Another important change in the management <strong>of</strong> endometrial cancer is the increasing<br />

use <strong>of</strong> chemotherapy. <strong>The</strong> efficacy <strong>of</strong> chemotherapy in this disease was supported by a large<br />

prospective study that showed that systemic chemotherapy was superior to abdominal radiation<br />

therapy in patients with advanced stage disease. In addition, over half <strong>of</strong> the recurrences in<br />

early stage disease include sites outside the pelvis, a fact that support the need for a systemic<br />

treatment.<br />

Nearly every retrospective and prospective study has confirmed that the extent <strong>of</strong> cytoreductive<br />

surgery and the amount <strong>of</strong> residual disease are among the most important factors impacting the<br />

survival <strong>of</strong> women with advanced ovarian cancer.<br />

Role <strong>of</strong> primary surgery: Large metanalyses have clearly demonstrated that the PFS and OS are<br />

proportional to the extent <strong>of</strong> the surgical surgical effort, and so with each 10% increase in the<br />
