NOMINATIONS FOR 2011 SAN DIEGO DIVISION AWARDS The awards committee needs your help in seeking out those outstanding San Diego Division members who have made specifi c contributions to the game and deserve recognition. This is your chance to reward and honor them. Members are allowed to nominate themselves. The nomination form should be sent with a copy of the nominee’s most current resume. Please act on this as soon as possible. The awards committee must have all nominations by December 20, 2011. Please mail or email the nomination form to: San Diego Division 914 Alyssum Rd. Carlsbad, CA 92011 or email to usptasd@hotmail.com SAN DIEGO AWARD CATEGORIES and GUIDELINES Pro of the Year: Member in good standing with signifi cant contributions to the San Diego Division and National USPTA. Active involvement in the tennis community, publications, special programs, coaching and volunteering. Player of the Year: Member in good standing with exceptional record of tournament play this past year. Coach of the Year: Member in good standing with strong coaching record for the year. Lifetime Achievement: Member in good standing with many years of tennis achievement with USPTA, USTA, and other organizations volunteering and contributions. Tournament of the Year: Any tournament deemed to have a signifi cant infl uence on tennis in the San Diego Division area. Facility of the Year: A tennis facility with affi liation with the USPTA that demonstrated outstanding support of the USPTA and tennis in the area. Community Service: A member in good standing that has volunteered this past year in support of tennis for the community. New Member of the Year: A member that has joined the USPTA in the past two years that has shown an outstanding level of commitment to enhancing tennis in their area with contributions to the USPTA, USTA or other tennis programs. 7
