CDM Study Identifi es Diabetes/Prediabetes
In High Percent of At-Risk Dental Patients
According to 2011 estimates from the National Diabetes Information
Clearinghouse, nearly 26 million people in the United States have
diabetes, but seven million are unaware of their disease. Diabetes,
the sixth major cause of death in the United States, can also lead to
blindness, kidney failure, nerve damage, lower-extremity amputations
and an increase of risk for heart disease and stroke. Like diabetes,
periodontal disease is a chronic infl ammatory condition. Evidence
suggests that periodontal changes are the fi rst clinical manifestation
of diabetes and that periodontitis is a possible negative factor in
glycemic control. This bidirectional relationship was one of the
reasons that CDM researchers decided to develop an identifi cation
protocol for high blood sugar levels in dental patients, a study
supported by a grant from Colgate Palmolive. A paper describing
the study was published in the July 2011 Journal of Dental Research.
Its authors suggest that because many diabetic patients remain
undiagnosed, oral health examinations, basic to the diagnosis of
periodontal disease, may also play a signifi cant role in detecting
diabetes and prediabetes, offering “an unrealized opportunity for
the identifi cation of affected individuals unaware of their condition.”

Explaining the dental setting’s potential for diagnosing
diabetes and prediabetes, Dean Ira Lamster,
the paper’s senior author, not only cited periodontal
disease as an early complication of diabetes, but
emphasized that about 70 percent of U.S. adults
see a dentist annually. He also pointed out that,
”prior research focused on identifi cation strategies
in medical settings,” adding “but oral healthcare
sites have not been evaluated before, nor have
the contributions of oral fi ndings ever been
tested prospectively.” (continued on page 18)

Remembering Irwin D. Mandel '45
Family, friends and colleagues gathered at a commemoration on
September 14th to celebrate the life and achievements of Professor
Emeritus Irwin D. Mandel, who died in May. The roster of speakers
included former students, colleagues, friends and family members,
who commented on the enormous influence Dr. Mandel had on
generations of students and the major role he played in shifting the
focus of dentistry from repair to prevention.
“Irwin fit the definition of a renaissance man...
widely read, erudite, scholarly, incredible sense
of humor, a polymath whose expertise seemed
to know no bounds. Amongst his 250 publications
are several dealing with the caricature of
the dentist in literature, theater and film; the
history of toothpicks; and the story of the 18th-
century scientist John Hunter, who was credited
with the naming of the teeth.”

in 1992 as a tribute to Dr. Mandel’s years of service to Columbia
and its students and to his significant impact on research in the

Appointed To ADA Commission
Dr. Thomas J. Cangialosi, Leuman Waugh Professor and
Director, Postdoctoral Orthodontics Program, has been
named to the Commission on Dental Accreditation of the
American Dental Association, representing the specialty
of Orthodontics and Dentofacial Orthopedics.

Appointed Chair, Section of Growth
and Development
Dr. Steven Chussid, associate professor of Clinical
Dental Medicine, has been appointed Chairman
of the Section of Growth and Development,
succeeding Dr. Thomas Cangialosi in this position.
He will continue as director, Division of
Pediatric Dentistry, a post he has held since 2000.
Dr. Chussid noted, “The Divisions of Pediatric
Dentistry and Orthodontics at CDM are among
the finest and most highly regarded in the nation,
attracting the best and brightest applicants.”
He added, "We are fortunate to have Dr. Sunil
Wadhwa succeed Dr. Thomas Cangialosi as
Director of the Division of Orthodontics. I look
forward to working with him in maximizing the
amazing potential of our Section and increasing
collaboration between our Divisions.”

Receives Two new Appointments
Dennis Mitchell, DDS, MPH, Senior Associate
Dean for Diversity and Director of Community-
Based Dental Education at CDM, and Dr. Rafael
Lantigua, professor of Clinical Medicine at
P&S, have been appointed as Dean Lee
Goldman’s Special Advisors for Community
Health Affairs. They will work with the Office of
Government and Community Affairs to engage
community health organizations and solidify
academic and community partnerships.
Dr. Mitchell also serves on the Health Disparities
work group for Governor Andrew M. Cuomo’s
Medicaid Redesign Team, which helps reduce
costs and increase quality and efficiency in
New York’s Medicaid Program

On CAMPuS Faculty
At a May 3 Congressional hearing on oral health issues
affecting American Indians and Alaska Natives, the American
Dental Association (ADA) thanked the House Appropriations
Committee for supporting the Indian Health Service (IHS)
dental program and the Obama administration for recommending
increased funding to the IHS Division of Oral Health. In oral
testimony, Matthew J. Neary ‘80, Perio ‘82, chair, ADA Council
on Government Affairs, told Committee Chair Rep. Mike
Simpson (R-Idaho), a dentist, "The ADA and its constituent
societies have worked during the year to advance oral health
outreach and awareness in Indian Country.” He added,
“However, the ADA believes more needs to be done to
improve access to dental care and reduce oral disease among
American Indians and Alaska Natives.”

William R. Calnon, DDS, was guest speaker for
the Class of 2011 Graduation ceremony. Dr. Calnon,
who has since been installed as president of the
American Dental Association, addressed CDM's
doctoral candidates on their future in the changing
world of dentistry. He reminded them that the highest
reward for a person’s effort is not what they get
for it, but what they become because of it.

Postdoctoral Graduation
The Allan J. Formicola Teaching Award was presented
to Assistant Clinical Professor in Prosthodontics Anthony

CDM Study Identifies Diabetes/Prediabetes In High Percent of At-Risk Dental Patients
The researchers recruited approximately 600 white, non-white,
and Hispanic patients in northern Manhattan, all 30 years or
older and all uninformed about their diabetic or prediabetic
condition. About 530 at-risk patients qualified for the study
protocol because of age and at least one self-reported risk
factor, including family history of diabetes, high cholesterol,
high blood pressure, and obesity. With this simple approach
to screening, 36% of the participants were identified with
diabetes or prediabetes. The addition of two dental parameters
(number of missing teeth and percentage of deep
periodontal pockets) was effective in correctly identifying
73% of unrecognized diabetes or prediabetes. When this
algorithm was expanded to give each at-risk patient a
periodontal examination and a fingerstick, point-of-care he-
moglobin A1c test, correct identification of such cases
rose to 92%.
Dr. Evanthia Lalla, an associate professor at CDM and lead
author on the paper, explains that because early recognition
of diabetes means earlier treatment to eliminate many serious
complications from disease, it has been the focus of efforts for
years."Now," she says, "Our findings provide a simple
approach that can be easily used in all dental care settings."
Dr. Carol Kunzel, associate professor in Community Health
at CDM and the Mailman School of Public Health, Dr. Sandra
Burkett, an examiner and project coordinator in the Division
of Periodontics, and Dr. Bin Cheng, assistant professor
in the Department of Biostatistics at the Mailman School of
Public Health, were also participants in the study.

Q. Dr. Geisinger, you’ve been on faculty at the University
of Alabama in Birmingham since 2008. You are now an assistant
professor and director of the Pre-doctoral Periodontal
Program at UAB. Did you intend to become an academic
when you chose dentistry?
A. I came to Birmingham because my husband, a
physician, took a position here. I visited the dental school
and volunteered with postdoctoral students in clinic and
seminars, resulting in an offer to join the UAB dental faculty.
I’m happy with the rewarding mix of teaching, clinical and
translational research, administration, service and
intramural practice at UAB.
Q. Why did you choose dentistry as your life’s work?
A. When I was fifteen, I thought I should decide on my
future. Because I liked science, I called all my parents’ friends
in health care and asked what they would say if their daughter
wanted to follow the same career. The physicians weren’t
sure, but the dentists thought it was a great idea.
Q. Are you glad you followed their advice?
A. Absolutely! And I knew it very early. Working as a
dental clinic assistant in college proved it to me. You know,
dentists get a bad rap, but actually more people visit their
dentist every year than go to see their physician. Dentists
are really very positive people—and we’re fast becoming the
gateway to better overall systemic health for our patients.

Q. Why did you decide to study dentistry at Columbia?
A. Reputation, of course, and strength in basic science
training. That’s extremely important to good dental
practice, a solid base for understanding and incorporating
scientific information. But Columbia also gave me financial
aid, support that was a big benefit in limiting my educational
debt, which allowed me to consider a career in academia
Q. How did periodontology become your specialty?
A. Being in a small group environment as a third-year
student with Dr. Evie Lalla was important, and I also
worked with Dr. Fraya Karsh in the clinic and in her private
practice. They both encouraged me to make a choice that
fitted my strengths and I’m glad it was periodontology.
It’s very exciting and currently the only subspecialty recognized
by ADA and CODA for predictably regenerating lost
dental tissue, including periodontal ligament, bone and
cementum, in vivo.
Q. Now you have a very new baby girl. Would you like her
to follow in her parents’ footsteps?
A. I’d like my daughter to choose for herself, based
on her own strengths. And I hope she will have the same
passion for her choice that I have for mine. I think it will
make a big difference to her happiness.
