Wash all internal windows with Actichlor plus and dry off with a paper towel. 10.All equipment, ie buckets, mop handles should be removed, thoroughly cleaned using a solution of Actichlor plus, dried and stored in the cleaning cupboard. The Actichlor Plus solution should be changed when it becomes visibly dirty. When cleaning is complete dispose of Actichlor Plus solution in the sluice. Replace clinical and domestic waste bin liners and check and refill/replace soap and alcohol wall dispensers. The room is now ready for use. Please note that the IPCT may recommend using Hydrogen Peroxide Vapour technology (currently Bioquell) to decontaminate a room after discharge. If there is an outbreak of MRSA the bay should be emptied and decontaminated using Hydrogen Peroxide Vapour. Winchester & Eastleigh Healthcare NHS Trust Policy for Isolation and Infectious Diseases Policy Appendix 4: Acinetobacter What is Acinetobacter? It is a Gram Negative Bacillus (GNB). It is an environmental organism that is both in and outside the hospital environment; it lives in water and damp conditions but can survive in dust. It needs little to survive in the environment and can survive for long periods and is relatively resistant to routine cleaning. In hospitalised patients colonisation of the gastro-intestinal tract and oropharnynx with GNBs is common. As with most bugs it is the susceptible patients i.e. the immunosuppressed and/ or the critically ill who are at risk. They have been implicated in outbreaks in ITU, neonatal and oncology units. Surveillance of patients and the environment will be carried out if cases of healthcare acquired aceinetobacter occur. Multi-resistant bacteria are seen more frequently in areas that have high usage of broad spectrum antibiotics and where patients have diminished immunity e.g. critical care and oncology units. GNBs commonly achieve antibiotic resistance by producing an enzyme (e.g. beta-lactamase) that counters the effects of specific antibiotics. The genes that confer antibiotic resistance that can spread to other bacteria. Multi-resistant Acinetobacter are defined as isolates which are resistant to any aminoglycosides and to any third generation cephalosporin. Some multi-resistant Acinetobacter strains are also resistant to carbapenem antibiotics (these strains are designated MRAB-C). How is it spread and how to prevent it? As with all Health Care Acquired Infections (HCAI), Acinetobacter can be part of the transient flora on hands, on equipment and the patient’s environment. Hand hygiene is therefore important in the prevention of spread. Hand hygiene (Policy CPO73), and environmental cleaning are the most important areas that need to be concentrated on. Isolate the patients to prevent spread to other patients, especially if sputum positive with a productive cough or incontinent of urine, in a high risk areas i.e. ITUs, HDU or immunocompromised patients.
