IMPORTANT INFORMATION:<br />
Eligibility: To be eligible to live in summer housing, you must be a matriculated student in an<br />
undergraduate or graduate degree program at <strong>Framingham</strong> <strong>State</strong> <strong>University</strong> (FSU) or at ano<strong>the</strong>r<br />
accredited U.S. Institution of Higher Education, and must: a) be registered in a minimum of two FSU<br />
summer session courses during <strong>the</strong> entire period of occupancy, or, be employed at FSU and working a<br />
minimum of 20 hours per week ; or be enrolled in one course and be employed at FSU working a<br />
minimum of 10 hours per week, b) have a completed immunization and health record form reviewed<br />
and approved by FSU Health Services, c) have a record of good citizenship including demonstrated<br />
good social standing at FSU or <strong>the</strong> institution at which student is matriculated; and, d) be 18 years old,<br />
or older.<br />
Assignments will not be made until payment is confirmed as complete by FSU’s Office of Student<br />
Accounts and <strong>the</strong> FSU <strong>University</strong> Health Form (immunization, health and physical examination<br />
record) is reviewed and approved by <strong>the</strong> <strong>University</strong>’s Health Services Office. Return this application<br />

Health Services: The summer <strong>2013</strong> immunization and health record form can be downloaded from <strong>the</strong><br />
Health Services section of <strong>the</strong> <strong>University</strong> Web Site at: http://www.framingham.edu/health-services<br />
The Health Services Office can be reached by telephone at (508) 626-4900.<br />
Health Insurance: All summer residents are required to have health insurance coverage equal to, or<br />
greater than <strong>the</strong> coverage provided by <strong>the</strong> FSU Insurance plan, or as deemed sufficient by FSU Health<br />
Services. Health Insurance information is submitted on <strong>the</strong> health form.<br />
Location: <strong>Summer</strong> <strong>Housing</strong> is scheduled to be housed in Linsley Hall. Location is subject to change<br />
without notice.<br />
Reservation/Damage Deposit: A $100 reservation/damage deposit is required of all summer resident<br />
students at <strong>the</strong> time of application. In <strong>the</strong> event <strong>the</strong> resident withdraws from summer housing this<br />
deposit is non-refundable and non-transferrable and will be forfeited. <strong>Housing</strong> charges, less <strong>the</strong><br />
reservation deposit are refundable until <strong>the</strong> day before first scheduled day of occupancy. After that<br />
time housing costs are non-refundable and non-transferable unless student is withdrawn from all FSU<br />
summer courses, in which case <strong>the</strong> Student Accounts refund schedule will<br />
<strong>Summer</strong> residents are responsible for any damage beyond normal wear and tear to <strong>the</strong> room and/or its<br />
furnishings and will be damage billed accordingly. <strong>Summer</strong> residents are also responsible for sharing<br />
residence hall floor and common area damages where those responsible are unable to be identified.<br />
Following <strong>the</strong> full occupancy period FSU will conduct a final damage assessment. Following this<br />
assessment <strong>the</strong> damage deposit, less damage charges, will be refunded in August.<br />
<strong>Housing</strong> Cost: Costs are listed below by scheduled summer sessions:<br />
o Session A (Move in May 19, Move out June 25) - $1,000;<br />
o Session C-1 (Move in May 19, Move out July 11) - $1,600<br />
o Session C-2 (Move in May 20, Move out July 12) - $1,600<br />
<strong>Housing</strong> costs listed do not include $100 Reservation/Damage deposit, or <strong>the</strong> cost of meals. Please<br />
note: housing is not available for Sessions B, C-3, D-1, D-2, D-3, D-4 or E.<br />

Meal Costs: <strong>Summer</strong> food service is available on a limited basis (M-F, excluding holidays). Linsley Hall<br />
has one shared lounge with a stovetop. Cooking appliances are prohibited in residence hall rooms for<br />
fire safety reasons. Evening and weekend meals are available for purchase at a number of local<br />
establishments.<br />
Room Capacity: Room Assignments are in multiple occupancy rooms. Single occupancy rooms are not<br />
available. If space permits, some double-as-single rooms may be offered. The cost for a double-assingles<br />
for session A is an extra $375 and for sessions C-1 and C-2 is an extra $600.<br />
Check-In: Unless o<strong>the</strong>rwise specified, check-in is at <strong>the</strong> Linsley Hall Security Desk between 6:00 p.m.<br />
and 10:00 p.m. on <strong>the</strong> scheduled move in day. All summer residents are required to complete <strong>the</strong><br />
check-in side of <strong>the</strong> Room Inventory Form upon arrival.<br />
Check-out: Check-out must be completed by 12 noon on <strong>the</strong> scheduled check-out date. <strong>Summer</strong><br />
residents are required to schedule a check-out appointment with a member of <strong>the</strong> staff to check-out,<br />
complete <strong>the</strong> Room Inventory Form and return keys. Alternatively, residents may check-out by<br />
completing an express checkout. Express check-out envelopes and key/express check-out envelope<br />
return lock box is located in <strong>the</strong> Linsley Hall lobby. Failure to check-out by 12:00 noon on <strong>the</strong><br />
scheduled check-out date will result in restriction from <strong>the</strong> hall and a $40 per day late check-out<br />
assessment.<br />
FSU ID Card: FSU ID Card serves as <strong>the</strong> residence hall front door key and must be presented to <strong>the</strong><br />
Security Desk Attendant (SDA) each time a resident enters <strong>the</strong> hall. SDA’s are not allowed to open<br />
residence hall front doors. The FSU Any summer resident who does not have a FSU ID Card is<br />
required to report to <strong>the</strong> ID Office, located next to <strong>the</strong> FSU Police Department in <strong>the</strong> D. Justin McCarthy<br />
Center between 3:00 p.m. and 5:00 p.m. on move-in day.<br />
Parking: The <strong>Framingham</strong> <strong>State</strong> <strong>University</strong> Police Department (508-626-4911) manages parking on<br />
campus. On campus residency does not guarantee parking. Contact <strong>the</strong> <strong>Framingham</strong> <strong>State</strong> <strong>University</strong><br />
Police Department, located in <strong>the</strong> D. Justin McCarthy Center with questions about parking or to obtain<br />
an application for a parking permit.<br />
Telephone/Cable TV: Rooms are not equipped with telephone service. Residents are advised to<br />
utilize cell phones for calls. Basic cable service is scheduled to be in service during <strong>the</strong> summer period.<br />
The campus movie channel will not be available during this period.<br />
Disability/Medical Accommodation of <strong>Housing</strong> Needs: Any student requiring housing<br />
accommodation due to a disability must register for disability services in CASA. In addition, a written<br />
request for accommodation accompanied by appropriate supportive documentation by a medical or<br />
mental health care provider should be provided to <strong>the</strong> Office of Residence Life at <strong>the</strong> time of<br />
application.<br />
Maintenance: Routine maintenance requests should be submitted to <strong>the</strong> Facilities Management<br />
Department through my<strong>Framingham</strong>. Be sure to provide your contact information and detailed<br />
information regarding <strong>the</strong> request. Emergency maintenance situations should be immediately reported<br />
to <strong>the</strong> <strong>Framingham</strong> <strong>State</strong> <strong>University</strong> Police Department and/or a member of <strong>the</strong> Residence Life staff.<br />

Residence Hall Policies and Student Code of Conduct Regulations: All policies and behavioral<br />
expectations noted in <strong>the</strong> Residence Hall License Agreement, <strong>the</strong> RAM Student Handbook and <strong>the</strong> Guide<br />
to Residence Living, unless o<strong>the</strong>rwise outlined in this application, are in effect during <strong>the</strong> summer<br />
housing period. Residents are expected to carefully review all <strong>University</strong> and residence hall policies.<br />
Please take note of <strong>the</strong> following policies:<br />
• For security purposes, valid FSU ID must be presented to staff upon entry to <strong>the</strong> hall.<br />
• No smoking is allowed in Linsley Hall rooms or on <strong>the</strong> front patio of Linsley.<br />
• No cooking appliances are allowed in student rooms.<br />
• Candles and incense are prohibited in all residence halls for fire safety reasons.<br />
• Alcohol, drugs, intoxication, violence and/or weapons of any type and are strictly prohibited.<br />
• Violation of residence hall and/or <strong>University</strong> policies may result in immediate loss of housing.<br />
• There is no refund of housing charges in <strong>the</strong> event of suspension or dismissal from housing.<br />
Neighbors: Please be considerate of our neighbors. Adams Road is a family neighborhood. Inform all<br />
guests to be courteous and respectful with regard to noise. In addition, summer residents are<br />
reminded that noise travels more easily during warm months when windows are open. The volume of<br />
music and voices are expected to be at a reasonable level and contained within assigned rooms at all<br />
times.<br />
Safety: When traveling to or from <strong>the</strong> hall summer residents are urged to consider <strong>the</strong>ir safety by not<br />
walking alone at night, keeping doors and windows locked, not allowing strangers into <strong>the</strong> hall, and<br />
knowing who is at your door before opening it. Take note of emergency call box locations and be alert<br />
for public safety alert notices which may be posted or distributed on campus. <strong>Summer</strong> residents are<br />
urged to review <strong>the</strong> FSU Police Department Web site for information on campus safety, safety and<br />
crime reports and statistics, safety tips, parking, special police units and o<strong>the</strong>r helpful and important<br />
information. The <strong>Framingham</strong> <strong>State</strong> <strong>University</strong> Police Department is available 24 hours a day at(508)626-4911<br />
(x4911 from a campus telephone).<br />
Refrigerators/MicroFridge Units: One personal refrigerator with a capacity not to exceed<br />
three cubic feet, is allowed per room. Personal refrigerators must be ENERTY STAR® rated and<br />
in good working order. Personal MicroFridge-type units are allowed, but must be inspected and<br />
approved by <strong>the</strong> Office of Residence Life before <strong>the</strong>y are brought past <strong>the</strong> residence hall’s security<br />
desk. Units not leased from <strong>Framingham</strong> <strong>State</strong> must meet <strong>the</strong> following specifications:<br />
o 700 watt maximum microwave<br />
o Maximum three cubic foot total capacity freezer/refrigerator<br />
o ENERTY STAR® rated<br />
o Utilizes current limiting circuitry, which is built into <strong>the</strong> unit and has only one plug to <strong>the</strong> wall<br />
MicroFridge units may be leased for $60 from <strong>the</strong> Office of Residence Life for use in Linsley Hall<br />
during <strong>the</strong> student’s summer occupancy period. Please email Residencelife@framingham.edu for more<br />
