−−−−De frequentie en reden <strong>van</strong> eventuele nadere onderzoeken worden met de patiëntebesproken en minimaal éénmaal gecombineerd met <strong>het</strong> bezoek aan de mammacareverpleegkundige, indien noodzakelijk vaker.Het is belangrijk dat een contactpersoon (frequent de mammacare verpleegkundige)wordt aangewezen. Er wordt aan de patiënte duidelijk aangegeven dat zij zich te allentijde tot deze contactpersoon kan wenden in geval <strong>van</strong> optreden <strong>van</strong> symptomen dieaanleiding geven tot ongerustheid en voor <strong>het</strong> vernemen <strong>van</strong> uitslagen <strong>van</strong> onderzoek.Oplossing <strong>van</strong> de gepresenteerde problemen wordt geboden door de daartoetoegeruste discipline (medisch specialist, maatschappelijk werk, fysiotherapeut ofBVN).Optreden <strong>van</strong> mogelijke, borstkankerspecifieke problemen wordt aangegeven, zoals:bewegingsbeperking <strong>van</strong> de arm, lymfoedeem, huidproblemen, verwerkingsproblematiek,klachten <strong>van</strong> vroegtijdige overgang, pijnklachten ten gevolge <strong>van</strong>beschadiging <strong>van</strong> de nervus intercosto brachialis of bestraling <strong>van</strong> de borst,vermoeidheid, problemen rondom prothesen en lichaamshouding en teleurstelling <strong>van</strong>de vrouw met betrekking tot <strong>het</strong> cosmetisch resultaat <strong>van</strong> de borst na de behandeling.Bij <strong>het</strong> afbouwen <strong>van</strong> de controlebezoeken wordt duidelijk besproken welkesymptomen aanleiding geven contact op te nemen. De informatie, genoemd onder <strong>het</strong>derde punt, wordt herhaald.AdviesHet doel <strong>van</strong> de follow-up na behandeling voor een invasief carcinoom en DCIS is <strong>het</strong> geven<strong>van</strong> voorlichting en begeleiding en <strong>het</strong> in een zo vroeg mogelijk stadium opsporen <strong>van</strong> eenlocoregionaal recidief of tweede primair carcinoom.Het volgende follow-up schema wordt geadviseerd:− eerste jaar: eens per drie maanden− tweede jaar: eens per half jaar− daarna: jaarlijks.De optimale duur <strong>van</strong> de follow-up is onbekend. Deze wordt in overleg tussen arts en patiëntbepaald. De keuze voor de duur kan niet gemaakt worden zonder invulling te geven aan deprimaire aspecten <strong>van</strong> follow-up, zoals voorlichting en zorg voor de patiënt, en aan deinschatting <strong>van</strong> recidief risico’s, zoals familiaire belasting, stadium primairemammacarcinoom etc.. De follow-up kan na een ziektevrij interval <strong>van</strong> (tenminste) 5 jaarmeestal worden afgesloten, met dien verstande dat mammografische controle volgens derichtlijn blijft plaatsvinden.Inhoudelijk wordt geadviseerd de follow-up te laten bestaan uit een anamnese en lichamelijkonderzoek. Het gebruik <strong>van</strong> HST wordt ontraden. Tot 60 jaar wordt een jaarlijksmammogram geadviseerd, daarna eens per twee jaar. Bij vrouwen ouder dan 75 jaar kanoverwogen worden deze controle te staken.Wie de follow-up coördineert is afhankelijk <strong>van</strong> de expertise en bereikbaarheid <strong>van</strong> departicipanten in de mammazorg: mammacareverpleegkundige/Nurse Practitioner/PhysicianAssistant, chirurg, internist, radiotherapeut.Het moet aan eerste lijn en patiënte duidelijk zijn waar ze zich, in geval <strong>van</strong> klachten, kanvervoegen voor nadere hulp en advies.Literatuur1.
