INSTRUCTIONS
PLEASE READ CAREFULLY BEFORE SENDING THE APPLICATION
NOTE: DO NOT ATTACH THIS PAGE WITH THE APPLICATION FORM
1. Ensure that the passport application is dully filled in and signed. Incomplete/incorrect applications will be summarily
rejected.
2. Passport in original (wherever applicable) and photocopy of first and last page.
3. Four photographs (front‐pose 3.5 cm X 3.5 cm Size). Two photographs must be pasted on the application form. The photo
on page 3 must be crossed signed/thumbed by applicant only. PLEASE DO NOT SIGN PHOTO PASTED ON FIRST PAGE OF THE
APPLICATION FORM.
4. For minors (below the age of 18), both parents must provide passports in original as well as photocopy of first and last
page, if applying for the first time. Otherwise, photocopies of first and last page.
5. Both parents must sign all applications for a minor below the age of 18. In case one of the parent is not available to sign
the application, attach notarized no objection certificate from the second parent.
6. Attach original residency proof of the country where you are residing or proof of acceptance of your application for
residency.
7. Keep in mind that for the following categories, applications by post will NOT be accepted/ entertained:
(a) Lost/stolen/damaged passports, (b) Change in appearance or change in signature (c) Emergency certificates (compulsory
to bring original copy of FIR and original air‐ticket issued by the airline), (d) First time application for children (biological
parents may also apply, even if not married).
यिद आपका पासपोटर् गुम/चोरी/क्षितग्रत हुआ है तो यह िकन पिरथित म हुआ इसका पूरा िववरण एक अलग सादे कागज पर द और
आिखर म इसे हतािक्षरत कर एवं गुम/चोरी हुए पासपोटर् की पोिलस िरपोटर् की मूल प्रित साथ लगाएं /क्षितग्रत पासपोटर् को साथ म
संलग्न कर / Attach a signed statement explaining briefly the circumstances of lost/stolen/damaged passport. In case of
lost/stolen passport attach the original police report lodged with the local police. In case of damaged passport, attach the
original passport.
8. If you have opted to have your newly issued passport sent back to you by mail/post, please attach self‐addressed stamped
envelope. Please keep in mind that your newly issued passport or any other related documents will be posted only to your
residential address provided in the application form. Requests for sending documents to a third party address will NOT be
entertained.

10. In case name of spouse does not figure in the old passport, attach photocopy of marriage certificate.
11. Submit notarized affidavit in all cases where existing passport has expired more than one year ago.
12. Submit notarized affidavit with photograph for change of appearance from beard to non‐beard, turban to non‐turban
(and vice‐versa) and from childhood photo to attaining young age.
13. Please fill‐in column numbers 1 to 10, 14 and 18 only, if applying for a service as mentioned against serial number 2,3,7
and 8.
If application is found in order and no prior reference is required to be made to the original Passport Issuing Authority, a new passport will
be issued within 10 working/business days for applications received at the counter and within 15 working/business days for applications
received by mail, from the date of receipt of application. If reference is needed, it would take 45 working/business days. Please enquire
about status of passport only after 45 working/business days in cases of outside issued passports.
