Useful Information
The NHS & Community Care Act came into force on 1 April
1993. Community Care is the network of services which
support people in need of care, whether those services are
provided by the NHS, Social Care and Health (Social
Services), or by the voluntary or private sector. This act
places a duty on Social Care and Health to assess any
person who appears to be in need of services. The aim is to
identify people who can be looked after at home rather than
going into residential and nursing home care, and who to
provide services in response to their needs.
Assessments are the first stage in sorting out what help is
needed. You will be eligible for assessment if you appear to
need services. The decision about whether to provide any
services, and if so which, is taken by Social Services as the
result of the assessment and will form part of an individual
care plan.
How to get the most out of a Care Plan.
Assessment is only the first stage of a process of planning
individual care. The process will also include devising an
individual plan which you should see and which should
include details of the services needed. The plan should
make provision for the situation and reviewing the
assessment regularly. For further information and

Personal Care: Social Care and Health Home Care
Assistants will provide help with all aspects of personal care
following an assessment completed by a Social Worker or
Care Manager. If the assessed need meets Social Care and
Health criteria, help with personal care may be provided.
Contact your local Social Care and Health office for details
of current charges.
Night Care/Sitting: A Night Care/Sitting Service is available
through Social Care and Health following an assessment by
a Social Worker or Care Manager. The service may be
useful to carers who find that their sleep is regularly
disturbed by the person that they care for. A care attendant
will be able to provide help with personal care and other
household duties, for example ironing, washing throughout
the night. Contact your local Social Care and Health office
for details of current charges.

Concessionary Travel – Purbeck
Purbeck District Council provides a range of travel
concessions to people who are permanent residents of the
area and who satisfy the eligibility conditions. An application
can be made at any time, but only one of the concessions
can be claimed.
Dorset Bus Pass
Allows you to travel for free within Dorset (including
Bournemouth and Poole) and also on journeys that start or
finish within Dorset, provided the journey does not involve a
change of bus outside of the Dorset area. You are eligible
for a Dorset Bus Pass if you are:

Dorset Country Cars
This covers a number of schemes in rural areas of Dorset.
The scheme is for people who are not able to use other
means of transport.
Contributions are between 25p and 35p per mile (this varies
from scheme to scheme) for journeys up to 50 miles round
trip. Bookings should be made at least 24 hours in advance

Can be used to visit your doctor, clinic, chiropodist, friends or
relatives in hospital, collect prescriptions and connect with a
bus, train or coach.
The scheme cannot be used for hospital appointments - the
hospital car scheme should be used instead.
Bookings should be with your local organiser listed below,
who will then check that the journey is eligible and arrange
for a driver.

Gillingham Area Voluntary Car Link Scheme
Help is available for transport to:
• Medical and related appointments
• Visiting a friend or relation in hospital
• Collecting shopping, library books, prescriptions etc
• To and from the hairdressers or barbers
• For social outings
The transport is organised by a group of local people
supported by volunteer drivers and co-ordinators, and offers
assistance to people in the SP8 postcode area who need
help with transport. Users of the service will be given an
envelope to make donations towards the running costs of

NORDCAT & SCAT Bus – North Dorset Community
Accessible Transport
A door-to-door on demand service for people of all ages who
cannot use normal public transport and for all people aged
over 60. Available in North and North West Dorset for
shopping, visiting friends, medical appointments and so on.
Users must register with an annual fee of £2. This includes
the cost of a newsletter and programme every four months.
The buses are also available for hire by community groups.

Concessionary Travel – North Dorset
North Dorset Concessionary Travel Scheme is able to
provide residents aged over 60 and disabled residents with
free travel for all bus journeys that start within the borough
through to their destination and return. Anyone who is
registered blind or partially sighted is entitled to a bus pass
which will enable them and a companion to travel free. To

Disabled Parking Badges – (Blue Badge Scheme)
The Blue Badge Scheme is a national arrangement of
parking concessions for people with severe walking
difficulties who travel either as drivers or passengers. The
scheme also applies to registered blind people, and people
with very severe upper limb disabilities who regularly drive a
vehicle but cannot turn the steering wheel by hand. The
scheme allows badge holders to park close to their
destination, but the national concessions apply only to onstreet
parking. Badges are only issued to people who meet
the eligibility criteria, not to relatives or carers. They can
however be used in any car the badge holder is driving or is
a passenger in.
The eligibility criteria for Blue Badges are set by law. The
local authority makes the final decision on eligibility for the
blue badge: this cannot be decided by a GP, although
evidence of your disability from your GP will be required.

You will be automatically eligible for a Blue Badge if you
receive
• Mobility Allowance
• War Pensioner’s Mobility Allowance
• Higher rate of Disability Living Allowance (DLA), that is
awarded “ Help getting around.”
Or if you:
• Are registered blind
• Use a disabled person’s car supplied by a government
department
• Receive a grant towards owning a vehicle under section
5(2)(a) of the NHS Act 1977, or section 46 of the NHS
(Scotland) Act 1978.
You will be assessed as eligible if you:
• Drive a motor vehicle regularly and have a severe
disability in both upper limbs and are unable to turn by
hand the steering wheel of a motor vehicle even if it is
fitted with a turning knob.
• Have a permanent and substantial disability, which
causes inability to walk or very considerable difficulty in
walking.

The service is only available to patients who have no other
way of getting to hospital.
Healthcare Travel Costs Scheme
If you are to travel to hospital independently, and are in
receipt of any of the benefits listed below, you are entitled to
claim back public transport costs from the Hospital General
Office:
• Income Support (IS)
• Income based Job Seekers Allowance (JSA (IB))
• Working Tax Credit (WTC) – NHS Tax Credit Exemption

Hospital Car Scheme – Going that extra mile for our
patients
As a hospital trust we only have to reimburse public
transport costs for those patients who are eligible for support
in getting to hospital. However, it is in our interest to make
sure that patients arrive at hospital on time for their
appointments so that our clinics and theatre lists run on time.
Because of this we have set up a service with Poole Radios
Cabs to help those eligible to receive a hospital car service.
You will need your GP to sanction your first request for
Hospital Car transport. All future requests will need to be
sanctioned by the hospital department you are visiting.
Bus services from Wareham to local hospitals
Wilts & Dorset Bus Company – service to Poole Hospital
From Wareham bus 142, 143, or 144 to Poole Bus Station,
then catch bus M1, M2, 101, 164, 165 (every few minutes) to
the hospital. Wilts & Dorset Bus Company – service to Royal
Bournemouth Hospital. From Wareham catch bus 142, 143,
or 144 to Poole Bus Station, then bus X3 or bus 165 to
Royal Bournemouth Hospital. Buses X3 run every hour on
the half hour and the 165 bus runs every half hour, on
Mondays to Saturdays.

National organisations able to help with
transport
British Red Cross Transport and Escort Service
For people who cannot use ordinary transport on their own.
You can book a vehicle with a trained driver and escort if
needed, or you can book an escort to help you use public

transport. Services include help with bags, attending
appointments, shopping or visiting friends.
Charges are 38p per mile from the volunteer’s door to the
end of your journey. There is also a 15% charge for
