Key <strong>is</strong>sues and concerns in Secondary/Tertiary care<br />
The organization <strong>of</strong> the health care <strong>system</strong> <strong>is</strong> dominated by hospital-based curative<br />
services. Low bed occupancy <strong>is</strong> a key <strong>is</strong>sue, especially in view <strong>of</strong> plans to expand the<br />
number <strong>of</strong> hospital beds. The State and Social Insurance Organization hospital<br />
investment programme increases the number <strong>of</strong> beds in secondary and tertiary care at<br />
the same time that occupancy levels are falling and new types <strong>of</strong> treatment facilities are<br />
needed to make use <strong>of</strong> developing technologies permitting less invasive interventions<br />
and shorter lengths <strong>of</strong> stay.<br />
The continued separation <strong>of</strong> PHC and hospital planning and management <strong>is</strong> a key<br />
obstacle to the development <strong>of</strong> cost effective and coherent health strategies.<br />
The performance <strong>of</strong> the curative sector <strong>is</strong> assessed in terms <strong>of</strong> bed occupancy and<br />
average length <strong>of</strong> stay. Th<strong>is</strong> must be replaced by more relevant measures <strong>of</strong><br />
performance.<br />
