rejected .


The abov- <strong>items</strong> are <strong>to</strong> be supplied before 05.01.2013 at 05.00 pm <strong>to</strong> the District<br />

Medical Office •f Health in 14 districts <strong>and</strong> in Direc<strong>to</strong>rate <strong>of</strong> Health Services as per the<br />

distribution pro osecl by the undersigned. Any delay in <strong>supply</strong> the <strong>items</strong> within the<br />

stipulated time, here will be a levy by reduction @ 2 % <strong>of</strong> <strong>to</strong>tal amount <strong>of</strong> <strong>print</strong>ing cost/<br />
