Application For Work Permit
All applicants must fill out an application for a work permit and have it signed
by a parent or legal guardian. Bring the completed application to the main
office and present the issuing officer a document that verifies the age of the
student. (Birth Certificate, Passport, Driver's License)

Parental Acknowledgement of Minor's Duties and Hours of Employment
For individuals who are under 16 years of age, a written statement by the
minor's parent or legal guardian acknowledging understanding of the duties
and hours of employment and granting permission to work is required. This
downloadable form is one way to satisfy that requirement.
