The list of drugs covered by our pharmacy benefit<br />
is called our formulary. Most drugs are included<br />
in our formulary.<br />
We use a variety of approaches <strong>to</strong> manage your<br />
pharmacy benefit. Some of these approaches<br />
include the three-tier pharmacy copayment, prior<br />
authorization, step therapy, quantity limitation,<br />
and designated specialty pharmacy programs.<br />
