Ultrasound Imaging <strong>Position</strong> <strong>Statement</strong><br />
The <strong>SDMS</strong> Board <strong>of</strong> Directors conducted <strong>its</strong><br />
annual Board meeting on September 19, 2011 in<br />
Atlanta, Georgia during the 2011 <strong>SDMS</strong> Annual<br />
Conference. During the meeting, the Board<br />
reviewed and approved the proposed Point-<strong>of</strong>-<br />
Care Ultrasound <strong>Position</strong> <strong>Statement</strong> submitted by<br />
Point-<strong>of</strong>-Care Ultrasound Imaging<br />
the <strong>SDMS</strong> Practice Management Committee and<br />
which now replaces the <strong>Limited</strong> <strong>Exam</strong> <strong>Position</strong><br />
<strong>Statement</strong>. This position statement as well as<br />
other <strong>SDMS</strong> position statements can be located<br />

The <strong>Society</strong> <strong>of</strong> Diagnostic Medical Sonography (<strong>SDMS</strong>) believes that point-<strong>of</strong>-care<br />
ultrasound imaging should be performed by:<br />
1. A diagnostic medical sonographer certified in the specialty area <strong>of</strong> the procedure to be<br />
performed; or<br />
2. A licensed health care provider who:<br />
a. Has acquired the necessary knowledge, skills and training in the specific point-<strong>of</strong>-care<br />
ultrasound imaging procedure to be performed; and<br />
b. Is providing the point-<strong>of</strong>-care ultrasound imaging procedure pursuant to the licensing<br />
jurisdiction’s scope <strong>of</strong> practice for the health care provider.<br />

<strong>Position</strong> <strong>Statement</strong> Background:<br />
Point-<strong>of</strong>-Care Ultrasound Imaging<br />
The following background information was prepared<br />
by the <strong>SDMS</strong> Practice Management Committee for the<br />
<strong>SDMS</strong> Board <strong>of</strong> Directors, <strong>SDMS</strong> members, and the public.<br />
However, the background information should not be<br />
considered to be part <strong>of</strong> the formal position statement<br />
above. Definitions have been provided at the end <strong>of</strong> this<br />
document to assist the reader.<br />
The <strong>Society</strong>’s traditional position has been that<br />
anyone holding an ultrasound transducer during<br />
a sonographic examination should be a certified/<br />
credentialed sonographer. However, miniaturization<br />
and technological advancements have resulted in<br />
portable ultrasound devices capable <strong>of</strong> use by other<br />
licensed health care pr<strong>of</strong>essionals for certain limited,<br />
point-<strong>of</strong>-care ultrasound imaging procedures. Some<br />
commentators have predicted that these portable<br />
devices will become as ubiquitous in medicine as<br />
stethoscopes, but such statements oversimplify the<br />
complexities, benef<strong>its</strong>, and risks associated with the<br />
use <strong>of</strong> ultrasound in medicine.<br />
The ultrasound technology evolution does not signify<br />
the demise <strong>of</strong> the diagnostic medical sonography<br />
pr<strong>of</strong>ession. Instead, the technology perm<strong>its</strong> the<br />
application <strong>of</strong> ultrasound in new ways that, if<br />
performed properly, can improve patient care and<br />
safety. The ultrasound technology evolution is likely<br />
to continue and the issues surrounding <strong>its</strong> use by<br />
other healthcare providers are also likely to increase.<br />
Therefore, it is critical to distinguish non-sonographers<br />
who use ultrasound technologies for limited point-<strong>of</strong>care<br />
ultrasound imaging from the diagnostic medical<br />
sonography services provided by sonographers.<br />
Sonography, unlike many other medical imaging<br />
modalities, does not use ionizing radiation but<br />
instead uses high-frequency sound waves (a type<br />
<strong>of</strong> non-ionizing radiation). Yet, despite no ionizing<br />
radiation risks, there can be harm from the application<br />
<strong>of</strong> ultrasound by untrained or inadequately trained<br />
individuals. Animal studies indicate that ultrasound<br />
can cause tissue changes or damage – <strong>of</strong> great<br />
concern when applied to a developing fetus<br />
in obstetrical sonography examinations. If the<br />
individual using ultrasound equipment does not fully<br />
understand the proper use <strong>of</strong> the equipment and<br />
how to properly perform an ultrasound examination<br />
(by minimizing the exposure time and energy level),<br />
patient harm may result. 1<br />

Perhaps a more observable harm is when an incorrect<br />
diagnosis results from an improperly performed<br />
sonographic examination. For example, an improperly<br />
performed carotid artery examination could indicate<br />
a severe stenosis requiring immediate surgery, even<br />
when no blockage is actually present. The risks<br />
associated with such an unnecessary surgery are<br />
serious and significant. Another example is when a<br />
poorly performed sonographic examination must be<br />
repeated – delaying potentially lifesaving treatment<br />
and increasing the cost to the patient, taxpayers, and<br />
society.<br />
Sonographers complete extensive education and<br />
training in the proper use <strong>of</strong> ultrasound including<br />
the study <strong>of</strong> human anatomy, ultrasound physics,<br />
and techniques to obtain the best image while<br />
minimizing exposure to ultrasound energy. However,<br />
the proper application <strong>of</strong> ultrasound in medical<br />
imaging is still largely dependent on the operator’s<br />
skill and knowledge. Sonographers have demonstrated<br />
minimum competence by successfully completing a<br />
national certification examination provided through<br />
a nationally recognized sonography credentialing<br />
organization. If other licensed health care providers<br />
wish to perform diagnostic medical sonography<br />
studies, there are pathways available for them to<br />
demonstrate minimum competence by documenting<br />
experience and successfully completing a national<br />
certification examination provided through a nationally<br />

recognized sonography credentialing organization.<br />
Some procedures may be performed by either<br />
sonographers or point-<strong>of</strong>-care ultrasound imaging<br />
providers. However, diagnostic medical sonographic<br />
examinations are typically procedures which are<br />
diagnostic in nature and are referred to a physician<br />
for final interpretation or diagnosis. Point-<strong>of</strong>-care<br />
ultrasound imaging is performed as part <strong>of</strong> a focused<br />
diagnostic examination and is not intended to replace<br />
comprehensive sonographer performed imaging. 4<br />
Point-<strong>of</strong>-care ultrasound imaging procedures permit<br />
a licensed health care provider to visualize gross<br />
anatomical structures or guide other procedures. In<br />
the past, such procedures would have been done<br />
by blind insertion or visualization with an ionizing<br />
radiation such as x-ray or fluoroscopy. In these cases,<br />
the use <strong>of</strong> point-<strong>of</strong>-care ultrasound imaging greatly<br />
reduces the risk <strong>of</strong> patient injuries and complications<br />
such as infection, pneumothorax, etc. However, point-
<strong>of</strong>-care ultrasound imaging procedures should only<br />
be performed by an individual who has acquired the<br />
necessary knowledge, skill, and training in the specific<br />
point-<strong>of</strong>-care ultrasound imaging procedure to be<br />
performed. Failure to properly perform a point-<strong>of</strong>-care<br />
ultrasound imaging procedure can result in significant<br />
harm to a patient.<br />
A point-<strong>of</strong>-care ultrasound imaging procedure creates<br />
real-time images that are useful for immediate or<br />
direct patient care at the moment or soon after the<br />
procedure is completed. For example, a paramedic<br />
might perform a point-<strong>of</strong>-care ultrasound imaging<br />
procedure to determine if a patient has an abdominal<br />
aortic aneurysm, which could help determine the<br />
immediate patient care required (e.g., start a largebore<br />
intravenous line, transport by medical helicopter,<br />
etc.). 5 Other examples <strong>of</strong> point-<strong>of</strong>-care ultrasound<br />
imaging include:<br />
• Determination <strong>of</strong> fetal presentation;<br />
• Determination <strong>of</strong> fetal cardiac activity;<br />
• Placenta location;<br />
• Aiding in establishment <strong>of</strong> an intravenous line;<br />
• Guidance <strong>of</strong> needle for nerve block injection; and<br />
• Rapid assessment for fluid in a trauma patient’s<br />
abdomen.<br />
Most diagnostic medical sonography procedures<br />
are billable when performed by an appropriately<br />
certified sonographer or a licensed physician. Most<br />
point-<strong>of</strong>-care ultrasound imaging procedures are not<br />
directly reimbursed by insurance companies as these<br />
procedures are frequently part <strong>of</strong> another procedure<br />
billing code. In addition, images from point-<strong>of</strong>-care<br />
ultrasound imaging procedures are not referred to<br />
a physician or other qualified licensed health care<br />
provider for interpretation or diagnosis.<br />
Some physician specialty residencies (e.g., emergency<br />
medicine) include specific training on point-<strong>of</strong>care<br />
ultrasound imaging (sometimes taught by<br />
sonographers). On the other hand, the national<br />
curricula, educational programs, and certification/<br />
licensing examinations <strong>of</strong> most other licensed health<br />
care providers do not begin to address this emerging<br />
use <strong>of</strong> medical imaging. The needed knowledge, skill,<br />
and training is unlikely to be gained from a weekend<br />
(or even week-long) continuing education course but<br />
rather a combination <strong>of</strong> education and supervised<br />
clinical experience.<br />

In addition to having the necessary knowledge, skill,<br />
and training, point-<strong>of</strong>-care imaging providers should<br />
be authorized pursuant to their jurisdiction’s licensing<br />
scope <strong>of</strong> practice. Ideally, such authorization should<br />
be explicitly provided through the licensed health<br />
care provider’s licensing statute or regulation. The<br />
Oklahoma Board <strong>of</strong> Nursing has adopted guidelines<br />
on limited obstetric ultrasound examinations<br />
performed by registered nurses. 6 These guidelines<br />
describe the registered nurse’s qualifications,<br />
including training beyond basic nursing preparation<br />
in the performing <strong>of</strong> limited ultrasound, and<br />
requirements for institutional policies such as<br />
availability <strong>of</strong> sonographers or other qualified staff if a<br />
complication is identified. 6<br />
In 2011, a bill was introduced in the New Mexico<br />
legislature that, as written, would have exempted all<br />
nursing from the state’s licensing requirements for<br />
medical imaging including radiography, sonography,<br />
computed tomography, etc. 7 Discussions with the<br />
various nursing organizations revealed that the<br />
primary motive behind the bill was to allow nurses<br />
to perform point-<strong>of</strong>-care ultrasound imaging. A new<br />
bill designed to address this and other issues has<br />
been prepared by the medical imaging community<br />
for introduction in the 2012 New Mexico legislative<br />
session. The new bill mirrors the elements listed in<br />
the <strong>SDMS</strong> <strong>Position</strong> <strong>Statement</strong> above. Similar statutory<br />
language may be needed in Oregon, the only other<br />
state that directly regulates who may perform medical<br />
ultrasound procedures. Regulation <strong>of</strong> sonography and<br />
point-<strong>of</strong>-care ultrasound imaging may be necessary in<br />
other states as well.<br />
It is important to recognize the patient benef<strong>its</strong> from<br />
the immediate availability <strong>of</strong> point-<strong>of</strong>-care ultrasound<br />
imaging. It enables bedside diagnostic testing for<br />
immediate patient care decisions. As point-<strong>of</strong>-care<br />
ultrasound imaging becomes part <strong>of</strong> other health<br />
care providers’ educational curricula and training,<br />
sonographers are uniquely positioned to train these<br />
health care providers. However, it is also incumbent on<br />
sonographers to serve as patient advocates and help<br />
other health care providers understand the limitations<br />
<strong>of</strong> point-<strong>of</strong>-care ultrasound imaging, the unique<br />
knowledge, skills, and training necessary to perform<br />
comprehensive diagnostic medical sonography<br />
studies, and the risks associated with improper use <strong>of</strong><br />
this imaging modality.
Definitions<br />
For purposes <strong>of</strong> this <strong>SDMS</strong> <strong>Position</strong> <strong>Statement</strong><br />
background document, the following definitions may<br />
help the reader:<br />
“Diagnostic medical sonographer” means a person<br />
who performs medical imaging procedures using<br />
ultrasound and who is currently certified by a<br />
recognized national or international sonography<br />
certification or credentialing organization.<br />
“Licensed health care provider” means a person who is<br />
licensed by the applicable state agency or authority to<br />
deliver health care services.<br />
“Point-<strong>of</strong>-care ultrasound imaging” means the limited<br />
use <strong>of</strong> ultrasound to create real-time images necessary<br />
for immediate direct patient care and which are not<br />
transmitted or referred for interpretation or diagnosis<br />
by physician or other authorized health care provider.<br />
“Point-<strong>of</strong>-care ultrasound imaging providers” means<br />
a person who has acquired the necessary knowledge,<br />
skills and training in the specific procedure to be<br />
performed and who provides point-<strong>of</strong>-care ultrasound<br />
imaging pursuant to their licensed scope <strong>of</strong> practice.<br />
“Sonography certification or credentialing<br />
organization” means a national or international<br />
organization that administers examinations and award<br />
sonography credentials to those who successfully pass<br />
examinations and meet certification requirements.<br />
Each <strong>of</strong> the organization’s sonography certification<br />
programs must be currently accredited by National<br />
Organization for Competency Assurance (NOCA) /<br />
National Commission for Certifying Agencies (NCCA)<br />
or American National Standards Institute (ANSI).<br />

“Ultrasound” means the use <strong>of</strong> nonionizing high<br />
frequency sound waves with specialized equipment to<br />
direct the sound waves into areas <strong>of</strong> the human body<br />
to generate images for the assessment, diagnosis and<br />
treatment <strong>of</strong> various medical conditions.<br />
Note: In the sonography pr<strong>of</strong>ession, the terms<br />
“certified”, “registered” and “credentialed” are <strong>of</strong>ten<br />
used interchangeably. The term “licensed” refers to<br />
authority granted by a state.<br />

Scholarship Program<br />
The <strong>SDMS</strong> Foundation Sonography Student<br />
Scholarship Program provides a $2,500 scholarship<br />
to a deserving sonography student who has been<br />
accepted or is currently enrolled in an educational<br />
program in diagnostic medical sonography or<br />
cardiovascular technology that has been accredited<br />
by the Commission on Accreditation <strong>of</strong> Allied Health<br />
Educational Programs (CAAHEP). Two (2) sonography<br />
student scholarships will be awarded this year.<br />
Scholarships can be used for tuition, books, or<br />
educational fees.<br />

<strong>SDMS</strong> as the Editor <strong>of</strong><br />
the <strong>Society</strong>’s journal, the<br />
JDMS. During Michelle’s<br />
term as journal editor the<br />
circulation <strong>of</strong> the JDMS<br />
increased by over 42%, a<br />
remarkable achievement.<br />
Additionally, the JDMS<br />
was supported by a “blue<br />
ribbon” panel <strong>of</strong> sonographers and sonologists from<br />
throughout the country under Michelle’s leadership.<br />
During her tenure as the JDMS Editor, Michelle also<br />
served a term <strong>of</strong> service as an At-Large Director on the<br />
<strong>SDMS</strong> Board <strong>of</strong> Directors, having been elected to the<br />
position by her <strong>SDMS</strong> member colleagues.<br />
Michelle continued to maintain an active record<br />

Ann’s at It Again in West Africa!<br />
For more than 10 years, Ann Polin, MS, RDMS, RDCS,<br />
RVT, has been volunteering in West Africa, training<br />
doctors and other medical personnel in sonograhy.<br />
She was fundamental in starting the first sonography<br />
program on the continent <strong>of</strong> Africa at Aninwah<br />
Medical Center in Kumasi, Ghana, where eighteen<br />
Ghanaian students and one Nigerian student made up<br />
the first class.<br />
Ann’s back at it and returning to Ghana this January<br />
to teach at the Aninwah Medical Center, which now<br />
boasts forty students in <strong>its</strong> sonography bachelor<br />
degree program.<br />
<strong>SDMS</strong> continues to support Ann and the work being<br />
done in West Africa. <strong>SDMS</strong> has donated textbooks,<br />
educational videos, scrub tops, and totebags for the<br />
students at the university.<br />

What inspired you to make a career <strong>of</strong> sonography?<br />
My educational background is aeronautical<br />
engineering with a particular interest in fluid<br />
mechanics. That led to the study <strong>of</strong> blood flow and<br />
hemodynamics as I found these more interesting than<br />
“rocket science,” and the only non-invasive way to<br />
really look at these in vivo was Doppler ultrasound.<br />
When pulsed Doppler ultrasound <strong>of</strong> the early 1970s<br />
was merged with imaging to make duplex ultrasound<br />
possible, the potential seemed unlimited – a view I<br />
still hold today. When one looks back, it seems pretty<br />
amazing the advances sonography has made over the<br />
past 30 to 40 years.<br />
Did you have a memorable role model/teacher?<br />
Who was it?<br />
I have been very fortunate throughout my career,<br />
from my undergraduate student days to the present,<br />
to have had a number <strong>of</strong> excellent teachers and role<br />
models. Of these, Dr. John Glover stands out as the<br />
person most important in my own career, not from<br />
a technical perspective but from a pr<strong>of</strong>essional one.<br />
I had the good fortune to collaborate and work with<br />
Dr. Glover, a vascular surgeon and Chief <strong>of</strong> Surgical<br />
Services, for more than 25 years, first at the Indiana<br />
University School <strong>of</strong> Medicine and later here at<br />
Beaumont Hospital prior to his retirement.<br />

This is a continuing series <strong>of</strong> interviews <strong>of</strong><br />
our distinguished <strong>SDMS</strong> Fellow members.<br />
In exchange for my teaching him a little sonography,<br />
he was always teaching, by word and example, the<br />
finer points <strong>of</strong> pr<strong>of</strong>essionalism and practicing patient<br />
care – by mixing just the right amounts <strong>of</strong> knowledge,<br />
humility, respect, compassion, and hard work, and<br />
an attitude <strong>of</strong> always questioning the status quo. His<br />
work instilled in me the concept that every patient<br />
complication or unexpected finding could (and <strong>of</strong>ten<br />
should) be a potential research project; he practiced<br />
evidence-based medicine long before the term<br />
became popular. There is no doubt in my own mind<br />
that I came out ahead in these exchanges.<br />
What was the most significant advance in the<br />
technology that you directly experienced?<br />
It has to be the transition from analog to digital<br />
processing <strong>of</strong> ultrasound signals. In the analog world,<br />
all your ultrasound settings would drift around,<br />
changing in unpredictable ways and strongly<br />
dependent on the surrounding environment. In the<br />
digital era in which we now live, the “ones” and “zeros”<br />
only change when we change them, providing a<br />
very stable imaging platform. Add to that the ever<br />
increasing memory capacity and speed <strong>of</strong> digital<br />
electronics, and it is clear why medical imaging, and<br />
ultrasound specifically, have such a bright future.<br />
While we cannot alter the fundamental physical<br />
principles at work, we can certainly extract more<br />
diagnostic information more quickly from smaller and<br />
smaller signals.<br />
Were there any memorable developments, cases,<br />
or events that changed the way you practiced<br />
sonography?<br />
Two come to mind. First is the development <strong>of</strong> color<br />
Doppler imaging. The first CDI display I ever saw<br />
was flashing blue and red on a screen about the size<br />
<strong>of</strong> one <strong>of</strong> today’s cell phones and purported to be<br />
a neonatal heart. (Much like the early static B-arm<br />
bistable images, it was nearly impossible to question.)<br />
That rapidly evolved into more modern displays that
now allow dynamic 2-D and 3-D representations <strong>of</strong><br />
blood flow patterns and hemodynamics, immensely<br />
speeding up the process <strong>of</strong> identifying tissue<br />
vascularity and cardiovascular abnormalities. The<br />
second is an individual case that presented early on<br />
in my association with Dr. Glover. He sent a patient in<br />
his mid-40s to the vascular laboratory for a peripheral<br />
arterial examination, based on his talking with the<br />
patient; the surgical resident involved in the case<br />
assured us it would be an easy normal examination.<br />
Ninety minutes and a good bit <strong>of</strong> work later, we had<br />
the diagnosis <strong>of</strong> arteria magna involving the entire<br />
aorto-iliac and femoro-popliteal systems bilaterally. It<br />
was one <strong>of</strong> those cases you never forget and a great<br />
learning experience early in my career that it is not<br />
just about the technology – sonography is a powerful<br />
diagnostic tool, but especially so if you listen to the<br />
patient and think like a clinician, not an engineer. (This<br />
lesson was not lost on the surgical resident either; he<br />
went on to have a very successful career as a vascular<br />
surgeon.)<br />
What changes do you predict in the immediate<br />
future?<br />
Over the next two to three years as reimbursement<br />
pressures increase, we will see a lot <strong>of</strong> technology<br />
dedicated to making sonography and sonographers<br />
more efficient, without sacrificing quality or diagnostic<br />
capabilities. My own feelings are that well-controlled<br />
outcome studies will show sonography to be much<br />
more cost-effective than CT and/or MR in many areas<br />
<strong>of</strong> medicine, leading to increased utilization. That<br />
will increase the demand for sonographers, and to<br />
maintain the high regard for our pr<strong>of</strong>ession it will<br />
be the responsibility <strong>of</strong> all <strong>of</strong> us in the field to be<br />
<strong>SDMS</strong> News Wave December 2011 12<br />
certain that our sonography schools are graduating<br />
competent sonographers and that quality, rather than<br />
quantity, remains the focus <strong>of</strong> our own efforts.<br />
What advice would you give to students/future<br />
sonographers?<br />
First, you will never stop learning, so get used to the<br />
idea early in your career! Keep an open mind and don’t<br />
hesitate to try out new ideas. And remember, in this<br />
digital age <strong>of</strong> the Internet, social media, etc., it is not<br />
hard to find help when you need it. Second, keep the<br />
patient at the center <strong>of</strong> all that you do. Sonographers<br />
work in a challenging environment as the direct<br />
human link between technology and the patient, and<br />
it can be easy to get caught up in the “busywork” <strong>of</strong><br />
high patient volumes, schedules, the paperwork (even<br />
in those so-called paperless departments), and so on.<br />
Embrace new technology as it evolves as something<br />
that will help you clinically and contribute to your<br />
providing the best patient care possible. And finally,<br />
as you take care <strong>of</strong> your patients, take care <strong>of</strong> yourself.<br />
Get in the habit early <strong>of</strong> good ergonomic practices – I<br />
have seen too many promising careers cut short by<br />
musculoskeletal injuries.<br />
What’s your favorite part <strong>of</strong> the job?<br />
Mentoring and teaching new sonographers, watching<br />
them grow and develop pr<strong>of</strong>essionally as they learn to<br />
integrate their technical and clinical skills and become<br />
good care-givers. My generation <strong>of</strong> sonographers<br />
is approaching retirement, and it is both fun and<br />
reassuring to see this next generation successfully<br />
meet the challenges presented by 21st century<br />
healthcare. It’s nice to know that the pr<strong>of</strong>ession is<br />
being left in good, caring hands.

The LRP awards loan repayment (up to $20,000<br />

bachelor’s degree, Ashley intends to continue her<br />
education by obtaining her master’s degree at OSU.<br />
When asked what inspired her to study sonography,<br />
Ashley replied “My inspiration to study sonography<br />
followed a shadowing experience at a cardiologist<br />
<strong>of</strong>fice during my senior year <strong>of</strong> high school.” Ashley<br />
values the patient interaction that is required <strong>of</strong> a<br />
sonographer and being able to help in the diagnosis <strong>of</strong><br />
disease and anomalies. She also enjoys the utilization<br />
<strong>of</strong> both human anatomy, the hands-on work ethic that<br />
sonography provides, and the exciting advancements in<br />
sonography within the past few years.<br />
Ashley stated the <strong>SDMS</strong> Webinar Series has been<br />
extremely helpful and her favorite membership benefit<br />
so far. She feels the webinars are a great tool that<br />
can be used as a student learning activity and also<br />
for advanced sonographers. The information keeps<br />
sonographers up-to-date in the latest techniques and<br />
practices in the field.<br />
