Tender orms -we-issued from UIP 1 section of this office from 10.12.2012 at
10.15 am onw rds during office hours. Last date of issue of tender forms is 20.12.2012
up to 01.00.p. . Last date of submission of tenders i52d-12.2012 up to 03.00 p.m. Date of
opening of Ten ers is on 22.12.2012 at 11.00 a.m.
Cheque/ ank Draft are not accepted towards the cost of tender forms. Tender
duly filled an' signed should be accompanied by an agreement executed in Kerala
Stamp paper orth P s 50 and EMD @ 1% of the quoted amount. Samples of materials
are to be prod ced.
Tend r should be addressed to the Additional Director of Health Services(FW),
Directorate of Health Services, Near General Hospital, Thiruvananthapuram, Kerala
state. At the tip of the envelop containing tender documents, it should be subscribed
that "Tender to print/ supply of Pulse Polio materials 2012-13 as per Tender No:
5, ate tenders and tenders not satisfying the above conditions shall be
rejected .

The abov- items are to be supplied before 05.01.2013 at 05.00 pm to the District
Medical Office •f Health in 14 districts and in Directorate of Health Services as per the
distribution pro osecl by the undersigned. Any delay in supply the items within the
stipulated time, here will be a levy by reduction @ 2 % of total amount of printing cost/
per day.
