Total 100% Fleet Miles
Estimated miles: If this is the first year an apportioned registration application is made, an estimated mileage percentage will be accepted. Estimated mileage is based on proposed operation of the fleet during the year. Mileage must
be clearly marked "E" for estimate. Only reasonable estimated miles will be accepted. Once you submit your mileage, you cannot amend your mileage to lower your bill. If estimating operation in a jurisdiction for second consecutive
registration year and there were actual operations in other jurisdictions, then you are required to pay that jurisdiction a mileage percent in excess of 100% registration. In cases where estimating for second year or doing a double
estimate, the jurisdiction is treated like an added jurisdiction. In the space provided, explain the type of operation, routes, frequency, and any information to support the estimate. Attach additional sheets if needed.
Explain in detail your operation if any mileage is estimated (use back of form if more space is needed)
Submit the original copy to Motor Carrier Services and keep a copy for your records.
