Useful Information<br />

The NHS & Community Care Act came in<strong>to</strong> force on 1 April<br />

1993. Community Care is the network of services which<br />

support people in need of care, whether those services are<br />

provided by the NHS, Social Care <strong>and</strong> Health (Social<br />

Services), or by the voluntary or private sec<strong>to</strong>r. This act<br />

places a duty on Social Care <strong>and</strong> Health <strong>to</strong> assess any<br />

person who appears <strong>to</strong> be in need of services. The aim is <strong>to</strong><br />

identify people who can be looked after at <strong>home</strong> rather than<br />

going in<strong>to</strong> residential <strong>and</strong> nursing <strong>home</strong> care, <strong>and</strong> who <strong>to</strong><br />

provide services in response <strong>to</strong> their needs.<br />

Assessments are the first stage in sorting out what help is<br />

needed. You will be eligible for assessment if you appear <strong>to</strong><br />

need services. The decision about whether <strong>to</strong> provide any<br />

services, <strong>and</strong> if so which, is taken by Social Services as the<br />

result of the assessment <strong>and</strong> will form part of an individual<br />

care plan.<br />

How <strong>to</strong> get the most out of a Care Plan.<br />

Assessment is only the first stage of a process of planning<br />

individual care. The process will also include devising an<br />

individual plan which you should see <strong>and</strong> which should<br />

include details of the services needed. The plan should<br />

make provision for the situation <strong>and</strong> reviewing the<br />

Personal Care: Social Care <strong>and</strong> Health Home Care<br />

Assistants will provide help with all aspects of personal care<br />

following an assessment completed by a Social Worker or<br />

Care Manager. If the assessed need meets Social Care <strong>and</strong><br />

Health criteria, help with personal care may be provided.<br />

Contact your local Social Care <strong>and</strong> Health office for details<br />

of current charges.<br />

Night Care/Sitting: A Night Care/Sitting Service is available<br />

through Social Care <strong>and</strong> Health following an assessment by<br />

a Social Worker or Care Manager. The service may be<br />

useful <strong>to</strong> carers who find that their sleep is regularly<br />

disturbed by the person that they care for. A care attendant<br />

will be able <strong>to</strong> provide help with personal care <strong>and</strong> other<br />

household duties, for example ironing, washing throughout<br />

the night. Contact your local Social Care <strong>and</strong> Health office<br />

Purbeck District Council provides a range of travel<br />

<strong>Dorset</strong> Country Cars<br />

This covers a number of schemes in rural areas of <strong>Dorset</strong>.<br />

The scheme is for people who are not able <strong>to</strong> use other<br />

means of transport.<br />

Contributions are between 25p <strong>and</strong> 35p per mile (this varies<br />

from scheme <strong>to</strong> scheme) for journeys up <strong>to</strong> 50 miles round<br />

trip. Bookings should be made at least 24 hours in advance<br />

Can be used <strong>to</strong> visit your doc<strong>to</strong>r, clinic, chiropodist, friends or<br />

relatives in hospital, collect prescriptions <strong>and</strong> connect with a<br />

bus, train or coach.<br />

The scheme cannot be used for hospital appointments - the<br />

hospital car scheme should be used instead.<br />

Gillingham Area Voluntary Car Link Scheme<br />

assistance <strong>to</strong> people in the SP8 postcode area who need<br />

help with transport. Users of the service will be given an<br />

envelope <strong>to</strong> make donations <strong>to</strong>wards the running costs of<br />

NORDCAT & SCAT Bus – <strong>North</strong> <strong>Dorset</strong> Community<br />

Accessible Transport<br />

A door-<strong>to</strong>-door on dem<strong>and</strong> service for people of all ages who<br />

cannot use normal public transport <strong>and</strong> for all people aged<br />

over 60. Available in <strong>North</strong> <strong>and</strong> <strong>North</strong> West <strong>Dorset</strong> for<br />

shopping, visiting friends, medical appointments <strong>and</strong> so on.<br />

Users must register with an annual fee of £2. This includes<br />

the cost of a newsletter <strong>and</strong> programme every four months.<br />

The buses are also available for hire by community groups.<br />

Concessionary Travel – <strong>North</strong> <strong>Dorset</strong><br />

<strong>North</strong> <strong>Dorset</strong> Concessionary Travel Scheme is able <strong>to</strong><br />

provide residents aged over 60 <strong>and</strong> disabled residents with<br />

free travel for all bus journeys that start within the borough<br />

through <strong>to</strong> their destination <strong>and</strong> return. Anyone who is<br />

registered blind or partially sighted is entitled <strong>to</strong> a bus pass<br />

which will enable them <strong>and</strong> a companion <strong>to</strong> travel free. To<br />

Disabled Parking Badges – (Blue Badge Scheme)<br />

The Blue Badge Scheme is a national arrangement of<br />

parking concessions for people with severe walking<br />

difficulties who travel either as drivers or passengers. The<br />

scheme also applies <strong>to</strong> registered blind people, <strong>and</strong> people<br />

with very severe upper limb disabilities who regularly drive a<br />

vehicle but cannot turn the steering wheel by h<strong>and</strong>. The<br />

scheme allows badge holders <strong>to</strong> park close <strong>to</strong> their<br />

destination, but the national concessions apply only <strong>to</strong> onstreet<br />

parking. Badges are only issued <strong>to</strong> people who meet<br />

the eligibility criteria, not <strong>to</strong> relatives or carers. They can<br />

however be used in any car the badge holder is driving or is<br />

a passenger in.<br />

The eligibility criteria for Blue Badges are set by law. The<br />

local authority makes the final decision on eligibility for the<br />

blue badge: this cannot be decided by a GP, although<br />

evidence of your disability from your GP will be required.<br />

The service is only available <strong>to</strong> patients who have no other<br />

way of getting <strong>to</strong> hospital.<br />

Healthcare Travel Costs Scheme<br />

If you are <strong>to</strong> travel <strong>to</strong> hospital independently, <strong>and</strong> are in<br />

receipt of any of the benefits listed below, you are entitled <strong>to</strong><br />

claim back public transport costs from the Hospital General<br />

Hospital Car Scheme – Going that extra mile for our<br />

patients<br />

As a hospital trust we only have <strong>to</strong> reimburse public<br />

transport costs for those patients who are eligible for support<br />

in getting <strong>to</strong> hospital. However, it is in our interest <strong>to</strong> make<br />

sure that patients arrive at hospital on time for their<br />

appointments so that our clinics <strong>and</strong> theatre lists run on time.<br />

Because of this we have set up a service with Poole Radios<br />

Cabs <strong>to</strong> help those eligible <strong>to</strong> receive a hospital car service.<br />

You will need your GP <strong>to</strong> sanction your first request for<br />

Hospital Car transport. All future requests will need <strong>to</strong> be<br />

sanctioned by the hospital department you are visiting.<br />

Bus services from Wareham <strong>to</strong> local hospitals<br />

Wilts & <strong>Dorset</strong> Bus Company – service <strong>to</strong> Poole Hospital<br />

From Wareham bus 142, 143, or 144 <strong>to</strong> Poole Bus Station,<br />

You can book a vehicle with a trained driver <strong>and</strong> escort if<br />

appointments, shopping or visiting friends.<br />

Charges are 38p per mile from the volunteer’s door <strong>to</strong> the<br />

end of your journey. There is also a 15% charge for<br />
