fit the definition <strong>of</strong> a renaissance man...<br />

widely read, erudite, scholarly, incredible sense<br />

<strong>of</strong> humor, a polymath whose expertise seemed<br />

to know no bounds. Amongst his 250 publications<br />

are several dealing with the caricature <strong>of</strong><br />

the dentist in literature, theater and film; the<br />

history <strong>of</strong> toothpicks; and the story <strong>of</strong> the 18th-<br />

century scientist John Hunter, who was credited<br />

with the naming <strong>of</strong> the teeth.”<br />

Dr. Steven Chussid, associate pr<strong>of</strong>essor <strong>of</strong> Clinical<br />

<strong>Dental</strong> <strong>Medicine</strong>, has been appointed Chairman<br />

<strong>of</strong> the Section <strong>of</strong> Growth and Development,<br />

succeeding Dr. Thomas Cangialosi in this position.<br />

He will continue as director, Division <strong>of</strong><br />

Pediatric Dentistry, a post he has held since 2000.<br />

Dr. Chussid noted, “The Divisions <strong>of</strong> Pediatric<br />

Dentistry and Orthodontics at CDM are among<br />

the finest and most highly regarded in the nation,<br />

attracting the best and brightest applicants.”<br />

He added, "We are fortunate to have Dr. Sunil<br />

Wadhwa succeed Dr. Thomas Cangialosi as<br />

Director <strong>of</strong> the Division <strong>of</strong> Orthodontics. I look<br />

Dennis Mitchell, DDS, MPH, Senior Associate<br />

Dean for Diversity and Director <strong>of</strong> Community-<br />

Based <strong>Dental</strong> Education at CDM, and Dr. Rafael<br />

Lantigua, pr<strong>of</strong>essor <strong>of</strong> Clinical <strong>Medicine</strong> at<br />

P&S, have been appointed as Dean Lee<br />

Goldman’s Special Advisors for Community<br />

Health Affairs. They will work with the Office <strong>of</strong><br />

Government and Community Affairs to engage<br />

community health organizations and solidify<br />

academic and community partnerships.<br />

Dr. Mitchell also serves on the Health Disparities<br />

work group for Governor Andrew M. Cuomo’s<br />

Medicaid Redesign Team, which helps reduce<br />

costs and increase quality and efficiency in<br />

New York’s Medicaid Program<br />

On CAMPuS Faculty<br />

At a May 3 Congressional hearing on oral health issues<br />

affecting American Indians and Alaska Natives, the American<br />

<strong>Dental</strong> Association (ADA) thanked the House Appropriations<br />

Committee for supporting the Indian Health Service (IHS)<br />

dental program and the Obama administration for recommending<br />

increased funding to the IHS Division <strong>of</strong> Oral Health. In oral<br />

testimony, Matthew J. Neary ‘80, Perio ‘82, chair, ADA Council<br />

on Government Affairs, told Committee Chair Rep. Mike<br />

Simpson (R-Idaho), a dentist, "The ADA and its constituent<br />

societies have worked during the year to advance oral health<br />

outreach and awareness in Indian Country.” He added,<br />

“However, the ADA believes more needs to be done to<br />

improve access to dental care and reduce oral disease among<br />

American Indians and Alaska Natives.”<br />

doctoral candidates on their future in the changing<br />

world <strong>of</strong> dentistry. He reminded them that the highest<br />

The Allan J. Formicola Teaching Award was presented<br />

to Assistant Clinical Pr<strong>of</strong>essor in Prosthodontics Anthony<br />

at the June 17 Postdoctoral Graduation Ceremony.<br />

were presented to 30 students during the ceremony.<br />

CDM Study Identifies Diabetes/Prediabetes In High Percent <strong>of</strong> At-Risk <strong>Dental</strong> Patients<br />

The researchers recruited approximately 600 white, non-white,<br />

and Hispanic patients in northern Manhattan, all 30 years or<br />

older and all uninformed about their diabetic or prediabetic<br />

condition. About 530 at-risk patients qualified for the study<br />

protocol because <strong>of</strong> age and at least one self-reported risk<br />

factor, including family history <strong>of</strong> diabetes, high cholesterol,<br />

high blood pressure, and obesity. With this simple approach<br />

to screening, 36% <strong>of</strong> the participants were identified with<br />

diabetes or prediabetes. The addition <strong>of</strong> two dental parameters<br />

(number <strong>of</strong> missing teeth and percentage <strong>of</strong> deep<br />

periodontal pockets) was effective in correctly identifying<br />

73% <strong>of</strong> unrecognized diabetes or prediabetes. When this<br />

algorithm was expanded to give each at-risk patient a<br />

periodontal examination and a fingerstick, point-<strong>of</strong>-care he-<br />

moglobin A1c test, correct identification <strong>of</strong> such cases<br />

rose to 92%.<br />

Dr. Evanthia Lalla, an associate pr<strong>of</strong>essor at CDM and lead<br />

author on the paper, explains that because early recognition<br />

<strong>of</strong> diabetes means earlier treatment to eliminate many serious<br />

complications from disease, it has been the focus <strong>of</strong> efforts for<br />

years."Now," she says, "Our findings provide a simple<br />

approach that can be easily used in all dental care settings."<br />

at CDM and the Mailman School <strong>of</strong> Public Health, Dr. Sandra<br />

Burkett, an examiner and project coordinator in the Division<br />

<strong>of</strong> Periodontics, and Dr. Bin Cheng, assistant pr<strong>of</strong>essor<br />

in the Department <strong>of</strong> Biostatistics at the Mailman School <strong>of</strong><br />

Public Health, were also participants in the study.

Q. Dr. Geisinger, you’ve been on faculty at the <strong>University</strong><br />

<strong>of</strong> Alabama in Birmingham since 2008. You are now an assistant<br />

pr<strong>of</strong>essor and director <strong>of</strong> the Pre-doctoral Periodontal<br />

Program at UAB. Did you intend to become an academic<br />

when you chose dentistry?<br />

A. I came to Birmingham because my husband, a<br />

physician, took a position here. I visited the dental school<br />

and volunteered with postdoctoral students in clinic and<br />

seminars, resulting in an <strong>of</strong>fer to join the UAB dental faculty.<br />

I’m happy with the rewarding mix <strong>of</strong> teaching, clinical and<br />

translational research, administration, service and<br />

intramural practice at UAB.<br />

Q. Why did you choose dentistry as your life’s work?<br />

A. When I was fifteen, I thought I should decide on my<br />

future. Because I liked science, I called all my parents’ friends<br />

in health care and asked what they would say if their daughter<br />

wanted to follow the same career. The physicians weren’t<br />

sure, but the dentists thought it was a great idea.<br />

Q. Are you glad you followed their advice?<br />

A. Absolutely! And I knew it very early. Working as a<br />

dental clinic assistant in college proved it to me. You know,<br />

dentists get a bad rap, but actually more people visit their<br />

dentist every year than go to see their physician. Dentists<br />

are really very positive people—and we’re fast becoming the<br />

gateway to better overall systemic health for our patients.<br />

Q. Why did you decide to study dentistry at <strong>Columbia</strong>?<br />

A. Reputation, <strong>of</strong> course, and strength in basic science<br />

training. That’s extremely important to good dental<br />

practice, a solid base for understanding and incorporating<br />

scientific information. But <strong>Columbia</strong> also gave me financial<br />

aid, support that was a big benefit in limiting my educational<br />

debt, which allowed me to consider a career in academia<br />

Q. How did periodontology become your specialty?<br />

A. Being in a small group environment as a third-year<br />

student with Dr. Evie Lalla was important, and I also<br />

worked with Dr. Fraya Karsh in the clinic and in her private<br />

practice. They both encouraged me to make a choice that<br />

fitted my strengths and I’m glad it was periodontology.<br />

It’s very exciting and currently the only subspecialty recognized<br />

by ADA and CODA for predictably regenerating lost<br />

dental tissue, including periodontal ligament, bone and<br />

cementum, in vivo.<br />

Q. Now you have a very new baby girl. Would you like her<br />

to follow in her parents’ footsteps?<br />

A. I’d like my daughter to choose for herself, based<br />

on her own strengths. And I hope she will have the same<br />

passion for her choice that I have for mine. I think it will<br />

make a big difference to her happiness.<br />
