Instructions for filling Preference form
1) Fill all the information with Black ball point pen.
2) Fill the column of "HSC Pass"
3) HSC PCB Marks : Please darken the appropriate circle. e.g. 257 out of 300, to
be written & marked as under
4) Signature of the Candidate: Candidate should sign in the place provided.
5) College Code : The candidates are expected to carefully read and then mark their
preferences of the college according to the college code.
The "College code" is a four digit number
i) The first digit denotes the 'course', MBBS - 1, BDS - 2, BAMS - 3, BHMS – 4,
BUMS - 5, BPTh - 6, BOTh - 7, BASLP - 8 and BP&/BSc Nursing – 9
ii) The second digit denotes the 'region', ROM - 1, Vidarbha - 2, Marathwada – 3
(Except B.Sc. Nursing)
iii) The third and fourth digit denote the college number
e.g. 1101 denotes Grant Medical College, Mumbai. Its first digit denotes the
course MBBS-1, second digit denotes the region ROM - 1, and the third and
fourth digits i.e. 01 denotes the college number.
6) Signature of Verifying Officer : The officer appointed by the Divisional Authority for
this purpose, to scrutinize / verify the details stated in the preference form by the
candidate.
