Policy for Isolation and Infectious Diseases Policy
Follow the steps stated in the Isolation policy to manage patients with
colonisations or infections that require barrier nursing.
Contacts will be screened if there is another case of ESBL on the
ward, or there is suspicion of an outbreak.
Treatment
Treatment of infection due to multiple antibiotic-resistant gram negative bacteria
should be based on susceptibility testing and the clinical situation. Clinical staff are
encouraged to refer to the Antimicrobial guidelines and for serous infections to
discuss with the consultant microbiologists
Giving antibiotics to asymptomatic patients to clear the colonisation is seldom
effective. Antibiotic treatment should only be given if patients show clinical signs of
infection.
Duration of treatment
Isolation should continue until the patient has completed treatment and culture of a
repeated specimen is negative. Patients maybe discharged to their own home if
clinically well. Ward staff must liaise with residential or nursing homes or other
hospitals if the patient is to be discharged there.
Screening for ESBL
If ESBL is suspected take a urine sample for culture and sensitivity on admission or
when the patient is symptomatic. Patients without a catheter do not need screening
for ESBL unless they are known, or suspected, to have previously had ESBL
coliforms.
