<str<strong>on</strong>g>Effects</str<strong>on</strong>g> <str<strong>on</strong>g>of</str<strong>on</strong>g> <str<strong>on</strong>g>Sleep</str<strong>on</strong>g> <str<strong>on</strong>g>Deprivati<strong>on</strong></str<strong>on</strong>g> <strong>on</strong> <strong>Fire</strong> <strong>Fighters</strong> <strong>and</strong> <strong>EMS</strong> Resp<strong>on</strong>ders<br />

Commercial vehicle safety is an arena where rules <strong>and</strong> regulati<strong>on</strong>s are prominent (Federal<br />

Registrar, 2006). However, publicati<strong>on</strong>s c<strong>on</strong>cerning that industry point out the general<br />

ineffectiveness <str<strong>on</strong>g>of</str<strong>on</strong>g> regulatory measures (Knipling et al., 2004). For instance, they cite the example<br />

<str<strong>on</strong>g>of</str<strong>on</strong>g> a worksite that used docking pay for safety rule violati<strong>on</strong>s. Rather than increasing the desired<br />

behavior, they found that there was a paradoxical decrease in the acti<strong>on</strong>s with punitive measures.<br />

Only positive feedback for compliance, not penalties for violati<strong>on</strong>s, resulted in a significant<br />

increase in the health promoting behavior (Zohar, Cohan & Azar, 1980).<br />

Recommendati<strong>on</strong>s <str<strong>on</strong>g>of</str<strong>on</strong>g> occupati<strong>on</strong>al experts <strong>and</strong> available evidence are clear that any effective<br />

work structure system involves workers <strong>and</strong> their families, employee representatives, local<br />

administrators, regulatory bodies <strong>and</strong> <str<strong>on</strong>g>of</str<strong>on</strong>g>ten expert c<strong>on</strong>sultants in designing programs that maximize<br />

physical <strong>and</strong> psychosocial health, safety <strong>and</strong> productivity outcomes.<br />

5.12 Summary <str<strong>on</strong>g>of</str<strong>on</strong>g> Studies <strong>on</strong> Work Hour Management<br />

<strong>Fire</strong> fighting <strong>and</strong> <strong>EMS</strong> resp<strong>on</strong>se work cover a wide spectrum <str<strong>on</strong>g>of</str<strong>on</strong>g> activities <strong>and</strong> c<strong>on</strong>diti<strong>on</strong>s, <strong>and</strong><br />

generalizing recommendati<strong>on</strong>s is difficult. Principles that apply across the range <str<strong>on</strong>g>of</str<strong>on</strong>g> worksites include<br />

becoming educated about the performance <strong>and</strong> health effects <str<strong>on</strong>g>of</str<strong>on</strong>g> fatigue <strong>and</strong> assessing work structures<br />

<strong>and</strong> job dem<strong>and</strong>s. Involving emergency medical services pers<strong>on</strong>nel <strong>and</strong> their families, management,<br />

representatives from labor organizati<strong>on</strong>s <strong>and</strong> nati<strong>on</strong>al administrative bodies, <strong>and</strong> sometimes outside<br />

c<strong>on</strong>sultants is important in the success <str<strong>on</strong>g>of</str<strong>on</strong>g> any fatigue management program. <str<strong>on</strong>g>Sleep</str<strong>on</strong>g> science <strong>and</strong> the<br />

informati<strong>on</strong> c<strong>on</strong>tained in this report are resources for fire fighters <strong>and</strong> <strong>EMS</strong> resp<strong>on</strong>ders in those<br />

efforts.<br />

• Those working l<strong>on</strong>g durati<strong>on</strong> shifts can improve their well being by leading healthy lifestyles.<br />

Chr<strong>on</strong>ic sleep deprivati<strong>on</strong> may not be recognized, <strong>and</strong> it is important for workers to<br />

acknowledge their need for <strong>and</strong> maximize their ability to achieve adequate restorative sleep.<br />

• Coping with l<strong>on</strong>g work hours may be facilitated by identifying workers at higher risk for<br />

difficulties in adjusting , such as those with sleep disorders.<br />

• Fatigue is a risk for motor vehicle crashes, <strong>and</strong> commuting home following l<strong>on</strong>g durati<strong>on</strong><br />

shifts may be an especially vulnerable time for workers.<br />

• Pers<strong>on</strong>nel, their families, management <strong>and</strong> c<strong>on</strong>sultants, working in collaborati<strong>on</strong>, are best<br />

able to structure work hours <strong>and</strong> circumstances to meet the needs <str<strong>on</strong>g>of</str<strong>on</strong>g> pr<str<strong>on</strong>g>of</str<strong>on</strong>g>essi<strong>on</strong>al excellence<br />

<strong>and</strong> employee well being that typify fire fighting <strong>and</strong> <strong>EMS</strong> work.<br />
