Additi<strong>on</strong>al reports <strong>on</strong> fatigue <strong>and</strong> scheduling issues can be found in the Aviati<strong>on</strong> Safety Reporting<br />

System (ASRS) database. The air crashes captured <strong>on</strong> those sites corroborate the potential dire<br />

c<strong>on</strong>sequences <str<strong>on</strong>g>of</str<strong>on</strong>g> pilot fatigue dem<strong>on</strong>strated in research settings.<br />

Pilot Fatigue Woven Through Circumstances <str<strong>on</strong>g>of</str<strong>on</strong>g> FedEx Crash<br />

Fatigue can defeat pilots’ proven performance records <strong>and</strong> progressive companies’ policies. The<br />

object less<strong>on</strong> comes from the July 26, 2002, collisi<strong>on</strong> with trees <strong>on</strong> final approach <str<strong>on</strong>g>of</str<strong>on</strong>g> a Federal<br />

Express B727 cargo jet at Tallahassee, Florida. The three pilots survived the crash, but their airplane<br />

was destroyed. The fatigue factor was woven throughout the Nati<strong>on</strong>al Transportati<strong>on</strong> Safety Board<br />

(NTSB) final hearing <strong>on</strong> the case. “Much more work needs to be d<strong>on</strong>e <strong>on</strong> the fatigue issue, especially<br />

<strong>on</strong> the back side <str<strong>on</strong>g>of</str<strong>on</strong>g> the clock,” said NTSB Member John Goglia. The “back side <str<strong>on</strong>g>of</str<strong>on</strong>g> the clock” is the<br />

expressi<strong>on</strong> <str<strong>on</strong>g>of</str<strong>on</strong>g>ten used to describe the time from midnight to about 6:00 AM, when the human<br />

body’s natural dem<strong>and</strong> for sleep is greatest. As <strong>on</strong>e <str<strong>on</strong>g>of</str<strong>on</strong>g> his last <str<strong>on</strong>g>of</str<strong>on</strong>g>ficial acts after nine years <strong>on</strong> the<br />

board, Goglia chaired the hearing. He noted with dismay that the accident occurred even though<br />

FedEx “is <strong>on</strong>e <str<strong>on</strong>g>of</str<strong>on</strong>g> the more enlightened companies regarding the fatigue issue.” Updating obsolescent<br />

flight time <strong>and</strong> duty time regulati<strong>on</strong>s has been <strong>on</strong> the NTSB “Most Wanted” list <str<strong>on</strong>g>of</str<strong>on</strong>g> aviati<strong>on</strong> safety<br />

improvements since its incepti<strong>on</strong> in 1990. The FedEx crash case has focused attenti<strong>on</strong> <strong>on</strong> this issue,<br />

as it marks at least the third crash in which the safety board has cited pilot fatigue as a c<strong>on</strong>tributing<br />

factor. The two previous fatigue-related crashes were:<br />

* The Aug. 18, 1993, crash <str<strong>on</strong>g>of</str<strong>on</strong>g> American Internati<strong>on</strong>al Airways Flight 808, a DC-8 cargo jet, at<br />

Guantanamo Bay, Cuba, at the end <str<strong>on</strong>g>of</str<strong>on</strong>g> a l<strong>on</strong>g day involving a last-minute runway change as in the<br />

case <str<strong>on</strong>g>of</str<strong>on</strong>g> the FedEx accident, <strong>and</strong><br />

* The June 1, 1999, crash <str<strong>on</strong>g>of</str<strong>on</strong>g> American Airlines [AMR] Flight 1420, an MD-82, at Little Rock, Ark.,<br />

at the end <str<strong>on</strong>g>of</str<strong>on</strong>g> a l<strong>on</strong>g day involving a last-minute runway change, as in the case <str<strong>on</strong>g>of</str<strong>on</strong>g> the FedEx crash.<br />

2.14 Possible Implicati<strong>on</strong>s for <strong>Fire</strong> <strong>Fighters</strong> <strong>and</strong> <strong>EMS</strong> Resp<strong>on</strong>ders<br />

Emergency services, l<strong>on</strong>g haul commercial vehicle operati<strong>on</strong> <strong>and</strong> seafaring may seem divergent<br />

occupati<strong>on</strong>s. However, they are united by their each having l<strong>on</strong>g work hours.<br />

Seafaring has overlap with emergency medical services resp<strong>on</strong>ders because both involve working<br />

in teams or crews in unique circumstances. Also for both sites, workers rely <strong>on</strong> other members’<br />

performance, <strong>and</strong> their tasks <str<strong>on</strong>g>of</str<strong>on</strong>g>ten are accomplished under unpredictable envir<strong>on</strong>mental c<strong>on</strong>diti<strong>on</strong>s.<br />

In additi<strong>on</strong>, for both occupati<strong>on</strong>s, the effects <str<strong>on</strong>g>of</str<strong>on</strong>g> worker fatigue can have catastrophic c<strong>on</strong>sequences.<br />

Seafarers’ efforts to acknowledge, identify <strong>and</strong> deal with l<strong>on</strong>g work hours <strong>and</strong> fatigue, especially as<br />

they relate to the Cardiff recommendati<strong>on</strong>s (Table 2.2) <strong>and</strong> the U.S. Coast Guard efforts have touch<br />

points with fire fighters <strong>and</strong> <strong>EMS</strong> resp<strong>on</strong>ders, <strong>and</strong> the c<strong>on</strong>clusi<strong>on</strong>s about countermeasures presented<br />

in Table 2.2 also would apply for those workers. One aspect missing in marine worker reform<br />

efforts, which is discussed further in Secti<strong>on</strong> 5 (page 63), is the need to involve c<strong>on</strong>siderati<strong>on</strong> <str<strong>on</strong>g>of</str<strong>on</strong>g><br />

workers’ families <strong>and</strong> other psychosocial comp<strong>on</strong>ents in planning <strong>and</strong> assessing work structures.<br />
