4.5.2.2 Selection of molecules <strong>in</strong> the regression analysis<br />
Among the 66 groups hav<strong>in</strong>g a high cost br<strong>and</strong>, we selected 12 groups to analyze the<br />
association between the use of low cost alternatives <strong>and</strong> patients’ <strong>and</strong> physicians’<br />
<strong>socioeconomic</strong> characteristics. All drugs hav<strong>in</strong>g a narrow therapeutic marg<strong>in</strong>, recognized<br />
as “no switch” 5 or hav<strong>in</strong>g few prescriptions were excluded from the analysis. Choice of<br />
molecules was validated by an expert <strong>and</strong> is summarized hereafter.<br />
Drugs <strong>in</strong> Group ATC A-Alimentary tract <strong>and</strong> metabolism<br />
In this group the two most prescribed molecules, gliclazide <strong>and</strong> lanzoprazole, were<br />
chosen.<br />
Drugs <strong>in</strong> Group ATC C-Cardiovascular <strong>system</strong><br />
Among the group of diuretics (C03BA, C03CA), the most prescribed molecule was<br />
analyzed <strong>in</strong> one group (<strong>in</strong>dapamide) <strong>and</strong> all other diuretics were taken together as a<br />
separate group (chlortalidone, furosemide, torasemide <strong>and</strong> spironolactone). In the class<br />
of beta-blockers (C07), the two most prescribed molecules, bisoprolol <strong>and</strong> atenolol,<br />
were analyzed as one group. Other beta-blockers were analyzed together <strong>in</strong> a separate<br />
group (propranolol, sotalol, metroprolol, acebutol <strong>and</strong> celiprolol). For calcium channel<br />
antaganosist (C08), only diltiazem was analyzed as it was the most prescribed molecule.<br />
Drugs <strong>in</strong> Group ATC J-Anti-<strong>in</strong>fective for <strong>system</strong>ic use<br />
Only drugs from the qu<strong>in</strong>olone group (J01FA) were analyzed.<br />
Drugs <strong>in</strong> Group ATC M-Musculo-skeletal <strong>system</strong><br />
All drugs <strong>in</strong> group M can be received over-the-counter. <strong>The</strong> only exception is<br />
piroxicam. For this reason only this molecule was analyzed.<br />
Drugs <strong>in</strong> Group ATC N-Nervous <strong>system</strong><br />
Tramadol was selected for analysis s<strong>in</strong>ce it is the most prescribed molecule <strong>in</strong> this<br />
group. Also citalopram was analyzed as it is the most prescribed antidepressant.<br />
Drugs <strong>in</strong> Group ATC R-Respiratory <strong>system</strong><br />
<strong>The</strong> most prescribed molecule, acetylcyste<strong>in</strong>e, was analyzed.<br />
Table 16 presents data on the <strong>reference</strong> supplement paid by patients <strong>in</strong> 2008. Mean<br />
<strong>reference</strong> supplement paid <strong>in</strong> 2008 for patients <strong>in</strong> our sample equaled €13.5. Among<br />
the molecules selected for the analysis, the mean <strong>reference</strong> supplement was smaller<br />
(€11.5), but for patients us<strong>in</strong>g lanzoprazole, diltiazem <strong>and</strong> ditalopram the mean<br />
<strong>reference</strong> supplement was more than €30.
