Failed to ascertain and/or brief Ground Contenders of the flow
rates through the tu rn stile s at the Leppings Lane end;
Failed adequately or at a ll to b rief police o fficers in
attendance at Hillsborough in Police Control, Club Control, and
Vest Stand and on the perimeter at the Vest end that i t u s
th eir duty to monitor the nunbers of spectators in the separate
pens on the Vest Terraces and in particu lar to observe and
report to Police Control if and when any p a rticu la r pen was
approaching capacity, particularly when they knew or ought to
have known that SWPC had ignored a police naming that the
capacity of the Vest Terraces as a whole was too high;
Failed adequately or a t a ll to monitor the nunbers of spectators
in the separate pens on the Vest Terraces frcm the Police
Control Rocm, and/or the Club Control Room and/or by Police
Officers in the Vest Stand and/or by Police O fficers on ' near
the perimeter fence, but to the contrary operated a policy of
allowing spectators ”to find th eir own level" cn the whole
terrace, and on the b asis that so long as spaces existed in any
part of a pen, that pen could not have reached capacity;
Failed to appreciate or ignored the fact that brick walls at the
exit fran the tunnel in to pens three and four and the position
and size of gates at the rear of the pens hindered in practice
"self levelling" of spectators throughout the whole terrace.
