Facoltà
La c<strong>la</strong>ssificazione internazionale dei<br />
disturbi <strong>del</strong> sonno (ICSD, 2005) chiama <strong>la</strong><br />
forma principale di insonnia pediatrica<br />
“Behavioral Insomnia of Childhood” che si<br />
divide in 3 tipi:<br />
Disturbo di inizio <strong>del</strong> sonno per<br />
associazione<br />
Disturbo da inadeguata definizione <strong>del</strong><br />
limite<br />
Il tipo combinato
Si definisce insonnia in età evolutiva <strong>la</strong> presenza<br />
per 3 o più notti a settimana, per almeno 3<br />
settimane, di uno dei seguenti sintomi:<br />
a) 45’ di <strong>la</strong>tenza <strong>del</strong> sonno;<br />
b) risvegli notturni con almeno 30’ per<br />
riaddormentarsi;<br />
c) risvegli precoci.<br />
La prevalenza <strong>del</strong> disturbo varia al variare<br />
<strong>del</strong>l’età: nei primi due anni è intorno al 20-30% e<br />
dai 3 anni in poi rimane costante intorno al 15%<br />
Spesso i problemi di sonno nel<strong>la</strong> prima infanzia<br />
persistono e si cronicizzano
La deprivazione di sonno o <strong>la</strong> frammentazione <strong>del</strong><br />
sonno ha effetti <strong>del</strong>eteri sullo sviluppo cognitivo<br />
(apprendimento, memoria, funzioni esecutive),<br />
sul<strong>la</strong> rego<strong>la</strong>zione <strong>del</strong>l’umore (irritabilità cronica,<br />
scarsa modu<strong>la</strong>zione degli affetti), <strong>del</strong>l’attenzione<br />
e <strong>del</strong> comportamento (aggressività, iperattività,<br />
scarso controllo degli impulsi) e sul<strong>la</strong> salute<br />
generale (funzioni metaboliche e immunitarie,<br />
incidenti)<br />
Sono stati dimostrati effetti anche sull’umore<br />
famigliare (maggiore rischio di depressione<br />
materna)

Le insonnie da cause organiche sono meno frequenti, (circa un<br />
quinto) rispetto a quelle da cause psicofisiologiche o<br />
comportamentali
CBCL e SDS, 5 asili di Roma<br />
I risvegli notturni corre<strong>la</strong>no significativamente<br />
con i disturbi esternalizzanti (disturbi<br />
comportamentali, con maggiore componente<br />

I DS a 3 anni sono predittori significativi di<br />
disturbi di internalizzazione (ansia, depressione,<br />
iso<strong>la</strong>mento sociale) e disturbi comportamentali<br />
globali a 6 anni (Verhulst et al., 1988)<br />
I DS sono sintomi precoci indicativi di problemi<br />
più tardivi: i DS che durano più di 1 mese,<br />
tendono a persistere e sono associati ad<br />
ambivalenza materna, o ad un evento stressante,<br />
depressione materna, ritorno al <strong>la</strong>voro <strong>del</strong><strong>la</strong><br />
madre e co-sleeping (Lozoff et al., 1985)
Comorbidità tra insonnia pediatrica<br />
e disturbi psichiatrici<br />
Il maggiore disturbo riferito dai pazienti<br />
affetti da depressione, ansia, mania è<br />
l’insonnia<br />
(Abad & Guilleminault, 2005).
DISTURBI D’ANSIA ED INSONNIA<br />
Nel<strong>la</strong> c<strong>la</strong>ssificazione dei disturbi mentali (DSM-IV) il<br />
disturbo d’ansia comprende:<br />
Il disturbo d’ansia da separazione<br />
Fobia sociale<br />
Fobie specifiche<br />
Il disturbo d’ansia generalizzato<br />
Il disturbo ossessivo compulsivo<br />
Il disturbo post-traumatco da stress<br />
Il disturbo da panico<br />
Il disturbo d’ansia NAS<br />
L’ansia si caratterizza per <strong>la</strong> presenza di uno stato di<br />
ipervigi<strong>la</strong>nza o iperarousal che incide sul<strong>la</strong> rego<strong>la</strong>zione <strong>del</strong><br />
sonno e ha molto a che fare con il sonno<br />
La presenza di problemi di sonno a 4 anni corre<strong>la</strong><br />
significativamente con ansia e depressione in adolescenza.
DISTURBI D’ANSIA ED INSONNIA<br />
Circa l’85% di bambini e adolescenti con<br />
disturbo d’ansia hanno problemi di sonno<br />
transitori, e più <strong>del</strong> 50% ha disturbi <strong>del</strong><br />
sonno cronici<br />
I più comuni DS sono:<br />
<strong>Disturbi</strong> di inizio e mantenimento <strong>del</strong> sonno,<br />
Risvegli notturni multipli<br />
Rifiuto ad andare a letto<br />
Cosleeping<br />
Incubi<br />
Paure notturne
DISTURBI D’ANSIA ED INSONNIA<br />
I sintomi sono associati con determinate abitudini<br />
<strong>del</strong> sonno come:<br />
Dormire con <strong>la</strong> luce accesa<br />
Avere bisogno di un giocattolo o di un oggetto per<br />
dormire<br />
Ipersensibilità ai rumori notturni<br />
Paura <strong>del</strong> buio e di dormire da soli<br />
Avere dei rituali all’addormentamento<br />
Incubi e pianto notturno<br />
Nel<strong>la</strong> fase acuta di un PTSD predominano sintomi<br />
di ipervigi<strong>la</strong>nza, difficoltà a dormire, e f<strong>la</strong>shbacks,<br />
mentre nel<strong>la</strong> fase cronica si ha distacco affettivo,<br />
tristezza, stati dissociativi e aumento <strong>del</strong> sonno.
DISTURBI D’ANSIA ED INSONNIA<br />
Studi attigrafici hanno dimostrato che i bambini ricoverati in<br />
un reparto psichiatrico per una storia di abusi fisici o<br />
sessuali presentavano un’efficienza di sonno minore<br />
rispetto ai bambini che non avevano subito abusi.<br />
Studi polisonnografici hanno dimostrato un aumento dei<br />
risvegli notturni e una riduzione <strong>del</strong> sonno ad onde lente nei<br />
bambini ansiosi<br />
Nei casi di insonnia severa associata ad ansia, l’uso<br />
di farmaci con azione sedativa è preferibile e va<br />
prescritta da un pediatra specialista di disturbi<br />
psichiatrici, da un neuropediatra o da un pediatra<br />
esperto di medicina <strong>del</strong> sonno.
MDD (depressione maggiore)<br />
DEPRESSIONE MAGGIORE ED INSONNIA<br />
Ci sono pochi studi sul<strong>la</strong> re<strong>la</strong>zione tra sonno e<br />
depressione in età pediatrica<br />
Circa i 2/3 di bambini con depressione ad<br />
insorgenza precoce in età prepuberale soffrono di<br />
DIMS e almeno il 50% riporta insonnia terminale<br />
Circa il 76% degli adolescenti che hanno avuto un<br />
disturbo <strong>del</strong> sonno nell’arco di un anno, hanno poi<br />
sviluppato una depressione<br />
I cambiamenti nel sonno REM sono simili a quelli<br />
<strong>del</strong>l’adulto con depressione, negli adolescenti con<br />
<strong>la</strong> forma “endogena” di depressione, che si<br />
associa ad un maggiore rischio di suicidio e di<br />
forme psicotiche
MDD (depressione maggiore)<br />
DEPRESSIONE MAGGIORE ED INSONNIA<br />
Il trattamento contemporaneo <strong>del</strong><strong>la</strong><br />
depressione e <strong>del</strong>l’insonnia sembra essere<br />
l’approccio migliore per avere un<br />
miglioramento rapido e duraturo <strong>del</strong><strong>la</strong><br />
depressione<br />
L’igiene <strong>del</strong> sonno e <strong>la</strong> terapia cognitivo<br />
comportamentale sono <strong>la</strong> prima scelta per<br />
<strong>la</strong> cura <strong>del</strong>l’insonnia, per il trattamento<br />
farmacologico il trazodone può essere una<br />
prima scelta.
MDD (depressione maggiore)<br />
DISTURBO BIPOLARE ED INSONNIA<br />
I DS sono comunemente osservati nei bambini durante un<br />
episodio maniacale.<br />
La riduzione <strong>del</strong> bisogno di dormire è il sintomo cardine <strong>del</strong><br />
disturbo bipo<strong>la</strong>re ad insorgenza prepuberale insieme ad un<br />
aumento <strong>del</strong> tono <strong>del</strong>l’umore, ipersessualità, grandiosità,<br />
fuga <strong>del</strong>le idee<br />
Circa il 40% dei bambini con una diagnosi di mania hanno<br />
una riduzione <strong>del</strong> bisogno di sonno, contro il 6.2% dei<br />
bambini con ADHD e l’ 1.1% di quelli sani<br />
La riduzione <strong>del</strong> bisogno di dormire è presente nel 95% di<br />
bambini con diagnosi di disturbo bipo<strong>la</strong>re tra i 3 e i 17 anni.<br />
I DS sono riportati più frequentemente durante gli episodi<br />
depressivi piuttosto che in quelli maniacali.
ADHD E INSONNIA<br />
La deprivazione di sonno e l’eccessiva sonnolenza diurna, soprattutto nei<br />
bambini, si manifesta con un corteo sintomatologico ed un profilo<br />
cognitivo simile all’ADHD.<br />
Le ipotesi che sono state formu<strong>la</strong>te sul legame tra ADHD e sonno si<br />
possono dividere in 4 categorie:<br />
1) molti bambini con ADHD presentano disturbi <strong>del</strong> sonno che fanno<br />
parte <strong>del</strong> corteo sintomatologico;<br />
2) i disturbi <strong>del</strong> sonno sono una conseguenza <strong>del</strong> disturbo <strong>del</strong><br />
comportamento e <strong>del</strong> trattamento farmacologico (aumento dei livelli<br />
di vigi<strong>la</strong>nza ed insonnia a causa <strong>del</strong><strong>la</strong> terapia con psicostimo<strong>la</strong>nti);<br />
3) l’ADHD è una disturbo primario dei livelli di vigi<strong>la</strong>nza (stato di “ipoarousal”)<br />
e di conseguenza si associa ad un disturbo <strong>del</strong> ritmo sonnoveglia,<br />
4) le alterazioni neurochimiche e neuro-strutturali spiegano sia i disturbi<br />
<strong>del</strong> sonno che l’ADHD.
ADHD E INSONNIA<br />
Studi soggettivi con questionari<br />
Gli studi con i questionari <strong>del</strong> sonno, compi<strong>la</strong>ti<br />
prevalentemente dai genitori, riportano una elevata frequenza<br />
(25-50% dei casi) di DIMS, con resistenza ad andare a letto,<br />
difficoltà al risveglio mattutino, sonno agitato e sonnolenza<br />
diurna (Corkum et al, 1998, Cohen Zion & Ancoli Israel, Sung<br />
et al, 2008).<br />
I disturbi <strong>del</strong> sonno nell’ADHD si associano ad un livello<br />
socio-economico basso, una tendenza ad arrivare tardi a<br />
scuo<strong>la</strong>, un aumento <strong>del</strong> distress familiare e dei disturbi<br />
psichiatrici in famiglia (Sung et al, 2008).<br />
Recentemente è stata riportata <strong>la</strong> presenza di sonnolenza<br />
diurna e tendenza a dormire di più, solo nel sottotipo<br />
inattentivo, mentre l’insonnia veniva più frequentemente<br />
riportata nei bambini con il sottotipo combinato (Mayes et
ADHD E INSONNIA<br />
I <strong>la</strong>vori attigrafici dimostrano <strong>la</strong> presenza di<br />
un’alterazione <strong>del</strong> ritmo sonno-veglia, con un ritardo<br />
nell’orario di addormentamento e tendenza a saltare il<br />
sonnellino pomeridiano nei bambini con riferita insonnia e<br />
ADHD, confermando <strong>la</strong> presenza di un picco ritardato<br />
<strong>del</strong><strong>la</strong> me<strong>la</strong>tonina (Van Der Heiyden et al, 2006).<br />
Al contrario, un altro <strong>la</strong>voro attigrafico su circa 80<br />
bambini con ADHD ha dimostrato che i bambini con<br />
ADHD differiscono dai controlli per un risveglio più<br />
precoce, una maggiore frammentazione <strong>del</strong> sonno, senza<br />
un aumento <strong>del</strong><strong>la</strong> <strong>la</strong>tenza al sonno (Owens et al, 2008).
Molti studi dimostrano l’efficacia <strong>del</strong><strong>la</strong><br />
me<strong>la</strong>tonina nei bambini con ADHD e<br />
insonnia (con principalmente orario<br />
ritardato di addormentamento)
L’associazione tra ADHD e difficoltà di<br />
addormentamento suggerisce una serie di ipotesi sul<strong>la</strong><br />
genesi <strong>del</strong> DS:<br />
La difficoltà di addormentamento può essere una<br />
manifestazione <strong>del</strong> fenotipo ADHD<br />
Può essere semplicemente in comorbidità<br />
Oppure può essere un segnale che <strong>la</strong> diagnosi di ADHD è<br />
sbagliata: Staton (2008) ha recentemente suggerito che il<br />
sintomo cardine <strong>del</strong> disturbo bipo<strong>la</strong>re nell’infanzia non sia<br />
una riduzione <strong>del</strong> bisogno di dormire, quanto piuttosto di<br />
un disturbo <strong>del</strong> ritmo sonno veglia con una fase di sonno<br />
ritardata, simile a quello che avviene nei bambini con<br />
ADHD e difficoltà di addormentamento.
RITARDO MENTALE E INSONNIA<br />
I DS nei bambini con RM presentano<br />
caratteristiche comuni (Quine, 1991; Espie,<br />
1991)<br />

attempting generalizations that risk being<br />
limited in meaning and clinical usefulness."

Qualsiasi bambino con un disturbo neurologico è a rischio di<br />
sviluppare un disturbo <strong>del</strong> ritmo sonno veglia<br />
I bambini con RM possono avere un’alterazione <strong>del</strong><strong>la</strong><br />
percezione dei comuni “zeitgeber" (ciclo luce-buio, orari dei<br />
pasti, stimoli materni)<br />
Possono anche avere <strong>del</strong>le disfunzioni endogene e ormonali<br />
che interferiscono con <strong>la</strong> sincronizzazione dei ritmi circadiani<br />
La manifestazione comportamentale di questa alterazione <strong>del</strong><br />
sonno è rappresentata prevalentemente dai DIMS (disturbi di<br />
inizio <strong>del</strong> sonno nel 51% dei casi e risvegli notturni nel 67%)
Negli ultimi anni molti studi hanno<br />
dimostrato l’efficacia <strong>del</strong><strong>la</strong> me<strong>la</strong>tonina<br />
(MLT) nei DS e nei disturbi <strong>del</strong> ritmo sonno<br />
veglia nei bambini con disturbi neurologici<br />
I DIMS e il ritmo free running sono spesso<br />
associati con una secrezione alterata <strong>del</strong><strong>la</strong><br />
MLT
Autismo e insonnia<br />
L’insonnia rappresenta il disturbo <strong>del</strong> sonno<br />
prevalente nei bambini con ASD e <strong>la</strong> sua natura è<br />
multifattoriale, con fattori neurochimici (anomalie<br />
nel<strong>la</strong> trasmissione serotoninergica e nei livelli di<br />
MLT), psichiatrici (ansia), e comportamentali<br />
(scarsa igiene <strong>del</strong> sonno) coinvolti.<br />
La prevalenza dei DIMS è molto più alta rispetto a<br />
quel<strong>la</strong> dei bambini con uno sviluppo mentale<br />
normale (44–83% dei bambini ASD in età<br />
sco<strong>la</strong>re)<br />

I caregivers descrivono un insonnia caratterizzata da:<br />

Alterazioni <strong>del</strong>l’igiene <strong>del</strong> sonno<br />
Risvegli precoci<br />
Molti studi attigrafici hanno confermato i dati dei questionari, gli<br />
studi PSG riportano una riduzione <strong>del</strong> tempo totale di sonno e<br />
alterazioni <strong>del</strong><strong>la</strong> microstruttura <strong>del</strong> sonno (corre<strong>la</strong>te con il<br />
livello di RM)<br />

Autismo e insonnia<br />
Le terapie comportamentali comunemente usate<br />
(tecniche di estinzione più o meno graduale)<br />
vengono utilizzate, anche se <strong>la</strong> maggior parte<br />
degli studi suggerisce una terapia combinata con<br />
<strong>la</strong> MLT.<br />
La MLT è efficace e non sono stati riportati effetti<br />
col<strong>la</strong>terali negli studi sui bambini con ASD, trattati<br />
anche per un lungo periodo<br />
Recentemente è stato dimostrato che un deficit<br />
primario <strong>del</strong><strong>la</strong> sintesi di MLT possa essere al<strong>la</strong><br />
base dei DS nell’autismo, e al<strong>la</strong> base <strong>del</strong>l’autismo<br />
stesso.<br />

TERAPIA COMPORTAMENTALE<br />
L'approccio comportamentale utilizza una serie di tecniche<br />
atte a modificare abitudini errate corre<strong>la</strong>te al sonno; è<br />
efficace ma non deve necessariamente escludere il<br />
trattamento farmacologico<br />
Le tecniche non vanno utilizzate in maniera rigida e<br />
singo<strong>la</strong>rmente; i migliori risultati si ottengono combinando<br />
le diverse tecniche e adattandole al b. o ai caretakers<br />
(genitori, nonni, baby-sitter, ecc.).<br />
La terapia farmacologica è utile nei periodi iniziali mentre <strong>la</strong><br />
terapia comportamentale fornisce risultati più stabili nel<br />
tempo

Consiste nel<strong>la</strong> rimozione di ogni risposta di rinforzo di comportamenti<br />
errati <strong>del</strong> bambino tramite una breve routine al bedtime con il minimo di<br />
interazione reciproca, il non intervento quando piange, il riportarlo a<br />
letto e farlo riaddormentare con le stesse modalità se si sveglia e si alza<br />
Approccio rapido, ma anche frustrante, spesso sensi di colpa e paure<br />
sull’incolumità <strong>del</strong> bambino, causa di resistenza al metodo da parte dei<br />
genitori<br />
Rischio di interruzione e ulteriore rinforzo <strong>del</strong> disturbo: indispensabile<br />
valutare a priori le possibilità di resistenza <strong>del</strong><strong>la</strong> famiglia<br />
Sconsigliato per quei b. che hanno sviluppato una avversione verso <strong>la</strong><br />
cul<strong>la</strong> o il letto e una difficoltà di separazione importante (Messer, 1993;<br />
France et al., 1996)
ESTINZIONE GRADUALE<br />
Consiste nel cercare di ottenere il comportamento desiderato tramite<br />
piccole conquiste successive, come ritardare (o anticipare) gradualmente<br />
l'orario di addormentamento, oppure abituare il b. al<strong>la</strong> progressiva<br />
distanza dai genitori al momento di andare a letto<br />
E’ necessario che i genitori abbiano molto tempo a disposizione. Ad ogni<br />
risveglio si valuta il tempo impiegato per <strong>la</strong> conso<strong>la</strong>zione, in modo da<br />
ridurlo gradualmente, fino a far assumere al bambino <strong>la</strong> capacità di<br />
addormentarsi da solo: ogni 1-2 sere il periodo di lontananza dal b. viene<br />
incrementato di circa 1-5’; con progressivo aumento <strong>del</strong><strong>la</strong> <strong>la</strong>tenza di<br />
risposta<br />
Il rischio è <strong>la</strong> non costanza per <strong>la</strong> necessità di tempi lunghi. Il vantaggio è<br />
<strong>la</strong> sua alta tollerabilità da parte dei genitori (ansiosi) che non<br />
tollererebbero metodi più incisivi. Lo svantaggio sono i tempi di<br />
applicazione (almeno un mese) e <strong>la</strong> necessità di una buona<br />
organizzazione familiare. E’ applicabile per bambini in buona salute e che<br />
non hanno l’abitudine <strong>del</strong> cosleeping
APPRENDIMENTO DISCRIMINATO<br />
Consiste nello stabilire una routine fissa<br />
all'addormentamento in modo tale che il bambino<br />
impari ad associare l'addormentamento con<br />
determinati eventi che si ripetono stabilmente ogni<br />
giorno; in tal modo il "mezzo" di addormentamento<br />
non sono più i genitori ma <strong>la</strong> cul<strong>la</strong> o il letto<br />
(Messer, 1993)<br />
In alcuni casi è utile associare il momento<br />
<strong>del</strong>l'addormentamento con il leggere una favo<strong>la</strong>,<br />
tenere un giocattolo speciale o un carillon nel<strong>la</strong><br />
cul<strong>la</strong>, consentire l'uso <strong>del</strong> ciuccio, <strong>la</strong>sciare una luce<br />

patients compared to patients with other forms of “insomnia.”<br />
In adolescents, failure to cooperate with a p<strong>la</strong>n to reschedule<br />

Prevalence: found evidence suggesting a 7% in adolescents<br />
Age of Onset: Adolescence appears to be the most common<br />
period, but childhood cases have been reported. Onset after<br />
age 30 is rare.<br />
Sex Ratio: Case series in the literature have varied between<br />
a 10:1 male:female ratio in adolescents to 1:1 in a mixed<br />
series of adults and adolescents.
Diagnostic Criteria:<br />
A. The patient has a comp<strong>la</strong>int of an inability to fall asleep at the desired clock time, an<br />
inability to awaken spontaneously at the desired time of awakening, or excessive<br />
sleepiness.<br />
B. There is a phase de<strong>la</strong>y of the major sleep episode in re<strong>la</strong>tion to the desired time for sleep.<br />
C. The symptoms are present for at least 1 month.<br />
D. When not required to maintain a strict schedule (e.g., vacation time), patients will exhibit<br />
all of the following:<br />
1. Have a habitual sleep period that is sound and of normal quality and duration<br />
2. Awaken spontaneously<br />
3. Maintain stable entrainment to a 24-hour sleep-wake pattern at a de<strong>la</strong>yed phase<br />
E. Sleep-wake logs that are maintained daily for a period of at least two weeks must<br />
demonstrate evidence of a de<strong>la</strong>y in the timing of the habitual sleep period.<br />
F. One of the following <strong>la</strong>boratory methods must demonstrate a de<strong>la</strong>y in the timing of the<br />
habitual sleep period:<br />
1. Twenty-four-hour PSG (or by means of 2 consecutive PSG and 1 MSLT)<br />
2. Continuous temperature monitoring showing that the time of the absolute temperature<br />
nadir is de<strong>la</strong>yed into the second half of the habitual (de<strong>la</strong>yed) sleep episode<br />
G. The symptoms do not meet the criteria for any other sleep disorder causing inability to<br />

questionnaire. The severity of Internet addiction was evaluated<br />

and possible Internet addicts were 2.5% and 53.7%, respectively.<br />
For girls, were 1.9% and 38.9%, respectively.<br />
With adjustment for duration of Internet use, duration of sleep<br />
time, age, gender, smoking, taking painkillers due to headache,<br />
insomnia symptoms, witnessed apnea, and nightmares, the odds<br />
of EDS were 5.2-fold greater (95% confidence interval [CI]: 2.7-<br />
10.2) in Internet addicts and 1.9-fold greater (95%CI: 1.4-2.6) in<br />
possible Internet addicts compared to non-addicts.<br />
CONCLUSION: Internet addiction is strongly associated with EDS in<br />
adolescents.
N. of studies have documented a tendency for teenagers<br />
and young adults to de<strong>la</strong>y their sleep schedule, but the<br />
re<strong>la</strong>tive contribution of endogenous and exogenous<br />
factors underlying this phenomenon have not been fully<br />

This retrospective study describes the effects of long-term<br />
treatment with me<strong>la</strong>tonin in 33 adolescents (age range, 10-<br />
18 years) with de<strong>la</strong>yed sleep phase syndrome (DSPS).<br />
Patients were treated with oral me<strong>la</strong>tonin, 3 to 5 mg/day<br />
for an average period of 6 months.<br />
During the treatment, sleep onset was advanced and sleep<br />
duration was longer.<br />
Treatment was also associated with a decrease in the<br />
proportion of patients reporting school difficulties.<br />
No adverse effects of me<strong>la</strong>tonin were noted.<br />
This study indicates that long-term treatment with<br />
me<strong>la</strong>tonin can be beneficial for adolescents with DSPS in<br />
terms of sleep-wake schedule and school performance.
We present here the unusual case of a young<br />
girl with symptoms of agitated depression who<br />
comp<strong>la</strong>ined severe insomnia and was referred to<br />
our Sleep Centre because her sleep was too<br />
short.<br />
She underwent overnight PSG that disclosed the<br />
presence of PLMS, thus supporting the clinical<br />
diagnosis of restless legs syndrome.<br />
Treatment with the dopamine agonist ropinirole<br />
improved significantly both sleep disturbance<br />
and psychiatric symptoms.

Depression Rating Scales dropped to<br />
10 and 6, respectively.<br />
She was drug-free and reported early insomnia<br />

Personality Inventory-2 showed depressive<br />
anxious traits, low self-esteem, social avoidance,<br />
impulsiveness, fears of being judged and<br />

She scored 24 on both Young Mania 8 and<br />
Hamilton Depression Rating Scales<br />
She had no history of smoking or substance<br />
abuse.<br />
She reported all 4 criteria of RLS (urge to move<br />
the legs to relieve uncomfortable and unpleasant<br />
sensations, symptom worsening during rest<br />
periods, symptoms partially or totally relieved by<br />
movement, and worsening at evening and night).

bruschi episodi di sonno. I genitori usualmente tendono a<br />
sottovalutare questi sintomi: eccessiva durata <strong>del</strong> sonno e numero di<br />
"sonnellini" diurni, specie se il rendimento sco<strong>la</strong>stico è nel<strong>la</strong> norma<br />
Sono gli episodi di cataplessia, in un secondo stadio <strong>del</strong><strong>la</strong> ma<strong>la</strong>ttia,<br />
che preoccupano i genitori (anche se nei bb più piccoli possono<br />
essere non correttamente diagnosticati es. errata diagnosi di<br />

The role of cyclic alternating pattern<br />
analysis
In disorders characterized by<br />
hypoarousability (narcolepsy, ADHD and<br />
PWS), there is a global decrease of<br />
NREM instability, represented by a<br />
reduction of CAP rate and of A indexes.
The main finding of this study is that the<br />
occurrence of the A1 CAP subtypes is impaired<br />
during NREM sleep in narcoleptic patients.<br />
Narcolepsy seems to be accompanied not only by<br />
alterations of REM but also of NREM sleep, as<br />
reflected by CAP and the corresponding EEG<br />
spectral analysis.<br />
Our findings might indicate that, in narcolepsy,<br />
very-slow oscil<strong>la</strong>tion processes expression of<br />
cortical activity are less effective than normal,<br />
probably due to orexin deficts

comportamentale. Sul piano poligrafico l’arousal, non<br />
dura in media più di 60”, si accompagna ad attività<br />
