<strong>Foster</strong> care worker to foster parent ratio: One Social Worker for 10 children. It does not go by<br />

homes by number of children.<br />

What types of services are available to youth places with <strong>Provider</strong>? (Both treatment and non‐<br />

treatment)<br />

Treatment:<br />

Social Workers do Case Management and refer to special services needed for a child<br />

with special needs.<br />

A nurse is on staff and follows care of children with medical staff.<br />

The agency knows how to apply and work with DD waivers.<br />

They have a Behavior Consultant.<br />

The Social Workers usually work with multiple service providers.<br />

The <strong>Provider</strong> sub contracts with a Psychiatrist.<br />

What types of services are available to foster parents working with <strong>Provider</strong>? (Both treatment<br />

and non‐treatment) There is monthly training for <strong>Foster</strong> Parents. Social Workers have weekly<br />

contact, twice a month in the home. Social Workers will go more often to the home if<br />

needed. There is a 24 hour on call service. Social Workers assist in linking <strong>Foster</strong> Parents to<br />

needed services for their foster children.<br />

<strong>Provider</strong> requirements for <strong>Foster</strong> Parents: <strong>Foster</strong> Parents need prior experience with<br />

Treatment of Medically Fragile youth. <strong>Foster</strong> Parents that have experience with DD youth<br />

and services. <strong>Foster</strong> Parents that can work with the medically fragile youth or the chronic<br />

condition youth.<br />

<strong>Foster</strong> Parent skills/characteristics <strong>Provider</strong>s are seeking: <strong>Foster</strong> Parents that can work with<br />

multiple service providers. <strong>Foster</strong> Parent with prior training or experience with DD youth and<br />

medically fragile children. <strong>Foster</strong> Parents that are willing to take children to visits in and out<br />

of county.<br />

<strong>Provider</strong> requirements for foster parents to transfer: A foster parent that has experience. The<br />

rest is the same as the State requires. Releases need to be signed to get information from<br />

FCCS. LifeStart is interested in <strong>Foster</strong> Parents who want to work with the Youth they serve.<br />
