RCVS versus PACNS
• 1950s: Primary angiitis of the CNS (PACNS)
uniformly poor outcome; requires prompt
immunosuppressive therapy
• Until late 2000s: RCVS under-recognized
(variable nomenclature - Call’s, BACNS, PPA,
vasospasm in eclampsia, TCH, migraine, drugs)
• 1950s-2000s: patients with RCVS misinterpreted
as having PACNS due to overlapping features
such as headache, cerebral angiographic
abnormalities, and ischemic stroke
patients with RCVS exposed to the risks of brain
biopsy, chronic immunosuppressive therapy
