ATTACHMENT C January 25, 2012 RFP #240427 - “Modified KW Basin Annex Construction” (FFU) PRICING TEMPLATE AND MEASUREMENT FOR PAYMENT Page 3 of 6 Pay Item Descriptions over $25,000.00; 5 percent administrative commission on total costs over $100,000.00 . • No more than three mark-ups, one performing commission and two administrative commissions, none of which exceed the percentages shown above, will be allowed regardless of the number of tiers of subcontractors or the subcontract instrument (ie. Purchase order, contract, etc.). • The contractor’s or subcontractor’s administrative commission shall be only applied to the net increase in the change order price after deductions have been taken. • See also Attachment B, Part II “Financial Terms”, of the 4 Part Draft Contract, for additional terms. MEASUREMENT FOR PAYMENT For all Pay Items (1-12) listed below, payment is made monthly on the basis of percentage of progress complete. Pay Item Descriptions are general and do not necessarily represent all activities which may be required to perform the specific task. All quantities are approximated estimates based on 90% design. All items listed in the pricing sheet template are to be considered furnished and installed unless otherwise noted, and are to include all required supports, hangers, fasteners and etc., and all costs as required by the specification(s) and design documents. All items are to include shipping, handling and delivery costs as required to furnish and install. Pay Item Line #1. Mobilization Measurement will be made on a Lump Sum basis upon Contractor’s successful completion and (CHPRC) approval to begin construction of the KW Basin Annex. Mobilization includes the following items: • Onsite office personnel (key personnel, administration) and home office support up to beginning construction activities • Delivery of all equipment, tools, materials, supplies, and personnel necessary to conduct construction of the Annex • Delivery and installation of field office and support facilities • Installation of any roads, staging, inspection and control areas • Work zone delineation and traffic control materials, equipment, and installation • Safety equipment, communication equipment, and industrial hygiene (IH) monitoring equipment • Task lighting • Other associated work activities required to begin construction activities Payment will constitute full compensation for all direct and indirect costs including any
