By Louis Malcmacher, DDS, MAGD
n I am asked all of the time what
the hottest topic is in dentistry. What
new technique or technology is going
to change the dental practice forever?
I believe the next big direction in
dentistry is actually going to take
place in the oral-systemic connection.
Most dentists are familiar with this
connection as being how oral health
affects systemic health. I’m going to
look at the oral-systemic connection
from a completely different angle: the
oral-systemic esthetic connection.
We all can do a magnificent job of
making teeth look great and giving
people a healthy and beautiful smile.
Esthetic dentistry has exploded during
the last 30 years with innovative
techniques such as teeth whitening
and no/minimally invasive Cristal
veneers (Aurum Ceramics). However,
if the teeth look good but we ignore
the surrounding areas of the face,
then the esthetic dentistry is not
complete and you have not given the
patient a great-looking smile.
It is time to give serious consideration
to extending the oral-systemic
connection to the esthetic realms
of the face, which dentists are more
familiar with than any other healthcare
practitioner. Dermal fillers, such
as hyaluronic acids (Juvederm, Restylane),
are commonly used to add
volume to the face in the nasolabial
folds, oral commissures and marionette
lines.
As we age, collagen is lost in these
facial areas, and these lines start
to deepen. These dermal fillers are
injected (in the same areas that dentists
now inject) to plump up these
areas so that these lines are much less
noticeable.
Botox is a wrinkle remover and
temporarily reduces or eliminates the
contraction of the muscles that create
the wrinkles in the skin of the face.
Once the muscle cannot contract, the
skin will smooth out all by itself. The
face looks more esthetic and is an
essential component to every esthetic
dentistry case.
education
Botox has been used therapeutically
in dentistry for almost 20 years
for the treatment of facial pain, TMJ
syndrome and bruxism. These concepts
are now taught at major dental
universities.
I have been performing Botox and
dermal filler procedures for years.
We as dentists give injections all the
time — this is just learning how to give
another kind of injection extra-orally
and is in the same area of the face that
we inject all the time.
Most state boards now allow general
dentists to provide Botox and
dermal fillers to patients for both
easily integrate such hot procedures
as Botox and dermal fillers, no-prep
as well as discuss overhead control and
significantly increase your production,
substantially lower your overhead and
have fun doing dentistry.
therapeutic and esthetic uses. Is there
a market for these services? In 2010,
close to $5 billion were spent on botulinum
toxin and dermal filler therapy
in the United States. Think about this.
That was money spent on non-surgical
elective esthetic procedures that
could have been spent on dentistry,
but the patient made a choice.
Interestingly, these procedures
become more popular in an uncertain
economy because patients want to do
something to look better that is more
affordable than surgical esthetic
options.
Like anything else a dentist does,
this requires comprehensive practical
training. The learning curve is
short for dentists because you already
know how to give comfortable injections.
The American Academy of
Facial Esthetics (www.FacialEsthetics.org),
with more than 50 local
courses a year, has trained nearly
6,000 dental professionals from 48
states and 28 countries through comprehensive
mentored one-on-one,
hands-on, live-patient, two-day dentofacial
esthetic training sessions with
Botox and dermal fillers.
The hottest topic in dentistry right
now that will influence dentistry for
the rest of your career is the integration
of Botox and dermal fillers into
surgical, restorative, prosthodontic,
periodontic, orthodontic and esthetic
dental treatment plans. This opens up
well-proven treatment options that
we legally, morally and ethically
have to offer patients. Get trained
these procedures. It is a perfect complement
procedures.
Greater New York Dental Meeting — Show Preview
Increasing chairside
production through
cosmetic simulation
At the GNYDM
Stop by the SNAP booth, No. 3606,
during the Greater New York Dental
Meeting and see for yourself how easy
it is to increase your cosmetic revenue
and profits through the art of cosmetic
simulation.
‘What would happen if you
gave all of your patients a
simulated before-and-after
picture? Would providing
these pictures ... increase or
decrease the number of
cosmetic cases that you book
on a weekly basis?’
Because the billable hourly rate is
much higher for cosmetic dentistry,
the idea is to convert non-cosmetic
patients into cosmetic patients. In
most cases, the simulated picture
makes its way back to the spouse, family
and friends, resulting in even more
bookings and higher case acceptance.
The bottleneck in this scenario has
always been the amount of training,
time and effort required to produce
these simulations. Now, with recent
advances in SNAPs automated technology,
even untrained personnel can
easily produce convincing and natural-looking,
side-by-side, full-face or
close-up simulations in just a minute
or two.
Turning non-cosmetic patients into
cosmetic patients is easier than you
might think. Hypothetically, what
would happen if you gave all of your
patients a simulated before-and-after
picture? Would providing these pictures
to your non-cosmetic patients
increase or decrease the number of
cosmetic cases that you book on a
weekly basis?
Obviously, this is a rhetorical question.
A picture is worth a thousand
words, and even disinterested
patients are bound to be swayed by an
actual picture of themselves, showing
how great they are going to look
if they take advantage of your full
range of services.
Many dentists are now placing
placards in the reception area (typically
near the magazines) that prompt
patients to ask for their free Smile
Evaluation Picture. Curiosity alone is
enough to garner additional inquiries
from your patients. However, providing
simulation services at venues
such as bridal shows and health clubs
will result in a noticeable influx of
new patients.

n The decision to pursue ceramic
dental restorations is a major one for
most patients. There may be a concern
that the results will turn out to be
different from what the patient imagined
— or perhaps the patient simply
cannot picture the vast improvement
to appearance that a well-designed
dental restoration can create.
Think how much more confident
patients would be if they experienced
their new smiles before they decided
to proceed with the actual restoration
process. That experience is now
available with PreNew PreView ® , an
innovative new technology that lets
your patients “test-drive” their new
smile in person.
Built from a simple study model,
PreNew PreView is a smile preview
that fits over patients’ existing teeth
to show them just how great they are
going to look. Before PreNew Pre-
View, dentists had to rely on digitially
manipulated before-and-after photos.
Today, dentists have PreNew Pre-
View, which can create a realistic
image of what a patient’s new smile
will look like once his or her final
dental restoration is complete.
The pioneer of this technology
Implants, a Dentatus division) are
designed to stabilize, cushion and
retain dentures and have their own
unique prosthetic design.
The 1.8 mm, 2.2 mm, 2.4 mm and
now 2.8 mm diameter Atlas implants
are ideal for patients with insufficient
bone and for those who may not be
able to undergo the lengthier and more
costly surgical procedures associated
with other systems. The low-profile
head tolerates substantial angulation,
which is beneficial as a patient’s existing
denture is often utilized.
The Atlas system uses no O-rings,
no housings and no adhesives.
“The key to the system is the Tuf-
Link silicone liner, which retains the
denture incredibly well,” Dr. Louis
Malcmacher said. “This is much different
than the other mini-implant
systems that rely on clumsy metal
housings in which more of the denture
needs to be removed and in
which the implants must be placed
perfectly parallel.
How does PreNew PreView
work?
PreNew PreView is a simple, two-step
process.
• Step one: During the patient’s first
visit to the dentist, a study model
is taken, along with two photos
(smile with lips and full-facial
smile). These are sent to the lab at
Jason J. Kim Dental Aesthetics.
• Step two: During the second visit,
the dentist applies the new look
over the patient’s existing teeth.
There’s no drilling and no pain.
The patient will get to experience a
“The Dentatus Atlas System gives
the general dentist much more leeway
and is significantly easier to use.”
The non-porous Tuf-Link silicone
is the interface between the patient’s
ridge and the denture. Tuf-Link inherently
ensures greater immobility of
the implant during the critical early
stages of treatment because it acts as
a shock absorber, distributing forces
of mastication throughout the ridge
rather than directly on the implants.
With this protocol , there is no need
for adhesives, which are known to
cause bacteria and, thus, halitosis.
The Tuf-Link is durable, ensuring
patients do not have to replace it
realistic preview of his or her new
smile.
This first-hand look boosts the
patient’s confidence about the dental
restoration process and adds certainty
to his or her decision to proceed.
PreNew PreView is a unique
way to intrigue and captivate current
before their scheduled annual visits,
although it generally lasts 18 to 24
months.
The Atlas Denture Comfort procedure
is minimally invasive, requires
no surgical incision, can be performed
in a general dentist’s office in less
than one hour and calls for only local
anesthesia. What’s more, patients can
walk out of the office wearing their
refitted dentures right away.
Fear of surgery, time constraints,
inadequate bone and high cost are
often cited as reasons why people do
not choose to have implants support
their dentures. With the implantretained
Group of reference dentists met in
the Strand Hotel in Stockholm where
Prof. Dan Ericsson proposed his idea
for a combined wedge and matrix in
response to a need in the market.
A number of prototypes were made
and trials undertaken, and by autumn
Directa CEO Olle Larsson and Directa consultant
Dr. Jan Johansson talk about the origins
and history of FenderWedge and FenderMate
Uppsala, Ericsson had his prototype
solution — a thin steel plate attached
to a wooden wedge.
The stainless-steel plate could not
be glued onto the wooden wedge satisfactorily,
but engineering consultants
presented a solution with a plastic
wedge that could be fused with the
metal plate.
A manufacturer in Huddinge, south
of Stockholm, took the idea onboard,
and in 2006, FenderWedge ® was born:
a combined wedge and protective
matrix for use in protecting the adjacent
tooth during preparation.
Encouraged by the technical solution
and successful acceptance of
FenderWedge in the market, Directa
returned to the task of finding a solution
to the original concept of a combined
wedge and sectional matrix for
use with composite fillings.
Many trials and clinical tests were
made, and the first version of Fender-
Mate ® was released at the International
Dental Show (IDS) in Cologne
in 2007.
At that time, FenderMate needed
a retentive ring to hold the stainlesssteel
plate in position. It was made
of dead-soft metal and needed to be
burnished to create a contact point.
It did not satisfactorily fulfill the
high expectations required, and further
work and developments were
made, under the guidance of Dr. Jan
Johansson.
Many further trials, tests and modifications
were made to fulfill Directa’s
goals of a combined wedge and sectional
matrix system that was simple
to use, required no retention ring
or burnishing, made a good contact
point, adapted to the shape of the
patient’s tooth and created no cervical
overhang.
At IDS in 2009, FenderMate as we
know it now was launched. Fender-
Mate accommodates around 60 percent
of Class II fillings and is used for
molars and pre-molars.
Because of the success of this
unique product, Directa continues to
make further developments to expand
its “Fender” line.
n The Rinn division of DENTSPLY
has provided service to the dental
X-ray market since 1923. While 2011
is notable as the company’s 88th year
in operation, it’s also the year that
Rinn is launching three universal
holders for intra-oral digital sensor
positioning: Uni-Grip ® 360, XCP-DS
FIT ® and Snap-A-Ray ® DS.
Since the introduction of Trophy
intraoral digital sensors in the 1980s,
the dental market has seen a variety
of uniquely shaped and sized digital
sensors. With the latest round
of thicker CMOS-based sensors, the
variations in size continue to grow.
Compared to X-ray film, which
is available in a limited number of
standardized sizes, size variations
present a challenge for practitioners
in finding the right holders for their
sensor.
However, digital users no longer
have to search far for custom baskets
because the three new universal
holder systems available from Rinn
are designed to adapt to all intraoral
sensors currently on the market and
to all expected future iterations.
For users who prefer an autoclavable
solution that integrates with
their existing Rinn arms and rings,
the XCP-DS FIT Universal sensor
holders are the ideal solution. The
XCP-DS FIT system comprises five
indication-specific biteblocks: anterior,
posterior, horizontal bitewing,
vertical bitewing and endo.
Each biteblock is made with a selfadjusting
clip that accommodates any
sensor brand, from the smallest sized
one to the largest. Soft silicone covers
soft-tissue contact points while
also minimizing risk of sensor cover
perforation. Because XCP-DS FIT biteblocks
are front-loading on the face
of the sensor, there is no plastic surrounding
the cord connection area on
the back.
exhibitors
Available in introductory kits or
refill packages with biteblocks only,
the XCP-DS FIT system has become a
popular choice for autoclavable sensor
holders.
Another autoclavable universal
sensor holder available is the Snap-
A-Ray DS. Designed along the lines
of the original Snap-A-Ray for film
(which has been around since the
1940s), Snap-A-Ray DS is made to
allow for quick and easy periapical
imaging.
A patented set of collapsing grips
made of soft plastic allows the Snap-
A-Ray DS to gently but firmly grip
digital sensors. An optional arm and
ring set is available for use with
the Snap-A-Ray DS to aid new users
with accurate alignment. The sensor
holder is available in two versions:
a standard hygiene version and an
endodontic version with reduced bite
surface area to allow room for files
and a rubber dam clamp.
If you prefer a disposable holder for
the ultimate in infection control, the
Uni-Grip 360 universal sensor holder
is an ideal solution. It attaches with
adhesive to covered sensors but does
not require tight-fitting sensor covers
for positioning stability.
Uni-Grip 360 utilizes four adhesive
tabs that provide secure mounting by
locking the sensor on both sides and
bottom. Perforations in the tabs allow
for easy removal after use.
